Laparoscopic Treatment of Ovarian Remnant Syndrome in a Queen (고양이에서 발생한 난소 잔존 증후군에 대하여 복강경을 이용한 치료 증례 1)

  • A 1-year-old, 2.35 kg spayed female American short hair cat was referred with episodic signs of heat at 3 months after ovariohysterectomy. Through the screening tests, bilateral cystic, ovary like masses were shown at the caudal to both kidneys with high serum estradiol concentration. It was considered that the patient was suffered from ovarian remnant syndrome. Laparoscopic exploration was performed, and each of mass lesions was resected by ultrasonic scalpel. Patient was recovered favorably and has been doing well without recurrence of estrus signs until 2 years after surgery.

Near-infrared Spectroscopy of Metal-enriched Supernova Ejecta in Cassiopeia A

  • The supernova remnant Cassiopeia A (Cas A) provides a unique opportunity to observe the fine details of the explosion of core-collapse supernova (SN). Previous optical and near-infrared (NIR) observations of Cas A have shown that the spatial distribution of the metal-enriched SN ejecta is very complicated, indicating that the SN explosion should have been asymmetric and turbulent, especially near the core. Recently, we obtained a long-exposure (~10 hr) image of Cas A by using the UKIRT 3.6-m telescope with a narrow-band filter centered at [Fe II] 1.644 um emission. This 'deep [Fe II] image' provides an unprecedented panoramic view of Cas A, revealing the distribution of dense SN ejecta over the entire remnant. We have carried out NIR multi-object spectroscopic observations of the dense ejecta knots in the northeastern (NE) and eastern (E) outer regions of the remnant using the MMIRS attached on the MMT 6.5-m telescope. A total of 67 ejecta knots are detected. By analyzing their spectra, we have found that the knots in the NE area show strong [S II]/[S III] and [Fe II] lines but little or no [P II] line, while those in the E outer region show strong [Fe II] lines but weak [S II]/[S III] lines. In this talk, we present the preliminary results of our NIR spectroscopic observations and discuss the implications.

A remnant choledochal cyst after choledochal cyst excision treated with a lumen-apposing metal stent: a case report

  • A lumen-apposing metal stent (LAMS) is a saddle-shaped stent with large flanges at both ends, thereby preventing stent migration and helping with approximation of the adjacent structures. We report the case of a 25-year-old female with remnant choledochal cyst which was successfully treated with LAMS after initial treatment failure with a plastic stent. Although complete excision of the cyst is the definite treatment of choledochal cysts, endoscopic ultrasonography-guided cystoduodenostomy can be considered in cases wherein surgery is not feasible and dysplasia is not present. LAMS may be preferred to plastic stents for effective resolution of remnant choledochal cyst and prevention of ascending infection.

  • After radical subtotal gastrectomy (RSTG) for stomach cancer, the remnant stomach is supposed to be perfused through the short gastric vessels. What if a patient who received previous RSTG is diagnosed with resectable distal pancreatic cancer? Can radical distal pancreatosplenectomy (DPS) be performed safely without ischemic damage to the remnant stomach? Unfortunately, there are limited studies on this specific clinical issue. Notably, in spite of rare clinical presentation, it is expected to increase due to prolonged survival of patients with resected gastric cancer. Therefore, we aimed to demonstrate the safety and feasibility of the radical DPS in patients with previous RSTG. In this study, we investigated perioperative and long-term survival outcomes of DPS for left-sided pancreatic cancer in patients with previous RSTG.

Patch Reconstruction with Radial Forearm Free Flap of Hypopharyngeal Cancer Using the Narrow Strip Pharynageal Wall (소폭의 잔존 하인두벽을 이용한 첩포형 전완유리 피판 인두 재건술)

  • Purpose: Various attempts of reconstruction for pharyngoesophageal defects after ablative surgery have been made to restore the function of the pharyngoesophagus. A fabricated tubed radial forearm free flap or free jejunal free flap was used when the width of remnant pharyngeal wall was less than 50% of the normal width. However there are many disadvantages such as stricture, saliva leakage and fistula formation on tubed radial forearm free flap. The jejunal free flap has the problem such as short pedicle, poor tolerance of ischemic time, wet voice and delayed transit of swallowed food due to the uncoordinated contraction. The authors studied the utility of patch-type radial forearm free flap using the remnant posterior pharyngeal wall of the hypopharynx. Methods: Retrospective reviews in Severance Hospital were made on 25 patients who underwent reconstruction surgery with patched radial forearm free flap because of the hypopharyngeal cancer between 1996 and 2005. The patients of Group I had the narrow posterior pharyngeal wall and its width was less than 3centimeters after the tumor was resected. Those of Group II had the partial pharyngectomy and the width of the remnant pharynx was larger than 3 centimeters. Results: Seven patients belonged to the group I and the flap of this group had 100% survival rate. One case of fistula and no swallowing discomfort due to stricture was reported. The Group II including 18 patients also had the 100% flap survival rate. Neither fistula nor stricture was seen but the lower diet grade was checked. Conclusion: The patch type radial forearm free flap using the remnant pharyngeal wall have the advantage of the radial forearm free flap, and furthermore this flap is the safe reconstructive method even if the width of the remnant pharyngeal wall is less than 30% of that of normal pharynx.

Characteristics of Habitats and Small Rodent Populations caused by Different Remnant Treatments on Forest Floor in Fired Coniferous Forest (산불피해지역에서 임상잔존물의 처리에 따른 서식환경과 소형 설치류 개체군 특성)

  • This study was conducted to clarify the characteristics of habitats and small rodent populations between remnant removed and left areas caused by different remnant treatment on forest floor in fired coniferous forest, Mt Gumbong, Samcheok, Korea, There were higher value of variables of understory coverage, snags, downed coarse woody debris (CWD) in natural restored area. Mean number of captured individuals and body weights of Apodemus peninsulae and Eothenomys regulus were more higher in remnant left area than in removed area, The differences in density and body weight of small rodents would be related with habitats between both study areas. The remove of remnant trees on the forest floor would not be good for small rodents. The more detailed and long-term researches would be needed for proper restoration in fired forest area.

Arthroscopic Posterior Cruciate Ligament Reconstruction with Preservation of the Remnant Posterior Cruciate Ligament (남아 있는 후방 십자 인대 다발을 보존한 관절경하 후방 십자 인대 재건술)

  • Purpose : The majority of PCL deficient knees have some intact remnant PCL bundles. In these cases, the remnant PCL bundles were removed fur easy passage of graft(especially in bone-patellar tendon-bone graft). The purpose of study is to report the results of PCL reconstruction by posterior transseptal portal technique and by preserving the remnant posterior cruciate ligament bundles. Materials & Methods : From November 1993 to June 1999, 44 arthroscopic posterior cruciate ligament reconstructions have been performed by one surgeon. We studied 37 knees those were followed up over 1 year among them. The mean age($14\~56$ years) was 31.8 years. The average follow-up period was 17.8 months($12\~61$ months). The graft materials were 29 double-loop hamstring autografts, 4 double-loop hamstring allografts and 4 Achilles tendon-bone allograft. Results : There are significant improvement of the preoperative subjective symptoms. The last follow-up shows that the average Lysholm knee score was markedly improved from 59.8 pre-operation to 89.2 post-operation and the average KT 2000 side difference was decreased from 11.1mm pre-operation to 2.3 mm post-operation. Follow-up MRI showed that the graft was healed with remnant posterior cruciate ligament bundles as one ligament. Conclusion : The posterior trans-septal portal makes it possible to locate the exact tibial tunnel. The arthroscopic PCL reconstruction preserving the remnant of original PCL bundles seems to provide the good result.

  • We have investigated the ferroelectric properties of multi-layered SrBi$_2$Ta$_2$$O_{9}$Pb(Zr,Ti)O$_3$, SBT/PZT, thin film capacitors. Specimens were prepared onto Pt-coated Si wafer by sol-gel method. Ferroelectric properties of these finns could be obtained only for thin SBT layers below 50nm in thickness. The values of dielectric constant and remnant polarization depend mainly on the thickness of SBT layer, which arises from the paraelectric interface layer between SBT and PZT due to the thermal diffusion of Pb. The value of remnant poarization of PZT/SBT is greater than that of SBT, and the plarization fatigue behaviors of PZT/SBT/Pt capacitors are somewhat improved as compared with those of PZT/Pt.t.
