Education, Role, and Prospects of Advanced Practice Nurses in Hospice and Palliative Care in South Korea

  • Hospice palliative care refers to holistic care provided by an interdisciplinary team aimed at improving the quality of life of patients suffering from life-threatening diseases and their families. Among interdisciplinary team members, hospice advanced practice nurses (APNs) trained as master's-level advanced nursing professionals are leaders who play an important role in providing patient-centered care and improving the quality of services. The Medical Service Act revised in 2018 requires the scope of practice of APNs in each field to be specified in the Ordinance of the Ministry of Health and Welfare. Accordingly, discussions on the role and scope of practice of hospice APNs are actively underway. In this review, the curriculum of hospice APNs, their work responsibilities and roles, and their current status are reviewed, and the future direction of the hospice APN system is also discussed.

A Study on Home Care Service and the Level of Client Satisfaction in Community (일 지역사회의 가정간호서비스 내용 및 만족도 조사)

  • Purpose : This study was done to examine the home care service provided by home care nurses and the level of client satisfaction in community. Method : Data were collected from 120 clients who received the home care service at the community home care service center in Pusan from 2th May. 2003 to 31th May 2003. The tool for measurement of satisfaction was composed of 16 items and was 4 score scale by Kim. Data were analyzed by using SPSS/WIN 10.5 program. Result: 1) 67.5% among 120 clients was over sixty years old and 30.6% of clients received home visits 20 times by home care nurse. 2) The total number of home care services was 15,783. And most of the clients received therapeutic nursing care. 3) The mean score of satisfaction on provided home care services was 3.31, among 16 items, "The home care nurses kept up the secret of patients and family" was the highest($3.67{\pm}0.48$). the total level of satisfaction of home care services was very high, with mean score $3.31{\pm}0.32$. Conclusion : The satisfaction level of home care services was very high. so we can expect that the prospect of home care services is very challengeable. Therefore we should try to expand the service recipients with promotional education to assure close ties with the public health centers for the continuous home care service linking and make more efforts to improve the quality of the service.

The Influence of Attitude, Experience and Empathy on the Nursing Care of the Elderly Who Have No Caregiver (돌봄 제공자가 없는 노인에 대한 태도, 경험, 공감이 간호실천에 미치는 영향)

  • Objectives : The purpose of this study was to identify the factors influencing nursing care behavior for hospitalized elderly patients who have no family caregiver. Methods : Participants were 170 nurses working in general hospitals in G city. Data were collected from November to December, 2015 and analyzed by exploratory factor analysis, t-test, ANOVA, Pearson's correlation coefficient, and multiple regression with the SPSS/WIN 20.0 program. Results : There were significant correlations among experience, empathy, and nursing care behavior. Experience and empathy were significant factors influencing nursing care behavior and explained 29.4% of the variance in the score; in particular, empathy had a positive effect on nursing care behavior. Conclusions : The results show that theempathy of nurses' is the most important factor in caring for elderly patients who have no family caregiver. Nurse education programs should be developed that increase the empathy of nurses to improve the quality of nursing care for the elderly.

Effect of Kegel Exercise on Urinary Incontinence after Radical Prostatectomy: Systematic Review of Randomized Controlled Trials (근치적 전립선적출술 후 요실금에 대한 케겔운동의 효과: 무작위 대조군 실험연구의 체계적 문헌고찰)

  • Purpose: This study is reviewed of the available literature to identify the evidence of the value of Kegel exercise programs as an intervention to decrease urinary incontinence and the improvement of the quality of life following a radical prostatectomy in localized prostate cancer. Methods: We searched studies of randomized controlled trials that utilized the Kegel exercise programs with patient with a radical prostatectomy. The review was conducted electronic bibliographic database of Ovid-Medline, Embase, Scopus, KoreaMed and NDSL, etc. Of 630 publications identified, seven studies that met the inclusion criteria, and all studies analyzed by meta-analysis. To ensure the quality of the studies, we used Cochrane's Risk of Bias. Results: Kegel exercise helped patient to achieve continence more quickly (after 1, 3, 6, 12 months) than men not using Kegel exercises. Especially, Kegel exercise significantly reduced the development of urinary incontinence at one month after prostatectomy. The effectiveness of Kegel exercise after prostatectomy was found to improve the quality of life at a significant level. Conclusion: Based on available evidence, Kegel exercise that nurses can teach improved the return to continence more than usual care in men with prostatectomy urinary incontinence.

A Study of the Effect of Medical Service Quality on Patient Satisfaction with a Focus on the Telemedicine Service of Colonoscopies

  • Recently many small and mid-sized hospitals are closing down or experiencing financial difficulties. As a result, in order to provide exceptional service that would increase their competitiveness, they implemented telemedicine service for the prescription of medicine necessary before a colonoscopy. This study is an analysis of how telemedicine service affects patient satisfaction. Through the focus group interview (FGI) of medical service providers, regarding telemedicine service and face-to-face service, it was appraised that telemedicine service was more effective than face-to-face service. Afterward, a second survey aimed at endoscopy patients was conducted in order to find out the value of telemedicine service. First, through the analysis of the three main factors of telemedicine service, conclusions were drawn(safety/reliability, convenience/rapidity, and economics). A follow-up analysis showed that convenience/rapidity had the greatest effect on telemedicine service satisfaction. Next the factors of the quality of medical service were analyzed and 3 main factors were deduced (the superiority of the external and internal environments/appropriateness, superiority of the medical team, telemedicine service). A follow-up analysis found that telemedicine service had the greatest effect on patient satisfaction. This study found that providing exceptional medical service that utilizes telemedicine service would improve patient satisfaction. Therefore, we would be able to form a strategic plan that would strengthen the competitiveness of small and mid-sized hospitals.

Effects of anxiety and smartphone dependency on sleep quality among pregnant women with preterm labor (조기진통 임부의 불안과 스마트폰 의존도가 수면의 질에 미치는 영향)

  • Purpose: The purpose of this study was to investigate the effects of anxiety and smartphone dependency on sleep quality in pregnant women with preterm labor. Methods: The participants of this study were 111 pregnant women who were between 20 and 37 weeks of gestation and experienced preterm labor. The data were collected from October 1, 2018 to October 25, 2019. The collected data were analyzed using descriptive statistics (frequency, percentage, and standard deviation), as well as the t-test, Pearson correlation coefficients, and hierarchical multiple regression. Results: Significant negative correlations were found between anxiety and sleep quality and between smartphone dependency and sleep quality. Participants' history of preterm birth, pregnancy method, bowel movements, anxiety, and smartphone dependency significantly affected sleep quality, with an explanatory power of 18%. Conclusion: In order to improve the quality of sleep, which is an important health-related factor for pregnant women experiencing preterm labor, it will be necessary to identify a history of premature birth, pregnancies achieved using artificial reproductive technology, bowel problems, and smartphone dependency in advance and to provide nursing interventions accordingly.

  • The purpose of this study was to identify nurses' communication style and the extent of their health literacy and to discern the impact of nurses' communication and health literacy on self-care behaviors, as perceived by elderly diabetic patients in comprehensive nursing-care service wards. This study provides basic data for establishing an efficient nursing-care service-system environment through high-quality self-management. The measurement and improvement of an accurate health literacy level of elderly diabetic patients is an important intervention method to increase the self-care behavior of patients. Therefore, by developing a distinguished nursing intervention plan, early treatment of diabetic patients and quality of care may be possible.

Patient's Satisfaction with Nursing Care Services in Military Hospital Emergency Department (군병원 응급실 간호서비스에 대한 환자만족도)

  • Purpose: The purpose of this study is to provide basic data for the improvement of the quality in the emergency nursing services by investigating the expectation level and actual received level with nursing care and identifying the factors that influence patient satisfaction with the emergency nursing care in the military hospitals. Method: Using a self report questionnaire, data were collected from 150 patients who had been admitted in military hospitals emergency department. The data collection was done between April 17 and April 30, 2006. At least, 145 questionnaires were analyzed and used in this study. Results: The average sum score was 61.93 out of 76 with standard deviation 9.590. The mean of the patient's expectation levels with the overall emergency nursing services was $(3.25{\pm}.504)$ out of the highest score 4. the average sum score was 63.27 out of 76 with standard deviation 9.902. The mean score of the patient's actual levels with the overall emergency nursing services was $(3.33{\pm}.521)$ out of the highest score 4. Conclusion: nursing management strategies must be established to remove patient's dissatisfaction factors and to improve the emergency nursing care quality through continuous measurement and assessment of the patient's satisfaction with emergency nursing services in military hospitals.

Determinants affecting patients loyalty in medical service: An empirical study on comparison with in-patients and out-patients using multi-group structural equation model (대학병원환자의 의료서비스 충성도에 영향을 미치는 요인에 관한 연구: 멀티그룹 공분산구조분석을 이용한 입원환자와 외래환자의 인식차이 비교분석)

  • The purpose of this research is to identify causalities among service quality, service value, customer satisfaction and customer loyalty and to compare the causalities between in-patients and out-patients. A multi-group structural equation model was used to test the hypothesis of conceptual model. Empirical results indicate that two factors such as assurance and empathy have direct impact on service value and customer satisfaction in both the patients. In the results of multi-group comparison test, however, the path coefficients from service value to customer loyalty and from customer satisfaction to customer loyalty were different across the two groups. In out-patients, customer loyalty is more positively related with service value. In in-patients, customer loyalty is more positively related with customer satisfaction. It is proven that service quality influences customer loyalty. Therefore To compete successfully in today's volatile & competitive medical markets, the hospital needs to manage the service quality, considering the type of patients.
