Evaluation of Validity Thyroid Scintigraphy Using Parallel Hole Collimator (갑상샘 신티그래피 검사 시 평행다공형 조준기 적용의 유효성 평가)

  • In this study, When acquisition thyroid scintigraphy images, a parallel hole collimator was applied, and the difference from the pinhole collimator was quantitatively analyzed under each image acquisition condition. Visual size, resolution, sensitivity, signal to noise ratio (SNR), and contrast to noise ratio (CNR) were evaluated using thyroid phantom and point source. When comparing visual size, it was confirmed that an image similar to the size of the pinhole collimator could be obtained only when a magnification ratio of about 2.00 to 2.09 times when applying a parallel hole collimator. There was no tendency in FWHM(mm) measurement using a point source, and sensitivity was high in the parallel hole collimator. SNR and CNR were high when using a low magnification ratio, matrix size of 128×128, and a parallel hole collimator. In images of similar size to the naked eye, when the matrix size was the same, both SNR and CNR were high in the pinhole collimator. Therefore, when performing a thyroid scintigraphy test, if appropriate conditions are set according to the situation of each hospital and a parallel hole collimator is applied, it can be a good option in terms of equipment utilization and work efficiency.

Analysis of Occupational Therapy Students' Awareness of Adolescent Occupational Therapy: A Comparative Study Between Korea and the United States (청소년 작업치료에 관한 작업치료전공 학생들의 인식도 분석: 대한민국과 미국의 비교 연구)

  • Objective : This study aims to compare the awareness of adolescent occupational therapy (AOT) among university students majoring in occupational therapy (OT) in Korea and the United States through an online survey. Methods : This online survey was conducted from January 9 to 13, 2023 to investigate the awareness about AOT among 55 students majoring in OT in Korea and the United States. An online survey was formulated in Korean and English. Data analysis was performed using descriptive statistics and an independent t-test. Results : In response to the question "Have you ever heard of AOT?", 65.5% of Korean students answered yes and 58.2% of the United States students answered no. In both countries, the average score for the question "Can AOT help improve the quality of life of teenagers?" was the highest, while the score for "Is AOT sufficiently advanced in your country?" was the lowest. For "Potential for future development of AOT," the scores for the two countries showed a significant difference (p < .05). Conclusion : Students in both countries acknowledged the necessity and importance of AOT, but there were differences noted in terms of prospects of the therapy.

  • Objective : This study aimed to acquire activities of daily living (ADL) skills through activity analysis and forward chaining in children with developmental disabilities. Methods : In this study, the performance of children with developmental disabilities was guided stepwise using a changing criterion design. The intervention was conducted in forward chaining after the therapist analyzed the activities of three tasks (shoes, shorts, and short-sleeved T-shirts) set as the Canadian Occupational Performance Measure (COPM). The performance rate at each stage of the three tasks was used as an independent variable, and COPM performance and satisfaction scores before and after the intervention were set as dependent variables. Results : The task performance rates of children wearing shoes, shorts, and short-sleeved T-shirts improved over time. Even at home, the scores for performance and satisfaction of all three tasks increased. Conclusion : Hopefully, activity analysis and behavioral chaining methods will be used not only for ADL but also for various tasks in children with developmental disabilities who have difficulty acquiring tasks.

Can ChatGPT Pass the National Korean Occupational Therapy Licensure Examination? (ChatGPT는 한국작업치료사면허시험에 합격할 수 있을까?)

  • Objective : This study assessed ChatGPT, an artificial intelligence system based on a large language model, for its ability to pass the National Korean Occupational Therapy Licensure Examination (NKOTLE). Methods : Using NKOTLE questions from 2018 to 2022, provided by the Korea Health and Medical Personnel Examination Institute, this study employed English prompts to determine the accuracy of ChatGPT in providing correct answers. Two researchers independently conducted the entire process, and the average accuracy of both researchers was used to determine whether ChatGPT passed over the 5-year period. The degree of agreement between ChatGPT answers of the two researchers was assessed. Results : ChatGPT passed the 2020 examination but failed to pass the other 4 years' examination. Specifically, its accuracy in questions related to medical regulations ranged from 25% to 57%, whereas its accuracy in other questions exceeded 60%. ChatGPT exhibited a strong agreement between researchers, except for medical regulation questions, and this agreement was significantly correlated with accuracy. Conclusion : There are still limitations to the application of ChatGPT to answer questions influenced by language or culture. Future studies should explore its potential as an educational tool for students majoring in occupational therapy through optimized prompts and continuous learning from the data.

Influence of varying cement types and abutment heights on pull-off force of zirconia restorations (시멘트의 종류 및 임플란트 지대주 높이가 지르코니아 수복물의 제거력에 미치는 영향)

  • Purpose: The purpose of this study is to evaluate Ti-base abutment's three different heights and three different cement types on the pull-off force of zirconia-based restorations. Materials and Methods: A total of 90 fixture lab analogs were embedded in auto polymerizing resin bloack. 90 Ti-base abutments heights of 3 mm, 5 mm, 7 mm were scanned and zirconia restoration were prepared from scanned files. Zirconia restoration were cemented with three different types of cements (temporary, semi-permanent, permanent) following manufacturer's instructions. All 90 specimens were placed and tested in a universal testing machine for pull-out testing. Retention was measured by recording the force at load drop. Statistical analysis was performed using Kruskal-Wallis test for detecting whether there are any statistical significance along cement types or abutment heights. After that, Mann-Whitney test was used for figuring out differences regarding abutment height and the comparison between 3 cements. Results: Temp bond showed significantly lower pull-off force compared to Fujicem regardless of any abutment height. However, there were significant differences between Cem-implant and Fujicem in abutment height of 3 mm and 7 mm, but there was no significant difference in 5 mm. Temp bond and Cem-implant had significant differences only in abutment height of 5 mm. Conclusion: Although Ti-base abutment height did not influenced zirconia restorations' retentiveness, cement types showed significant differences.

Fabrication of surveyed crown and repairing the artificial teeth for existing removable partial denture using digital technology: a case report (디지털 방식을 이용한 기존 국소의치 맞춤 보철 제작과 심미적인 인공치 수리 증례)

  • It often happens that a removable partial denture needs to be repaired due to tissue changes in the remaining alveolar ridge, fracture of the denture, or fracture of the abutment tooth. There are several advantages to retrofitting a customized surveyed crown under the existing RPD. Retrofitting a crown to the RPD decreases the economic burden to the patient and avoids the need for several appointments to fabricate a new RPD. It is difficult for artificial teeth used to repair dentures due to fractured natural teeth to have a shape similar to that of natural teeth, and to repair aesthetic artificial teeth, it is necessary to manufacture customized artificial teeth similar to the shape of each patient's teeth. Recently, CAD/CAM technology has been used to fabricate customized prosthetics on existing RPD to achieve high retention and fitness accuracy, and by manufacturing customized artificial teeth, more aesthetic and harmonious artificial tooth repair is possible. This is a case in which a denture was repaired using a digital method to fabricate a customized prosthesis on an existing partial denture and customized artificial teeth that mirrored the adjacent dentition, saving time and cost, simplifying the process, and achieving aesthetically and functionally satisfactory results.

Complete denture fabrication of a skeletal class III edentulous patient considering anterior neutral zone: a case report (골격성 III급 무치악 환자에서 전치부 중립대를 고려한 총의치 제작 증례)

  • In the case of skeletal class III edentulous patients, the stability of dentures can be achieved by using a crossbite by considering the residual ridge relationship when fabricating complete dentures. Forming the anterior teeth in a normal occlusal relationship in a skeletal class III ridge relationship may reduce the stability of the denture by increasing the anterior cantilever. However, when patients use complete dentures, not only functional aspects but also aesthetic aspects are important. The aesthetics of complete dentures depends on how the anterior artificial teeth are arranged, and cases of complete denture fabrication using normal occlusion or edge-to-edge bite in edentulous patients with skeletal class III ridge relationships have been reported. In this case, complete dentures were fabricated for an edentulous patient with a skeletal class III edentulous patients by forming anterior edge-to-edge bite considering neutral zone in maxillary anterior teeth, and good aesthetic and functional results were obtained.

Automated Detection and Segmentation of Bone Metastases on Spine MRI Using U-Net: A Multicenter Study

  • Objective: To develop and evaluate a deep learning model for automated segmentation and detection of bone metastasis on spinal MRI. Materials and Methods: We included whole spine MRI scans of adult patients with bone metastasis: 662 MRI series from 302 patients (63.5 ± 11.5 years; male:female, 151:151) from three study centers obtained between January 2015 and August 2021 for training and internal testing (random split into 536 and 126 series, respectively) and 49 MRI series from 20 patients (65.9 ± 11.5 years; male:female, 11:9) from another center obtained between January 2018 and August 2020 for external testing. Three sagittal MRI sequences, including non-contrast T1-weighted image (T1), contrast-enhanced T1-weighted Dixon fat-only image (FO), and contrast-enhanced fat-suppressed T1-weighted image (CE), were used. Seven models trained using the 2D and 3D U-Nets were developed with different combinations (T1, FO, CE, T1 + FO, T1 + CE, FO + CE, and T1 + FO + CE). The segmentation performance was evaluated using Dice coefficient, pixel-wise recall, and pixel-wise precision. The detection performance was analyzed using per-lesion sensitivity and a free-response receiver operating characteristic curve. The performance of the model was compared with that of five radiologists using the external test set. Results: The 2D U-Net T1 + CE model exhibited superior segmentation performance in the external test compared to the other models, with a Dice coefficient of 0.699 and pixel-wise recall of 0.653. The T1 + CE model achieved per-lesion sensitivities of 0.828 (497/600) and 0.857 (150/175) for metastases in the internal and external tests, respectively. The radiologists demonstrated a mean per-lesion sensitivity of 0.746 and a mean per-lesion positive predictive value of 0.701 in the external test. Conclusion: The deep learning models proposed for automated segmentation and detection of bone metastases on spinal MRI demonstrated high diagnostic performance.

Functional Magnetic Resonance Imaging and Diffusion Tensor Imaging for Language Mapping in Brain Tumor Surgery: Validation With Direct Cortical Stimulation and Cortico-Cortical Evoked Potential

  • Objective: Functional magnetic resonance imaging (fMRI) and diffusion tensor imaging-derived tractography (DTI-t) contribute to the localization of language areas, but their accuracy remains controversial. This study aimed to investigate the diagnostic performance of preoperative fMRI and DTI-t obtained with a simultaneous multi-slice technique using intraoperative direct cortical stimulation (DCS) or corticocortical evoked potential (CCEP) as reference standards. Materials and Methods: This prospective study included 26 patients (23-74 years; male:female, 13:13) with tumors in the vicinity of Broca's area who underwent preoperative fMRI and DTI-t. A site-by-site comparison between preoperative (fMRI and DTI-t) and intraoperative language mapping (DCS or CCEP) was performed for 226 cortical sites to calculate the sensitivity and specificity of fMRI and DTI-t for mapping Broca's areas. For sites with positive signals on fMRI or DTI-t, the true-positive rate (TPR) was calculated based on the concordance and discordance between fMRI and DTI-t. Results: Among 226 cortical sites, DCS was performed in 100 sites and CCEP was performed in 166 sites. The specificities of fMRI and DTI-t ranged from 72.4% (63/87) to 96.8% (122/126), respectively. The sensitivities of fMRI (except for verb generation) and DTI-t were 69.2% (9/13) to 92.3% (12/13) with DCS as the reference standard, and 40.0% (16/40) or lower with CCEP as the reference standard. For sites with preoperative fMRI or DTI-t positivity (n = 82), the TPR was high when fMRI and DTI-t were concordant (81.2% and 100% using DCS and CCEP, respectively, as the reference standards) and low when fMRI and DTI-t were discordant (≤ 24.2%). Conclusion: fMRI and DTI-t are sensitive and specific for mapping Broca's area compared with DCS and specific but insensitive compared with CCEP. A site with a positive signal on both fMRI and DTI-t represents a high probability of being an essential language area.

Association between High Diffusion-Weighted Imaging-Derived Functional Tumor Burden of Peritoneal Carcinomatosis and Overall Survival in Patients with Advanced Ovarian Carcinoma

  • Objective: To investigate the association between functional tumor burden of peritoneal carcinomatosis (PC) derived from diffusion-weighted imaging (DWI) and overall survival in patients with advanced ovarian carcinoma (OC). Materials and Methods: This prospective study was approved by the local research ethics committee, and informed consent was obtained. Fifty patients (mean age ± standard deviation, 57 ± 12 years) with stage III-IV OC scheduled for primary or interval debulking surgery (IDS) were recruited between June 2016 and December 2021. DWI (b values: 0, 400, and 800 s/mm2) was acquired with a 16-channel phased-array torso coil. The functional PC burden on DWI was derived based on K-means clustering to discard fat, air, and normal tissue. A score similar to the surgical peritoneal cancer index was assigned to each abdominopelvic region, with additional scores assigned to the involvement of critical sites, denoted as the functional peritoneal cancer index (fPCI). The apparent diffusion coefficient (ADC) of the largest lesion was calculated. Patients were dichotomized by immediate surgical outcome into high- and low-risk groups (with and without residual disease, respectively) with subsequent survival analysis using the Kaplan-Meier curve and log-rank test. Multivariable Cox proportional hazards regression was used to evaluate the association between DWI-derived results and overall survival. Results: Fifteen (30.0%) patients underwent primary debulking surgery, and 35 (70.0%) patients received neoadjuvant chemotherapy followed by IDS. Complete tumor debulking was achieved in 32 patients. Patients with residual disease after debulking surgery had reduced overall survival (p = 0.043). The fPCI/ADC was negatively associated with overall survival when accounted for clinicopathological information with a hazard ratio of 1.254 for high fPCI/ADC (95% confidence interval, 1.007-1.560; p = 0.043). Conclusion: A high DWI-derived functional tumor burden was associated with decreased overall survival in patients with advanced OC.
