A HISTOLOGICAL STUDY ON THE EFFECT OF SEVERAL HEMOSTATIC PULPOTOMY PROCEDURES (지혈적 치수 절단술의 효과에 관한 조직학적 연구)

  • The efficacy of several pulpotomy methods were evaluated histologically on animal model using 6 beagles. At 1, 4, 6 weeks after pulpotomy, animals were sacrificed by perfusion method, Histomorphometric analysis was performed using computerized image analyzing system. Statistical comparisons were done using SPSS program. The following results were obtained: 1. Tissue responses after ferric sulfate treatment mainly consisted of fibrous surface layer with the underneath pulpal tissue layer containing well-preserved odontoblasts. 2. Bleeding, fibrosis and necrosis are the main reactions obsereved in electrosurgical pulpotomy and the normal pulpal tissues were limited to the apical portion. 3. In the aspect of preserving the normal pulpal tissue, ferric sulfate pulpotomy was evaluated to be superior to formocresol or electrosurgical pulpotomy.

Considerations for Pulp therapy in primary teeth (유치의 치수치료 : 치료법 선택의 근거)

  • According to the results of the questionnaire survey and the HIRA data released in KAPD conference in 2019, the pulpotomy in primary teeth is gradually decreasing while the one-visit pulpectomy is rapidly increasing. However, these changes seem to be for a more definite prognosis rather than based on academic grounds, and a more accurate diagnosis and conservative approach are needed. In addition, many studies have suggested that MTA pulpotomy was superior to any other materials and which are currently preferred as medicaments for pulpotomy. In the long term period, MTA pulpotomy is expected to be cost effective option.

A preliminary report on histological outcome of pulpotomy with endodontic biomaterials vs calcium hydroxide

  • Objectives: The purpose of the study was to evaluate human dental pulp response to pulpotomy with calcium hydroxide (CH), mineral trioxide aggregate (MTA), and calcium enriched mixture (CEM) cement. Materials and Methods: A total of nine erupted third molars were randomly assigned to each pulpotomy group. The same clinician performed full pulpotomies and coronal restorations. The patients were followed clinically for six months; the teeth were then extracted and prepared for histological assessments. The samples were blindly assessed by an independent observer for pulp vitality, pulp inflammation, and calcified bridge formation. Results: All patients were free of clinical signs/symptoms of pulpal/periradicular diseases during the follow up period. In CH group, one tooth had necrotic radicular pulp; other two teeth in this group had vital uninflamed pulps with complete dentinal bridge formation. In CEM cement and MTA groups all teeth had vital uninflamed radicular pulps. A complete dentinal bridge was formed beneath CEM cement and MTA in all roots. Odontoblast-like cells were present beneath CEM cement and MTA in all samples. Conclusions: This study revealed that CEM cement and MTA were reliable endodontic biomaterials in full pulpotomy treatment. In contrast, the human dental pulp response to CH might be unpredictable.

PULPOTOMY IN PRIMARY MOLAR TEETH USING ELECTROSURGERY AND MTA : A RETROSPECTIVE STUDY OF SURVIVAL RATES (유구치에서 electrosurgery와 MTA를 이용한 치수절단술 : 성공률에 대한 후향적 연구)

  • This study investigates the success rate of pulpotomy using electrosurgery operated on a primary molar in caries. The pulpotomy using electrosurgery was done on 253 primary molars of 111 young patients from 2 to 9 years old during the period of the first day of January 2011 to the last day of December 2015. After the amputation of pulp and hemostasis of primary molar were done using electrosurgery, MTA as pulp capping material was applied to the primary molar and the tooth was restored with the stainless steel crown. The follow up period after the treatment ranged from 4-46 months. The clinical and radiographic success rate ranged from 92.1 - 94.3%. Which is comparable to formocresol and ferric sulfate pulpotomy. Due to its non-pharmacological characteristic, electrosurgery can minimize harmful effect on the pulp tissue. Its fast bleeding control makes it easy and safe to use in pediatric and disabled patients in comparatively simple manner. Electrosurgery can be an alternative for pulp therapy considering the side effects of pharmacological ways.

Pain Occurrence after Partial Pulpotomy by using Endocem MTA and ProRoot MTA: a Clinical Study (Endocem MTA와 ProRoot MTA를 이용한 부분치수절단술 후 통증 발생에 관한 임상 연구)

  • Objectives: This study aimed to compare the postoperative pain and clinical performance after partial pulpotomy by using ProRoot MTA and Endocem MTA. Materials and Methods: Twenty-eight teeth requiring partial pulpotomy due to deep dental caries or traumatic injury were included in this study. After 2mm removal of exposed pulp and bleeding control, the ProRoot MTA or Endocem MTA was randomly adjusted to the exposed site. 1, 2, 4, and 12 weeks after the final restoration, the patients were recalled to check the postoperative pain or another unfavorable signs. Pearson's chi-square test was used for statistical analysis to evaluate any differences among tested materials. Results: 3 of 28 teeth showed postoperative pain and cold positive during follow-up period (10.7%). There were no statistically differences in pain occurrence between two tested materials (P > 0.05). Conclusions: In the limitations of this study, partial pulpotomy by using Endocem MTA showed the advantages of short setting time and lower postoperative pain incidence, allowing one visit treatment.

YouTube as a source of information about pulpotomy and pulp capping: a cross sectional reliability analysis

  • Objectives: The purpose of this study was to critically evaluate the quality, reliability and educational content of the information of vital pulp treatment videos available on YouTube. Materials and Methods: The keywords "pulpotomy" and "pulp capping" were searched on YouTube on 5th July 2020, until 60 English language videos of each search term with a duration shorter than 15 minutes were acquired. Video characteristics were recorded and Video Power Index (VPI) was calculated. Reliability and educational quality of videos were evaluated using the Modified DISCERN score, the Journal of American Medical Association (JAMA) benchmark criteria and Global Quality Scores (GQS). Videos were categorized by uploading source. Results: Regarding pulpotomy, 31.7% of the videos were uploaded by specialists and 68.3% were directed by non-specialists. In the case of pulp capping, the corresponding percentages were 45% and 55%, respectively. Videos uploaded by specialists had significantly higher modified DISCERN, JAMA and GQS scores compared to those uploaded by non-specialists. Endodontists tended to have the highest reliability and VPI scores. Conclusions: YouTube videos on vital pulp treatment contain low educational quality or incomplete information. Low popularity of dental pulp capping and pulpotomy videos may be attributed to the specialized nature of these procedures. As YouTube represents an important source for patient information about different health topics, reliable informative videos should be uploaded by specialized dental professionals.

AN ELECTRONMICROSCOPIC STUDY ON THE HEALING PROCESS OF THE REMAINING PULPAL TISSUES AFTER PULPOTOMY BY Nd-YAG LASER (Nd-YAG레이저에 의한 치수 절단후 잔존 치수 조직의 치유과정에 관한 전자현미경적 연구)

  • The purpose of this study was to investigate a fragment of possibility of pulpotomy with the Nd-YAG laser by the observation of pulpal healing process and the fine structural changes of the fibroblasts of the remaining pulpal tissues. Class V cavities on !55 teeth from 4 adult dogs were prepared and the pulp chambers were opened with a sterilized round bur. In the control group(19 teeth), the exposed coronal pulps were excised by a sharp excarvator. After bleeding was controlled with the sterilized cotton pellets, calcium hydroxide powder was applied on the remaining pulpal tissues and the cavities were sealed with Z.O.E. In the experimental group 1 : the pulpotomy with laser-calcium hydroxide powder application group(l9 teeth), the exposed coronal pulps were excised by Nd-YAG laser(10 watts power, 2 psi water, 20 psi air) for 2 or 3 seconds and calcium hydroxide powder was applied on the remaining pulpal tissues and the cavities were sealed with Z.O.E. In the experimental group 2 : the pulpotomy with laser-no calcium hydroxide powder application group(17 teeth), after amputating the coronal pulps with Nd-YAG laser as the experimental group 1, the remaining pulpal tissues were covered with stenilized aluminum foil and the cavities were filled with Z.O.E. The animals were sacrificed at the intervals of 1, 2, 3 and 4 weeks. All the teeth were rouutinely processed and the remaining pulpal tissues were observed by the light microscope and electron microscope. The results were as follows : 1. In light microscopic findings, there was no significant difference of the inflammatory response in the remaining pulpal tissues between the control group and the experimental groups. In both of the experimental group 1 : pulpotomy with laser-calcium hydroxide powder application group and the control group, the dentin bridges were observed after 2 weeks and the structure of the dentin bridge was almost same. In the experimental group 2 : pulpotomy with laser-no calcium hydroxide powder application group, the fibrous layers instead of dentin bridge were observed on the superficial portion of the remaining pulpal tissues after 2 weeks and they were consisted with densely crowded active fibroblasts. 2. In the electronmicroscopic findings, the active fibroblasts in the experimental groups were more frequently observed than in the control group at 1 week. But active fibroblasts were found with same frequency after 2 weeks in all of the control group and the experimental groups. 3. General distortions of the cell such as loss of the cell membrane, vaculoization of the cell etc. were observed at the suberficial layer of the remaining pulpal tissues and the carbonization was found in the dentinal wall in 1 week of the experimental groups. 4. In the experimental group 2 : pulpotomy with laser-no calcium hydroxide powder application group, the activity and the density of the fibroblasts in the fibrous layer were more than those in the deep portion of the remaining pulpal tissues after 2 weeks. 5. In the control group, bacteria such as cocci and bacilli were observed frequently, but in the experimntal groups, they could not be observed.

  • Traumatic crown fracture in mixed dentition or early permanent dentition is relatively common. Crown fracture is classified into simple or complicated crown fracture by the presence of pulp exposure. The condition of pulp must be considered in treatment of crown fractures with pulp exposure. Treatment of immature crown-fractured incisor with pulp exposure is more complex because of its incomplete root formation. Pulp capping, partial pulpotomy, cervical pulpotomy can be used for continuous root development in immature crown-fractured incisor with pulp exposure. The success rate of partial pulpotomy is very high and there are several advantages of partial pulpotomy because the cell-rich coronal pulp tissue is preserved. This paper reports 2 cases of crown-fractured permanent incisors with pulp exposure that had been treated by partial pulpotomy successfully.

Clinical and radiographical evaluation of mineral trioxide aggregate, biodentine and propolis as pulpotomy medicaments in primary teeth

  • Objectives: The purpose of this study was to evaluate the efficacy of mineral trioxide aggregate (MTA), Biodentine and Propolis as pulpotomy medicaments in primary dentition, both clinically and radiographically. Materials and Methods: A total of 75 healthy 3 to 10 yr old children each having at least one carious primary molar tooth were selected. Random assignment of the pulpotomy medicaments was done as follows: Group I, MTA; Group II, Biodentine; Group III, Propolis. All the pulpotomized teeth were evaluated at 3, 6, and 9 mon clinically and radiographically, based on the scoring criteria system. Results: The clinical success rates were found to be similar among the three groups at 3 and 6 mon where as a significant decrease in success rate was observed in Group III (84%) compared to both Group I (100%) and Group II (100%) at 9 mon. Radiographic success rates over a period of 9 mon in Groups I, II, and III were 92, 80, and 72%, respectively. Conclusions: Teeth treated with MTA and Biodentine showed more favorable clinical and radiographic success as compared to Propolis at 9 mon follow-up.

A STUDY OF THE EFFECTS OF AUCUBIN ON THE PULP TISSUE AFTER PULPOTOMY IN DOGS (치수절단 후 Aucubin이 잔존치수조직에 미치는 영향에 관한 연구)

  • Aucubin, an iridoid glucoside isolated from Aucuba japonica, has pharmacological effects such as antimicrobial effect, liver protective function and inhibition of liver RNA and protein biosynthesis, etc. This study was performed to observe the effect of aucubin on the pulp tissue after pulpotomy. Aucubin was converted to aucubinogenin as an active form by ${\beta}$-glucosidase. In 3 Mongrel dogs, the pulps were mechanically exposed with a sterile round bur and excised with sterile excavator. After bleeding control, aucubin was applied on remaining pulp tissues and then they were sealed with IRM in experimental group. In control group, $Ca(OH)_2$ powder was applied on remaining pulp tissues and then they were sealed with IRM. After interval of 1 and 12 weeks, the dogs were sacrificed. The teeth were prepared for histologic evaluation and examined by light microscope. Aucubin 1 week group showed that mild inflammation and vascular congestion in most of the specimen. More various degree of inflammation was found in experimental group than in control group. Premature calcified mass were found in the both 1 weeks groups. Continuous well-formed dentin bridge was found in both 12 weeks groups. Collectively, this study suggests that the possibility of aucubin as a medicament after pulpotomy.
