Evaluation of Applications of Adaptation of the Evidence-Based Nursing Practice Guidelines Patients with Acute Stroke (급성 뇌졸중 환자 대상 근거중심 간호 가이드라인 수용개작의 적용 평가)

  • Purpose: This study was done to evaluate nursing guidelines for patients with acute stroke, developed by adapting the guidelines of Registered Nurses Association of Ontario, Canada to clinical settings on a large scale and evaluating the effectiveness as a research study. Method: The general characteristics of the 319 patients and the effectiveness of guideline application were evaluated in terms of structure, process, and outcome using questionnaires on the guidelines application with reference to the medical records of patients with acute stroke hospitalized on a ward of the stroke center of S General Hospital in Seoul. Results: Structures as a guidance system for assessment were consistent with the recommendations. With respect to the process of the guidelines, for items on nursing assessment, improved performance was found to be statistically significant. For outcomes of the guidelines, complications occurred in 8 patients (5.3%) prior to application of the guidelines and 11 patients (6.5%) after application of the guidelines, but this result was not statistically significant (p=.841). Conclusion: The results of the study indicate that for the effectiveness of the guidelines, accessibility to the guidelines and effectiveness of quality improvement need to be evaluated, in addition to complications of a stroke.

Spatial-temporal distribution of carabid beetles in wetlands

  • In this study, we investigated carabid beetles residing in the wetlands to understand their ecological adaptation and strategy selection associated with restricted resources and habitat limitation. The species richness, abundance, seasonal activity, and spatial distribution of the carabid beetles between the Mujechi Wetlands (wetland sites) and Mt. Jeongjok (mountain sites) have been compared. A total of 1,733 individual beetles from 30 species were collected and classified at the studied sites. The wetland sites were identified as having lower species richness and abundance for carabid beetles when compared with the adjacent mountain sites, whereas these beetles were observed to be dominant in the wetland sites than in the adjacent mountain sites. Calosoma inquisitor cyanescens, Carabus sternbergi sternbergi, and Carabus jankowskii jankowskii species were dominant in both the wetland and mountain sites. These species showed significantly different seasonal activity patterns in the wetland sites relative to the mountain sites. Although the three listed carabid species were observed to be widely distributed throughout the wetland sites, they still showed preference for drier sites, which clearly shows a distinction in their habitats. The results of the spatial-temporal distribution of carabid beetles in the wetland sites reflect their special strategies regarding space and time partitioning for maintaining their population. The distribution patterns of carabid beetles in the wetland sites also showed the desiccation gradient and environmental changes prevalent in wetlands. Ecological surveys, which use carabid beetles in the wetlands, can then be performed when restoring wetlands and for establishing management practices for improving the habitat quality.

Effects of dietary fermented spent coffee ground on nutrient digestibility and nitrogen utilization in sheep

  • Objective: The objective of the study was to determine the effect of fermented spent coffee ground (FSCG) on nutrient digestibility and nitrogen utilization in sheep. Methods: Fermentation of spent coffee ground (SCG) was conducted using Lactobacillus plantrum. Fermentation was performed at moisture content of 70% and temperature of $39^{\circ}C$ with anaerobic air tension for 48 h. Four adult rams (initial body weight = $56.8{\pm}0.4kg$) were housed in a respiration-metabolism chamber and the treatments were: i) control (Basal diet; 0% SCG or FSCG), ii) 10% level of SCG, iii) 10% level of FSCG, and iv) 20% level of FSCG in $4{\times}4$ Latin square design. Each dietary experiment period lasted for 18-d with a 14-d of adaptation period and a 4-d of sample collection period. Results: In SCG fermentation experimental result, acid detergent insoluble nitrogen (ADIN) concentration of FSCG (64.5% of total N) was lower than that of non-fermented SCG (78.8% of total N). Digestibility of dry matter and organic matter was similar among treatment groups. Although crude protein (CP) digestibility of the control was greater than FSCG groups (p<0.05), the 10% FSCG group showed greater CP digestibility and nitrogen retention than non-fermented 10% SCG group (p<0.05). Body weight gain and average daily gain were linearly decreased with increasing FSCG feeding level (p<0.05). When the feeding level of FSCG was increased, water intake was linearly increased (p<0.05). With an increasing FSCG level, dry matter intake did not differ among groups, although the gain to feed ratio tended to decrease with increasing level of FSCG (p<0.10). Conclusion: Microbial fermentation of SCG can improve protein digestibility, thereby increasing CP digestibility and nitrogen utilization in sheep. Fermentation using microorganisms in feed ingredients with low digestibility could have a positive effect on improving the quality of raw feed.

Development of Nursing Practice Guideline on Intermittent Urinary Catheterization by Using Remake Process (수용개작방법을 활용한 간헐도뇨 간호실무지침 개발)

  • Purpose: The purpose of this study was aimed to develop evidence-based nursing practice guideline for preventing intermittent urinary catheterization (IUC) related complications using guideline remake process. Methods: Guideline remake process was conducted according to guideline adaptation manual developed by Gu et al (2012) which consisted of three main phases and 9 modules including a total of 24 steps. Results: Newly developed IUC guideline consists of introduction, overview of intermittent catheterization, summary of recommendations, recommendations, references, and appendices. There were 50 recommendations in 5 sections including assessment, equipments, catheterization, complications management, and education/consult. Three recommendations (6%) were graded A, and five (10%) and 41 recommendations(82%) were B and C, respectively. Conclusion: The IUC remade-guideline was developed based on evidence-based nursing and therefore, this guideline is recommended to be disseminated and utilized by nurses nationwide to improve the quality of care for IUC and to decrease the IUC related complications.

Distribution of Epilithic Diatom Assemblages in an Urban Stream in Busan: Effected of Urban Climatic Conditions (도시 기후 변화가 도시하천의 부착규조류 군집에 미치는 영향)

  • This study illustrates changes in the epilithic diatom assemblages in response to urban climatic conditions. We further assess the impact of abnormal urban climate to the urban stream environment. Epilithic diatoms, water chemical and physical variables were sampled every quarter, and assessed at 3 Oncheon stream sites, for a period of two years(from 2013~2014). The variation of physiochemical properties such as BOD, COD, T-N and T-P, show that the water quality was strongly influenced with long periods of drought and flood disturbance. Epilithic diatom assemblages were separated along the stream sites; however, the physical disturbance from urban drought and stormwater changed the composition of diatom assemblages instead of decreasing the taxonomic richness. Thus, our results suggest that epilithic diatom assemblages are altered in response to urban climatic changes, resulting in variations of stream conditions. Hence, strategies of climate change adaptation are required when considering urban stream environments.

  • The aging of population is a social phenomenon commonly observed in advanced countries. It causes various problems. Thus the aged problem must be approached from quality of life, which the elderly pursue instinctively regardless of their life cycle, and the utilization of their spare time or leisure hours. However, there have been research reports that, if the elderly participate in voluntary and active meaningful leisure, they can achieve successful adaptation to their aging and the life in their old age. In particular, we need to give our attention to the fact that, compared to other leisure activities, recreational sports make significant contributions to successful aging. When the aged participate in active leisure and recreational sports regularly, their satisfaction with spare time is enhanced and their loneliness is relieved, which resultantly brings high life satisfaction. This suggests that recreational sports activities enable the aged to achieve successful aging and contribute to the solution of the aged problem. In order to promote physical activity for aging population, we must utilize MINIMAX strategies.

  • Using solar power in wireless sensor networks (WSNs) requires adaptation to a highly varying energy supply and to a battery constraint. From an application's perspective, however, it is often preferred to operate at a constant quality level as opposed to changing application behavior frequently. Reconciling the varying supply with the fixed demand requires good tools for allocating energy such that average of energy supply is computed and demand is fixed accordingly. In this paper, we propose a probabilistic observation-based model for harvested solar energy. Based on this model, we develop a time-slot-based energy allocation scheme to use the periodically harvested solar energy optimally, while minimizing the variance in energy allocation. We also implement the testbed and demonstrate the efficiency of the approach by using it.

Spouse Support, Sexual Satisfaction and Marital Intimacy of Post-mastectomy Patients (유방절제술 환자의 배우자 지지, 성생활 만족도 및 부부 친밀도)

  • Data were collected using questionnare to 102 post-mastectomy patients visiting the out-patient department at Catholic University St. Mary Hospital and Kang Nam St. Mary Hospital from October, 1998 to January, 1999 in order to provide the basic data for development of nursing intervention which can help patients' emotional and sexual adaptation to treatment outcome as well as increase quality of life by studying spouse support, sexual satisfaction and marital intimacy of patients. Spouse support and sexual satisfaction were measured with the tool developed by Soon-bok Jang (1989). Marital intimacy was measured with the tool developed by Waring (1984) and improved by Sook-nam Kim (1998). Data were analyzed using t-test, ANOVA, multiple comparison by Duncan. Pearson correlation coefficients, and stepwise multiple regression. The results were as follows ; 1. Spouse support in the group of 6 month or less post-operative period was higher than that of 13 to 36 month period. 2. Sexual satisfaction in the group college graduation or above was higher than that of high school and middle school graduation. 3. Marital intimacy in the group of 61 month or above post-operative period was higher than that of 6 month or less and 13 to 36 month period. 4. The higher spouse support results in the higher sexual satisfaction and the higher marital intimacy according to a positive correlation. 5. The main effective variable on marital intimacy was spouse support. and the effectiveness showed $31.09\%$, while it was $43.06\%$ including sexual satisfaction and post-operative period.

The Experience of Nurses Who are working in the Government-Funded Subsidized Occupational Health Program for Small Scale Industries : Focus Group Interview (소규모사업장 보건기술지원사업에서의 간호활동경험 : 포커스그룹 인터뷰)

  • Purpose: This study was conducted to provide the fadamental information to prepare better Occupational Health Program for SSI based on the evaluation of the experience of nurse who were working in the Government-funded subsidized occupational health program for SSI. Method: The focus group method was used. Data was collected using focus group interviews and analyzed in the framework of the Grounded theory method as mapped by Strauss and Corbin(1990). The subjects were 14 nurses. Result: We identified 60 concepts, 30sub-categories and 6 categories(Table 1). Categories were Various services, Difficulties in implement Services, Successful strategies, Program Evaluation, Alternative Plans and Adaptation to new field. Conclusion: Nurses evaluated the effect of this program that autonomous health management ability has not changed although the possibility for the small scale industries to start the health management arose by acknowledging the importance of the health management of the workplace. In spite of many difficulties in implement services, nurses provided various health services using successful strategies. They suggest that various programs such as a model set-up suitable for the quality of workplaces and a role model set-up of the occupational health nurse for SSI as well as various efforts for the activated this program were needed. Nurses had a hard time in unfamiliar field when they began this program and overcame this difficulties with various strategies.

TCP-Friendly Rate Control for Vertical Handover over Wireless Heterogeneous Network (무선 이종망 환경에서 Vertical Handover를 위한 TCP-Friendly 비트율 제어)

  • Streaming video has become a popular form of transferring media over the wired/wireless network. TCP-friendly rate control (TFRC) is used as a streaming media transport protocol. Using the TCP congestion response function and current network conditions, TFRC adjusts its sending rate to yield the maximum TCP-friendly throughput. Since TFRC was designed for applications that would prefer to maintain a slowly-changing sending rate, it is less responsive to changes in handover between wireless heterogeneous networks such as 3G network, WLAN, and so on. This paper shows a new TFRC method for vertical handover over wireless heterogeneous network. The proposed TFRC method has features of low quality interruption and fast rate adaptation to a new target network. The simulation results show that the proposed one provides better QoS and throughput support than the traditional TFRC scheme during vertical handover.
