Although surgical resection remains the best option as potentially curative therapy for hepatocellular carcinoma, radiofrequency thermal ablation has begun to receive much attention as an effective minimally invasive technique for the local control of unresectable malignant hepatic tumors. Most recent radiofrequency devices equipped with a powerful generator and larger needle electrode permit larger thermal lesions, up to 5 cm in diameter, with a single ablation. In this article, the author reviews the technical developments and early clinical results obtained with radiofrequency ablation techniques.
Background: Facet joint have been implicated as a source of chronic low back pain. Radiofrequency denervation has demonstrated the most solid evidence. To increase safety and efficacy of treatment, computed tomography (CT) guidance injection has been used in several disease. The purpose of this study was to evaluate the efficacy of CT-guided radiofrequency rhizotomy in the treatment of facet joint pain. Methods: A total of 40 patients were randomized to undergo radiofrequency facet joint denervation under CT guidance or C-arm guidance. All patients were examined visual analogue scale (VAS) score before treatment, 1 month, and 3 months after treatment. Results: The VAS in both groups showed significant improvement over the 1-month interval. No significant difference in the VAS score among the group was observed. Conclusions: In this study there was no significant difference between CT guidance lumbar rhizotomy and C-arm guidance lumbar rhizotomy. Therefore CT-guided radiofrequency denervation of the lumbar facet joint was a minimally invasive technique that appears effective.
Background: This study tests the hypothesis that of bipolar radiofrequency thermocoagulation of the thoracic facet joint capsule may provide a safe and effect method of pain control from thoracic facet origin. Methods: Among patients suffering from localized mid back pain, nine patients with thoracic facet disease confirmed by magnetic resonance image and diagnostic thoracic facet block were enrolled. Bipolar radiofrequency ablation in the inferior aspect of the thoracic facet joint was done. Visual Analog Scale (VAS) was measured pre-intervention and 1 month post-intervention. Any complications and changes in amount of pain medication were recorded. Results: Significant 47.6% reduction in VAS was noted at 1 month. There were no serious complications. Conclusions: Intra-articular bipolar radiofrequency thermocoagulation of the thoracic facet joint may be a technically easier and valid method of treating mid back pain of thoracic facet origin.
When medical therapy fail to relieve pain at tolerable level for patients confirmed with trigeminal neuralgia, presence of mass lesion excluded, surgery is indicated. Innumerable surgical strategies have been attempted for the treatment of trigeminal neuralgia but only four have proven appropriate: (1)stereotactic radiofrequency gasserian ganglionotomy, (2) percutaneous glycerol gangliolysis, (3) percutaneous microcompression, (4) microvascular decompression. Radiofrequency thermocoagulation of the gasserian ganglion stems from the efforts of Sweet. This technique is the surgical treatment of choice around the world for surgical treatment for trigeminal neuralgia. Since 1986, over 14,000 cases have been reported utilizing this technique. To improve the treatment method further, an electrode with a flexible curved tip has been developed for easier and more precise electrode placement and lesion production during the thermocoagulation of gasserian ganglion. This operation was performed recently on three patients at Hallym University Hospital. using a curved tip electrode. Complete relief of pain was achieved for all patient. However, some complications were noted.

A C2-3 zygapophygeal joint is a major source of cervicogenic headache. Radiofrequency (RF) neurotomy is preformed widely for zygapophygeal joint pain. Conventional RF denervation technique is generally performed under fluoroscopic control. Recently, ultrasound-guided radiofrequency on zygapophygeal joint has emerged as an alternative method. We report our experiences of two successful ultrasound-guided pulsed radiofrequencies on 39-year-old and 42-year-old males, who complained occipital headache and posterior neck pain.
Destruction of the gasserian ganglion can be carried out by creating a radiofrequency lesion under biplanar fluoroscopic guidance. This procedure is reserved for patients who have failed various interventions for intractable trigeminal neuralgia including retro-gasserian injection of glycerol and whose physical status otherwise precludes more invasive neuro-surgical treatments such as microvascular decompression. Radiofrequency thermocoagulation of the gasserian ganglion provides a safe method of achieving long-standing relief from trigeminal neuralgia with low risk. This technique is currently emerging worldwide as the surgical treatment of choice for trigeminal neuralgia. Recently we performed a successful radiofrequency gasserian ganglionotomy, without any complication, under fluoroscopic guidance. The procedure was successful and complete pain relief was achieved for a patient who already had treatments of various interventions including microvascular decompression but never experienced pain relief.

Complex regional pain syndrome secondary to brachial plexus injury is often severe, debilitating and difficult to manage. Percuteneous radiofrequency sympathectomy is a relatively new technique, which has shown promising results in various chronic pain disorders. We present four consecutive patients with complex regional pain syndrome secondary to brachial plexus injury for more than 6 months duration, who had undergone percutaneous T2 and T3 radiofrequency sympathectomy after a diagnostic block. All four patients experienced minimal pain relief with conservative treatment and stellate ganglion blockade. An acceptable 6 month pain relief was achieved in all 4 patients where pain score remained less than 50% than that of initial score and all oral analgesics were able to be tapered down. There were no complications attributed to this procedure were reported. From this case series, percutaneous T2 and T3 radiofrequency sympathectomy might play a significant role in multi-modal approach of CRPS management.
배경: Cox maze 수술은 거의 20년 가까이 심방세동의 표준적인 외과적 치료방법으로 이용되었다. 최근 저자들은 Cox maze 수술에서 대부분의 절개선을 양극고주파전극(Cox maze IV procedure)으로 대치하였다. 저자들은 지속성 심방세동의 수술적 치료에 양극고주파전극을 이용한 Cox maze 수술의 결과를 조사하고 절개-봉합법을 이용한 Cox maze III 수술의 결과와 비교하였다. 대상 및 방법: 2005년 4월부터 2007년 7월까지 40명의 환자가 양극고주파전극으로 Cox maze 수술을 받았다. 그 결과를 절개-봉합법으로 수술 받은 35예의 결과와 비교하였다. 모두 다른 심장수술과 함께 심방세동 수술을 받았으며, 수술 후 1내지 2개월마다 추적 조사하였다. 결과: 수술 후 6개월에 정규리듬의 전환율은 양극고주파전극방법과 절개-봉합방법 사이에 차이가 없었고(95.0%/97.1%, p=1.0), 마지막 추적시간까지 정규리듬 전환율도 차이가 없었다(92.5%/91.6%, p=1.0). 다변량 분석(Cox-regression)으로 양극고주파군에서 심방세동의 지속 및 재발의 위험인자는 수술 후 좌심방의 내경이었고(hazard ratio 31, p=0.005), 양군 전체의 환자에서 수술 후 심방세동의 지속 및 재발의 위험인자(Cox-regression)는 수술 6개월에 심방세동의 출현(hazard ratio 92.24, p=0.003)과 수술 후 좌심방의 내경(hazard ratio 16.05, p=0.019)이었다. 대동맥차단시간과 체외순환시간은 양극고주파전극군에서 더 짧았다. 결론: Cox maze 수술 시 양극고주파전극의 사용은 절개-봉합법과 같이 우수한 정규리듬 전판율을 보이며, 수술 후 좌심방의 크기가 심방세동의 지속 및 재발에 영향을 주는 독립인자였다.
Objectives : This report is intended to suggest that lentigo can be removed using the cauterization. Methods : Handle of the needle, which is distal part to the tip and shaft, was heated with an alcohol lamp to cauterize the lentigo one by one to form a crust, and wet dressing was applied to the treatment sites by using Hwangryunhaedok-tang pharmacopuncture solution. Results : Ten days after the cauterization, the lentigo was cleared from the normal skin without adverse reactions. Conclusions : The cauterization is a useful technique for removing lentigo, but the conventional method is somewhat inconvenient, so a radiofrequency technique or a laser-based cauterization may be more useful Instead of the traditional cauterization.
