The Changes in the Public Health Laws and in the Legal Policies of the National Health Insurance over the Past Decade (최근 10년 보건의료법 환경 및 건강보험법정책의 변화)

  • Korea has gained the much more performances in the fields of pubic health laws and related policies on the basis of the substantial economic achievements. In 1977, the social medical insurance was established for companies with more than 500 employees, and in 1989, Korea successfully achieved the national medical insurance system covering the total population within only 12 years beginning with multiple insurers. There remained some problems, however, to be improved such as both the low level of contribution rates and benefit packages due to the inefficiency in utilizing limited medical resources. In 2000, all insurers were unified into a single insurer (National Health Insurance Corporation), and special independent Health Insurance Review & Assessment Service (HIRA) was also established. From the origin of medical insurance system in 1977, the Korean reimbursement system has been fee-for-service system, and after the establishment of HIRA, it has been providing objective and expert medical cost review services and health quality assessment services.

  • With the development of Internet of Things (IoT) technology, the combination of ICT and medical services has been increasing to improve the quality of medical services. Using the IoTs, we can collect personal health information continuously in a patient's everyday life. We expect that this will improve the quality of medical service through analysis. However, the problem of ensuring the protection of personal information within the personal health information has been hampering the research, development, and application of such services. Other problems include lack of IoT devices and lack of user convenience for collecting health information about a patient's everyday life. Therefore, in this study, we construct a daily health information management service that can collect the health related information at any time and store this data in personal storage. This data is then only provided to the healthcare worker when necessary. We built a test bed for an IoT-based smart home platform and are currently conducting user experiments. Based on the results of this study, we are attempting to provide a high quality medical trial service based on daily health information through linkage with medical device manufacturers, medical clinics, insurance companies, etc. We expect the proposed health information management service will contribute to the revitalization of smart health care services via activating various health related IoT devices and analyzing daily health information.

  • This paper explored that medial consumers' trusts in both hospitals and doctors are developed by and during investigating hospital websites and that these trusts directly build the consumers' purchase intentions of the hospitals' offline medical services. 208 questionnaires based on the websites of two virtual hospitals, captured on survey brochures, were collected and analyzed. This research indicated information quality of hospital websites affected more on developing consumers' trusts than service quality.

Effects of the Health Status and Health Behavior on Health-related Quality of Life of the Elderly Living Alone and Living with Their Families: Using Data from the 2014 Community Health Survey (독거노인과 가족동거노인의 건강상태와 건강행위 경험이 건강 관련 삶의 질에 미치는 영향: 2014 지역사회 건강조사 자료 활용)

  • Purpose: The purpose of this study is to identify factors influencing the quality of life of the elderly living alone and living with their families with regard to their health statuses and health behavior experiences. Methods: We used source data from the 2014 Community Health Survey. The subjects of this study included some elderly people aged 65 and over, and analyzed the data of 13,373 elders living alone and 13,322 elders living with family. Results: Factors influencing the quality of life of the elderly living alone and living with their families include gender, age, education, household income, current occupation, subjective stress level, depression, number of diagnosed diseases, walking exercise, the experience of health screening, and the experience of not having necessary medical services (p<.001). Region was a significant variable influencing the quality of life of the elderly living with their families (p<.001). Conclusion: In order to improve the quality of life of the elderly, it is necessary to provide sound conditions for working, emotional support, walking exercise and promotion of health screening, and to supplement the environment and institution for them to receive necessary medical services.

Establishing Weights among Indicators of Rural Residents' Quality of Life Using AHP (AHP를 이용한 농촌주민 삶의 질 지표의 중요도 설정)

  • This study aims to establish the indicator system of the quality of life of rural residents and to estimate the weights among criteria or sub-criteria that constitute the indicator system of the quality of life using AHP(Analytic Hierarchy Process) method. Indicator system consists of four criteria, that is, health care, eduction & culture, living environment, and income and jobs. Each criterion include three sub-criteria that make up corresponding criterion. The results show that income and jobs was most highly evaluated among the four criteria. It was followed by health care, living environment, and eduction & culture. Among the total 12 sub-criteria, annual income was most highly evaluated, followed by medical service level, job satisfaction, and public transportation environment in terms of global weight which was deduced from expert questionnaire survey. This study implies that rural development policy needs to focus on increasing rural residents' income by creating good quality occupations and increasing the level of medical services, and expanding public transportation services in rural areas. This study has failed to collect local residents' opinions regarding their quality of life in spite of the efforts to reflect the opinions from various expert groups, which is the limitation of this study.

The Effects of Depression, Anxiety and Stress on Sleep Quality in College Students (보건계열 대학생의 우울, 불안, 스트레스가 수면의 질에 미치는 영향)

  • Purpose: The purpose of this study was to determine the level of depression, anxiety, and stress among health college students and to identify factors affecting sleep quality to provide a basis for improving sleep quality among health college students. Methods: Data was collected using a structured questionnaire from 181 college students at a university in City D from October 1 to October 31, 2023. Results: Sleep quality was significantly and positively correlated with depression (r=.703, p<.001), anxiety (r=.680, p<.001), and stress (r=.708, p<.001), and its predictors were caffeine consumption (β=-.174, p=.044), Satisfaction with college life (β=-.137, p=.009), depression (β=.251, p=.013). Stress (β=.277, p=.007), with an explanatory power of 56%. Conclusion: To improve the sleep quality of college students, we should encourage them to reduce their caffeine intake, run various programs to improve their satisfaction with college life, and expand mental health counseling programs on campus to monitor their mental health status regularly so that they can actively manage depression and stress.

  • The purpose of this study is to explain intention to use untact medical diagnosis and consultation services. There is a need for untact medical diagnosis and consultation services in order to increase social interests and a global trend for public protection and convenience enhancement. We carry out the analysis of the survey data using Smart PLS 3.0 to test the hypotheses. According to the empirical analysis results, this study confirms that health consciousness have significant effects on the cost saving, quality of telemedicine service and self-efficacy have significant effects on the accessibility, compatibility. cost saving, quality of telemedicine service, accessibility and compatibility have significant effects on Perceived usefulness. Perceived usefulness have significant effects on use Intention of untact medical diagnosis and consultation services. This study has its meaning because it found out that it deals structurally and expansively with use intention through Expected Benefit and usefulness based on individual characteristics for untact medical diagnosis and consultation services. In addition, The results of this study suggest that if the related policies are institutionalized based on the Expected Benefits covered in this study, the perception of untact medical diagnosis and consultation services can be changed progressively.

Literature review on the domestic studies of the Outcomes of Home Care Service for 1981-1996 (국내 가정간호 사업의 성과 측정 연구의 고찰)

  • The purpose of this study is to identify the domestic research results related to the outcomes of home care services for 1981-1996. 12 studies were analized by the characteristics of the subjects, home care services provided, and outcome variables and tools during the period of Apirl-June, 1997. The results were as follows: the subjects of 8 studies conducted before 1993 were discharged patients comparing to early discharged patients of 2 studies conducted after 1994. The main home care service delivered to the clients and their families were education before 1993. The outcome variables were used the level of sat. isfaction of home care services, quality of care, cost-effectveness, health status, quality of life, and self -care ability. A variable, health status, were mainly measured by the global outcome measures such as quality of life, health hehelief, health perception, activity of daily living, health management pattern but also foused outcome measures that could be specified by the medical diagnoses.

Key Elements for the Improvement of Healthcare Services for Foreigners in Korea: A Delphi Study (외국인 의료서비스 향상을 위한 주요 요소: 델파이 연구)

  • Purpose: This study was performed to identify the key elements for the improvement of healthcare services for foreigners in Korea. Method: Delphi technique was used for this study. As the members of an expert panel for this study, 32 healthcare professionals, who were physicians, nurses, administrators, and care coordinators, with at least 6 months of experiences in international clinics and healthcare services in five metropolitan areas in Korea participated. Data collections were conducted three times from August to October, 2009. The priority and the importance were analyzed using descriptive statistics in SPSS Win 15.0. Result: The key element selected most frequently by the experts was the 'Healthcare providers' abilities for foreign languages' followed by 'Guidelines for facing medical accidences and disputes', 'Information and guide for healthcare services written in English', 'Informed consent preventing medical disputes', 'System of healthcare service fees for foreigners'. Conclusions: The key elements for the improvement of healthcare services for foreigners in Korea were mostly the requirements for effective communication with the foreign clients and the systemic support. The key elements identified in this study can be applied usefully for the development of strategies to improve the quality of healthcare services for foreigners.
