저자들은 폐결핵의 병력이 있던 2명의 환자에서 기침과 객담을 주소로 내원하여 단순 흉부 방사선과 흉부 전산화 단층 촬영 결과 흉막비후와 기관지흉막루를 의심할 수 있는 공동형성을 보이고 흉막 천자액 배양과 수술후 조직배양검사결과 Aspergillus fumi-gatus가 확인되어 수술 및 항진균제를 사용하여 치료한 2례를 경험하였기에 이를 보고하는 바이다.

폐격리증이란 기형적 동맥에 의해 영양공급을 받는 대생 폐조직의 일부를 일컫는 용어이다. 이는 두 가지로 대별되며 -외협형과 내엽형- 다른 임상양상을 보인다. 본 과에서는 폐국균증을 동반한 내엽형 폐격리증 1례를 경험하였다. 환자는 32세 여자로 특별한 증상은 없었으며, 우연히 낭종성 병변을 지닌 폐종물이 발견되었다. 수술시야에서 하폐인대의 비정상적 동맥을 보어 폐격리증으로 진단하고, 우하엽 절제술을 시행하였다.

A 33 year old male patient was admitted with 20 years history of recurrent hemoptysis. On clinical examination, mild left chest discomfortness and foul odored sputum with occasional rusty hemoptysis were principal complaints noted. Chest X-ray film revealed moderately advanced active tuberculosis lesion on both upper lung fields, and hen-egg sized mass surrounded with linear crescent of air shadow in a cavity on his left upper lung field. On left thoracotomy, dense pleural adhesions on left apicoposterior segmental surface with multiple lymphnode enlargements were noted, and the soft encapsulated mass of $5{\times}5{\times}8cm$ was localized in the apicoposterior segment of the left upper lobe. Apicoposterior segment with anterior segment of the left upper lobe was resected. Cavity was opened to find a rusty grayish colored, fragile mass, which was confirmed as "fungus ball" of aspergillosis by histological section slide with Gomori staining. The authors report one case of pulmonary aspergilloma superinfected with previous long standing pulmonary tbc.

Between July 1988 and August 1991, 8 cases of pulmonary resection for pulmonary asp-ergilloma had been performed in the department of thoracic surgery, Korea Cancer Center Hospital. The patients were consisted of 4 males and 4 females and were evenly distributed from fourth decade to sixth decade. Hemoptysis was the most common chief compla int. In chest roentgenogram, patch infiltration was noted in 4 cases[50%] and intracavi-tary fungus ball was noted in only 2 cases[25%]. A. fumigatus was identified pre-operatively in 2 cases by bronchoscopic washing and in 1 case by culture of pleural effusion, Eight pulmonary resections were done by 5 lobectomies, 1 segmentectomy, 1 wedge resection and 1 pneumonectomy, Postoperative pathologic findings showed that 6 cases [75%] were combined with bronchiectasis, 1 case with tuberculosis and 1 case with pneumonia. We experinced 1 case of postoperative pulmonary edema but there was no mortality case.

흉막 아스페르길루스증은 매우 드물고 사망률이 높은 질환으로 알려져 있다. 저자들은 흉막생검으로 진단한 흉막 아스페르길루스증에서 불량한 전신상태로 인해 수술적 치료는 시행하지 않고 항진균제만을 투여하여 임상적 호전이 관찰된 1예를 경험하였기에 문헌 고찰과 함께 보고하는 바이다.

아스페르길루스 감염증은 드물기는 하지만 면역 억제된 상황에서는 비교적 잘 발생하며 치사율도 높은 것으로 알려져 있다. 경구 또는 정맥내 치료제가 있기는 하지만 합병증이 많고 장기간 사용해야 하기 때문에 치료효과는 크게 기대할 수 없는 경우가 많다. 따라서 환자에 따라서는 외과적 요법을 병행하여 치료효과를 높이는 방법도 사용하고 있다. 세종병원에서는 지금까지 실시한 15례의 심장이식환자중 2 례의 폐 아스페르길루스증을 경 험하였기 에 보고하는 바이다. 두 환자 모두 나이와 수술시기가 비슷하였 으며 수술후 약 3주후 외래에서 흉부 X-선상 폐병변을 발견하였다 경피적 천자생검상 Aspergillus fumigatus로 확인되어 정맥내 Amphotericin B와 경구용 Itraconazole을 병용한 항진균제요법을 실시하 였으며 두번째 환자는 병변이 우하엽에 국한되어 외과적 절제술을 병용하였다. 첫번째 환자는 이식후 14개월째 양호한 상태로 추적중이며 두번째 환자는 치료 완료후 3개월째 비세포성 거부반응으로 사망하였다.

Aspergillus fumigatus causes a variety clinical syndrome in lung including aspergilloma, chronic necrotizing aspergillosis, invasive pulmonary aspergillosis, and allergic bronchopulmonary aspergillosis. Aspergilloma develops by a colonization and growing of Aspergillus inside lung cavities with underlying lung disease. There is a few report of endobronchial aspergilloma without lung parenchymal lesion. We experienced a case of endobronchial aspergilloma did not fit any category of Aspergillus-induced lesion, who show minimal fibrostreaky denstities on chest PA and chest CT. Massive hemoptysis was improved by a removal of the aspergilloma in this patient. Here, we report a rare case of endobronchial aspergilloma showing massive hemoptysis with review of literatures.

아스페르길루스 기관-기관지염은 침습성 폐 아스페르길루스의 매우 드문 형태 중 하나로 주로 기관-기관지에 국한되어 거짓막이나 궤양을 형성하거나 폐쇄를 유발하는 질환이다. 거짓막성 아스페르길루스 기관-기관지염은 아스페르길루스 기관-기관지염 중 가장 심한 형태로 대게는 면역저하자에서 발병하고 예후가 좋지 않다. 현재까지 이 질환에 대해 몇 개의 국내보고가 있으나 영상 소견에 대한 보고는 드물다. 이에 저자들은 기관지경 검사상 거짓막성 아스페르길루스 기관-기관지염으로 진단되고 적절한 항진균제 투여로 성공적으로 치료된 환자의 증례를 특징적인 영상 소견과 함께 보고하고자 한다.
