In Sasang constitutional medicine, doctors diagnose and treat patients according to their Sasang consitutition (SC) type. In this work, by a clinical test with a radial pulse tonometer, we investigated radial pulse properties which were significant in distinguishing unhealthy subjects from healthy subjects for each SC type. We measured radial pulse properties on left and right Gwan locations with a pulse tonometer for 299 elderly female subjects of age of 50 years old or above. We used a newly developed SCAT system to determine subjects' SC types. Subjects' health levels of either healthy or unhealthy were determined independently by two Korean medical doctors. To investigate the statistical differences, we used either of Student's t-test or Mann-Whitney U test depending on the normality of distribution of test statistic. For TE type, unhealthy subjects were characterized with significant increases in heart rate, systolic to diastolic period, pulse depth, while they showed significant decreases in width of pulse in normal direction, pulse area in diastolic period, and high harmonic components (6th, 7th) of power spectral density. For SE type, unhealthy subjects were characterized with significant increases in pulse pressure and pulse depth, while no significant differences were found for SY type. We investigated the characteristic differences in radial pulse properties due to change in health levels on each SC type for elderly females. Pulse variables which were significantly different between healthy subjects and unhealthy subjects were found to vary between SC types. For TE type subjects, many variables were found significant at the left Gwan location, which is in support of the theory of hyperactive liver functioning for TE type. Irrespective of the constitution, the related changes in the pulse properties due to worsened health level were in support of elevated blood flow amount in compensation with weakened blood circulatory function.
We measured radial arterial pulse signals using a prototype of a clip-type pulsimeter equipped with a permanent magnet and a Hall device, which produced signals through a voltage-detecting circuit. The systolic peak time and the reflective peak time for a temporally pulsed signal were analyzed for an arbitrary pulse wave at one position of a small permanent magnet. The measured value of the peripheral pulse wave velocity was about 1.25-1.52 m/s, demonstrating the accuracy of this new method. To measure the peripheral blood flow velocity, we simultaneously connected the radial artery pulsimeter to a photoplethysmography meter. The average value of the peripheral blood flow velocity was about 0.27-0.50 m/s.
In this paper, we developed a feature point detection algorithm that detects upstroke point(S), peak point(P), incisura(C) point from radial pulse waveform which obtained by using the developed radial pulse transducer. As the results of experiment the three kinds of parameters can extracted with effectively from normal radial pulse waveform.
Objectives: This study was aimed to evaluate the hemodynamic feasibility using pulse parameters as a way to establish safe dose guidelines for herbal prescription containing Ephedra herb (Ephedra intermedia Schrenk & C.A.Mey) on weight loss, and to provide a foundation for developing evidence-based guidelines for clinical use. Methods: Forty-two volunteers were recruited to participate in a study examining the changes in pulse wave characteristics following the ingestion of Gambi-hwan, a herbal prescription containing ephedra, over a period of 4 weeks, and pulse wave measurements were taken before and after the administration. Pulse wave parameters were measured in this study using a 3-dimensional radial pulse tonometry device (DMP-Lifeplus). In addition, questionnaire, blood pressure, temperature, and body composition were also measured as secondary measures. Results: Fifteen minutes after administration of Gambi-hwan, the non-adverse event group (non-AE) exhibited a statistically significant increase in several power and pressure-related parameters, including h1, h5, systolic area, pulse area, and pulse width, while the AE group showed a trend of decreasing stroke volume and increasing Radial Augmentation Index (RAI), w, and w/t. After 4 weeks of administration, both groups exhibited significant changes in pulse rate, w/t, RAI, t3/t, stroke volume (SV), and stroke volume Index (SVI). Notably, there are significant differences between AE group and non-AE group in w/t, SV, and SVI. Conclusions: These findings suggest that pulse parameters may be a useful way to establish safe dosing guidelines for weight loss herbal prescription containing ephedra. Further research is needed to confirm these results and to develop evidence-based guidelines for clinical use.
본 연구에서는 집게형 맥진기와 심전도계를 이용하여 전기적인 심전도 파형과 요골동맥파형을 동시 측정하여 얻은 맥파전달속도를 분석하였다. 두 신호의 동시 측정에 의한 맥파전달속도는 평균 6 m/s의 값을 보여 주었으며, ECG 파형과 홀소자 맥진파형으로 동시 측정한 방법은 기존 방법들 보다 정확하게 맥파전달속도를 측정할 수 있는 새로운 방법임을 입증하였다. 따라서 이결과는 한방의료기기인 맥진기에서 측정된 데이터를 임상적으로 활용할 수 있음을 보인다.
Objectives : The purpose of this study is to find parameters to compare the effects of Sa-Am acupuncture with stomach-tonification (ST) and Sa-Am acupuncture with spleen-tonification (SP) on radial pulse. Methods : Forty healthy subjects participated in this study. Radial pulse was measured using 3 dimensional pulse imaging system (DMP-3000) before, right after, 30 minutes after, and 60 minutes after acupuncture. Results : 1. The parameters analysed in this study were changed in the same direction with different degrees by ST and SP. 2. In males, SP manipulation showed significantly increased pulse period and (T-T4)/T, and significantly decreased T1/T, T4/T, T4/(T-T4), and F1~F3 frequencies compared to ST. 3. In females, ST manipulation showed significantly increased (T-T4)/T, and significantly decreased F1~F3 frequencies compared to SP. Conclusions : The parameters analysed in this study may be used to differentiate the effects between ST and SP on radial pulse. These parameters are more strongly affected by SP than ST in males whereas more strongly affected by ST than SP in females.
Objectives : The purpose of this study is to find parameters to compare the effects of Sa-Am acupuncture with gall bladder jeonggyeok and seunggyeok on radial pulse in healthy subjects. Methods : 60 healthy subjects participated in this study and divided into the gall bladder jeonggyeok group (GB+ group), the gall bladder seunggyeok group (GB- group) and the control group. Radial pulse was measured by a 3 dimensional pulse imaging system (DMP-3000) before, immediately after, 30 minutes after and 60 minutes after acupuncture on the Cun, Guan and Chi in the GB+, GB- group and at the same time for the control group. Results : 1. The GB+ and GB- group exhibited significantly increased T, M4/M1 and decreased T1/T, T2/T, T5/T, M1, M2 compared to the control group. 2. The GB+ and GB- group exhibited significantly increased H4, Ad/Ap, RAI/HR and decreased As/Ap, Aw/Ap in the right Guan and decreased pulse power volume/min in the left Guan compared to the control group. 3. The GB- group exhibited significantly decreased M3, M3/M1 and increased M4/M1, M5, M5/M1 compared to the GB+ group. The GB+ group exhibited significantly decreased T4/(T-T4) compared to the GB- group. 4. The GB- group exhibited significantly decreased H1 in the right Guan, Chi, H4, pulse area in the right Chi, H5, pulse power volume/min in the left Chi, right Chi, and increased main peak angle in the right Guan, Chi compared to the GB+ group. Conclusion : The effect of gall bladder jeonggyeok and seunggyeok in healthy humans may be observed on various parameters. The parameters analyzed in this study may be used to differentiate effects between gall bladder jeonggyeok and seunggyeok on radial pulse. Further studies on the effects of Sa-Am acupuncture using radial pulse are needed.
Arrhythmia is basically diagnosed with the electrocardiogram (ECG) signal, however, ECG is difficult to measure and it requires expert help in analyzing the signal. On the other hand, the radial pulse can be measured with easy and uncomplicated way in daily life, and could be suitable bio-signal for the recent untact paradigm and extensible signal for diagnosis of Korean medicine based on pulse pattern. In this study, we developed an irregular radial pulse detection algorithm based on a learning model and considered its applicability as arrhythmia screening. A total of 1432 pulse waves including irregular pulse data were used in the experiment. Three data sets were prepared with minimal preprocessing to avoid the heuristic feature extraction. As classification algorithms, elastic net logistic regression, random forest, and extreme gradient boosting were applied to each data set and the irregular pulse detection performances were estimated using area under the receiver operating characteristic curve based on a 10-fold cross-validation. The extreme gradient boosting method showed the superior performance than others and found that the classification accuracy reached 99.7%. The results confirmed that the proposed algorithm could be used for arrhythmia screening. To make a fusion technology integrating western and Korean medicine, arrhythmia subtype classification from the perspective of Korean medicine will be needed for future research.
This study was conducted to find objective diagnostic indicators for the Eight Principle Pattern Identification using a pulse wave analyzer. Typology Complexion Pulse and Symptom data from 500 women over the age of 18 were used. Five experts made a diagnosis of Eight Principle Pattern Identification and divided the subjects into 'Yin-Yang', 'Exterior-Interior', 'Cold-Heat', 'Deficiency-Excess' groups. Their pulse waves were measured in the left and right radial arteries, and it was investigated whether there was a significant difference between groups in the pulse wave parameter values. 'Yin' group showed a significantly lower value in the left radial artery for Ener, Emin, EIX, T4T, T4TT parameters and in the Right for T2, T2T, T5. The Vmag, As and Ad parameters were significantly different between the 'Exterior' and 'Interior' groups. 'Heat' group showed a significantly higher value in the right radial artery for RAI/t parameter. 'Deficiency' group showed a significantly higher value in the right radial artery for W, Angl parameters. Through this study, significant pulse wave parameters were found, and they can be used as objective diagnostic indicators for Eight Principle Pattern Identification.
