Study of the Development of the Pulmonary Arteries following the Blalock-Taussig Shunt in Tetralogy of Fallot (활로씨 사징증 환자에서 Blalock-Taussig 단락술후 폐동맥의 발달에 관한 연구)

  • Primary intracardiac repair of tetralogy of Fallot with low mortality and early good results, has been accomplished in recent years. But palliative procedures have been reserved for those hypoplastic pulmonary arteries, a hypoplastic left ventricle or anomalies of the coronary artery would make total correction difficult. And the Blalock-Taussig shunt operation is recognized as a standard and popular palliative procedure. I undertook a retrospective determination of the effect of the Blalock-Taussig shunt operation on the development of the main pulmonary artery and the right and left pulmonary arteries. Between January, 1980, and April, 1987, at the Severance Hospital, 16 patients were studied by cardiac catheterization and angiocardiography, before undergoing Blalock-Taussig shunting procedures for the palliation of severe symptoms of tetralogy of Fallot, and some time later, usually prior to a second procedure. The mean interval between catheterizations was 22.25 months. Patients with tetralogy of Fallot and pulmonary atresia or with an occluded shunt were not included. The primary and secondary angiograms of each patient were reviewed, and measurements of the diameter of the main pulmonary artery, the right and left pulmonary arteries, and the descending thoracic aorta were taken. The results are as follows; 1. The hematocrit decreased from 56.39% to 50.34%[p< 0.05], and the arterial oxygen saturation increased from 62.00 % to 81.31 %[p< 0.001] following shunt procedures 2. The ratio of the diameter of the right pulmonary artery plus the left pulmonary artery to the diameter of the descending thoracic aorta increased 1.30 k 0.28 times [p< 0.01]; but the ratio of the diameter of the main pulmonary artery to the diameter of the descending thoracic aorta increased 1.10 * 0.33 times, which was not. significant[p< 0.05]. 3. The interval between shunting and second catheterization was not related to the magnitude of change in the pulmonary arteries[r=0.141, p >0.05]. 4. The changes in the ratio of the diameter of the right pulmonary artery plus the diameter of the left pulmonary artery to the diameter of the descending thoracic aorta was inversely related to the initial ratio[r=0.757, p >0.001], but the change in the ratio of the diameter of the main pulmonary artery to the descending thoracic aorta was not related[r=0.059, p >0.05]. 5. There were no differences in enlargement of the pulmonary artery on the side of the shunt [ipsilateral] versus enlargement on the opposite side [p >0.05], nor according to the size of the shunt[p >0.05]. In conclusion, this study suggests that the Blalock-Taussig shunt is effective for the development of the right and left pulmonary arteries but not effective for the main pulmonary artery.

Estimation Error Analysis on the COD Loads due to the Sampling Intervals (관측간격에 따른 COD 오염부하 추정오차 분석)

  • The change patterns of the water quanity, water quality(WQ, exclusively COD in this study) and monthly / annual pollutant loads(PL) estimated using the daily effluent discharges and WQ measurement data are analyzed in the Nakdong river estuarine seadike. The contribution factor defined by the ratio of the water quantity range and WQ range shows that the PL pattern in this estuary is classified as the strongly flow(water quantity)-dominated situation. The estimated PL(EPL)s with respect to the increasing sampling periods, e.g., 2-days, 5-days, 7-days, and so on, show that the mean values of the EPLs remain nearly same, whereas the standard deviations of the EPLs have an obvious increasing trend. The PL values using the monthly-averaged water quantity and WQ measurement data could have approximately 100% estimation error in annual mean and $300\sim400%$ estimation errors in summer season because its confidence level is relatively low. It is recommended that the PL should be estimated using at least $10\sim20$ day interval data sets and also the water quantity(river discharges) and WQ should be measured at the intervals of at least 1 day interval and 5 to 7 days, respectively in summer.

  • Several algorithms have been developed to classify arrhythmia which either rely on specific ECG(Electrocardiogram) database. Nevertheless personalized difference of ECG signal exist, performance degradation occurs because of carrying out diagnosis by general classification rule. Most methods require accurate detection of P-QRS-T point, higher computational cost and larger processing time. But it is difficult to detect the P and T wave signal because of person's individual difference. Therefore it is necessary to design efficient algorithm that classifies different arrhythmia in realtime and decreases computational cost by extracting minimal feature. In this paper, we propose arrhythmia classification method using QRS Pattern of ECG signal according to personalized type. For this purpose, we detected R wave through the preprocessing method and define QRS pattern of ECG signal by QRS feature Also, we detect and modify by pattern classification, classified arrhythmia duplicated QRS pattern in realtime. Normal, PVC, PAC, LBBB, RBBB, Paced beat classification is evaluated by using 43 record of MIT-BIH arrhythmia database. The achieved scores indicate the average of 99.98%, 97.22%, 95.14%, 91.47%, 94.85%, 97.48% in PVC, PAC, Normal, BBB, Paced beat classification.

  • Reliability based design and assessment (RBDA) methodology is one of the newest directions of natural gas pipeline design method. Reliability targets are used to ensure that safety levels are met relevant limit states in the stage of design and maintenance. The target reliability for ultimate limit states such as large leak and rupture were developed using tolerable risk criteria for individual and societal risk. This paper shows the reliability target can be met through the implementation of periodic maintenance measures during the life cycle of the pipelines. The case study involves the calculation of the failure probability due to equipment impact, the calculation of the failure probability due to corrosion, and the estimation the re-inspection interval for domestic natural gas transmission pipelines.

  • A possibility of the drag coefficient estimation in numerical water basins was discussed where the numerical solution were calculated by the 3-dimensional hydro-dynamical model (FLOW-$3D^{(R)}$) with the RNG $k-{\varepsilon}$ turbulence model. On the known cases of the drag coefficients for a rectangle, the numerical drag coefficients got $1.34{\sim}1.52$ and the wind tunnel values were $1.3{\sim}1.5$. For a cylinder, the numerical values were calculated as $0.75{\sim}0.78$ in the range of 0.5

The Effect of Maternal Pre-pregnancy Body Mass Index on Very Low Birth Weight Infants

  • Purpose: The pre-pregnancy body mass index (BMI) is associated with adverse neonatal outcomes. However, studies on very low birth weight (VLBW) infants are rare. This study aimed to investigate the effect of maternal pre-pregnancy BMI on VLBW infants. Methods: This retrospective study evaluated singleton VLBW infants born at the CHA Gangnam Medical Center from 2006 to 2016. The neonates were classified into three groups according to the maternal pre-pregnancy BMI: underweight (<$18.5kg/m^2$), normal weight (${\geq}18.5$ to <$23kg/m^2$), and overweight or obese (${\geq}23kg/m^2$). Clinical characteristics and morbidities of mothers and infants were analyzed. Results: A total of 181 infants belonging to underweight (16.6%), normal weight (58.6%), and overweight or obese (24.8%) groups were enrolled. The pre-pregnancy BMI had a significant negative correlation with gestational age (r=-0.198, P=0.001) and a significant positive correlation with the z-score of the birth weight (r=0.078, P=0.001) and body length (r=0.067, P=0.008). The number of extremely preterm infants was significantly higher in the overweight or obese group. The proportion of risk of small for gestational age infants was higher in the underweight group (adjusted odds ratio [OR], 2.958; 95% confidence interval [CI], 1.113 to 7.864), whereas that of infants with severe retinopathy of prematurity was higher in the overweight or obese group (adjusted OR, 9.546; 95% CI, 1.230 to 74.109). Conclusion: In our population of VLBW infants, the pre-pregnancy BMI was associated with gestational age, intrauterine growth, and adverse neonatal outcomes. Therefore, proper weight control before pregnancy is important.

Development of an IAQ Index for Indoor Garden Based IoT Applications for Residents' Health Management (실내거주자 건강 관리를 위한 IoT기반 실내정원용 IAQ지수 개발)

  • Objectives: In this study, we started to develop an indoor garden integrated IoT solution based on IAQ (indoor air quality) and interconnection with an environmental database for smart management of indoor gardens. The purpose of this study was to develop and apply an integrated solution for customized air purification from an indoor garden through big data analysis using IoT technology. Methods: An IoT-based IAQ monitoring system was established in three households within a new apartment building. Based on real-time and long-term data collected, $PM_{2.5}$, $CO_2$, temperature, and humidity changes were compared to those of indoor garden applications and the analyzed results were indexed. Results As a result of the installation, all three households had no results exceeding the standard for indoor air pollution on average $PM_{2.5}$ and $CO_2$ indices. In the case of indoor garden installation, the IAQ index increased to the "Good" section after the installation, and readings in the "Bad" section shown before the installation disappeared. The comfort index also did not dip into the "Uncomfortable" section, where it had been preinstallation, and significantly lowered the average score from "Uncomfortable for sensitive groups" to "Good". Overall, the IAQ composite index for the generation of installations decreased the "Good" interval, but "Bad" did not appear. Conclusions In this study on developing an integrated solution for IAQ based on IoT indoor gardens, big data was analyzed to determine IAQ and comfort indexes and an IAQ composite index. Through this process, it became understood that it is necessary to monitor IAQ based on IoT.

Evaluation of the Korean Version of Physical Activity Scale for the Elderly (K-PASE) (한국판 노인 신체활동 측정도구(Korean version of Physical Activity Scale for the Elderly: K-PASE)의 평가)

  • Purpose: The purpose of this study was to develop a Korean version of Physical Activity Scale for the Elderly (K-PASE) from PASE and to evaluate the validity and reliability of the K-PASE. This scale was originally developed by Washburn et al. (1993). A valid and reliable physical activity scale for the elderly is needed to assess accurately physical activity of the elderly, because there has been no valid and reliable physical activity scale for the elderly in Korea. Methods: The K-PASE was developed from PASE through linguistic validation and cultural adaptation for use with this population. Convenient sampling was used to recruit participants. Reliability was evaluated by conducting the test-retest and convergent validity was evaluated by Pearson correlation. Results: Test-retest reliability, assessed over a 2 week interval, was r=.94. Convergent validity was established by correlating the KPASE scores between related variables which were the Tokyo Metropolitan Institute of Gerontology Index of Competence (TMIG-IC) and hand-grip strength. Scores of the K-PASE had a low significant correlation with TMIG-IC (r=.246, p<.001), and hand-grip strength (r=.251, p<.001). The mean score of physical activity of Korean elderly assessed by the K-PASE was 96.5. Conclusion: We conclude that the K-PASE is a reliable and valid instrument to assess physical activity for Korean elderly.

QTL analysis of for micronutrient content in rice grain

  • Micronutrients such as zinc (Zn), iron (Fe), manganese (Mn) have important roles for development and growth in plants but it also have roles in animals and humans. In previous studies, a Korean weedy rice, KH2J was selected to have tolerance to heavy metal, lead (Pb) compared with a cultivar, Milyang23. To identify QTLs for micronutrients concentration in grain, an F2 population (120 plants) were developed from a cross between KH2J and an indica rice cultivar, Milyang23. To measure the concentration of eight ions, Zn, Fe, Mn, Pb, calcium (Ca), copper (Cu), cadmium (Cd) and arsenic (As), grains were collected and digested with 65% nitric acid, and the ion contents were measured using inductively coupled plasma mass spectrometry. A total 27 putative quantitative trait loci (QTLs) were detected on 12 chromosomes by single point analysis and 22 putative QTLs were detected by composite interval mapping. The co-locations of QTL for Zn, Fe and Mn were observed on chromosome 5. The QTLs for Cd, Cu and Zn were co-localized on chromosome 10, and QTLs for Zn, As and Mn was on chromosome 12. The Zn concentration in F2 generation showed significant correlation with concentrations of As (r = -0.4), Cu (r = 0.5) and Fe (r = 0.2) (P < 0.01). Also, the Ca concentration was significantly related with Mn and Fe concentrations (P < 0.01). Fine mapping of these QTLs is underway to analyze their functional relationship.

Reliability and Responsiveness of the Korean Version of the Trunk Impairment Scale for Stroke Patients

  • Purpose: The purposes of this study were to develop the Korean version of the trunk impairment scale (K-TIS) and to examine reliability and responsiveness of the K-TIS in patients with stroke. Methods Subjects of the study were 51 stroke patients (mean age: 57.78 years) recruited from two stroke clinics. For the interrater and test-retest reliability, two raters measured the K-TIS two times using video clips with an interval of 2 weeks. For the responsiveness, intensive physical therapy training was provided to all participants 2 times a day for one month or three months depending on the onset of the stroke and the admission rules of the two clinics. Inter-rater reliability and test-retest reliability of the K-TIS three subscales (static sitting balance, dynamic sitting balance, and coordination) scores and total scores were examined using intra-correlation coefficient ($ICC_{3,1}$) and Pearson's correlation coefficient (r). To examine responsiveness, the minimally important difference (MID) was calculated with effect size. Results: Inter-rater reliability of the K-TIS subscales and total scores were all high (ICC3,1=0.920-0.983 and r=0.924-0.984). For the test-retest reliability, $ICC_{3,1}$=0.805-0.901 and r=0.806-0.903, and the MID for acute and post-acute as well as chronic stroke patients remained in the mean change range. Conclusion: It is suggested that the K-TIS might be used for clinical and research purposes as a standardized tool for stroke patients. In addition, it can also be useful in establishment of treatment goal(s) and planning treatment program(s) for patients with stroke.
