토끼의 갑상선 측정 (Determination of Thyroid Secretion Rate in Rabbit)

  • A method for determination of thyroid secretion rate in rabbit by means of radioactive iodine presented. After injection of radioactive iodine, in vivo determination so f radioactivity in thyroid gland were made during a 19 day-experimental period. In the same period blood samples were drawn and analyzed for protein-bound iodine (PBI) and for protein-bound radioactive iodine(PBI181). A rate constant for secretion of thyroid hormone was calculated from the disappearance rate of radioactive iodine in thyroid gland. The secretion rate of radioactive hormone iodine was calculated by multiplying this rate constant by the amount of radioactive iodine present in thyroid gland. Assuming that the specific radioactiveness of the circulating thyroid hormone and of the hormone just secreted were identical , thyroid secretion rate was calculated by the equation. {{{{ { Secreted hormone-iodine , gamma /hr} over { Secreted hormone-I^131, % dose/hr }= { PBI, ${\gamma}$/ml.Serum} over { PBI^131 , % dose/ml . A method for properties evaluation of activated charcoal sorbents in iodine capture under dynamic conditions

  • Experimental equipment for studying the sorption properties of iodine sorbents using radioactive methyliodide has been developed. The sorption capacity index ${\alpha}$ is proposed as a criterion parameter for assessing the quality of impregnated activated charcoals. It was found that this parameter does not depend on the dynamic conditions during the sorbent test. It was shown that values of the sorption capacity index allow to recommend iodine sorbents for industrial gas cleaning processes.

갑상선암 환자의 방사성요오드 치료경험 (The Experience of Receiving Radioactive Iodine Therapy among Thyroid Cancer Patients)

  • Purpose: The purpose of this study was to explore the meaning of the experience of receiving radioactive iodine therapy among patients with thyroid cancer. Methods: A qualitative research design was adopted. The participants were ten women diagnosed with thyroid cancer who had received radioactive iodine therapy within one year. Data were collected through in-depth interviews from October of 2015 to April of 2016. Individual interviews were recorded, and transcribed data were analyzed using Colaizzi's method. Results: The six categories of the experience of receiving radioactive iodine therapy were "Finally realizing having cancer," "The lonely fight that feels like prison life," "Narrower scope of life," "Lack of understanding by others," "Enduring a short, yet difficult journey," and "A turning point for a new life." Conclusion: This study provides deep insight into the experience of thyroid cancer patients who had received radioactive iodine therapy. Nurses should concern their distress during radioactive iodine treatment and manage psychological difficulties as well as physical symptoms. Support from family and health care providers may help them to overcome the hard journey.

갑상선암에서 방사성 요오드 치료 후 발생한 코눈물길 폐쇄 (Nasolacrimal Duct Obstruction after Radioactive Iodine Therapy for Thyroid Cancer)

  • 목적: 갑상선암에서 방사성 요오드 치료 후 발생한 코눈물길 폐쇄의 임상양상에 대해 알아보고자 하였다. 대상과 방법: 2009년 1월부터 2011년 12월까지 갑상선암 수술 후 방사성 요오드 치료를 받은 환자 총 622명 중 눈물흘림 증상으로 성형안과에서 관류검사, 더듬자 검사 및 눈물주머니 조영술을 통해 코눈물길 폐쇄를 진단 받은 14명(18안)을 대상으로 하였다. 대상환자의 방사성 요오드 치료량과 횟수, 코눈물길 폐쇄의 임상양상, 치료법을 의무기록을 후향적으로 분석하여 확인하였다. 결과: 코눈물길 폐쇄가 진단된 환자 14명은 코눈물길 폐쇄가 발생하지 않은 환자군에 비해 평균 치료 용량($215.7{\pm}23.1mCi$, p=0.01)과 치료횟수($1.36{\pm}0.50$회, p<0.001)가 유의하게 높았다. 눈물흘림 증상 발생까지 평균 10.2개월, 진단까지 평균 18.4개월이 걸렸다. 14명(18안) 중 공통눈물소관폐쇄는 3명(3안), 코눈물관폐쇄는 11명(15안)이었다. 10명(13안)은 눈물길의 완전폐쇄가 확인되어 내시경하눈물주머니 코안연결술을 시행 받았으며, 4명(5안)은 부분폐쇄를 보여 내시경하 실리콘 관을 삽입하였다. 결론: 갑상선암에서 방사성 요오드 치료 후 코눈물길 폐쇄가 드물게 발생할 수 있다. 방사성 요오드 치료 후 눈물흘림 증상을 호소하는 환자가 있을 경우 합병증으로 코눈물길 폐쇄가 발생할 수 있음을 인지하고, 초기에 성형안과에 의뢰하여 전문적인 평가 및 치료가 시행되어야 한다.

  • Purpose: This study was to identify the symptom severity, interference and their psychological predictors in thyroid cancer patients hospitalized for radioactive iodine administration. Methods: One hundred seventy-seven thyroid cancer patients admitted to the isolation room for Iodine ($I^{131}$) therapy were recruited. Subjects were asked to complete the questionnaire on core symptoms, thyroid cancer symptoms, interference, depression and state anxiety in the evening after receiving radioactive iodine therapy. Data was analyzed using frequency, percentage, mean, Pearson's correlation, and multiple regression with SPSS vs. 19. Results: Lack of appetite, drowsiness, sleep disturbance, fatigue, and nausea were the 5 most core symptoms. More than 20% of patients experienced moderate to severe thyroid cancer symptoms including feeling cold, hoarseness, swallowing difficulty, and feeling hot. More than 30% of subjects experienced moderate to severe interferences in mood, general activity, and 22% in walking. Depression and state anxiety were identified as predictors of core symptoms, thyroid symptom severity and interference. Conclusion: Nursing interventions to reduce the symptom severity and interference need to be developed by considering thyroid cancer patients' depression and anxiety when hospitalized in the isolation room for radioactive iodine administration.

Selection of a carrying agent for obtaining radioactive methyliodide vapors under dynamic conditions

  • A method for preparing "reagent" for radioactive methyliodide vapors production using an isotopic exchange reaction has been developed. Based on the obtained data of the isotopic exchange efficiency and hydraulic resistance, white fused alumina (700-840 ㎛) was selected as the carrying agent material for "reagent" production. The radioiodine isotopic exchange dependences on such parameters as temperature, gas flow velocity, and the methyliodide concentration in it were determined. Optimal conditions have been selected to achieve 85% of the isotopic exchange rate in 1 h of the experiment. The obtained data allowed to develop an approach to the test of iodine filters for nuclear power plants and to determine their efficiency.

술후 방사성 요오드 사멸요법을 받은 갑상선암 환자들의 재발 관련인자에 관한 연구 (The Study on Recurrence-Related Factors of the Thyroid Cancer Patients Received Postoperative Radioactive Iodine Ablation Therapy)

  • Background and Objective: Factors that are associated with the recurrence after radioactive iodine ablation therapy have not been identified yet. The aim of this study is to elucidate the factors that are related to the recurrence after thyroid surgery of the thyroid papillary cancer followed by radioactive iodine ablation therapy. Patients and Methods: Fifty four cases who had underwent thyroid cancer surgery and postoperative radioactive iodine ablation therapy were included in this study. Mean followup duration was 7 years. There were 41 women and 13 men. Data analysis was done retrospectively with medical record review. Chi-square test and Fisher's exact test was used for the statistical analysis. Results: Age over 40, capsular invasion, and loca invasion were the factors that were associated with the high rate of recurrence. But sex, size of the tumor, multiplicity and extent of the surgery were not related to the recurrence. Conclusion: Without the curative resection of the tumor, radioactive iodine ablation therapy cannot lower the recurrence rate. So aggressive resection of the thyroid papillary cancer is important.The more data accumulated and the longer the followup, the easier we can reveal the recurrence-related factors of postoperative radioactive ablation therapy.

Removal of radioactive methyliodide from the gas stream with a composite sorbent based on polyurethane foam

  • A composite iodine sorbent was obtained in the form of porous polymer matrix with activated carbon particles impregnated with triethylenediamine deposited on its surface. A comparative assessment of the radioactive methyliodide capturing efficiency by the composite sorbent and a sample of industrial charcoal sorbent was conducted. It was shown that under the selected testing conditions, the hydraulic resistance of the composite sorbent is lower, and the sorption capacity is higher than that of the industrial charcoal sorbent. A method for comparing the effectiveness of iodine sorbents, based on the calculation of the ratio of the sorption capacity index to the minimum capacity index, needed for the required purification degree was proposed.

방사성옥소 투여에 따른 말초혈액 림프구 수의 변화 및 염색체이상 빈도의 관찰 (Observation on The Frequency of Chromosomal Aberration and Changes in Number of Peripheral Lymphocytes in Radioactive Iodine Treatment)

  • 배경 : 방사선 외부 피폭에 대한 생물학적 선량측정방법에는 혈액학적 지표인 림프구 변화와 세포유전학적 지표인 염색체 분석을 통한 Ydr값이 가장 많이 사용되고 있다. 방사성 옥소 투여시와 같은 방사선 내부 피폭에 대해서는 생물학적 선량측정 방법에 대한 연구가 미흡하여 피폭정도는 물론 방사성 옥소 투여용량을 반영할 수 있는지의 여부도 알려져 있지 못하다. 목적 : 갑상선 질환자에게 방사성 옥소 투여후 림프구 변화와 Ydr값을 추적관찰하여 이들을 방사선 내부 피폭에 대한 지표로 이용할 수 있는지 알아보고자 하였다. 방법 : 갑상선 기능항진증 5명과 갑상선암 수술을 받은 35명의 환자를 대상으로 방사성 옥소 투여후 말초혈액 림프구 수를 2개원이상 추적검사 하였고, 림프구의 염색체 분석을 통해 Ydr값을 구했다. 결과: 1) 림프구 수는 방사성 옥소 투여 2주후 부터 감소되기 시작하여 6주와 8주후에 최대로 감소된후 점차 회복되었다. 2) 방사성 옥소량이 증가할 수록 림프구 수는 감소했다 (P<0.01) 3) Ydr값은 2주-8주 사이에는 대체로 일정하였다. 4) 방사성옥소 투여량에 따른 최대 Ydr값의 변화는 유의한 상관관계를 보였다 (p<0.00) 5) 2주째 Ydr값은 방사성옥소 투여량이 증가할 수록 증가하였다 (p<0.00) 6) 2주째 Ydr값은 2주째 림프구 수의 감소정도와 비례관계를 보였다 (p<0.00) 결론: 1) 방사성 옥소의 통상적 치료용량은 일시적 골수부전과 경도의 염색체이상을 초래하므로 8주 이상의 면밀한 추적관찰이 요구된다. 2) 생물학적 선량측정 방법으로서의 최대 림프구 감소치와 2주째 Ydr값과 최대 Ydr값은 방사성옥소 투여용량을 반영하는 지표로 사용할 수 있다.
