A Study of Radiation Dose Evaluation and Optimization Methods for Intra Oral Dental X-ray in Pediatric Patient (소아 구내촬영 시 방사선량 평가 및 최적화 방안에 대한 연구)

  • Although intra oral dental x-ray is a lower dose than other radiological examinations, pediatric patients are known to have a higher risk of radiation damage than adults. For this reason, pediatric dental x-ray requires management of dose evaluation and imaging conditions during the examination. In this study, the dose calculation program ALARA-Dental(child/adult) was used to evaluate the organ dose and effective dose exposed to each examination site during intra oral imaging of children during dental radiographic examination, and dose analysis according to the imaging conditions was performed. As a result, the highest organ dose distribution was shown at 0.044 ~ 0.097 mGy in all are as of the mucous membrane of oral cavity except for the maxillary incisors and canines. Also, in the case of the thyroid gland, the maxillary canine and maxillary premolar examination showed 0.027 and 0.020 mGy, respectively, and the dose distribution was 15.4% to 70.0% higher than that of the mandibular examination. As for the effective dose calculated during intra oral imaging, the maxillary anterior and canine examinations showed the highest effective doses of 0.005 and 0.004 mSv, respectively, and the maxillary area examination showed a higher dose distribution on average than the mandible.

Importance of Job Tasks, Job Training Requirement and Work Satisfaction felt by Dental Hygienists (치과위생사의 수행업무의 중요성·업무에 대한 교육훈련의 필요도와 만족도 조사연구)

  • The study aimed to estimate the importance of job tasks, job training requirement and work satisfaction felt by dental hygienists to help dental hygienists work efficiently and effectively. A total of 142 dental hygienists working in dental hospitals located in J participated in the study. A SPSS 10.10 for Windows was used for statistical analysis. The statistical significance was defined as ${\alpha}$=.05. The results of the study are summarized as follows: 1. The mean score for the importance of job tasks was 4.09. The respondents considered dental clinic management and assistance to dentists the most important among their job descriptions showing the mean score of 4.44. The need for job training was 4.15 in average. The respondents wanted to have training on how to make a dental health insurance claim to receive payment other than any training with the mean score of 4.42. The work satisfaction of the respondents was 3.65 in average. The respondents were most satisfied with their work in dental assistance. 2. By the category of job tasks, the levels of importance of job tasks, job training requirement and work satisfaction were higher in plague removal with an ultrasonic scaler related to clinical oral prophylaxis, pit and fissure sealants related to preventive dental treatment and instruction in the correct usage of toothbrush related to public dental health education 3. In dealing with dental radiation machines, the radiation protection was considered the most important. While the training for periapical radiography was most needed, the respondents were satisfied with working in the same area. The levels of job importance, training requirement and work satisfaction were high in preventive dental care in the community performed in relation to the public dental health care. 4. In the dental health insurance claim category, the levels of importance of job task, training requirement and work satisfaction were higher in insurance claiming. The levels of job importance and training requirement were high in dental clinic management and assistance to dentists performed for preventive dental treatment. The work satisfaction was higher in maintaining medical chart records. The levels of job importance, training requirement and work satisfaction were higher in dental assistance in relation to dental protection. 5. There was significant relationship between the level of importance and work experience. Those with a longer period of work experience had higher levels of job importance in dental health insurance claim, dental clinic management and assistance to dentists and the overall task(pE0.05). 6. A significant relationship was observed between the need for job training in dental cleaning and preventive dental treatment and work experience. Those with more than 8 years of work experience demonstrated that job training is most needed(pE0.05). 7. The work satisfaction level was higher in the group with more than 8 years of work experience, compared with those in other work experience periods(pE0.05). 8. A positive correlation was seen between the level of importance and the need for job training among three variables.

  • Red tide(harmful algae) in the Korean Coastal Waters has a given a great damage to the fishery every year. However, the aim of our study understands the influence of meteorological factors (air and water tempaerature, precipitation, sunshine, solar radiation, winds) relating to the mechanism of red tide occurrence and monitors red tide by satellite remote sensing, and analyzes the potential area for red tide occurrence by GIS. The meteorological factors have directly influenced on red tide formation. Thus, We want to predict and apply to red tide formation from statistical analyses on the relationships between red tide formation and meteorological factors. In future, it should be realized the near real time monitoring for red tide by the development of remote sensing technique and the construction of integrated model by the red tide information management system (the data base of red tide - meteorological informations).

  • Malignant glomus tumor is a very rare disease originating from the paraganglia system through the body. Glomus tumor, also known as paraganglioma, usually are considered benign, and arises in a variety of head and neck locations, most of which include the carotid body, the vagus nerve, and the jugulotympanic area. The most widely accepted management of benign glomus tumor is surgical extiration. Here, we report a case of recurrent laryngeal glomus tumor which is proven malignant and metastatic to the brain and the lungs. We have treated the patient with combination chemotherapy and radiation to the brain, the result of which is partial response in terms of decreased size of metastatic lung lesions.

Synchronous Primary Cancer in Hypopharyngeal Cancer (하인두암에 병발한 동시성 암종)

  • Multiple primary malignant neoplasms occur relatively frequently today and are important especially in the head and neck area for they usually carry a bad prognosis. Detection of a synchronous primary tumor at the time of initial work-up is crucial both for management and final outcome. The first case was a T1 hypopharyngeal cancer with a mid-esophageal second primary who complained of a huge neck node. The second case was a T3 hypopharyngeal cancer who was initially seen by the chest surgeons for a large lower esophageal tumor. The third case was a patient previously operated for stomach adenocarcinoma three years ago, who had newly developed symptoms like dysphagia and hoarseness, and was diagnosed as hypopharyngeal T3 with oropharyngeal second primary cancer. Three cases were all heavy smokers and had histories of heavy alcohol consumption. They were all treated at the same sitting by en-block resection of the involved organs and postoperative radiation therapy. The authors have recently experienced 3 cases of synchronous second primary cancers in association with hypopharyngeal cancer and a report is made.

Independent Generation System Design for the Economic Management of Electrical Charging Stations (전기충전소의 경제적 운영을 위한 독립발전 시스템 설계)

  • This paper presents the optimal energy generation systems for economical EVs(Electric Vehicles) charging stations located in an island area. The system includes grid electricity, diesel generator and renewable energy sources of wind turbines and PV(Photovoltaic) panels. The independent generation system is designed with data resources such as annual average wind speed, solar radiation and the grid electricity price by calculating system cost under different structures. This sensitive analysis on the varying data resources allows for the configuration of the most economical generation system for charging stations by comparing initial capital, operating cost, NPC(Net Present Cost) and COE(Cost of Energy). Depending on the increase of the grid cost, the NPC variation of the most economical system which includes renewable energy generations and grid electricity can be smaller than those of other generation systems.

Preliminary Surgical Results of Open Sella Method with Intentionally Staged Transsphenoidal Approach for Patients with Giant Pituitary Adenomas

  • Objective: This study is designed to evaluate the clinical outcome, the safety and the effectiveness of the open sella methods(OSM) with intentionally staged transsphenoidal approach(TSA) for giant pituitary adenomas(GPA). Methods: Eight patients with GPA were managed by the OSM with intentionally staged TSA. There were 5 nonfunctioning adenomas, 2 prolactin-secreting adenomas, and 1 growth hormone-secreting adenoma. Among them, 6 patients underwent two times of TSA, one patient underwent three times of TSA, and the other patient underwent two times of TSA followed by radiation therapy. The mean time interval between staged operations was 3.9 months except for one case. Results: Seven out of the eight patients with GPA treated with the OSM with intentionally staged TSA showed that the tumors were completely removed on magnetic resonance imaging and that they were free from headache and visual problem suffered previously. Only one patient experienced severe complications including panhypo-pituitarism, cerebrospinal fluid rhinorrhea and permanent diabetes insipidus. Conclusion: With the surgical treatment for 8 cases of GPA, which extended to the suprasellar and parasellar area, we suggest that the OSM with intentionally staged TSA is a safe and effective method in management for GPA.

Characterization of Particulates Containing Naturally Occurring Radioactive Materials in Phosphate Processing Facility (인광석 취급 산업체에서 발생하는 천연방사성물질 함유 입자의 특성 평가)

  • Phosphate rock, phosphogypsum, and products in phosphate processing facility contain naturally occurring radioactive materials (NORM). Therefore, they may give rise to enhanced radiation dose to workers due to inhalation of airborne particulates. Internal dose due to particle inhalation varies depending on particle properties. The objective of the present study was to characterize particle properties at the largest phosphate processing facility in Korea. A cascade impactor was employed to sample airborne particulates at various processing areas in the plant. The collected samples were used for characterization of particle size distribution, particle concentration in the air, and shape analysis. Aerodynamic diameters of airborne particulates ranged 0.03-100 ${\mu}m$ with the highest concentration at the particle size range of 4.7-5.8 ${\mu}m$ (geometric mean = 5.22 ${\mu}m$) or 5.8-9.0 ${\mu}m$ (geometric mean = 7.22 ${\mu}m$). Particle concentrations in the air varied widely by sampling area up to more than two orders of magnitude. The large variation resulted from the variability of mechanical operations and building ventilations. The airborne particulates appeared as spheroids or rough spherical fragments across all sampling areas and sampled size intervals. Average mass densities of phosphate rocks, phosphogypsums, and fertilizers were 3.1-3.4, 2.1-2.6, and 1.7 $gcm^{-3}$, respectively. Radioactivity concentration of uranium series in phosphate rocks varied with country of origin, ranging 94-866 $Bqkg^{-1}$. Among the uranium series, uranium was mostly concentrated on products, including phosphoric acid or fertilizers whereas radium was concentrated on byproducts or phosphogypsum. No significant radioactivity of $^{226}Ra$ and $^{228}Ra$ were found in fertilizer. However, $^{40}K$ concentration in fertilizer was up to 5,000 Bq $g^{-1}$. The database established in this study can be used for the accurate risk assessment of workers due to inhalation of airborne particles containing NORM. In addition, the findings can be used as a basic data for development of safety standard and guide and for practical radiation safety management at the facility.

Evaluation of the Accuracy for Respiratory-gated RapidArc (RapidArc를 이용한 호흡연동 회전세기조절방사선치료 할 때 전달선량의 정확성 평가)

  • The position of the internal organs can change continually and periodically inside the body due to the respiration. To reduce the respiration induced uncertainty of dose localization, one can use a respiratory gated radiotherapy where a radiation beam is exposed during the specific time of period. The main disadvantage of this method is that it usually requests a long treatment time, the massive effort during the treatment and the limitation of the patient selection. In this sense, the combination of the real-time position management (RPM) system and the volumetric intensity modulated radiotherapy (RapidArc) is promising since it provides a short treatment time compared with the conventional respiratory gated treatments. In this study, we evaluated the accuracy of the respiratory gated RapidArc treatment. Total sic patient cases were used for this study and each case was planned by RapidArc technique using varian ECLIPSE v8.6 planning machine. For the Quality Assurance (QA), a MatriXX detector and I'mRT software were used. The results show that more than 97% of area gives the gamma value less than one with 3% dose and 3 mm distance to agreement condition, which indicates the measured dose is well matched with the treatment plan's dose distribution for the gated RapidArc treatment cases.

Actual Condition of Quality Control of X-ray Imaging System in Primary Care Institution: focused on Gwangju Metropolitan City (1차 의료기관의 엑스선 발생장치 정도관리에 관한 현황조사:광주광역시 지역을 중심으로)

  • With the expanded use of radiation in modern medical practices, the most important issue in regards to efforts to reduce individual exposure dose is quality assurance. Therefore in order to study the present condition of quality assurance, the Gwangju Metropolitan City area was divided into five districts each containing ten hospitals. Four experiments were conducted: a reproducibility experiment for kVp, mA, and examination time (sec) intensity of illumination; half-value layer (HVL) measurement; and beam perpendicularity test matching experiment. The tube voltage reproducibility experiment for all fifty hospitals resulted in a 95.33% passing rate and mA and examination time both resulted in a 77.0% passing rate. The passing rate for intensity of illumination was 86.0% and 52.0% for HVL, which was the lowest passing rate of all four factors. For the beam perpendicularity test matching experiment, generally the central flux is matched to within $1.5^{\circ}$. Of all fifty hospitals 30.0% were beyond $3^{\circ}$. The results of the survey showed that 58% responded that they knew about quality assurance cycle. All fifty respondents stated that they have not received any training in regards to quality assurance at their current place of employment. Although quality assurance is making relative progress, the most urgent issue is awareness of the importance of quality assurance. Therefore, the implementation of professional training focusing on safety management and accurate quality assurance of radiation will reduce the exposure to radiation for radiologists and patients and higher quality imaging using less dosage will also be possible.
