Dosimetric Analysis of Lung Stereotactic Body Radiotherapy Using Halcyon Linear Accelerator

  • Purpose: In this study, the dosimetric characteristics of lung stereotactic body radiotherapy (SBRT) plans using the new Halcyon system were analyzed to assess its suitability. Methods: We compared the key dosimetric parameters calculated for the Halcyon SBRT plans with those of a conventional C-arm linear accelerator (LINAC) equipped with a high-definition multileaf collimator (HD-MLC)-Trilogy Tx. A total of 10 patients with non-small-cell lung cancer were selected, and all SBRT plans were generated using the RapidArc technique. Results: Trilogy Tx exhibited significant superiority over Halcyon in terms of target dose coverage (conformity index, homogeneity index, D0.1 cc, and D95%) and dose spillage (gradient). Trilogy Tx was more efficient than Halcyon in the lung SBRT beam delivery process in terms of the total number of monitor units, modulation factor, and beam-on time. However, it was feasible to achieve a dose distribution that met SBRT plan requirements using Halcyon, with no significant differences in satisfying organs at risk dose constraints between both plans. Conclusions: Results confirm that Halcyon is a viable alternative for performing lung SBRT in the absence of a LINAC equipped with HD-MLC. However, extra consideration should be taken in determining whether to use Halcyon when the planning target volume setting is enormous, as in the case of significant tumor motions.

흉부 및 복부에서 AEC 적용에 따른 MDCT의 선량 감소 효과 (Radiation Dose Reducing Effect during the AEC System in the Chest and Abdomen of the MDCT Scanning)

  • CT검사에 따른 방사선 선량의 유해성에 대한 관심이 고조되고 있어, 고정관전류기법과 AEC(automatic exposure control)를 적용하여 선량의 감소 효과를 비교하였다. 64MDCT를 이용하여 인체조직등가물질 팬텀으로 흉부 및 복부 부위를 고정관전류기법과 AEC를 적용하여 CTDIvol과 DLP를 비교하였고, 영상의 평가는 관심영역으로 CT감약계수와 노이즈를 측정하여 비교하였다. 흉부에서 고정관전류기법과 AEC적용에 따른 선량감소 효과는 CTDIvol 35.2%, DLP 49.3%, 복부에서는 CTDIvol 5.9%, DLP 3.2% 감소 효과가 있었다. CT검사에서 자동노출장치인 AEC를 이용하여 선량의 감소효과가 있다. CT 검사에 따른 환자의 피폭선량을 줄이기 위해서는 AEC를 이용하여야 한다.

방사선치료에서 환자맞춤형 선량품질보증 장치의 비교 (A Comparison of Patient-specific Delivery Quality Assurance (DQA) Devices in Radiation Therapy)

  • This study aimed to compare the results of delivery quality assurance (DQA) using MapCHECK and OCTAVIUS for radiation therapy. Thirty patients who passed the DQA results were retrospectively included in this study. The point dose difference (DD) and gamma passing rate (GPR) were analyzed to evaluate the agreement between the measured and planned data for all cases, Plan complexity was evaluated to analyze dosimetric accuracy by quantifying the degree of modulation according to each plan. We analyzed the monitor units (MUs) and total MUs for each plan to evaluate the correlation between the MUs and plan complexity. We used a paired t-test to compare the DD and GPRs that were obtained using the two devices. The DDs and GPRs were within the tolerance range for all cases. The average GPRs difference between the two devices was statistically significant for the brain, and head and neck for gamma criteria of 3%/3 mm and 2%/2 mm. There was no significant correlation between the modulation index and total MUs for any of the cases. These DQA devices can be used interchangeably for routine patient-specific QA in radiation therapy.

부산지역 의료기관의 흉부촬영 조건과 피폭선량에 관한 조사연구 (A Study on Radiographical Conditions and Exposure Doses During Chest Radiography at Medical Facilities in Pusan)

  • This study was carried out to investigate radiographical and operating conditions of X-ray units and exposure doses to patients during chest radiography, so that the results could provide basic data used for reducing the exposure dose and for providing the diagnostic information with better quality. The conditions and exposure doses of 100 X-ray units mainly used for chest radiography were examined and also 100 radiological technologists mainly handling those apparatus at 76 medical facilities in Pusan were surveyed using a questionnaire from October 1 to December 31 in 1995. The following results were obtained from the study : 1. It was found that most units were capable of taking a high tube voltage radiography by showing 67% of the units equipped with the maximum tube voltage of 150 kV, 94% with more than 500 mA for the rating capacity and 85% with the full wave type of a signal phase. 2. For actual chest radiographical conditions, however, 80% of the units were operated at $60{\sim}100\;kVp$ and only 14% at 100 kVp and over for the high tube voltage. 3. The average exposure time was less than 0.1 second, and eighty four percent of the units adapted the X-ray tube currents ranging from 200 to 300 mA, 80% the focus-film distances between 180 and 210 cm, and 63% the focus sizes of more than 2.0 mm. 4. Most units(98%) employed additional filters made of aluminum, 75% the thickness of filters less than 2.0 mm, and only 2 units the compound filters. 5. Ortho chromatic system was only adopted in 13% of screen film system for the units, and 73% used the grid ratio at 8 : 1 for the low tube voltage during chest radiography. 6. The average exposure dose of all X-ray units during chest radiography was $371\;{\mu}Sv$ with a difference of about 16 times between the minimum to the maximum, and $386\;{\mu}Sv$ both at hospitals and at health centers, followed by $380\;{\mu}Sv$ at general hospitals and $263\;{\mu}Sv$ at university hospitals without showing any statistically significant differences. In conclusion, since patients during chest radiography at medical facilities in Pusan exposed to high levels of radiation, it is recommended that appropriate added filters and grids necessary for the high tube voltage radiography and high-speed screen systems should be adopted and used as soon as possible in order to reduce exposure dose to the patients.

Dosimetric comparison of intensity-modulated radiotherapy (IMRT) and volumetric modulated arc therapy (VMAT) in total scalp irradiation: a single institutional experience

  • Purpose: Total scalp irradiation (TSI) is a rare but challenging indication. We previously reported that non-coplanar intensity-modulated radiotherapy (IMRT) was superior to coplanar IMRT in organ-at-risk (OAR) protection and target dose distribution. This consecutive treatment planning study compared IMRT with volumetric-modulated arc therapy (VMAT). Materials and Methods: A retrospective treatment plan databank search was performed and 5 patient cases were randomly selected. Cranial imaging was restored from the initial planning computed tomography (CT) and target volumes and OAR were redelineated. For each patients, three treatment plans were calculated (coplanar/non-coplanar IMRT, VMAT; prescribed dose 50 Gy, single dose 2 Gy). Conformity, homogeneity and dose volume histograms were used for plan. Results: VMAT featured the lowest monitor units and the sharpest dose gradient (1.6 Gy/mm). Planning target volume (PTV) coverage and homogeneity was better in VMAT (coverage, 0.95; homogeneity index [HI], 0.118) compared to IMRT (coverage, 0.94; HI, 0.119) but coplanar IMRT produced the most conformal plans (conformity index [CI], 0.43). Minimum PTV dose range was 66.8%-88.4% in coplanar, 77.5%-88.2% in non-coplanar IMRT and 82.8%-90.3% in VMAT. Mean dose to the brain, brain stem, optic system (maximum dose) and lenses were 18.6, 13.2, 9.1, and 5.2 Gy for VMAT, 21.9, 13.4, 14.5, and 6.3 Gy for non-coplanar and 22.8, 16.5, 11.5, and 5.9 Gy for coplanar IMRT. Maximum optic chiasm dose was 7.7, 8.4, and 11.1 Gy (non-coplanar IMRT, VMAT, and coplanar IMRT). Conclusion: Target coverage, homogeneity and OAR protection, was slightly superior in VMAT plans which also produced the sharpest dose gradient towards healthy tissue.

Benefit of volumetric-modulated arc therapy over three-dimensional conformal radiotherapy for stage I-II extranodal marginal zone B-cell lymphoma of mucosa-associated lymphoid tissue in the stomach: a dosimetric comparison

  • Purpose: To retrospectively analyze dosimetric parameters of volumetric-modulated arc therapy (VMAT) and three-dimensional conformal radiotherapy (3D-CRT) delivered to extranodal marginal zone B-cell lymphomas of mucosa-associated lymphoid tissue in the stomach (gastric MALT lymphoma) to find out advantages of VMAT and conditions for definite benefits of VMAT. Materials and Methods: Fifty patients with stage I-II gastric MALT lymphoma received VMAT (n = 14) or 3D-CRT (n = 36) between December 2005 and April 2018. Twenty-seven patients were categorized according to whether the planning target volume (PTV) overlaps kidney(s). Dosimetric parameters were analyzed by dose-volume histogram. Results: Radiation dose to the liver was definitely lower with VMAT in terms of mean dose (p = 0.026) and V15 (p = 0.008). The V15 of the left kidney was lower with VMAT (p = 0.065). For those with PTV overlapping kidney(s), the left kidney V15 was significantly lower with VMAT. Furthermore, the closer the distance between the PTV and kidneys, the less the left kidney V15 with VMAT (p = 0.037). Delineation of kidney(s) by integrating all respiratory phases had no additional benefit. Conclusions: VMAT significantly increased monitor units, reduced treatment time and radiation dose to the liver and kidneys. The benefit of VMAT was definite in reducing the left kidney V15, especially in geometrically challenging conditions of overlap or close separation between PTV and kidney(s).

방사선 분포측정용 CsI(Tl) 검출기 설계 및 신호처리에 관한 연구 (The Study of CsI(Tl) Scintillation Detector Design and Signal Processing for the Measurement of the Radiation Distribution)

  • 본 논문에서는 공간상에 분포된 방사선원에 대해 화소단위로 방사선 신호를 탐지하기 위한 섬광 검출기를 설계하고, 탐지신호를 취득하기 위한 모듈을 설계하여 방사선 영상화를 위한 연구를 수행하였다. 방사선 분포측정을 위해 적용한 검출기는 CsI(Tl) 섬광체와 결합효율이 높은 수광센서로 구성하였으며, 수광센서를 통해 획득된 신호를 처리하기 위해 펄스 신호를 증폭, 성형 한 후 임계치를 기준으로 펄스를 계수하기 위한 장치를 구현하였다. 제작된 검출기의 성능을 검증하기 위해 감마선 교정시설을 이용하여 검출기에 대한 성능 시험을 수행하고 그 결과를 분석하였다. 본 논문의 결과는 제작된 검출기에 방향성을 부여하기 위한 차폐체 및 조사부를 적용한 후 방향을 이동시켜 개별적인 영역의 값을 2차원 적으로 나타냄으로써 선원의 분포측정을 위한 기초자료로 활용될 것이다.

이동용 치과 X선 발생장치의 누설 및 산란 선량에 관한 연구 (Leakage and scattered radiation from hand-held dental x-ray unit)

  • Purpose: To compare the leakage and scattered radiation from hand-held dental X-ray unit with radiation from fixed dental X-ray unit. Materials and Methods: For evaluation we used one hand-held dental X-ray unit and Oramatic 558 (Trophy Radiologie, France), a fixed dental X-ray unit. Doses were measured with Unfors Multi-O-Meter 512L at the right and left hand levels of X-ray tube head part for the scattered and leakage radiation when human skull DXTTR III was exposed to both dental X-ray units. And for the leakage radiation only, doses were measured at the immediately right, left, superior and posterior side of the tube head part when air was exposed. Exposure parameters of handheld dental X-ray unit were 70 kVp, 3 mA, 0.1 second, and of fixed X-ray unit 70 kVp, 8 mA, 0.45 second. Results: The mean dose at the hand level when human skull DXTTR III was exposed with portable X-ray unit $6.39{\mu}Gy$, and the mean dose with fixed X-ray unit $3.03{\mu}Gy$ (p<0.001). The mean dose at the immediate side of the tube head part when air was exposed with portable X-ray unit was $2.97{\mu}Gy$ and with fixed X-ray unit the mean dose was $0.68{\mu}Gy$ (p<0.01). Conclusions: The leakage and scattered radiation from hand-held dental radiography was greater than from fixed dental radiography.

동물병원 영상의학적검사 실태조사 (Survey for Diagnostic Radiography Examination in Veterinary Hospital)

  • 동물병원에서 사용되는 전리방사선뿐만 아니라 초음파 등의 비전리방사선을 포함한 모든 영상의학적인 검사에 대한 실태조사를 실시하였다. 2021년 6월 1일부터 6월 20일 사이에 00광역시의 동물병원에 설치 사용 중인 영상의학적인 장치 및 검사 등에 대해 설문조사를 실시하였다. 설문항목에는 동물병원 위치와 주로 내원하는 동물 순위, 영상의학장치가 설치 여부, 구매 계획, 현재 설치 사용하는 장치 등이 포함되었다. SPSS ver. 26.0을 사용하여 명목변수에 대해서는 백분율(%)로 나타내었고, 연속변수는 평균과 표준편차로 나타내었다. 동물병원 내원동물 1순위로 개 61.3%로 가장 높았으며 고양이는 12.9%로 나타났다. 87.1%가 영상의학장치가 설치 사용되고 있으며 4개 병원에는 영상의학장치가 없었다. 영상의학장치가 설치된 27개 병원 중에서 48.1%가 검사실과 조정실을 분리 사용하고 있었고, 내원동물의 19.8%가 영상의학적검사를 받는 것으로 나타났다. 방어복 중 앞치마는 27개 모든 병원에서 구비하고 있었고, 55.6% 동물병원이 피폭 선량 측정 및 관리를 하고 있었다. 전체 27개 병원 중에서 92.6%가 검사교육이 필요하다고 응답했으며, 검사교육을 무료로 실시한다면 85.2%가 참여 한다고 하였다. 이상의 결과는 국내 동물병원 영상의학장치 사용에 대한 규정을 개정하는데 도움이 될 수 있을 것으로 사료된다.

통신해양기상위성 통신 탑재체의 우주 방사선 환경 모사 및 영향 추정 (Environment Simulation and Effect Estimation of Space Radiation for COMS Communication Payload)

  • 통신해양기상위성의 통신 탑재체가 겪게 될 우주 방사선 환경을 포획된 입자, 태양 양성자 그리고 우주선으로 구분하여 각각 NASA AP8/AE8 모델, JPL91 모델, 그리고 NRL CREME 모델을 사용하여 전산 모사 하였다. 이러한 우주 방사선 환경이 위성 내 통신 탑재체에 미치는 영향을 추정하기 위해 총 이온화 방사선 효과의 분석에 필요한 Dose-Depth 곡선 및 단일사건효과 발생률 계산에 필요한 LET 스펙트럼 구하였다. 통신 탑재체 내의 각 장치별 차폐 효과 차이를 고려한 총 이온화 방사선 효과의 예측을 위해서 기계 구조 모델을 만든 후 구형 분할 방법을 적용하였다. 이를 통해 통신 탑재체 내 각 장치의 위치별 총 이온화 방사선 효과를 예상하였으며 동일한 외부 방사선 환경에 대해서 차폐 효과에 따라 최고 8배까지 방사선 효과가 다르게 나타났다.
