Prevalence Related Characteristics and Management Status in Children with Atopic Dermatitis in Ulsan (울산 일 지역 영유아 및 학령기 아동의 아토피 피부염의 유병관련특성 및 관리실태)

  • Purpose: This study was performed to investigate prevalence related characteristics and management status in children with atopic dermatitis(AD) for developing health promotion strategies led by community. Methods: Data were collected from 6,562 children with AD in preschool and elementary schools in a district of Ulsan city. Results: The majority of children(44.4%) were onset in less than 2 years after the birth. Seventy six point seven percent of subjects had skin rash and the pathologic lesion of them were flexion of upper extremities and extension of lower extremities. The prevalence rate of the respondents was 30.3%. In bathing, the children in AD group had used soap more frequently than children in normal group. The most preference type of complementary therapy in frequency order were aloe, aroma oil and Pasture. Conclusion: More effective management strategy for alleviating symptom and preventing advanced disease such as asthma and allergic rhinitis should be developed. And also scientific evidence for using complementary therapy is necessary to be provided.

Consumer Socialization on Adolescent Impulsive Buying Behavior through School and Parents: A Random Effects Model (학교와 부모를 통한 소비자사회화가 청소년 및 대학생소비자의 충동구매행동에 미치는 영향: 랜덤효과 모형)

  • This study examines the effects of consumer socialization on Korean adolescent impulsive buying behavior. The current study used the third and sixth waves from the Korean Education and Employment Panel (KEEP) survey that has been administered by the Korea Research Institute for Vocational Education and Training since 2004. The subjects were high school juniors and university sophomores in 2006 and 2009, respectively. The final sample for panel regression analysis included 1,718 individuals. Two major agents of socialization (school and parents) were utilized in our model. Parent financial behavior (if the parents had savings) and the effectiveness/helpfulness of economics education in middle or high school were included in our estimation model. Two categories were included as individual factors: (1) psychological aspects and personal traits covering variables such as stress from self-image, academic stress, self-regulation, and a tendency of risky behavior and (2) financial behavior and attitudes, which include work experience, amount of money in hand, shopping habits, and if parental financial support is expected after high school graduation. The results from a random effects model revealed that the effects of consumer socialization through school was marginally significant, while through parents was not. Stress from self-image and the level of self-regulation were found to be significant. Neither risky behavior nor academic stress were a significant factor for impulsive buying behavior. The amount of money available in hand and shopping habits showed a significant influence. Implications for educators, parents and policy makers are identified.

The Impacts of the Recasting of Non-precious Metal Alloy for Porcelain Fused to Metal Crowns on Strength (도재소부금관용 비귀금속 합금의 반복주조가 강도에 미치는 영향)

  • This study compared and analyzed changes to the mechanical characteristics to nonprecious metal alloy for porcelain fused to metal crowns when it's repetitively used without the addition of new alloy. Metal samples were made with the Verabond V nonprecious metal alloy. Those samples to measure tensile and yield strength were made in the standardized design(ISO 22674), those to measure bond strength in the $25mm{\times}3mm{\times}0.5mm$ format, and those to measure hardness in the $10mm{\times}10mm{\times}1mm$ format. A ceramic to measure bond strength was made at the center of the metal sample in the length of $8{\ss}{\AE}$ by using Noritaker Super Porcelain EX-3. Ten samples were prepared for one, three and five repetitions of casting each. The test results were as follows: 1. The more casting was repeated, the more significantly tensile strength dropped. 2. The more casting was repeated, the more significantly yield strength dropped. 3. Repetitive casting didn't cause significant changes to bond strength. 4. The Vickers hardness significantly fell with increasing repetitions of casting. There were no changes to bond strength observed with the increasing number of repeating casting. But tensile strength, yield strength, and Vickers hardness decreased. Those results indicate that repeated casting can affect durability and that careful attention should be paid by avoiding repetitive use or excessive increase of uses when no new alloy is added.

  • Objectives : To Investigate Severity Measurement and the Correlation between Acupoints on the Face and the Upper limb in Bell's Palsy Patients by Using of DITI. Methods : We recreated the persons in the Wonkwang oriental hospital, KwangJu and the public health center, Hwasoon from April 2005 to October 2005. the 60 persons randomized allocated were not showed the acupuncture and treated in the acupuncture points used for treatment of the shoulder pain. the 30 persons were treated with real acupuncture and the others were treated with Kiln Sham Acupuncture. Conclusion : This report suggest that we must except Kyon-lyo(TE14), kyon-u(LI15), and hu-gye(SI03) from the shoulder pain study with Kim Sham Acupuncture because the persons significantly distinguished Real acupuncture from Sham acupuncture at the Kyon-lyo(TE14), kyon-u(LI15) and hu-gye(SI03) and so We cannot blind a person.

Effects of Personal Exposure to Nitrogen Dioxide on Peak Expiratory Flow in Asthmatic Patients (이산화질소 개인 노출량이 기관지천식 환자의 최대호기유속에 미치는 영향)

  • Objectives : Nitrogen dioxide $(NO_2)$ has been inconsistently associated with gradual decreases in lung function. Here, we studied the effects of $NO_2$ exposure in asthmatics by examining the association between changes in lung function and concentrations of $NO_2$ which were personally measured. Methods : Peak expiratory flow (PEF) and daily personal exposures to $NO_2$ were recorded on 28 patients with asthma (confirmed by methacholine provocation test) over 4 weeks. We used generalized estimating equations to assess the relationship between personal $NO_2$ exposure and PEF, adjusting for potential confounders such as age, gender, outdoor particulate matter, temperature, humidity, and exposure to environmental tobacco smoke. Results : The personal $NO_2$ exposures were higher than the corresponding ambient levels. The mean personal: ambient ratio for $NO_2$ was 1.48. The personal $NO_2$ exposures were not associated with the morning PEF, evening PEF, or the diurnal PEF variability. However, environmental tobacco smoke was negatively associated with both the morning and evening PEF. Conclusions : Among the asthmatic adults who participated in this study, we found no apparent impact of personal $NO_2$ exposures on the peak expiratory flow.

Throat Carriage Rate and Antimicrobial Resistance of Streptococcus pyogenes In Rural Children in Argentina

  • Objectives: The aim of this study was to determine the prevalence of asymptomatic carriers of group A ${\beta}-hemolytic$ streptococci (GAS) in children living in a rural community and to investigate the association between episodes of acute pharyngitis and carrier status. Methods: Throat swabs were collected from September to November 2013 among children 5-13 years of age from a rural community (Maria Ignacia-Vela, Argentina). The phenotypic characterization of isolates was performed by conventional tests. Antimicrobial susceptibility was assayed for penicillin, tetracycline, chloramphenicol, erythromycin, and clindamycin (disk diffusion). The minimum inhibitory concentration was determined for penicillin, cefotaxime, tetracycline, and erythromycin. Results: The carriage of ${\beta}-hemolytic$ streptococci was detected in 18.1% of participants, with Streptococcus pyogenes in 18 participants followed by S. dysgalactiae ssp. equisimilis in 5. The highest proportion of GAS was found in 8 to 10-year-old children. No significant association between the number of episodes of acute pharyngitis suffered in the last year and the carrier state was detected (p>0.05). Tetracycline resistance (55.5%) and macrolide-resistant phenotypes (11.1%) were observed. Resistance to penicillin, cefotaxime, or chloramphenicol was not expressed in any streptococcal isolate. Conclusions: The present study demonstrated significant throat carriage of GAS and the presence of group C streptococci (S. dysgalactiae ssp. equisimilis) in an Argentinian rural population. These results point out the need for continuous surveillance of GAS and non-GAS carriage as well as of antimicrobial resistance in highly susceptible populations, such as school-aged rural children. An extended surveillance program including school-aged children from different cities should be considered to estimate the prevalence of GAS carriage in Argentina.

Cimicifuga heracleifolia Extract Induces iNOS Expression via a Nuclear Factor-${\kappa}B$-dependent Pathway in Mouse Peritoneal Macrophages

  • Cimicifuga heracleifolia extract (CHE) was investigated for its effects on the release of nitric oxide (NO) and at the level of inducible nitric oxide synthase (iNOS) gene expression in mouse macrophages. We found that C. heracleifolia elicited a dose-dependent increase in NO production and the level of iNOS mRNA. Since, iNOS transcription has been shown to be under the control of the transcription factor $NF-{\kappa}B$, the effects of CHE on $NF-{\kappa}B$ activation were examined. Transient expression assays with $NF-{\kappa}B$ binding sites linked to the luciferase gene revealed that the increased level of iNOS mRNA, induced by CHE, was mediated by the $NF-{\kappa}B$ transcription factor complex. By using DNA fragments containing the $NF-{\kappa}B$ binding sequence, CHE was shown to activate the protein/DNA binding of $NF-{\kappa}B$ to its cognate site, as measured by electrophoretic mobility shift assay. These results demonstrate that C. heracleifolia stimulates NO production and is able to up-regulate iNOS expression through $NF-{\kappa}B$ transactivation.

Peroneal Flap: Clinical Application and Cadaveric Study

  • Background The goal of this study was to investigate the anatomy of the peroneal artery and its perforators, and to report the clinical results of reconstruction with peroneal artery perforator flaps. Methods The authors dissected 4 cadaver legs and investigated the distribution, course, origin, number, type, and length of the perforators. Peroneal artery perforator flap surgery was performed on 29 patients. Results We identified 19 perforators in 4 legs. The mean number of perforators was 4.8 per leg, and the mean length was 4.8 cm. Five perforators were found proximally, 9 medially, and 5 distally. We found 12 true septocutaneous perforators and 7 musculocutaneous perforators. Four emerged from the posterior tibia artery, and 15 were from the peroneal artery. The peroneal artery perforator flap was used in 29 patients. Retrograde island peroneal flaps were used in 8 cases, anterograde island peroneal flaps in 5 cases, and free peroneal flaps in 16 cases. The mean age was 59.9 years, and the defect size ranged from $2.0cm{\times}4.5cm$ to $8.0cm{\times}8.0cm$. All the flaps survived. Five flaps developed partial skin necrosis. In 2 cases, a split-thickness skin graft was performed, and the other 3 cases were treated without any additional procedures. Conclusions The peroneal artery perforator flap is a good alternative for the reconstruction of soft tissue defects, with a constant and reliable vascular pedicle, thin and pliable skin, and the possibility of creating a composite tissue flap.

Factors influencing sperm retrieval following testicular sperm extraction in nonobstructive azoospermia patients

  • Objective: Azoospermia owing to testicular disorders is the most severe manifestation of male infertility. The main concern for patients with nonobstructive azoospermia (NOA) is the probability of successful sperm retrieval following testicular sperm extraction (TESE). Therefore, the goal of this study was to determine predictive factors correlated with sperm retrieval. Methods: We assessed the testicular histopathological patterns, the choice of TESE surgical procedure, hormone levels, and chromosomal abnormalities in patients with NOA (n=170). The histopathology specimens were analyzed based on the histopathological patterns of hypospermatogenesis, maturation arrest, and Sertoli cell-only syndrome. Results: The mean rate of sperm retrieval was 48.8%. The rate of sperm retrieval was significantly higher in the hypospermatogenesis group than in the other groups (p<0.001). There was a positive correlation between micro-TESE (vs. conventional TESE) and the sperm retrieval rate (odds ratio, 8.077; p<0.01). A logistic regression model demonstrated that high levels of follicle-stimulating hormone (FSH) and small testicular volume were significantly associated with lower chances of successful sperm retrieval. Conclusion: Some parameters, including testicular histopathology patterns, FSH levels, testicular volume, and method of TESE surgery, may be able to predict the chances of obtaining spermatozoa in patients with NOA. However, despite the efficiency of some predictive models, the hope of retrieving any functioning spermatozoa may be sufficient to disregard predictive factors of the success of intracytoplasmic sperm injection in these patients.

A Study on the Stages of Change of Exercise and Its Related Factors in Patients with Diabetes Mellitus - Application of Transtheoretical Model - (당뇨병 환자의 운동행위 변화단계와 관련요인 연구 -범이론적 모델 적용-)

  • Purpose: This study was conducted to compare the process of change, decisional balance, and self-efficacy according to the stages of change of exercise on the basis of the Transtheoretical Model in order to investigate factors associated with the change of exercise in adult diabetic patients. Method: Data were collected from January to April 2005. The subjects were 160 patients in the G university hospital and public health center in J city. Results: The entire process of change showed the significant difference depending on the stage of change(F=20.007, p=.000). For each process of change, the Consciousness Raising(F=14.602, p=.000), Dramatic Relief(F=7.751, p=.000), Environmental Re-evaluation(F=11.843, p=.000), Self Re-evaluation(F=16.035, p=.000), Social Liberation(F=10.968, p=.000), Counter-conditioning (F=24.090, p=.000), Helping Relationships(F= 7.625, p=.000), Reinforcement Management(F= 16.693, p=.000), Self Liberation(F=11.990, p= .000) and Stimulus Control(F=4.020, p=.002) demonstrated significant differences depending on the stages of change of exercise. For the decisional balance, the Pros showed the significant difference depending on the stage of change(F=14.121, p=.000). For the self efficacy showed significant difference depending on the stage of change(F=17.137, p=.000). Conclusion: In order to proceed the stage of change of exercise in patients with Diabetes Mellitus, intensive use of a specific process of change, a stage of change matching is needed.
