만성 구강안면통증의 사회심리적 영향 (Psychosocial Impact of Chronic Orofacial Pain)

  • 본 연구는 6개월 이상의 만성 비치성 구강안면통증(orofacial pain, OFP) 환자들을 대상으로 통증으로 인한 사회심리적 영향을 평가하고, 대학병원의 구강안면통증 전문클리닉에 내원한 OFP 환자들의 임상적 특성(유발사건, 이전 치료병력 등)을 조사하고자 하였다. 연구를 위하여 2008년 9월부터 6개월간 단국대학교 치과대학 부속 치과병원 구강내과 구강안면통증클리닉에 내원한 초진환자 중 OFP 환자들의 초진시 진료기록과 면담 전 대기실에서 작성한 만성통증척도(Graded Chronic Pain Scale, GCPS) 설문지를 조사분석하였다. 본원에 지난 6개월간 내원한 다양한 비치성 구강안면통증 572 명의 환자들 중 63%는 이전 치료 경험이 있었고 약물치료, 상담, 물리치료의 순으로 빈도가 높았고, 전문과목은 일반치과와 정형외과의 순서였다. 환자들의 89.2%는 턱관절장애였고 6.4%는 삼차신경통을 포함한 신경병성동통, 4.5%는 2가지 이상의 OFP가 공존하는 복합성 안면통증(mixed OFP)이었다. 통증의 발생과 관련한 유발요인에 대해서는 환자들의 약 35%가 인지하고 있었으며, 외상, 치과치료의 순으로 빈도가 높았다. OFP 환자의 약 반 정도(46%)는 만성 통증을 가진 환자였으며, 이 중 40%에 이르는 많은 환자들이 구강안면통증으로 인하여 일상적인 활동과 사회적 활동 및 업무능력에 상당히 제한(high disability)을 받는다고 보고하였다. 여성일수록, 나이가 많을수록, 통증병력이 길수록(>5년) 그리고 턱관절장애보다는 신경병성 통증과 복합성 구강안면통증 환자들이 더 심한 사회심리적 활동제한을 보였다.(p=0.000) 이러한 연구결과는 환자가 자신의 만성통증에 수반되는 사회적, 심리적 장애(disability)를 잘 이해할 수 있도록 사회심리적 지원이 필요하며, 가능한 한 조기에 전문가에게 내원하여 정확한 진단을 받을 필요가 있음을 보여준다.

치과위생사의 치석제거 작업자세와 근골격계질환 자각증상의 융복합적 관련성 (Convergence Relationship between Scaling Work Posture and Symptoms of Musculoskeletal Disorders in Dental Hygienists)

  • 본 연구는 치과위생사의 치석제거 시 작업자세와 근골격계질환 자각증상의 관련성을 알아보기 위함이다. 치과위생사 264명을 대상으로 일반적 특성, 건강행태, 치석제거 업무, 작업자세, 근골격계 자각증상의 설문을 구성으로 융합연구 하였다. 자료 분석은 R 2. 15. 1 프로그램을 이용하였으며 분석한 결과, 치석제거 시 올바른 작업자세는 29.9%, 대체적으로 올바른 작업 자세는 37.5%, 올바르지 않은 작업 자세는 32.6%로 나타났으며, 근골격계의 자각증상을 호소하는 부위로는 어깨가 3.32배(OR,3.32;95%CI, 1.58~6.98), 발은 2.97배(OR,2.97;95%CI, 1.18~7.48), 손은 2.84배(OR,2.84;95%CI, 1.35~5.98), 목은 2.82배(OR,2.82;95%CI, 1.35~5.91), 등은 2.41배(OR,2.41;95%CI, 1.02~5.68)로 유의성 있게 높은 결과를 보였다. 이는 올바른 작업자세의 중요성을 제공함으로서 작업환경 개선 및 효율적인 예방프로그램 개발과 지속적인 교육계획을 수립하는데 기여할 것이며, 추후 사회 심리적 요인을 포함한 조사 연구가 필요하겠다.

순환교대근무와 주간고정근무 여성 간호사의 회복탄력성에 대한 스트레스, 수면, 우울증의 영향 (Effects of Perceived Stress, Sleep, and Depression on Resilience of Female Nurses in Rotating Shift and Daytime Fixed Work Schedules)

  • 목 적 : 건강한 수면은 중요하며 회복탄력성에 긍정적인 영향을 줄 수 있다. 본 연구에서는 교대근무 간호사와 주간 고정 근무 간호사 두 집단에서 회복탄력성의 차이를 비교하고 회복탄력성에 영향을 줄 수 있는 요인들을 조사해보고자 하였다. 방 법 : 두 개의 종합병원에 근무하는 만 20세 이상의 여성 교대근무 간호사와 주간고정근무 간호사 400명을 대상으로 하였으며 2017년 6월 12일부터 2018년 6월 12일까지 설문조사하였다. 한글판 스트레스 자각척도, 스트레스 대처전략 축약형, 사회심리적 건강측정도구, 한국판 역학연구센터 우울척도, 상태불안척도, 피츠버그 수면의 질 지수, 엡워스 주간졸음 척도, 불면증 심각도 척도, 코너 데이비슨 리질리언스 척도 질문지를 적용하였다. 결 과 : 교대근무간호사와 고정근무간호사 두 집단 간의 변수 통계량을 비교한 결과 스트레스 자각, 수면의 질, 주간 졸음증을 제외한 모든 변수에서 통계적으로 의미 있는 차이를 나타내었다. 회복탄력성과 유의한 상관을 보인 요소들은 근무자들의 스트레스 대처전략, 사회 심리적 건강수준, 우울증, 불면증 심각도 이었다(p < 0.05). 다중회귀분석에서는 긍정적 재구성1이 커질수록(β = 0.206, p < 0.001), 우울증이 줄어들수록(β = -3.45, p < 0.001), 사회심리적 건강수준이 높아질수록(β = 0.193, p < 0.001), 수용적 대처2가 증가할수록(β = 0.129, p < 0.05), 주간졸음이 줄어들수록(β = -1.17, p < 0.05), 적극적 대처2가 증가할수록(β = 0.118, p < 0.05), 긍정적 재구성2가 증가할수록(β = 0.110, p < 0.05) 회복탄력성이 커지는 것으로 나타났다. 결 론 : 본 연구에서는 교대근무자에 비해 주간고정 근무자의 회복탄력성이 높음을 밝힐 수 있었다. 또한 특정 스트레스 대처 전략과 사회심리적 건강수준, 수면, 우울증이 회복탄력성과 연관이 있는 것으로 확인되었다. 향후 더 많은 수면과 회복탄력성과의 관계를 밝히는 전향적 연구들이 필요할 것으로 생각된다.

한 대기업 근로자들의 직무스트레스와 정신건강과의 관련성 (Association between Job Stress and mental health among Workers in a Large Company)

  • Objectives: This study was conducted to investigate the association between job stress and mental health among male and female workers in a large electric manufacture company. Methods: A cross-sectional study was carried out on 3,228 employees who participated in annual medical check-up working in a large electric manufacture company in Gyeonggi Province. Medical check-up and self-administrated questionnaire were performed at the same time. Korean Occupational Stress Scale Short Form (KOSS-SF) and Psychosocial Wellbeing Index Short Form (PWI-SF) were applied to assess occupational stress and mental health. Hierarchical multiple linear regression and multiple logistic regression were performed to estimate the association between job stress and mental health. Results: The proportion of high risk of mental health was 17.1% in male, and 46.9% in women. Job stress had a greater effect on mental health than other general and work characteristics. All subscales of job stress were revealed to affect mental health. Bad occupational climate and lack of reward are the strongest risk factors in mental health of male and female respectively. Conclusions: Our results suggest that job stress could affect mental health among large electronic manufacture workers.

작업관련성 근골격계 질환의 국내 연구동향분석 (Research Trends of Work-Related Musculoskeletal Disorders in Korean Literature)

  • Objectives : The purpose of this study is to analyze internal research trends of work-related musculoskeletal disorders(WMSDs) and provide problems of researches forward. Methods : 6 Korean databases were searched for articles of WMSDs published from 2000 to 2012, and 264 research were systematic reviewed. An analytical method was used descriptive statistics, an actual number and percentage. Results : The results of distribution by year were reported more than 20 articles after 2004, and 45 articles in 2009. Industrial classification distribution of research subjects the manufacturing industry was many most at the 84. Research contents in most description were 147 whether it was risk factor and relation. Research design type of articles was most description survey research 226. In the research field 89 articles were reported to the journal related to technologies. Conclusions : It is considered to be necessary that ergonomic approach would cooperate with other approaches such as integrated health management system as well as industrial medicine considering psychosocial factors.

A Study on Relationship between Physical Elements and Tennis/Golf Elbow

  • Objective: The purpose of this research was to assess the agreement between job physical risk factor analysis by ergonomists using ergonomic methods and physical examinations made by occupational physicians on the presence of musculoskeletal disorders of the upper extremities. Background: Ergonomics is the systematic application of principles concerned with the design of devices and working conditions for enhancing human capabilities and optimizing working and living conditions. Proper ergonomic design is necessary to prevent injuries and physical and emotional stress. The major types of ergonomic injuries and incidents are cumulative trauma disorders (CTDs), acute strains, sprains, and system failures. Minimization of use of excessive force and awkward postures can help to prevent such injuries Method: Initial data were collected as part of a larger study by the University of Utah Ergonomics and Safety program field data collection teams and medical data collection teams from the Rocky Mountain Center for Occupational and Environmental Health (RMCOEH). Subjects included 173 male and female workers, 83 at Beehive Clothing (a clothing plant), 74 at Autoliv (a plant making air bags for vehicles), and 16 at Deseret Meat (a meat-processing plant). Posture and effort levels were analyzed using a software program developed at the University of Utah (Utah Ergonomic Analysis Tool). The Ergonomic Epicondylitis Model (EEM) was developed to assess the risk of epicondylitis from observable job physical factors. The model considers five job risk factors: (1) intensity of exertion, (2) forearm rotation, (3) wrist posture, (4) elbow compression, and (5) speed of work. Qualitative ratings of these physical factors were determined during video analysis. Personal variables were also investigated to study their relationship with epicondylitis. Logistic regression models were used to determine the association between risk factors and symptoms of epicondyle pain. Results: Results of this study indicate that gender, smoking status, and BMI do have an effect on the risk of epicondylitis but there is not a statistically significant relationship between EEM and epicondylitis. Conclusion: This research studied the relationship between an Ergonomic Epicondylitis Model (EEM) and the occurrence of epicondylitis. The model was not predictive for epicondylitis. However, it is clear that epicondylitis was associated with some individual risk factors such as smoking status, gender, and BMI. Based on the results, future research may discover risk factors that seem to increase the risk of epicondylitis. Application: Although this research used a combination of questionnaire, ergonomic job analysis, and medical job analysis to specifically verify risk factors related to epicondylitis, there are limitations. This research did not have a very large sample size because only 173 subjects were available for this study. Also, it was conducted in only 3 facilities, a plant making air bags for vehicles, a meat-processing plant, and a clothing plant in Utah. If working conditions in other kinds of facilities are considered, results may improve. Therefore, future research should perform analysis with additional subjects in different kinds of facilities. Repetition and duration of a task were not considered as risk factors in this research. These two factors could be associated with epicondylitis so it could be important to include these factors in future research. Psychosocial data and workplace conditions (e.g., low temperature) were also noted during data collection, and could be used to further study the prevalence of epicondylitis. Univariate analysis methods could be used for each variable of EEM. This research was performed using multivariate analysis. Therefore, it was difficult to recognize the different effect of each variable. Basically, the difference between univariate and multivariate analysis is that univariate analysis deals with one predictor variable at a time, whereas multivariate analysis deals with multiple predictor variables combined in a predetermined manner. The univariate analysis could show how each variable is associated with epicondyle pain. This may allow more appropriate weighting factors to be determined and therefore improve the performance of the EEM.
