Respiratory Review of 2014: Tuberculosis and Nontuberculous Mycobacterial Pulmonary Disease

  • Since tuberculosis (TB) remains a major global health concern and the incidence of multi-drug resistant (MDR)-TB is increasing globally, new modalities for the detection of TB and drug resistant TB are needed to improve TB control. The Xpert MTB/RIF test can be a valuable new tool for early detection of TB and rifampicin resistance, with a high sensitivity and specificity. Late-generation fluoroquinolones, levofloxacin, and moxifloxacin, which are the principal drugs for the treatment of MDR-TB, show equally high efficacy and safety. Systemic steroids may reduce the overall TB mortality attributable to all forms of TB across all organ systems, although inhaled corticosteroids can increase the risk of TB development. Although fixed dose combinations were expected to reduce the risk of drug resistance and increase drug compliance, a recent meta-analysis found that they might actually increase the risk of relapse and treatment failure. Regarding treatment duration, patients with cavitation and culture positivity at 2 months of TB treatment may require more than 6 months of standard treatment. New anti-TB drugs, such as linezolid, bedaquiline, and delamanid, could improve the outcomes in drug-resistant TB. Nontuberculous mycobacterial lung disease has typical clinical and immunological phenotypes. Mycobacterial genotyping may predict disease progression, and whole genome sequencing may reveal the transmission of Mycobacterium abscessus. In refractory Mycobacterium avium complex lung disease, a moxifloxacin-containing regimen was expected to improve the treatment outcome.

DNA손상 및 돌연변이에 대한 명지버섯의 방어효능 (The Protective Effects of Ganoderma lucidum on the DNA Damage and Mutagenesis)

  • Ganoderma lucidum is commonly known as medically potent mushroom, which has been widely used in China and other oriental countries for the treatment of various diseases, including cancer. In this report, we investigated the anti-oxidant and protective effect of Ganodema lucidum extract (GLE) against the DNA damage induced by free radical and U.V. In the assay of cell growth inhibition, the inhibitory cell growth rate induced by hydroxyl radical was dose-dependently decreased by GLE. This results support that GLE has a detoxifying activity against cytotoxicity of hydroxyl radical in E. coli cell. GLE also protected ColE1 plasmid DNA damage in the concentration of 200$\mu\textrm{g}$ per reaction on the DNA fragmentation assay. The nuclear tailing by hydrogen peroxide in single cell gel electrophoresis(SCGE) was decreased by GLE in the concentration of 50$\mu\textrm{g}$/ml. These data indicate that Ganoderma lucidum has an anti-oxidative activity to hydrogen peroxide. The mutation rate after irradiation of U.V. was reduced by 50$\mu\textrm{g}$/ml GLE and total number of Rif (Rifampicin) resistant mutants was decreased in a concentration dependent manner when added the GLE exogenously in a culture media. According to the results, it is likely that GLE has not only an anti-oxidative activity to hydroxyl radical but also an anti-mutagenic activity to U.V. mutagenesis.

봉인볼트용 UHF RFID Tag Antenna를 이용한 차량인식에 관한 연구 (A Study on Detecting System of Illegal Automobile Using a Seal-Bolt UHF RFID Tag Antenna)

  • 본 논문은 UHF 대역의 소형 RFID 태그 Antenna를 설계하여 자동차 후방 번호판 봉인볼트나 고정용 볼트에 적용하여 자동차를 인식 하여서 대포차량을 포함한 불법 차량을 속출 할 수 있는 시스템을 구현 하였다. 해당 태그의 크기는 100원짜리 동전정도의 지름이 24mm 정도의 크기이며 태그의 반사계수와 인식거리를 측정하였다. 기존의 볼트형 태그보다 인식거리를 대폭 개선하였다. 이 볼트형 태그를 번호판에 부착하면 경찰은 차량의 정보를 쉽게 알아낼 수 있으며 더 나아가 차량관리 시스템을 통한 손쉬운 범죄차량 추적 및 검거가 용이하다.

CMT 양성 유즙에서 유방염 원인균 분리 및 분리균의 항균제 감수성 (Isolation of causative agents from CMT-positive mastitic milk and antimicrobial susceptibility of isolates)

  • This study was carried out to isolate of causative agents from CMT-positive and mean somatic cell count(SCC) $\geq$500,000 cells/ml mastitic milk, and evaluate to antimicrobial susceptibility of isolates in Iksan branch area from January to November, 1996. 1. The CMT-positivity(SCC 500,000 cells/ml) of 610 heads was 36.2% (221), and of 2,373 quarter milks was 16.1% (383). 2. The Gram-positive isolates were 153 strains which was Staphylococcus sp (115), Micrococcus sp (18), Streptococcus sp (10), Listeria monocytogenes (5) and Enterococcus faecalis(5). 3. The Gram-negative isolates were 66 strains including E coli(14), Yersinia sp (13), Shigella sp(8), Enterobacillus sp(8), Cedecea sp(5), Pseudomonas aeruginosa(5), Proteus sp(5), Klebsiella sp(4), Salmonella sp(2), kluyvera ascorbate(1) and Tatumella ptyseos (1). 4. The Gram positive strains of isolates were moderately susceptible to T/s, Cp, Fd, Imp, Aug, Rif, Cft and Va. And the Gram negative strains of Isolates were moderately susceptible to T/s, Cp, Imp, Pi and Ti, In order. 5. Multiple antimicrobial resistant patterns were encountered 62 and 36 from Gram positive and negative isolates, respectively.

LPG 충전시설에 대한 신뢰도 분석과 정량적 위험성 분석에 관한 연구 (A Study on Reliability Analysis and Quantitative Risk Analysis for Liquefied Petroleum Gas Station)

  • 본 연구에서는 LPG 충전소에 대하여 Fussel-Vesely 중요도와 RDF 및 RIF 중요도를 수행하여 위험성 등급(Risk Rank)을 설정하였다 설정된 위험성 등급에서 위험성이 가장 큰 장치 및 설비에 대하여 정량적 위험성 분석을 수행하였다. LPG 충전소에 대한 중요도 분석결과 외부사고가 가장 위험하다고 확인되었으며, ekda 으로 구조물 결함과 파이프 배관의 부식의 위험등급이 높았다. 정량적 위험성 분석 결과로는 LPG 충전소 저장탱크의 완전히 파열하였을 경우에 대한 BLEVE의 경가 발생하였을 경우에 복사열에 의해 공정 설비에 손해를 입히기에 충분한 거리는 46.3m로 분석되었다.

상하악 동시 악교정술시 안정성에 관한 연구;[Ⅰ] 강선 고정에 의한 방법 (STABILITY OF SIMULTANEOUS MAXILLARY AND MANDIBULAR SURGERY;[Ⅰ]Wire osteosynthesis)

  • A series of 19 cases with maxillary hyperplasia and mandibular retrognathia were operated on by simultaneous superior repositioning of the maxilla after Le Fort I osteotomy and anterior repositioning of the mandible after bilateral sagittal split ramus osteotomies with or without osteotomy of the inferior border of the mandible. These were evaluated by retrospective cephalometric and computer analysis for the longitudinal skeletal and dental changes for an average of 17.1 months after surgery. For stabilization of the osteotomized segments, the authors used wire osteosynthesis by means of bilateral infraorbital and zygomatic buttress suspension wire at the maxilla, and direct interosseous wire at the split segments of the mandibular rami. Results show generally good stability after simultaneous maxillary and mandibular surgery with wire osteosynthesis, and a minimal to moderate tendency toward skeletal and dental relapse. This article is a preliminary study to defy the efficiency of the wire osteosynthesis (wo)compared with rigid internal fixation (RIF) for simultaneous maxillary and mandibular surgery. 1. The vertical relapse rate of the A point after superior repositioning of the maxilla is 2.2%. 2. The horizontal relapse rate of the B point after advancement of the mandible is 18.3%. 3. The condyle is distracted inferiorly and slightly posteriorly at the immediate postoperative period. 4. At the long term follow up examination, the condyle presents tendency of return to the preoperative position. 5. Condylar segment angle is decreased at the immediate postoperative period, and at the long term follow up evaluation, the angle is increased. 6. Gonial angle is increased at the immediate postoperative period, and then is decreased at the long term follow up evaluation. 7. The dentition is satisfactory with acceptable movement at the long term follow up evaluation. 8. At the mandibular free body analysis, genioplasty shows good stability. 9. Wire osteosynthesis provides excellent stabilization for the simultaneous maxillary and mandibular surgery.

  • Rhizobium fredii USDA193은 대두(Peking)의 근모 세포벽에 침투하여 근류를 형성한다. 본 실험에서는 전보 (1)에서 분리 보고한 R.fredii의 pectate lyase 유전자 clone(SY1)으로부터 $\Omega$변이를 시켰다. 이것을 R.fredii USDA193에 다시 marker-exchange시켜 얻은 변이주(R.fredii USDA193$\Omega$와 R.fredii USDA193omega1)의 pectate lyase 활성이 완전히 block되지 못하였다. R.fredii 내에서는 다른 종류의 pel 유전자가 존재하 것으로 생각되며 pelB 및 pelE의 Rhizobium mutants들은 근류형성 초기단계에서 직접적으로 영향을 미치지 못하였다.

Towards Understanding Tuberculosis-Related Issues in North Korea: A Narrative Review of North Korean Literature

  • Background: North Korea is one of the 30 countries with the highest tuberculosis (TB) and drug-resistant TB burdened. To understand the medical issues and research trends associated with TB in North Korea, we performed a comprehensive review of articles related to clinical and laboratory research on TB published in North Korean medical journals. Methods: We reviewed all types of TB-related articles published in nine North Korean medical journals (Yebang ŭihak: Preventive medicine; Koryo ŭihak: Korea Medicine; Chosŏn ŭihak: Chosun Medicine; Naekwa: Internal Medicine; Soa, sanbuinkwa: Pediatrics, Obstetrics, and Gynecology, Surgery; Ŭihak: Medicine; Kich'o ŭihak: Basic Medicine; and Chosŏn yakhak: Chosun Pharmacy). We classified the articles according to the type and field of study and analyzed the data qualitatively to gain insights. Results: We reviewed 106 articles (one- or two-page length) written in Korean, including reviews (n=43), original articles (n=52), and case reports (n=8). They were classified as follows: articles on diagnosis (n=52, 49%) and treatment (n=39, 37%). None of the studies investigated the commercialized molecular diagnosis systems such as Xpert MTB/RIF. Directly Observed Treatment, Short-course was reported as the basic treatment approach. Furthermore, six studies used Korean traditional medicines for treating TB, with one of them containing snake venom. Conclusion: The articles were not sufficiently detailed. Original articles on the treatment of multi-drug resistant TB were not found, and those on latent tuberculosis infection and nontuberculous mycobacteria were limited. To understand the current medical issues associated with TB in North Korea, articles from these nine journals were not sufficient.

Influence of latitude wind pressure distribution on the responses of hyperbolodial cooling tower shell

  • Interference effects are of considerable concern for group hyperboloidal cooling towers, but evaluation methods and results are different from each other because of the insufficient understanding on the structure behavior. Therefore, the mechanical performance of hyperboloidal cooling tower shell under wind loads was illustrated according to some basic properties drawn from horizontal rings and cantilever beams. The hyperboloidal cooling tower shell can be regarded as the coupling of horizontal rings and meridian cantilever beams, and this perception is beneficial for understanding the mechanical performance under wind loads. Afterwards, the mean external latitude wind pressure distribution, CP(${\theta}$), was artificially adjusted to pursue the relationship between different CP(${\theta}$) and wind-induced responses. It was found that the maximum responses in hyperboloidal cooling tower shell are primarily dominated by the non-uniformity of CP(${\theta}$) but not the local pressure amplitude CP or overall resistance/drag coefficient CD. In all the internal forces, the maximum amplitude of meridian axial tension shows remarkable sensitivity to the variation of CP(${\theta}$) and it's also the controlling force in structure design, so it was selected as an indicator to evaluate the influence of CP(${\theta}$) on responses. Based on its sensitivity to different adjustment parameters of CP(${\theta}$), an comprehensive response influence factor, RIF, was deduced to assess the meridian axial tension for arbitrary CP(${\theta}$).

POINTWISE CROSS-SECTION-BASED ON-THE-FLY RESONANCE INTERFERENCE TREATMENT WITH INTERMEDIATE RESONANCE APPROXIMATION

  • The effective cross sections (XSs) in the direct whole core calculation code nTRACER are evaluated by the equivalence theory-based resonance-integral-table method using the WIMS-based library as an alternative to the subgroup method. The background XSs, as well as the Dancoff correction factors, were evaluated by the enhanced neutron-current method. A method, with pointwise microscopic XSs on a union-lethargy grid, was used for the generation of resonance-interference factors (RIFs) for mixed resonant absorbers. This method was modified by the intermediate-resonance approximation by replacing the potential XSs for the non-absorbing moderator nuclides with the background XSs and neglecting the resonance-elastic scattering. The resonance-escape probability was implemented to incorporate the energy self-shielding effect in the spectrum. The XSs were improved using the proposed method as compared to the narrow resonance infinite massbased method. The RIFs were improved by 1% in $^{235}U$, 7% in $^{239}Pu$, and >2% in $^{240}Pu$. To account for thermal feedback, a new feature was incorporated with the interpolation of pre-generated RIFs at the multigroup level and the results compared with the conventional resonance-interference model. This method provided adequate results in terms of XSs and k-eff. The results were verified first by the comparison of RIFs with the exact RIFs, and then comparing the XSs with the McCARD calculations for the homogeneous configurations, with burned fuel containing a mixture of resonant nuclides at different burnups and temperatures. The RIFs and XSs for the mixture showed good agreement, which verified the accuracy of the RIF evaluation using the proposed method. The method was then verified by comparing the XSs for the virtual environment for reactor applicationbenchmark pin-cell problem, as well as the heterogeneous pin cell containing burned fuel with McCARD. The method works well for homogeneous, as well as heterogeneous configurations.
