상악동 편평세포암종의 수술적 치료에 있어서 측두아래오목과 날개구개오목 절제의 의의 (The Significance of Pterygopalatine Fossa and Infratemporal Fossa Dissection in the Surgical Treatment of Maxillary Squamous Cell Carcinoma)

  • Objectives: Maxillary cancer is notorious for its poor prognosis because it is usually detected lately and the majority of patients have advanced disease. Especially when the cancer extended to infratemporal fossa or pterygopalatine fossa it is very difficult to remove all the involved structure by conventional maxillectomy. In these cases we have used radical maxillectomy through lateral approach. We tried to figure out the clinical significance of this procedure. Material and Methods: This study retrospectively analyzed 23 patients with squamous cell carcinoma of the maxillary sinus who underwent surgical treatment for cure. Among them 17 patients(group A) were treated with initial surgery and 6 patients(group B) underwent salvage surgery after radiotherpy. Radical maxillectomy was performed in 12 patients and conventional total maxillectomy in 11 patients. Results: In group A, 3 out of 9 total maxillectomy case and none of 8 radical maxillectomy case were recurred. In group B one patient died of postoperative complication and among the other 5 patients only one out of 3 radical maxillectomy was salvaged and 2 total maxillectomy cases were died of intercurrent disease. Conclusion: Wide surgical dissection of pterygopalatine fossa and infratemporal fossa may improve the survival rate in patients with posterior wall invasion of maxillary sinus.

익돌구개와에 발생한 신경초종 1예 (A Case of Schwannoma of the Pterygopalatine Fossa)

  • Schwannomas are benign tumors arising from the nerve sheaths of peripheral, autonomic and cranial nerves. Between 25% and 45% of all schwannomas occur in the head and neck region but only 4% involve the nasal cavity and paranasal sinus. Moreover, schwannoma of the pterygopalatine fossa is extremely rare. We present a case of schwannoma of the pterygopalatine fossa in a 43-year-old woman. The patient was surgically managed with endoscopic transnasal approach combined with transantral approach. After 16 months, the patient shows no evidence of recurrence.

  • Neurilemmoma is a benign tumor of the nerve sheath that arises on cranial and spinal nerve roots as well as along the course of peripheral nerves. A case of a neurilemmoma that arose in the left infratemporal fossa of a 29-year-old male was presented. Plain radiographs, enhanced computed tomography scan, and magnetic resonance imaging demonstrated a large, well-circumscribed, heterogeneously enhanced mass with extension into the pterygopalatine fossa. Displaced by the large mass, bowing-in of the posterior maxillary antral wall was noted and a provisional diagnosis of a benign soft tissue tumor was made. The mass was completely excised and a diagnosis of neurilemmoma was confirmed.

  • 본 증례는 좌안의 통증과 시력저하를 호소한 83세 남자 환자에게서 진단된 면역글로불린 G4 (immunoglobulin G4; 이하 IgG4) 관련 질환의 CT와 MRI 소견에 대한 보고이다. 이는 CT에서 주변의 골미란을 동반한 날개입천장 오목의 등밀도 연부조직 종괴로 관찰되었으며 이병변은 자기공명영상 T2 강조영상에서 등신호에서 저신호강도를 보이며 비균질한 조영증강을 보였다. 환자는 벌집굴 절제술과 조직검사를 시행하였고 조직학적 검사상 IgG4 관련 질환으로 진단되었다. 환자는 기저질환으로 당뇨병이 있었고, 이런 면역저하 환자군에서 흔하게 보이는 침습적 진균성 부비동염과의 영상학적 감별이 어려운 경우였다. 저자들은 침습적 진균성 부비동염과 유사한 영상 소견을 보였던 IgG4 관련 질환의 증례를 문헌 고찰과 함께 보고하고자 한다.

Sphenopalatine ganglion block for relieving postdural puncture headache: technique and mechanism of action of block with a narrative review of efficacy

  • The sphenopalatine ganglion (SPG) is a parasympathetic ganglion, located in the pterygopalatine fossa. The SPG block has been used for a long time for treating headaches of varying etiologies. For anesthesiologists, treating postdural puncture headaches (PDPH) has always been challenging. The epidural block patch (EBP) was the only option until researchers explored the role of the SPG block as a relatively simple and effective way to treat PDPH. Also, since the existing evidence proving the efficacy of the SPG block in PDPH is scarce, the block cannot be offered to all patients. EBP can be still considered if an SPG block is not able to alleviate pain due to PDPH.

전산화 단층 촬영상에 의한 상악동 악성종양에 관한 연구 (THE STUDY OF THE MALIGNANT TUMORS OF THE MAXILLARY SINUS BY COMPUTED TOMOGRAPHY)

  • CT findings of proven 25 malignant tumors of the maxillary sinus were retrospectively analyzed to be of help in the diagnosis and treatment. The results were follows: 1. Average age was 54 years old, and eighteen were males and seven were females with a ratio of 2.6:1 2. The most common histopathologic feature was squamous cell carcinoma (19 cases) and others were two cases of adenoid cystic carcinoma, one case of malignant fibrous histiocytoma, mucoepidermoid tumor, histiocytic lymphoma, unidentified malignant tumor. 3. CT findings were sinus opacificaqtion (4%), soft tissue mass (92%), low densities within soft tissue mass (44.%), air densities within soft tissue mass (24%), osteosclerosis (4%), bone destruction (92%), bone displacement (32%), fat plane obliteration (76%). 4. CT in the malignant maxillary sinus tumors approved the value in evaluation of tumor extension to nasal cavity, ethmoid sinus, orbit, infratemporal fossa, pterygopalatine fossa, pterygoid fossa, pterygoid muscle, cheek skin and intracranial cavity. 5. Twenty four cases (96%) were stage Ⅲ, stage Ⅳ according to AJCC TNM classification. 6. Bone findings were destruction, displacement, sclerosis and most frequent site of bone destruction was the medial wall of the antrum(92%). 7. Tumor growth pattern showed destructive pattern in 18 cases(72%), and squamous cell carcinoma showed destructive pattern. (P<0.05)

  • The surgical removal of the wisdom teeth is obligate when forceps extraction fails or when the wisdom teeth are impacted. The surgical removal of impacted maxillary third molars is a commonly performed procedure usually associated with few complications & little morbidity. The most frequent complications are tooth root fracture, maxillary tuberosity fracture, tooth displacement into the maxillary sinus & oroantral fistula formation. A rarely reported complication is the displacement of a tooth into the infratemporal fossa. The method of prevention of this complication is by the placement of either a finger or periosteal elevator posterior to the tooth during extraction. To remove the displaced upper third molar is very difficult & has many complications, e.g., persistent bleeding & nerve damage. When the wisdom teeth is displaced, it is initially necessary to gain access to bone by developing a mucoperiosteal path of delivery is developed by additional bone removal or, preferably planned sectioning of the tooth. There are many approaching techniques to remove the displaced upper third molar. This following report describes the surgical technique of displaced upper third molar in the pterygopalatine fassa by the midpalatal &transpharyngeal approach.

신경주위 전파를 수반한 유선낭종암 (PERINEURAL SPREAD IN ADENOID CYSTIC CARCINOMA)

  • This is a report of adenoid cystic carcinoma occurred in the palate in 30-year-old patient with a complaint of exophytic mass. The authors diagnosed it as adenoid cystic carcinoma by the clinical examination, radiographic findings and histopathological findings. The obtained results are as follows: 1. In clinical examination, asymptomatic exophytic mass of palate was observed. 2. In radiographic findings, soft tissue mass infiltrated the left maxillary sinus, nasal cavity, infraorbital fossa, hard palate, pterygopalatine fossa and pterygoid plate, and enhanced soft tissue mass was also observed in CT. 3. In histopathological findings, tubular and solid patterns of glandular structures were observed and the infiltration of tumor cells into the nerve fibers was also observed. 4. Two years after radical surgery, radiation therapy and chemotherapy, the perineural spread to orbital area was observed. 5. Much longer follow-up than 5 years is needed for early diagnosis of recurrence and distant metastasis.

  • This is a case report of calcifying aponeurotic fibroma occurred in the right pterygopalatine fossa & ramus area accompanied by adenoid cystic carcinoma of the right sublingual gland of a 44-year-old female. Calcifying aponeurotic fibroma is benign tumor, but it is characterized by poorly marginated, infiltrated growth pattern and a stubborn tendency to local recurrence, but there is no record of malignant transformation or metastasis, and surgical management should be conservative(excision and reexcision). Most cases been reported at the hands and feet, but no reported case occuring in the head region is found in the literature. Adenoid cystic carcinoma is a slow-growing infiltrative tumor with high recurrence rate, and it's treatment requires radical excisin and radiotherapy. Wide surgical excision of tumor, RND and partial resection of mandible were done. And then, immediate mandibular reconstruction was performed by means of reimplantaion technique after autoclaving of the resected bone.
