The Relationships Between Patient's Perceived Quality of Healthcare Service and Intention to Re-visit (대상자가 인지한 의료서비스 질과 재이용 의도와의 관계)

  • Purpose: This study was performed to identify the relationship between patient's perceived quality of healthcare services and intention to re-visit. Method: The study subjects were 242 patients from three local clinics. The data were analyzed using SPSS PC+ 12.0 program for descriptive, Pearson's correlation coefficient, and multiple regression analysis. Result: Patients' perceived quality of healthcare services was measured 5.73$\pm$0.71 using the 7-point scale. The patients' perceived quality was moderately correlated with patient satisfaction (r=0.647, p<0.001) and intention to re-visit (r=0.604, p<0.001). Also, patient satisfaction was highly correlated with intention to re-visit (r=0.788, p<0.001). 3) the reliability factor of patients' perceived quality of healthcare services and patient satisfaction were found significant predictors of intention to re-visit. Conclusion: In conclusion, service quality evaluated by patients influenced on their intention of re-visit and especially, reliability was the most significantly influential variable.

Analysis of Healthcare Quality Indicators using Data Mining and Development of a Decision Support System (데이터마이닝을 이용한 의료의 질 측정지표 분석 및 의사결정지원시스템 개발)

  • Background : This study presented an analysis of healthcare quality indicators using data mining and a development of decision support system for quality improvement. Method : Specifically, important factors influencing the key quality indicators were identified using a decision tree method for data mining based on 8,405 patients who discharged from a medical center during the period between December 1, 2000 and January 31, 2001. In addition, a decision support system was developed to analyze and monitor trends of these quality indicators using a Visual Basic 6.0. Guidelines and tutorial for quality improvement activities were also included in the system. Result : Among 12 selected quality indicators, decision tree analysis was performed for 3 indicators ; unscheduled readmission due to the same or related condition, unscheduled return to intensive care unit, and inpatient mortality which have a volume bigger than 100 cases during the period. The optimum range of target group in healthcare quality indicators were identified from the gain chart. Important influencing factors for these 3 indicators were: diagnosis, attribute of the disease, and age of the patient in unscheduled returns to ICU group ; and length of stay, diagnosis, and belonging department in inpatient mortality group. Conclusion : We developed a decision support system through analysis of healthcare quality indicators and data mining technique which can be effectively implemented for utilization review and quality management in a healthcare organization. In the future, further number of quality indicators should be developed to effectively support a hospital-wide Continuous Quality Improvement activity. Through these endevours, a decision support system can be developed and the newly developed decision support system should be well integrated with the hospital Order Communication System to support concurrent review, utilization review, quality and risk management.

The Customer Satisfaction Index Model: An Empirical Study of the Private Healthcare Sector in Malaysia

  • The main purpose of this study was to gauge the patient satisfaction index and subsequently discuss the Importance-Performance (IP) matrix analysis of the inpatient services in the context of the private hospital setting. The Malaysian Customer Satisfaction Index Model was employed as the theoretical framework for the above purposes. This study involving 242 patients in Malaysian's private healthcare sector used a Web-based survey as the main method of data collection. Partial least square structural equation modeling (PLS-SEM) was utilized for data analysis. Using Fornell et al. (1996)'s formula, the resulting patient satisfaction index was slightly lower than the "very satisfied" category, the target level required for positioning as one of the world's premier medical tourism players. The IP matrix showed that medical quality is the main competitive advantage of the private hospitals that can propel their growth in the global healthcare marketplace. The results also indicate that outcome quality, patient rights, and privacy, and service quality are the three quality domains that need to be prioritized for further improvement. On the other hand, the servicescape quality domain needs to be strategized as the unique selling proposition as the performance of the private hospitals in this regard is already extremely good.

  • Recently, the smart healthcare industry that grafted the Internet of Things (IoT) onto the healthcare and medical services is being highlighted. Smart healthcare refers to the healthcare and medical services offered on the basis of information communication technologies including application, wearable devices and platforms, etc. The number of next generation smart healthcare devices are increasing in the smart healthcare industry through the combination of information technology (IT) and Bio Technology (BT), which are the most active areas among the 6T, which are the areas of the next generation industry. With the emergence of a diverse range of smart healthcare systems or devices, whether the smart healthcare system can be linked with other systems organically and the security and quality of the system have become important elements of evaluation. In this Study, the characteristics and service types of smart healthcare systems were examined, and the trends in the technology and industry of the smart healthcare system were analyzed. Moreover, this Study aims to develop the evaluation method for security and standards for quality evaluation by deducing the factors for the evaluation of smart healthcare system on the basis of the results of aforementioned examination. It is anticipated that this can induce improvement of quality and development of highly reliable products of a smart healthcare system, and will become the core technology to substitute the technology protection barrier.

  • Purpose - The purpose of this study is to shed light on the healthcare management systems in correlation with the advancing technological world, as well as the many diversified systems in different Asian countries. Research design, data, and methodology - This is a comprehensive study on the past researches that have been conducted on the field of healthcare management in different Asian countries. The different types of healthcare coverages were examined by country. Additionally, incorporating the healthcare system with technology is also investigated in this research. Results - The results have shown that there is a rapid incorporation of the technology in today's society with the healthcare systems. This has promising effects for the future, and for the different healthcare systems that exist within other Asian countries around the world. Conclusion - Quality healthcare for people are still elusive, and the healthcare management should improve on ways to better take care of their patients for the near future. By shedding light on the healthcare systems of Asian countries, an idea of the similarities and differences each hold can be assessed and utilized for future goals. By incorporating technology into healthcare systems, this will help in improving the overall care and quality-of-life that patients receive for the future.

Effect of Healthcare Quality on Recommended Intention in Vietnam A Hospital : Focusing on Customer Satisfaction Mediated Effects (베트남 A 병원의 의료서비스 품질이 추천의도에 미치는 영향 : 고객 만족도 매개효과를 중심으로)

  • This study aims to analyze the quality and satisfaction of healthcare perceived by patients using hospitals and to provide basic data necessary for expanding and settling Vietnamese healthcare services by analyzing the impact on recommendation intentions. The research method selected one hospital in Vietnam and collected data from patients using the hospital and used a total of 286 effective samples as data for hypothesis verification. The research model and hypothesis verification were analyzed with the statistical data from SPSS and AMOS. The findings show that, first, tangibility, accessibility, and reliability all have a positive effect on the quality of healthcare. Second, it has been shown that only accessibility among the quality of healthcare has a positive effect on recommendation intentions. Third, customer satisfaction has been shown to have a positive effect on recommendation intentions. Fourth, when looking at the mediating effect, reliability among the quality of healthcare was shown as a full-mediated effect, but accessibility was shown to have a partial mediating effect and tangibility to have no mediating effect. Contact management is important because customer satisfaction is highly regarded when customers feel positive emotions at the interface from the provision of convenience facilities that support medical services to the reduction of waiting time for patients, employees kindness, treatment, medication, and inspection. It is also confirmed that the demand for convenient and rapid use of hospitals is increasing in Vietnam. In addition, if customer satisfaction is increased through friendly medical staff's response, the intention of recommendation will be even greater.

Effect of Unmet Healthcare Needs on Quality of Life (미충족 의료 경험이 삶의 질에 미치는 영향)

  • The purpose of this study was to investigate the potential causal relationship between unmet healthcare needs and metrics related to quality of life. To examine the relationships between these variables, the study conducted an analysis by processing additional irradiated raw materials of the Korean Medical Panel. The metrics related to quality of life included six variables: athletic ability, self-care, daily activities, pain/trouble, uneasiness/depression, and subjective health conditions. Linear regression analysis revealed that the unmet healthcare needs had statistically significant negative effects on all six quality of life variables. A review of the magnitude of the linear regression β values for those variables showed that the relative level of influence on the quality of life variables decreased in the following order: pain/trouble, subjective health conditions, uneasiness/depression, daily activities, athletic ability, and self-care. Based on the results, practical applications related to strengthening working-level links between health and welfare is considered an effective policy response to the continued presence of unmet healthcare needs; such applications could contribute to improving the quality of life of those with unmet healthcare needs.

The Effect of Occupational Stress among Occupational Groups Related to Healthcare Accreditation on Turnover Intention (의료기관인증평가 관련 직무 스트레스가 직군간 이직의도에 미치는 영향)

  • Purpose: This study aims to investigate the factors influencing health personnel's occupational stress on turnover intention regarding healthcare accreditation. Methods: A survey was conducted from May 17 to May 31, 2021, among participants to examine occupational stress and turnover intention among health personnel working at a 188-bed specialized hospital in Seoul that is preparing for healthcare accreditation. Results: Occupational stress regarding healthcare accreditation was found to have a positive correlation with turnover intention (r=.698, p <.001), and influenced turnover intention the most, which explains the variance of 55.8% (F=29.015, p <.001). There were significant differences between occupational groups in job stress (F=13.292, p<.001) and turnover intention (F=10.930, p <.001) in the healthcare accreditation. Conclusion: Occupational stress regarding healthcare accreditation is higher in nursing than in other occupations, indicating the need to lower the turnover intention of nurses by preparing a national institutional standard for nursing manpower and also put in place an appropriate compensation system for each hospital seeking accreditation.

  • Purpose:This study aims to suggest the future direction for applying service design to improve the quality of healthcare as part of hospital service innovation and present implementation plans in Korea, based on a review of quality improvement activities and the current status of service design applications. Methods: Through a literature review, we examined the status of service design introduction and application in the healthcare field, focusing on cases in the US and Europe. The possibility and limitations of service design in the healthcare field were examined through a comparison of oversea and domestic cases. Results: Recently, service design has begun to be applied to the healthcare field worldwide. Service design shows the possibility of an alternative that alleviates and complements the limitations of existing quality improvement activities. It also offers the possibility of creating new organizational improvement and innovation approaches through integration and convergence with existing quality improvement activities and management innovation. Conclusion: To effectively apply service design to hospitals, it is necessary to integrate internal organizations related to service improvement, combine methods, and objectively measure and evaluate performance. To this end, we propose the operation of a nationwide education and training center for quality improvement and service design led by academic society. Service design will provide an opportunity to change the management innovation and organizational culture of hospitals beyond the scope of the current quality improvement, which deals only with micro-subjects of individual hospitals.

  • Purpose: The purpose of this study is to lay groundwork for future research on the outcomes of national quality awards for secondary-care healthcare organizations. Methods: For this study, a secondary-care hospital was selected through a convenience sampling method and all of its organizational members participated in complete enumeration survey using 109 survey questions derived from the MBNQA criteria for healthcare through structural equation modeling (SEM) Results: As a result, Leadership was shown to drive Foundation and Direction, which affect System that creates Results with 12 hypotheses supported out of 18 hypotheses established. Conclusion: The findings of this study will provide valuable implications to the top management of secondary-care hospitals for self-examining quality management and promoting sustainable competitiveness.
