의료 기술의 발달은 암 환자의 생존율을 크게 향상시켰으며, 암치료는 종료되었지만 암생존자들은 지속되는 신체적, 심리적, 사회적 문제로 고통 받고 있다. 따라서 암생존자들이 경험하는 문제에 총체적으로 접근하기 위해서는 다양한 통합 재활프로그램이 개발되어야 한다. 본 연구는 암생존자를 위한 재활 프로그램의 내용 및 구성을 확인하고 재활의 효과를 분석하여 향후 통합재활프로그램 개발의 자료로 사용하고자 한다. 문헌검토를 통해 검색된 13개의 연구 논문을 고찰한 결과 암생존자를 위한 재활 프로그램은 운동, 심리사회적, 통합 프로그램으로 분류되었으며, 운동 프로그램은 신체 기능 향상과 피로, 이차암 또는 암 재발과 관련된 호르몬의 변화를 통해 재활의 효과를 측정하였다. 심리사회 프로그램은 삶의 질, 우울, 불안 등과 같은 정서의 변화, 긍정적 자기 변화를 표현하는 외상 후 성장으로 효과를 측정하였고 긍정적인 결과를 보여주었다. 통합재활프로그램은 신체적, 심리사회적 측면을 모두 향상시켰다. 향후 암생존자를 위한 재활 프로그램은 신체적, 심리사회적 측면뿐만 아니라 직업 재활 측면까지 고려되어야 하며 암생존자의 다양성에 맞추어 프로그램을 탄력적으로 적용할 수 있도록 구성되어야 한다.
연구목적 : 저자는 만성 B형 간질환 환자의 정신과학적 측면과 사회학적 측면을 관찰해 보고자 본 연구를 실시하였다. 방법 : 저자는 1966년부터 1999년까지의 Medline database에 등록된 B형 간질환에 관한 모든 문헌들을 고찰하였다. 결과 : 만성 B형 간질환 환자와 관계되는 정신과학적 측면은 섬망, 전반적인 의학적 상태에 기인하는 정신병(특히 조증), 불안, 우울, 적응장애, 알코올 남용과 의존, 및 약물남용과 의존 등이다. 만성 B형 간질환 환자와 관계되는 사회학적 측면은 간염보균자로서의 낙인문제, 감염과 연관된 죄책감, 증가된 가족들의 부담에 대한 죄책감, 대인관계의 소외와 철수, 성장애, 직업상실 및 의료진의 치료 거부등이다. 결론 : 만성 경과를 거치는 B형 간염환자의 여러 가지 정신과학적 사회학적 문제로 볼 때, 적절한 조기의 교육적 상담 개업이 필요하다고 하겠다. 적절한 교육적 상담 개입은 간염으로부터 간경화 간암으로의 진행과정에 B형 간염환자의 치료의 순응도를 높이고 나아가 악화의 과정을 예방할 수 있다.
Objectives : We examined quality of life, psychosocial adjustments to illness, changes in sexual functioning, and prevalence of psychiatric disorders in AIDS patients compared to patients with chronic hepatitis B virus infection (CHB). Methods : Thirty-one men with AIDS and 50 men with CHB were enrolled. The Short-Form 12-Item Health Survey (SF-12), the Psychosocial Adjustment to Illness Scale (PAIS), and the Changes in Sexual Functioning Questionnaire short form (CSFQ-14) were administered. Results on these assessments were compared between the 31 AIDS patients and 50 CHB patients. The Structured Clinical Interview for DSMIV (SCID) was administered to determine the psychiatric diagnosis only for the AIDS patients. Results : The Physical Component Summary score (PCS) was lower in AIDS patients than in CHB patients (p<0.001). In the section examining sexual relationships, AIDS patients exhibited a lower level of adjustment (p<0.05) and had more changes in sexual function (p<0.05) than did CHB patients. Administration of the SCID to AIDS patients indicated that the lifetime prevalence of any psychiatric disorder was 56.7% ; 43.3% for mood disorders, 33.3% for alcohol use disorders, 26.7% for anxiety disorders, and 20% for adjustment disorder. Patients who had experienced any psychiatric disorder had more severe psychosocial distress (p=0.004) and evidenced a lower level of overall psychosocial adjustment (p=0.030) than patients who had not. Conclusion : We showed that AIDS patients have a high prevalence of psychiatric disorders, and that AIDS patients with psychiatric disorders were particularly low in levels of psychosocial adjustment. Thus, careful attention should be given to psychiatric aspects of AIDS patients emphasizing the early diagnosis and treatment of psychiatric disorders.
Purpose: This study was designed to construct and test a structural equation model on sexual function in women with gynecologic cancer. Methods: The model was constructed and tested under the hypotheses that women's physical changes in sexual function after gynecologic cancer treatment did not automatically lead to sexual dysfunctions. Women's psychosocial factors were considered to be mediating variables. Two hundred twelve women with cervical, ovarian, and endometrial cancer were recruited and asked to complete a survey on their physical factors, psychosocial factors and sexual function. Data was analyzed using SPSS WIN 12.0 and Amos WIN 5.0. Results: Predictors of sexual function in the final model were sexual attitude affected by physical distress and couple's age, sexual information affected by physical distress and couple's age, depression affected by physical distress, and marital intimacy affected by physical distress. Tumor stage and time since last treatment directly affected women's sexual function without any mediating psychosocial variables. However, body image did not affect women's sexual function. Conclusion: Nursing professionals should develop a tailored educational program integrating both physical and psychosocial aspects, and apply it to women and their spouses in order to promote sexual function in women with gynecologic cancer.
Purpose: Purpose of the this study is to define the hemodialysis treatment-related stressors and stress coping methods in hemodialysis noncompliant patients. Method: Hemodialysis treatment-related stressors scale and stress coping style scale were used with 178 patients that received continuous hemodialysis treatment, met the noncompliance criteria. Collected data were analyzed by using the SPSS 12.0 program. Result: In hemodialysis noncompliant patients, Psychosocial stress were higher than physical stress, 'aesthetic thinking' was the most used. As recognition of health status was lower, hemodialysis treatment-related stressors were higher. In terms of causative disease, 'hypertension' pt used more 'problem-focus coping' and 'social support pursuit' than the other groups. As subjective recognition level of health status was higher, 'problem-focus coping' and 'social support pursuit' were more used. It was showed positive correlation between psychosocial stressors and 'aesthetic thinking'. Conclusion: Hemodialysis noncompliant patients had high psychosocial stress, accordingly, used 'aesthetic coping'. It means that they has negative coping to disease and hemodialysis treatment. Therefore, Nurses need to manage psychosocial aspects, which influence on noncompliant behaviors.
The main purposes of this study were to investigate the content of health behaviors and to examine factors influencing the health behaviors of the elderly. Data regarding the health behavior of 126 people over 65 years of age living in community settings were used. All subjects were interviewed using a semi-structured questionnaire. The questionnaire consisted of sociodemographic variables, health behaviors, present chronic disease, subjective health status, a scale for worthy life, a scale for self-esteem, and a scale for loneliness. Health behaviors included 27questions on diet, exercise, stress-coping, smoking, drinking, a regular medical check-up, social activities, etc. The data analysis procedure included stepwise regression using health behavior as the dependent variable, and sociodemographics, illness, and psychosocial variables as independent variables. Stepwise regression revealed that factors such as feelings of worthy life($\beta$=-0.350, p<0.0001), communicating with otjers or the lack there of ($\beta$=0.183, p<0.05), and self-esteem($\beta$=0.196, p<0.05) were independently and significantly associated with health behaviors. For example, individuals who showed higher levels of worthy life and who had confiding relationships with others tended to practice more health behaviors. Subjects who had a higher level of self-esteem showed the same tendency. These results suggest the necessity of a intervention that considers psychosocial aspects should be included in care of the elderly so as to promote positive health behavior.
This review article explores the psychological characteristics, comorbid mental disorders, and psychosocial assessments throughout the solid organ transplant journey, spanning the pre-transplant, peri-transplant, and post-transplant phases for transplant recipients. The psychological burden and anxiety in the pre-transplant phase are high for organ failure patients with complex physical difficulties who are deciding to undergo transplantation and are on the waiting list. The pre-transplant psychosocial evaluation covers various aspects, including the patient's readiness, awareness of, and commitment to transplant treatment, medical compliance, psychopathological conditions such as cognitive function and personality disorders, lifestyle factors, including substance abuse, as well as various psychosocial factors like social support. During the peri-transplant phase, mental health problems such as postoperative delirium should be carefully recognized and addressed. After transplantation, it is essential to assist patients in coping with the various stressful experiences they encounter, manage psychiatric symptoms such as depression, anxiety, and insomnia, and improve treatment adherence and quality of life during long-term care for the transplanted organ. Managing psychiatric problems in post-transplant patients requires a deep understanding of immunosuppressant medications and a keen awareness of associated risks, including adverse effects and potential drug interactions. This comprehensive review emphasizes the significance of proactive mental health care and psychosocial evaluation, highlighting the necessity of a multidisciplinary approach to enhance the quality of life and overall success of transplant patients throughout all phases of transplantation.

Subjects and Methods: As participant, 67 children with CI attending mainstream schools were included. A survey was conducted using a structured questionnaire on academic performance in the native language, second language, mathematics, social studies, science, art, communication skills, self-esteem, and social relations. Additionally, auditory and speech performances on the last follow-up were reviewed retrospectively. Results: Most implanted children attending mainstream school appeared to have positive self-esteem and confidence, and had little difficulty in conversing in a quiet classroom. Also, half of the implanted children (38/67) scored above average in general academic achievement. However, academic achievement in the second language (English), social studies, and science were usually poorer than general academic achievement. Furthermore, half of the implanted children had difficulty in understanding the class content (30/67) or conversing with peers in a noisy classroom (32/67). These difficulties were significantly associated with poor speech perception. Conclusions: Improving the listening environment for implanted children attending mainstream schools is necessary.

유방암은 진단과 치료과정에서 환자뿐만 아니라 배우자에게 심리사회적 디스트레스를 초래하게 된다. 이러한 배우자의 디스트레스는 다시 유방암환자의 적응, 삶의 질에 영향을 미치게 된다. 이에 따라 부부관계 맥락에서 유방암의 심리사회적 영향을 살펴보아야할 필요성이 제기된다. 이를 위해 유방암환자와 배우자를 대상으로 심리사회적 영향을 조사한 국외 연구 33편, 국내 연구 1편 총 34편을 방법론적 측면과 내용적 측면으로 분석하였다. 분석 결과를 토대로 후속 연구와 임상적 실천에 있어서의 제언점을 제시하였다.
