국내 의료기관의 질 향상 사업의 활성화에 영향을 미치는 요인 - 조직동인적 관점에서 (Factors Associated with the Degree of Quality Improvement Implementation)

  • Objectives : To assess the degree of quality improvement (QI) implementation and to identify its associated factors. Methods : A mailed questionnaire survey of the QI staffs at hospitals with 400 beds or more was conducted between September 15 and October 30, 2000. Of the 108 hospitals eligible for inclusion in our study, 79 participated, yielding a response rate of 73.1%. After excluding 12 hospitals that did not perform any QI activities, 117 responses from 67 hospitals were used in our analysis. The degree of QI implementation was measured using the Malcolm Baldrige National Quality Award Criteria (MBNQAC). Factors evaluated for association with the degree of QI implementation were cultural, technical, strategic, and structural factors of the hospitals. Results : The average 01 implementation score across the 7 dimensions of MBNQAC was 3.34 on a 5-point scale, with the highest score for the area of customer satisfaction (3.88) followed by information and analysis (3.59) and quality management (3.35). The results of regression analysis showed that hospitals with a ofter information system (p<0.05) and using scientific and systematic problem solving approach (p<0.01) tended to perform a higher degree of QI implementation. While statistically insignificant, positive associations were observed for the factors of group or developmental culture, the degree of employee empowerment, and the use of prospective strategy. Conclusions : It appears that the most important factors contributing to active implementation of QI in Korean hospitals were the use of scientific skills in decision making, and having a quality information system to produce precise and valid information.

기 손 반사요법이 입원환자의 자율신경계 활성도와 스트레스 저항도에 미치는 영향 (Effects of Qi Hand Reflexology on Autonomic Nervous System Activity and Physical Resistance to Stress in Inpatients)

  • Purpose: This study was done to identify the effects of Qi hand reflexology on autonomic nerve activity in patients admitted to hospital. Method: A non-equivalent control group and quasi-experimental research with pre/post test design were used and 59 patients on a medical ward were assigned to either the experimental group (32) or the control group (27). The study variables included vital signs, autonomic nerve activities (ALF (absolute low frequency), AHF (absolute high frequency), nLF (normalized low frequency), nHF (normalized high frequency), SDNN (standard deviation of all normal-normal intervals)) and were evaluated using the QECG-3 system. The experimental group was treated with Qi hand reflexology for 10 minutes per day, for 3days. Results: Skin temperature and systolic blood pressure in the experimental decreased with treatment. ALF, AHF and SDNN for the experimental group increased with regulating proportion (nLF: nHF=6:4) and a significant difference between the groups was observed. Conclusion: The results indicated that application of Qi hand reflexology for patients in the hospital is effective in regulating the autonomic nervous system activation. Implication of finding should be useful for future research.

한의학의 관점에서 살펴본 건강의 개념 (Concept of Health from the View of Korean Medical Science)

  • Medical science is an area of study that focuses on maintaining health and treating disease. However, modern medicine focuses on treating disease and neglects methodology of maintaining health. Although the definition of heal this defined by the World Health Organization (WHO) and as the concept of harmonizing Yin and Yang in Korean medicine, there is no specific details regarding the definition. Thus, the following conclusion was obtained after the concept of health is viewed through Yin-Yang(陰陽), Vital Essence(精), Qi(氣), and Healthy Person(平人). Although Yin and Yang have opposite properties, they work very closely, maintaining the overall balance and harmony. Secondly, Vin and Yang would each function by the effects of one another, and the body could maintain normal physiological activities through this function. Thirdly, it maintains normal physiological state through the ascending Kidney-Water and descending Heart-Yang(水升火降). Fourthly, Vital Essence is divided into the congenital Essence(先天之精) and the acquired Essence(後天之精) in which the former is related with reproduction and the latter, nourishing. Fifthly, Qi is resistant to pathogens and related with the physique so that Qi and physique should be balanced. Sixthly, Healthy Person is the state where the exchange of Qi-Blood is active.

Elevated plus-maze를 이용한 신기환, 보중익기탕 및 사물탕의 항불안 효과 (Anxiolytic Effects of the Three Kinds of Traditional Chinese Medicine, Shin-Ki-Hwan, Bo-Jung-lk-Ki-Tang, and Sa-Mul-Tang, Using the Elevated Plus-maze Test)

  • Shin-Ki-Hwan (Shen-Qi-Wan, SKH), Bo-Jung-Ik-Ki-Tang (Bu-Zhong-Yi-Qi-Tang, BJIKT), and Sa-Mul-Tang (Si-Wu-Tang, SMT) have been used for various kinds of deficiency syndromes, such as 'yang', 'qi', and 'blood', respectively. The objects of this study were to determine the effects of water extracts of three different kinds of traditional Chinese medicine (TCM), SKH, BJIKT, and SMT, on the anxiolytic activities in the elevated plus-maze test and to clarify the differences among 'yang', 'qi', and 'blood'. The water extracts of SKH, BJIKT, and SMT were orally administered to male SD rats, at 1.0 g/kg for 10 days. All rats were subjected to behavioral tests for the anxiolytic activity at 10 days. SKH, for the benefiting 'yang'agents, significantly increased the ratio of open arms entry to the total arms entry and time spent in the open arms (p<0.05), suggesting anxiolytic effect. However, both BJIKT and SMT decreased the ratio of open arms entry to the total arms entry and increased times spent in the closed arms (p<0.05). From these findings, it can be speculated that SKH only exhibits anxiolytic effect and that the different anxiolytic effects in the elevated plus-maze test may be come from the meanings of 'yang', 'qi', and 'blood'in oriental diagnostics though the cases are restricted.

일반촬영실의 환자대기건수에 대한 원인별 분석 - QI 활용을 중심으로 - (The Causes Analysis for the Number of Patients Waiting in General X-ray Room (Focus on Utilization of Quality Improvement (QI)))

  • 영상의학과에서 검사한 환자들의 불편사항을 사전설문조사를 통해 개선사항을 조사하고 팀원과 Brain storming을 실시하여 가장 많은 답변인 촬영대기시간단축을 주제로 선정하여 QI활동을 효율적으로 수행할 수 있는 방안을 제시하고자 한다. 2006년 9월부터 2007년 11월까지 세부원인 분류에 따라 각 항목별로 3개월간 QI 실행전과 후의 환자건수를 비교하였다. 환자건수는 환자 한 명 당 촬영한 검사 한 건으로 하였으며 QI실행 후는 QI실행 전 대기건수를 QI팀 회의를 걸쳐 1개월간 개선방안을 모색한 후 개선방안에 따라 시행 하면서 같은 방법으로 1개월간 측정하여 QI활동 전후의 차이를 각각 백분율로 비교하였다. 1. 재촬영원인에 대한 QI활동 전 후 환자촬영 대기건수 비교하였을 때 3.9%의 감소효과 2. 안내부족 원인에 대한 QI활동 전 후 환자촬영 대기건수 비교하였을 때 1.1%의 감소효과 3. 처방착오입력 원인에 대한 QI활동 전 후 환자촬영 대기건수 비교하였을 때 1.1%의 감소효과 4. 응급환자, 통증이 심한 환자, 몸이 불편한 환자 등의 원인에 대한 QI활동 전 후 환자촬영 대기건수 비교하였을 때 12.0%의 감소효과 5. 외래와 병동환자의 촬영중복시간 원인에 대한 QI활동 전 후 환자촬영 대기건수 비교하였을 때 4.7%의 감소효과 환자의 촬영대기건수를 단축하기 위해서는 많은 요인들이 존재한다. 이 요인들을 QI를 통해 영상의학과의 많은 문제점을 찾아내 수정하고 개선하는 것이 QI의 첫걸음이며 영상의학과 QI팀이 결성되어 QI활동을 실행하는 이유라고 할 수 있다.

QI전담자의 주요 업무 및 역할 규명 (The Task and Role of the Quality Improvement Facilitator)

  • Objectives: To outline overall duties of quality improvement (QI) performers within a health care organization, thus describing their key tasks, including task element-related frequency, importance and difficulty in enough detail. Methods: A DACUM (Developing A CurriculUM) workshop took place to outline overall job activities of QI performers. To examine the scope of their duty and task, we performed a questionnaire survey of 338 QI performers from 111 hospitals. Results: The results of our survey showed that for the task assigned to each QI performer, there were 10 duties, 31 tasks and 119 task elements. Respondents cited a project planning as the most frequent/important duty, and a research was the highest level of difficulty in their duty. They also said that the most frequent task was index management, the most important task was a business plan, and the highest level of difficulty was a practical application of QI research. QI performers added that the most frequent task element was receipt of patient safety reporting in patient safety system, the most important task element was an analysis for patient safety and its improvement, and the highest level of difficulty was a regional influence analysis related to the patient safety and its improvement. Conclusion: To ensure that QI performers play a pivotal role as a manager to better improve patient safety and the quality of health care services, proper training program for them should be developed by reflecting the results of our study.

보중익기탕가미방(補中益氣湯加味方) 투여를 통한 뇌교병변 현훈과 보행실조 환자 치험 2례 (Two Cases of Improved Dizziness and Ataxia due to Pontine Stroke in Bojungikki-tang-gamibang)

  • Bojungikki-tang-gamibang is a widely used herbal prescription in traditional medicine in Korea. The aim of this study is to investigate the effectiveness of Bojungikki-tang-gamibang for patients who have pontine stroke. They have problems with type of Qi deficiency(氣虛) in the rehabilitation stage. The type of Qi deficiency caused the dizziness and ataxia for the patients. In this study there are two types of patients. The first case is a patient with dizziness and ataxia because of potine hemorrhage. The second case is a patient who has the same symptoms as the first one but caused by pontine infraction. We treated the two patients with Bojungikki-tang-gamibang. The Improvement of symptom was evaluated by Korean vestibular disorders activities of daily living scale(K-VADL). As a result the degree of dizziness decreased noticeably while other symptoms improved as well. This study suggests that Bojungikki-tang-gamibang may be an effective treatment for patients who suffer stroke with type of Qi deficiency(氣虛) in the rehabilitations stage. However, more cases and methods of diagnosis are required to prove that the oriental medicine, Bojungikki-tang-gamibang can apply to patients universally.

  • The above study is based upon the Theory of Five Woon(Reasons) and Six Qi(Climates), namely Five dynamic Yin and Yang, the principle of five-six combination, the rules of ten periodically changing Woon and twelve periodically changing Qi, and its reference to the main chapters of the theory by several experts. After careful, comprehensive research and study, the followings are concluded. 1. The Theory of Five Woon and Six Qi controls all of the physical phenomena of the universe, yet it originates from Essence of the Sky that is replete and in constant motion within the abysmal expanse of the universe. So it is natural to claim that it is the Essence of the universe that is the source of the variety of phenomena in nature. 2. There is order of dynamics in the Essence and it is Yin Yang Five Movement and expressed by Five Woon and Six Qi. Therefore the Five Woon and Six Qi, which is the climatic changes of nature, is the basis and condition for all of the natural, physical occurrences including the organic phenomena of human beings. 3. Since the Sky embodied the number Five, and the Earth the number Six, Qi of the Sky itself is consequently Five Woon and Qi of the Earth itself is Six Qi. 4. In Sixty Repetition Intervals (Yuk Sip Kap Ja), the Sky took the number Six, designating the six times repetition of the ten Intervals of the Sky and the Earth chose the number Five, symbolizing five times recurrence of the twelve Intervals of the Earth. Though the number for the Sky is Five and the number for the Earth is Six, the two combine and interact in order for the Sky to exploit the number Six and for the Earth to use the number Five. This interplay implies the relationship of operation and principle of Yin in Yang and Yang in Yin. 5. There seems to be clear and intimate association between five dynamics, five bearings and five Qi' in nature and five organs, five stamina, and five emotions in physiological activities of human beings. Such an correlation apparently demonstrates the idea of climatic changes of the universe in balance among human beings, the Sky, and the Earth. 6. Because nature is of an endlessly dynamic organism itself, to have changes must mear there being movements. Thus the climatic changes in nature are the subsequent consummation of the interaction in mutual balance and unbalance of the rise and fall of the Five Woon and Six Qi. 7. The interpretation, by Chang Ji Chong of "Yin and Yang of the Sky are for birth and growth and Yin and Yang of the Earth for demise and interment" as an explanation that the first half is mainly for life and the second half for death, appears to be correct by his view that there is balance and harmony between the long and short and the strong and weak. 8. In addition to Three Yang and Three Yin, as attributes of the Intervals of the Sky and the twelve Intervals of the Earth, Five Woon and Six Qi are utilized in the annual observation of the perpetual changes of climates.
