Immediate effects of neuromuscular control exercise on neck pain, range of motion, and proprioception in persons with neck pain

  • Objective: This study aimed to investigate the immediate effects of intervention using neuromuscular control, self-stretching (SS), and neck stabilization exercises (NSEs) on neck pain, range of motion (ROM), and proprioception of position sense in adults with neck discomfort. Design: Three-group pretest-posttest design. Methods: Forty-four adults who complained of neck pain participated in the experiment. They were randomly assigned to the following groups: neuromuscular control exercise (NMCE) group (n=15), SS group (n=14) and NSE group (n=15). The NMCE group did rolling with only upper limb pattern on both sides. The SS group performed neck stretching on each side, 3 sets of 30 seconds for each muscle. The NSE group had the pressure biofeedback applied with increases in pressure by 2 mmHg at a time from 20-30 mmHg while in the hook-lying position. All groups performed exercises for 10 minutes. Neck pain, ROM, and proprioception were measured to determine differences between the intervention methods. Results: Intra-group comparisons showed significant improvement after exercise in pain, ROM, and proprioception in the NMC group (p<0.05). In the comparison between groups, the NMC group had a significant decrease in pain compared to the other two groups (p<0.05). There was no difference in ROM between the groups but the NMC group showed significant improvement in left rotation compared to the stabilization exercise group (p<0.05). For proprioception, the NMC group had significantly lower error than the other two groups (p<0.05). Conclusions: NMCEs through upper extremity pattern rolling exercise is effective in improving neck pain, ROM, and proprioception.

  • Objectives : The purpose of this report is to prove the effect of The Daoyin exercise program combined with Korean medicine treatment on supraspinatus tendon tear. Methods : Two patients, diagnosed with Supraspinatus tendon tear by shoulder MRI, were treated with The Daoyin exercise program and Korean medicine treatment. VAS(Visual analog scale), ROM(Range of motion) and Physical examination were used to evaluate patients. Results of application : The Daoyin exercise program combined with Korean medicine treatment reduced shoulder pain and improved shoulder ROM of the two patients significantly. Conclusions : The Daoyin exercise program combined with Korean medicine treatment is proved to be effective treatment for reducing pain and improving shoulder ROM of supraspinatus tendon tear patients.

  • Purpose: The study aims to examine the effects of the proprioceptive neuromuscular facilitation pattern exercise and scrambler therapy on pain, range of motion (ROM), and shoulder dysfunction in breast cancer patients. Methods: In total, 30 breast cancer patients were recruited and randomized to group I (n = 10), group II (n = 10), and group III (n = 10). Pain was measured using a visual analogue scale, ROM was measured using a goniometer, and shoulder dysfunction was measured using a shoulder pain and disability index. Group I practiced the proprioceptive neuromuscular facilitation pattern exercise and underwent scrambler therapy, group II underwent scrambler therapy only, and group III practiced the proprioceptive neuromuscular facilitation pattern exercise only. Results: A within-groups comparison showed that all groups demonstrated significant differences in pain, ROM, and shoulder dysfunction after the experiment (p<0.05). Further, according to a comparison of the three groups, group I showed a more significant difference in effectiveness than groups II and III in terms of pain, ROM, and shoulder dysfunction before and after the experiment (p<0.05). Conclusion: This study showed that the proprioceptive neuromuscular facilitation pattern exercise and scrambler therapy are effective in treating pain, ROM issues, and shoulder dysfunction in breast cancer patients.

Investigation of the range of motion of the shoulder joint in subjects with rotator cuff arthropathy while performing daily activities

  • Background: Patients who have rotator cuff arthropathy experience a limited range of motion (ROM) of the shoulder joint and experience problems in performing their daily activities; however, no evidence is available to suggest the exact ROM of the shoulder joint in this population. Therefore, this study sought to determine the degree of motion of the shoulder joint in three planes during different activities. Methods: Five subjects with rotator cuff injuries participated in this study. The motion of the shoulder joints on both the involved and normal sides was assessed by a motion analysis system while performing forward abduction (task 1), flexion (task 2), and forward flexion (task 3). The OpenSIM software program was used to determine the ROM of the shoulder joints on both sides. The difference between the ranges of motion was determined using a two-sample t-test. Results: The ROMs of the shoulder joint in task 1 were 93.5°±16.5°, 72.1°±2.6°, and 103.9°±25.7° for flexion, abduction, and rotation, respectively, on the normal side and 28°±19.8°, 31°±31.56°, and 48°±33.5° on the involved side (p<0.05). There was no significant difference between the flexion/extension and rotation movements of the shoulder joint when performing task 1. However, the difference between flexion and rotation movements of the shoulder joints for the second task was significant (p>0.05). Conclusions: Those with rotator cuff arthropathy have functional limitations due to muscle weakness and paralysis, especially during the vertical reaching task. However, although these individuals have decreased ROM for transverse reaching tasks, the reduction was not significant.

The Relationship Between the Range of Hip Rotation and the Quadriceps Angle in Subjects With and Without Flat Foot

  • Background: Alignment of the lower limb is an important factor, influencing balance and gait in kinematics and kinetics, in patients with and without a flat arched foot. Flat arched foot are associated with the range of motion (ROM) of the hip and alignments of the knee joints, is strongly influenced. Objects: The purpose of this research was to investigate the relationship between hip joint ROM and quadriceps angle (Q-angle), by dividing them into two groups according to the presence or absence of flat feet, using a navicular drop test (NDT) and resting calcaneal stance position (RCSP). Methods: Forty elderly patients were allocated to the experimental group (flat foot group, n1=20) or the control group (non-flat group, n2=20). Universal and digital goniometer, tractograph and tape measure were used to determine the related changes in the hip ROM, Q-angle, NDT and RCSP. Results: Data were analyzed using the Pearson correlation coefficients. Active internal ROM of the hip joint (right, r=.803, p<.001), (left, r=.951, p<.001) were highly correlated with NDT, and also, was moderately correlated with Q-angle (right, r=.562, p=.019), (left, r=.757, p<.001). Passive internal ROM of the hip joint (right, r=.742, p=.001), (left, r=.922, p<.001) were highly correlated with NDT, and also, was moderately correlated with RCSP (right, r=-.530, p=.029) and with Q-angle (right, r=.710, p=.001), (left, r=.698, p=.002) in the flat foot group. However, no strong correlation among the hip ROM, NDT, RCSP and Q-angle were found in the non-flat foot group. Conclusion: This research may provide evidence of the correlations between hip internal ROM and flat foot.

재활환자를 위한 IoT 기반의 실시간 모니터링이 가능한 ROM 측정 장치 개발 (Development of ROM Measurement Device capable of IoT-based Real-time Monitoring for Rehabilitation Patients)

  • 본 논문에서는 관절 각도계를 사용하여 관절가동범위의 측정하던 것을 각도계를 사용하지 않고 플렉서블 센서를 이용하여 측정하고 측정된 값을 스마트 디바이스와 연동하여 실시간 모니터링이 가능하게 하였다. 현재의 관절가동 범위 측정은 관절 각도기를 활용하여 사람이 직접 측정하고 있다. 이러한 방법은 측정하는 사람의 측정 방법과 위치에 따른 오류로 일관성 있는 측정이 어려워 오차가 발생할 수 있다. 그래서 측정해야 할 관절에 센서를 부착하여 관절의 운동 범위를 측정하였다. 측정을 위한 센서는 플랙서블 형태로 관절의 움직임에 따라 변화되는 저항 값을 측정하는 센서로서 센싱 된 값을 ROM 센서노드를 통하여 무선으로 스마트 디바이스로 전송될 수 있도록 하였다. 개발 된 ROM 측정 장치는 일반적인 관절 각도기를 활용한 측정보다 일관성 있는 측정이 이루어 질 수 있고 스마트 디바이스와 연동하여 실시간 모니터링 함으로써 관절의 움직임에 따른 신속한 진단으로 환자의 빠른 치료와 재활의료 발전에 도움이 될 것이다.

  • Purpose: This study investigated the effects of proprioceptive neuromuscular facilitation (PNF) exercise on the range of motion (ROM), pain, and functional activity of patients who received total knee arthroplasty (TKA). The purpose of this study was to provide fundamental data regarding the use of PNF exercise among patients with musculoskeletal disease. Methods: Fourteen patients who received TKA were randomly divided into an experimental group (n=7) that took part in PNF exercise and a control group (n=7) that performed general rehabilitation exercise. Both groups performed the respective exercises for 30 minutes, five times a week for 2 weeks. For the measurement of ROM, the range of knee flexion was measured using a clinometer smartphone application. A visual analogue scale (VAS) was used for the measurement of the level of pain. The timed up and go test (TUG) was conducted to measure functional activity. A paired t-test was performed to compare within-group changes before and after the PNF exercise. Differences between the experimental group and control group were analyzed by an independent t-test. For all tests, the level of statistical significance was set at ${\alpha}=0.05$. Results: After the exercise, there was a significant within-group change in VAS and TUG scores in the experimental group and control group (p<0.01). There was also a significant between-group difference in VAS and TUG scores after the intervention (p<0.05). Conclusion: General rehabilitation exercise is commonly applied as a treatment for TKA patients and is relatively effective. The application of PNF exercise may be useful in such patients, considering its effects on ROM improvement, pain reduction, and functional enhancement.

트레드밀보행 시 경사도에 따른 족저압과 발목관절의 관절가동범위의 변화 (The change of ankle of plantar pressure and range of motion joint according to treadmill gradients)

  • Purpose : The purpose of this study was to investigate the change of the peak plantar pressure distribution under the foot areas and the range of motion (ROM) of ankle joint according to gradients in treadmill gait. Method : Thirty normal subjects (15 male and 15 female) walked on treadmill at three gradient conditions ($0^{\circ}$, $10^{\circ}$, and $15^{\circ}$) in normal speed. The ankle ROM was measured using the CMS70P that is three dimensional analyzer for excursion of ankle ROM, plantar flexion, and dorsi flexion. The peak plantar pressure distribution under the hallux, 1st metatarsal head (MTH) and heel was measured using the F -Scan system with an in-shoe sensor. Data was collected from 9 steps of left sife foot in at each gradient condition while all subjects walked. Result : As the treadmill gradient increased, the excursion of ankle joint was significantly increased (p<.05). Also, plantar flexion and dorsi flexion was significantly increased according to treadmill gradients (p<.05). The peak plantar pressure under the 1st MTH was significantly increased (p<.05) and the peak plantar pressure under the heel was significantly decreased (p<.05) as the treadmill gradient increased. No significant different in the peak plantar pressure under the hallux was observed. Conclusion : This study suggests that physical therapy for patients who have limited ankle ROM should be considered sufficient range of motion for functional ambulation. And individuals that have painful forefoot syndromes, including metatarsalgia, hallux valgus, and plantar ulceration should be careful in walking to uphill, as there is high plantar pressure under the forefoot.

Comparison of Knee Muscle Strength and Ankle Dorsiflexion Range of Motion Between Standing Workers With and Without Patellofemoral Pain Syndrome

  • Background: Prolonged standing during work causes a lower extremity pain and disorders. Patellofemoral pain syndrome (PFPS) is one of the common diagnoses of the knee pain. Although the etiology of PFPS is not completely understood, it is considered to be multifactorial. Objects: The purpose of this study was to investigate difference in strength of knee muscles, quadriceps:hamstring muscles strength ratio (Q:H ratio), asymmetry ratio of knee muscles strength and dorsiflexion range of motion (ROM) between standing workers with and without PFPS. Methods: Twenty-eight standing workers with PFPS and 26 age-, height-, and weight-matched standing workers without PFPS participated in this study. A tension sensor measured knee muscle strength, and motion sensor measured dorsiflexion ROM. The asymmetry ratio of knee muscles was calculated by a specific formula using the knee muscles strength of the dominant side and the sound side. An independent t-test was used to identify significant differences in the strength, ROM, Q:H ratio, and asymmetry ratio between the PFPS and normal groups. Results: The standing worker with PFPS have significantly lower dorsiflexion ROM (p < 0.000) and higher asymmetry ratio of the hamstring muscles strength (p < 0.000) compare to the standing worker without PFPS. No significant differences were seen in the strength of quadriceps muscle and hamstring muscles, Q:H ratio, and asymmetry ratio of quadriceps muscle strength. Conclusion: There was a significant difference in the asymmetry ratio of the isometric hamstring muscle strength. This finding suggests that the asymmetry ratio of isometric hamstring muscle strength may be more important than measuring only the hamstring muscle strength of the PFPS side. Furthermore, the results of this study showed a significant difference in dorsiflexion ROM between the standing industrial workers with and without PFPS. Dorsiflexion ROM and isometric hamstring muscle strength should be considered when evaluating the subjects with PFPS.

수축-이완 기법 방법에 다른 최대 힘과 무릎관절 가동 범위에 미치는 즉각적인 효과 비교 -예비연구- (Immediate Effects of Various Contract-Relax Techniques on the Peck Force and Range of Motion of Knee Extension -A Pilot Study-)

  • Purpose: This study aimed to compare the immediate effects of various contract-relax (CR) techniques on the peck force and passive range of motion (ROM) of knee extension. Methods: This study recruited 17 adult males and females who obtained below $90^{\circ}$ in straight leg raising. The subjects randomly performed one of three different CR techniques (direct CR, indirect CR, and modified CR) on the right knee extension four times per day for three days. The subjects' peck force during each CR and their passive knee extension on the hip at a $90^{\circ}$ flexed position before and after each CR were measured. Results: The peck force was significantly higher in the direct and modified CR than in the indirect CR. The ROM in the passive knee extension significantly increased after direct and indirect CR. Comparing between each CR, the passive knee extension range was significantly more improved in the direct and indirect CR than in the modified CR. Conclusion: Direct and indirect CR are more effective than modified CR in the ROM of passive knee extension. The study results suggest that the modified CR did not have the same effect on the ROM of the knee extension as the direct and indirect CR and that further research is needed to recruit more subjects and to determine the effects on different muscles after modified CR.
