Abstract. An area of particularly rapid technological growth in the last 15 years has been medical imaging (conventional X-ray, ultrasound, X-ray computed tomography (CT), magnetic resonance imaging (MRI). As the number and complexity of imaging studies rises, it becomes ever more important to distribute these images and the associated diagnoses in a timely and cost-effective fashion. The purpose of this paper is to describe the requirements for a future digital radiology system which will efficiently handle the large volume of images that generated, add new functionality to improve productivity of physicians, technologists, and other health care providers, and provide enough flexibility to allow the system to grow as medical image technology grows.

Nonalcoholic fatty liver disease, characterized by excessive accumulation of fat in the liver, is the most common chronic liver disease worldwide. The current standard for the detection of hepatic steatosis is liver biopsy; however, it is limited by invasiveness and sampling errors. Accordingly, MR spectroscopy and proton density fat fraction obtained with MRI have been accepted as non-invasive modalities for quantifying hepatic steatosis. Recently, various quantitative ultrasonography techniques have been developed and validated for the quantification of hepatic steatosis. These techniques measure various acoustic parameters, including attenuation coefficient, backscatter coefficient and speckle statistics, speed of sound, and shear wave elastography metrics. In this article, we introduce several representative quantitative ultrasonography techniques and their diagnostic value for the detection of hepatic steatosis.
To the detect of SPIO-labelled hMSC, in vitro study on various cell concentration and in vivo molecular magnetic resonance imaging(MRI) technique using T2, $T2^*$ and SWI are compared with pathology. Cell concentration was $1.56{\times}10^4$, $3.13{\times}10^4$, $6.25{\times}10^4$, $1.25{\times}10^5$, $2.5{\times}10^5$, $5{\times}10^5\;cells/m{\ell}$ and for control $5{\times}10^5\cells/m{\ell}$. MRI technique using T2, $T^2$ and SWI. Photothrombotic infarction was located 2.5mm from bregma right, posterior. Cell injected through the tail vein of rat for 8 rats. MRI performed pre injection and post injection of 1, 3, 7 and 14days and sacrifice for pathology. MRI analysed on quantitatively. In vitro result, SWI was highest CNR as compared with $T2^*WI$, T2WI and $2.5{\times}10^5\;cells/m{\ell}$ cell concentration. In vivo result among the T2WI, $T2^WI$, SWI, T2WI is highest CNR between normal and infarction. CNR in normal-SPIO and infarction-SPIO is high score in SWI. Therefore, T2WI is good distinguish between normal and infarction, SWI are well detect SPIO-labelled hMSC from normal and infarction. Nowaday, SWI are mostly used on hemorrhage, calcification etc. in clinically, but for the future, stem cell therapy is commonly application at all disease which is good observing tool for SPIO-labelled stem cells.
Objectives : We evaluated the use of FDG PET/CT for the identification of cervical nodal metastases of SCC of the oral cavity and oropharynx with histological correlation. Material and Methods : We reviewed 46 medical records, from January 2004 to July 2007, of patients who underwent FDG PET/CT and CT/MRI for SCC of the oral cavity and oropharynx before surgery. We recorded the lymph node metastases according to the neck level affected and the system used for the imaging-based nodal classification. Results : The FDG PET/CT had a sensitivity of 75.6% and a specificity of 96.7% ; it had a higher sensitivity than the CT/MRI for identification of cervical metastases on the side of the neck(26/28 vs. 20/28, p=0.031) and at each of the cervical levels(34/45 vs. 26/45, p=0.008). There was a significant difference in the $SUV_{max}$ between the benign and malignant cervical lymph nodes($3.31{\pm}3.23$ vs. $4.22{\pm}2.57$, p=0.028). The receiver-operating-characteristic (ROC) curve analysis for differentiating the benign from the malignant cervical lymph nodes, showed that the area under the curve(AUC) of the FDG PET/CT was 0.775. The cut-off value for the $SUV_{max}$ was 2.23 based on the ROC curve. There was a significant correlation between the $SUV_{max}$ and the size of the cervical lymph nodes(Spearman r=0.353, p=0.048). Conclusion : FDG PET/CT images were more accurate than the CT/MRI images. In addition, the $SUV_{max}$ cut-off values were important for evaluating cervical the cervical nodes in the patients with SCC of the oral cavity and oropharynx.

Purpose: As increasing complexity of modern radiotherapy technique, more developing dosimetry is required. Polymer gel dosimeters offer a wide range of potential applications with high resolution and assured quality in the thee-dimensional verification of complex dose distribution such as intensity-modulated radiotherapy (IMRT). The purpose of this study is to find the most sensitive and suitable gel as a dosimeter by varying its composition ratio and its condition such as temperature during manufacturing. Materials and Methods: Each polymer gel with various ratio of composition was irradiated with the same amount of photon beam accordingly. Various polymer gels were analyzed and compared using a dedicated software written in visual C++ which converts TE images to R2 map images. Their sensitivities to the photon beam depending on their composition ratio were investigated. Results: There is no dependence on beam energy nor dose rate, and calibration curve is linear. Conclusion: The polymer gel dosimeter developed by using anti-oxidant in this study proved to be suitable for dosimetry.

Abstract: RF Birdcage Resonator는 높은 자장 균질성과 신호 대 잡음비의 특성이 좋음으로 인해 MRI와 MRS 연구에 보편적으로 사용되고 있다. 본 연구는 인체를 비롯한 동물실험을 위한 다양한 실험대상에 따라 코일의 크기를 예측하여 Birdcage Resonator를 개발하였다. Unloaded와 Loaded 상태에서의 전기적인 성향을 Lumped Element Circuit Theory를 따라 일반적인 분석을 하였을 뿐 아니라 실제적인 실험을 통한 분석도 하였다. Introduction: Birdcage Resonator 는 자장 균일성이 뛰어나고 신호대잡음비가 좋아 Magnetic Resonance Imaging과 Spectroscopy에 유용하게 사용되어지고 있다. 이 연구에서는 실험대상에 따른 여러 가지의 Birdcage Resonator를 개발하기 위해 Lumped Element Circuit Theory를 사용한 일반적인 이론적 방법론을 이용하여 코일을 분석하였을 뿐 아니라 실제 제작한 후 실험을 통한 실질적인 영상분석도 병행하여 진행하였다. Method: 실질적인 개발에 앞서 Lumped Element Circuit Theory를 통해 이론적인 분석으로부터 나온 여러 parameter들을 정리하였다. 제작 후 실험적인 분석은 Network Analyzer (HP 4195A, 미국)를 통한 1차적인 분석을 한 후 최종적으로 3T MRI 장비 (메디너스, 한국)에서 수행하였다. 제작된 Birdcage Resonator의 종류는 기본적으로 Low-Pass Filter를 적용한 12-column 구조이다. 각각의 직경과 element의 길이는 다음과 같다. 1) 직경 13cm, element의 길이 22cm, 2) 직경 15cm, element의 길이 22cm, 3) 직경 17 cm, element의 길이 25cm. Result and Discussion: 여러 parameter들을 이론적으로 계산하여 실제 Birdcage Resonator를 제작하여 실험적인 분석을 한 결과 많은 차이가 있음을 알 수 있었다. 그 이유는 이론적인 계산에서는 생각되지 않았던 요인들이 실제 제작에 있어서는 많은 영향을 미쳤기 때문인 것이다. Network Analyzer를 통한 1차적인 분석 후에도 실제의 Magnet Field내에서 또 다시 차이가 나는 것 또한 같은 이유에서이다. 그러므로 Birdcage Resonator 제작에 있어서 이러한 여러 요인들을 충분히 고려하여야 할 것이다. Objective : Endoscopic third ventriculostomy is gaining popularity as a minimally invasive surgical option for certain types of hydrocephalus as an alternative to shunting. The authors have tried to fenestrate down to the subdural space passing through the prepontine cistern to lessen or avoid the chance of redoing due to healing. Materials and Method : A 48-year-old male patient with several years of intractable headache was presented. Magnetic Resonance Image(MRI) of the brain revealed marked ventricular dilatation with stenotic cerebral aqueduct. A 2.3mm flexible steerable endoscope($Neuroview^{(R)}$) was introduced via precoronal route and accessed to the third ventricular floor. Using 3-French Fogarty balloon catheter, thin third ventricular floor and the arachnoid membrane of the prepontine cistern were fenestrated, so called "double fenestration". To confirm the fenestration, subdural compa-rtment of the left abducens nerve was identified during the procedure. Forceful pulsating flow through the orifice convinced the patency of the opening. Results : The patient was discharged on the third postoperative day without any postoperative complications. The postoperative follow-up MRI of the brain, at second and sixth months, clearly demonstrated the flow void through the third ventricular floor. Conclusions : Endoscopic third ventriculostomy was successfully performed on an adult hydrocephalus patient with aqueduct stenosis. The third ventricular floor and arachnoid membrane of the prepontine cistern were fenestrated to achieve double fenestration to minimize the chance for failure. The details of this procedure and results are described.

Advancement of surgical techniques has made it necessary to accurately diagnose internal derangements. Arthrography and computerized tomography have been used to diagnose the majority of temporomandibular joint disorders, however, these methods have had their disadvantages. Magnetic resonance imaging utilizing surface coils has greatly improved the ability to diagnose meniscus abnormalities without using intrarticular injections or ionizing radiation. Ninety-two patients (184 joints) were evaluated by means of magnetic resonance imaging(MRI). Thirty-one patients (39 joints) were diagnosed as having meniscus perforation. Retrospective review of fifteen patients (20 joints) with a perforated meniscus diagnosed by magnetic resonance imaging pre-operatively demonstrated a sixty-five percent correlation between the radiographic diagnosis and the surgical findings.

Matrix metalloproteinases (MMPs) play an important role in the normal morphogenesis, maintenance, and repair of matrix and also have important functions in pathologic conditions characterized by excessive degradation of extracellular matrix, such as rheumatoid arthritis, osteoarthritis, periodontitis and in tumor invasion and metastasis. In this study, expression of MMP-1 and -2 mRNA in retrodiscal tissue of the temporomandibular joint (TMJ) was examined and compared with magnetic resonance imaging (MRI) and surgical findings. MMP mRNAs in the retrodiscal tissue samples were detected by reverse transcription - polymerase chain reaction. TMJ internal derangement (ID) was categorized as normal disc position, disc displacement with reduction, early stage of disc displacement without reduction (DDsR) and late stage of DDsR. TMJ osteoarthrosis (OA) was classified with normal, mild and advanced OA. The amount of synovial fluid collection was divided into not detected, small, large and extremely large amount on MR T2-weighted imaging. Perforation and adhesion were examined during open surgery of the TMJ. Six out of 37 samples were excluded because of little amount of extracted total mRNA. MMP-2 mRNA was detected whole joints, and so the MMP-2 mRNA seems to be expressed normally in retrodiscal tissue. However, MMP-1 mRNA was expressed in 8 of 31 joints. Frequencies of MMP-1 mRNA expression according to the TMJ IDs, amount of synovial fluid and surgical findings made no significant difference. MMP-1 mRNA was detected more frequently in OA groups (7/16 joints, 43.8%) than in normal bony structure group (1/15 joints, 6.7%). Expression of MMP-1 mRNA in retrodiscal tissue might be related with OA of the TMJ.
Objectives : The correction of clinical and radiologic abnormalities in patients with symptomatic ossification of the posterior longitudinal ligament (OPLL) is the current mainstay of treatment. This study aimed to identify radiographic predictors of severity of myelopathy in patients with symptomatic OPLL. Methods : Fifty patients with symptomatic cervical OPLL were enrolled. Based on Japanese Orthopedic Association (JOA) scores, patients were divided into either the mild myelopathy (n=31) or severe myelopathy (n=19) group. All subjects underwent preoperative plain cervical roentgenogram, computed tomography (CT), and MR imaging (MRI). Radiological parameters (C2-7 sagittal vertical axis, SVA; C2-7 Cobb angle; C2-7 range of motion, ROM; OPLL occupying ratio; and compression angle) were compared. Compression angle of OPLL was defined as the angle between the cranial and caudal surfaces of OPLL at the maximum level of cord compression Results : The occupying ratio of the spinal canal, C2-7 Cobb angle, C2-7 SVA, types of OPLL, and C2-7 ROM of the cervical spine were not statistically different between the two groups. However, the OPLL compression angle was significantly greater (p=0.003) in the severe myelopathy group than in the mild myelopathy group and was inversely correlated with JOA score (r=-0.533, p<0.01). Furthermore, multivariate regression analysis demonstrated that the compression angle (B=-0.069, p<0.001) was significantly associated with JOA scores (R=0.647, p<0.005). Conclusion : Higher compression angles of OPLL have deleterious effects on the spinal cord and decrease preoperative JOA scores.
