A Study on the Development Plan of Education & Training for Public Health Nursing Personnel (보건간호인력 교육훈련 현황과 발전 방안에 관한 연구)

  • Purpose: This study evaluated the education and training for public health nursing personnel and we suggest a development plan for their education and training. Methods: The data was collected from the annual planning reports of the Department of Health and Welfare Training in the Korea National Institute of Health from 1985 to 2004. Results: The summary of problems on the education and training included 1) unspecific educational goals and objectives, 2) a shortage of education and training programs for improving practical skills and knowledge on health promotion and chronic disease management, and 3) ineffective teaching methods based on lecture. In order to overcome these problems, education & training for public health nursing personnel should 1) establish dear and specific goals and objectives, 2) develop educational programs that focus on the trainee's needs and develop a long term educational program for reinforcing practical competency along with elementary courses for novices & advanced courses for experts, 3) utilize effective teaching methods such as case study, e-learning and applied learning programs. Conclusion: The education and training for public health nursing personnel should be improved in order to reinforce their competency and their ability to cope with the changing health care environment of the 21st century.

Perception of Importance and Performance in the Infection Control of Visiting Nursing Personnel in Public Health Centers (보건소 방문간호요원의 감염관리에 대한 중요성 인지도 및 실천도)

  • The work of visiting nursing personnel consists mostly of direct nursing and it is absolutely necessary that training of the nursing personnel for infection control be emphasized because most of patients are composed of elderly people with chronic diseases and malnutrition who are highly susceptible to infection. Accordingly. this study was designed to provide basic data for the development of an infection control program in public health centers as well as the promotion of perception and performance of infection control by analysing the perception of importance and performance in the infection control of visiting nursing personnel who carry out visiting nursing services. The subjects of this study consisted of a number of visiting nursing personnel working for 15 public health centers. 15 health-center branches. and 16 health clinics. The collection of data was grounded on a questionnaire method. The questionnaire breaks down to 8 questions about general characteristics and 108 questions in connection with the perception and performance of infection control. The results of this study are as follows: 1. 82 subjects (78.1%) responded that it is necessary to control infection. 79 subjects(75.3%) responded that education is necessary for infection control. 50 respondents (47.6%) answered in the affirmative in regard to the role of infection carriers. 2. The mean score for the perception of importance in infection control was 4.55 and that performance accounts for 4.03. this turns out to be very high. 3. In terms of the perception of importance per each field of infection control. medication. the management of contamination and sterilization, the control of environments and instruments. washing hands and individual hygiene as well as the prevention of cross-infection and the supervision of infection stand at 4.72. 4.64. 4.60. 4.58. 4.52 and 4.21 respectively. In terms of performance. the supervision of infection. the control of environments and instruments, individual hygiene and the prevention of cross infection. medication. washing hands, and the management of contamination and sterilization was 4.21. 4.12. 4.10. 4.03. 3.91 and 3.81 respectively. 4. In terms of the correlation between the perception of importance and the performance of infection control. the score stands high in all fields: this means the high perception of importance leads to high performance statistically (P< .05). 5. In terms of the perception of importance in general characteristics. there is no significant difference while the form of working places. working career and the career as visiting nursing personnel show a significant difference in terms of performance(P< .05). From the results. it is suggested that the visiting nursing personnel should be constantly educated with the aid of the development of infection control programs and that instructions and instruments for the management of infection should be worked out so as to enhance the performance of infection control on the part of nurses in applicable communities.

Influence of Shiftwork on Greek Nursing Personnel

  • Background: The aim of this study was to investigate the burden experienced by nursing personnel working irregular shifts in Greece and to conduct the first test of a Greek version of the Standard Shiftwork Index (SSI). Methods: A cross-sectional survey was carried out. The SSI was completed by 365 nurses and nursing assistants working shifts, including nights. Results: Female nursing personnel and those suffering from a chronic disease were most affected by working rotating shifts as they had elevated scores on the majority of the SSI scales, such as sleep, chronic fatigue, digestive and cardiovascular problems, general health questionnaire, cognitive and somatic anxiety, shift time satisfaction, engagement and disengagement strategies, languidity, flexibility, and neurotisicm. Nurses with longer working experience and those with family responsibilities also scored higher on some of the SSI scales, such as the sleep, shift time satisfaction, social and domestic disruption, disengagement strategies, morningness, and languidity scales. Conclusion: Shiftwork affects female nurses, those with chronic disease, older age, and domestic responsibilities more severely. Therefore management should take these factors into account when designing work schedules to alleviate the burden caused by shiftwork.

Multilevel Analysis of Factors Associated with Perceived Good Health and Multimorbidity among Older Adults: Using the 2017 Community Health Survey

  • Purpose: This study aimed to identify individual- and regional-level factors associated with perceived good health and multimorbidity among older adults. Methods: Secondary analysis of the 2017 Korea Community Health Survey was conducted on a sample of 67,532 older adults. The individual level data set was combined with regional-level factors from the administrative data released on the Korea National Statistical Office website. Distribution of perceived good health and multimorbidity in 254 public health centers were calculated using sampling weights and presented as percentages. Multilevel logistic regression analyses were used to identify individual- and regional-level factors associated with perceived good health and multimorbidity. Results: Overall, 21.1% of subjects perceived their health to be good, ranging from 9.3% to 39.4% by region. The prevalence of multimorbidity was 15.9%, and varied between 6.6% and 22.6% by region. At the individual level, perceived good health was associated with men, younger age, higher educational levels, higher income levels, and those married and living with a partner and maintaining a healthy lifestyle. At the regional level, higher rates of health center personnel among public officials and higher levels of financial independence were associated with perceived good health. Multimorbidity was associated with marital status and healthy lifestyle, and higher rates of health center personnel among public officials. Conclusion: Regional factors such as health care personnel and local economy could affect population health. Our findings suggest the need to consider individual- and regional-level factors to promote good health among older adults and reduce the health gap by region.

A Study for Reorientation of Home Care Service at Community (일개 보건소 방문간호사업의 업무 분석)

  • The purpose of this study is to evaluate the community based home care service, and make reorientation for better service. The data was collected from the public health center, which was operated for one year, 1997. This case is evaluated and reoriented according to five elements of public health care system; system of resources for public health, organization and administration, health care delivery system of financing, management. In resources for public health, available health personnels are 15 physicians, 17 nurses, 11 nurse aides and 2 other persons. One professional health personnel take care of 609 clients, The equipments used for elderly and the disabled are 6 wheelchairs, 4 walkers, 1 hairwashers and 30 viberations. But these equipments are not enough to deal all clients. In organization and administration, planning and setting goals for community home care are made by the director, supervisor of family department and public health nurse. So there is no regular commitee for home care services in this community. The form of delivery of health care is focused on preventive health care. The important works of public health nurse are health education, preventive care for hypertension, D.M. and vaccination of communicable desease. In finaning system, funds come from central government(8.3%), local goverment(16.7%) and health center itself(3.8%), The services consist of health education, vaccination, clinical test and equipment. There are several local volunteers, which are local hospitals, a college, a christian association, a catholic association, a drivers association and a disabled association. The volunteer groups give physical and mental support to the clients. In management, this health center has three evaluation methods. One is done by local government, one is done by health center itself, and the other is done by clients with questionair. But the evaluation tools are deferent between agency. Home care services must be planned and evaluated. This public health center has to have more personnel, equipments, education for professional kowledge and meetings with community volunteer agencies.

Factors Influencing Cooperation between Nurses and Social Workers (간호사와 사회복지사의 상호 협력에 영향을 미치는 요인)

  • Purpose: The study investigated the level of mutual cooperation among those engaged in public and private nursing and social welfare to understand contributing factors. Method: The subjects were 289 nurses and 279 social workers. The level of mutual cooperation between nurses and social workers was evaluated. taking into account related personal factors and organizational environmental factors. Results: The level of mutual cooperation between nursing and social welfare organizations showed the following sequence, in decreasing order; public nursing, public social welfare, private nursing and private social welfare personnel. The factors affecting nurses' cooperation with social welfare personnel were expected duties, understanding of social workers' service, service autonomy, service appropriateness, and the training environment of the organization. Factors influencing the level of cooperation of social workers with nurses included understanding of nursing service, understanding of the efficiency gained by cooperation, service appropriateness and the training environment of the organization. Conclusion: The development of training programs to increase the level of cooperation between the nursing and social welfare fields is strongly recommended.

Challenges in Public Health Programs for People Living in Rural and Remote Areas (농촌지역 공공보건기관의 보건사업 기능개편 방안)

  • Purpose: The purpose of this study was to suggest new directions for public health programs in rural and remote areas. Method: For this purpose, a literature review was done including articles, research reports, and master theses and doctoral dissertations. Results: Public health programs in rural remote areas were found to be very insufficient in terms of professional personnel and program diversity. Especially, there is a lack of adequate manpower and infra-structure in the public health sub-centers at the township and sub county level. Although community health practitioners at the village level are providing public health service beyond medical care, their coverage rate is very low. Conclusion: The results suggest a need to strengthen the function of public health sub-centers to provide comprehensive public health service based on the life-cycle approach. For this new change, legal and political support must be developed.

  • This study was performed to investigate the conditions of physical activity and exercise programs in Public Health Center. For this study 244 Public Health Centers in Seoul and all of the countries were surveyed from October 15 1999 to June 30 2000. The study was done by telephone, e-mail, fax and questionnaire. First, the 25 Public Health Centers in Seoul were investigated, and then 107 Public Health Centers in other cities were investigated. Public Health Centers in Seoul and Public Health Centers in other cities are very different in exercise program and equipment, budgets, personnel. There were many kinds of exercise program for people who have health problems, but a few exercise programs for healthy people. Exercise programs for people who have health problems were to prevent hypertension, arthritis. obesity, diabetes and back pain. Physical activity and exercise programs for healthy people were stretching for pregnancy and elderly. There were $24(96.0\%)$ Public Health Centers in Seoul which had physical activity and exercise program, $80(36.5\%)$ Public Health Centers in other cities which had physical activity and exercise program.

A Study of Present Conditions of Domiciliary Elder Care Utilizing Public Health Care Institutions in Rural Area (일 도단위 농촌지역 재가노인복지사업 운영 실태조사)

  • Purpose: The purpose of this study was to suggest new direction for domiciliary care for elders provided by public institutions in rural areas. Method: The participants in the study were elders using one of 11 public health care institutions, of which 8 operated day care services exclusively, and 3 operated both day care and short-term respite care services. A survey was conducted using a structured questionnaire that included items on general characteristics of the service users, conditions of the services, personnel, financial status, facilities, and perception of the tasks of the staff. Result: The service content of the day care centers included Western and Chinese medical service, physical services, activities of ADL, nursing care services, meal services and transportation services. Domiciliary care centers provided a wide variety of health and social welfare service for elders. Personnel consisted of 3 to 8 staff for day care centers and 7 to 10 for domiciliary care centers. Both types of centers rely on financial support from local government for operation. The perception of the staff was the need for operation of these centers by public health facilities such as public health centers and sub-centers. Conclusion: The result suggest a need to activate the function of public institutions to provide domiciliary care for elders. For this new change, the role as a social support system must be developed.

Evaluation Study of Home Health Care Program Operated in G Public Health Center (인천광역시 일 보건소 대도시 방문보건사업의 평가 연구)

  • Purpose: The purpose of this study was to evaluate the achievements of the project, and also to find out its strengths and weaknesses. Method: This evaluative study employed system theory and analytic techniques by using criteria which were relevance, appropriateness, adequacy, progress, effectiveness and satisfaction for input, process, output, and outcome of program. Study subjects were participants in the home health care program implemented in G public health center in Inchon metropolitan city in 2003. Results: Input factors including recruitment personnel, and support organization development were not adequately met for the program. However. the goal and objectives of the project were really appropriate for the community needs as well as government's policy. For the Process evaluation, home health care record form and computer data base had not progressed as scheduled, but overall program activities were finished on time. However, cooperation between organizations in the health center during service activity were not supportive. Managerial ability of program charged personnel about coordination and integration of team members was not affirmative. Output and outcome evaluation showed that people improved self care ability were 221 (17.5%). and 71 (5.62%) of subjects were moved into category of possible self-care. Client's satisfaction for this project showed a high degree. Conclusions: Based on the above results support organization and staff personnel for this program should be developed. Also, a community network of resources should be established and case management services should be focused continuously in community based home health care.
