Purpose: This study was performed to systematically review the recently published nursing intervention studies. Methods: The literature was identified through the Korean Education and Research Information Service (KERIS), the Korean Information Service System (KISS), and National Assembly Library websites. Key words such as breast cancer, nursing, and intervention were used. The factors analyzed are as follows: 1) the characteristics of studies and study populations, 2) the classification of interventions, 3) outcome indicators and their effects, and 4) effective interventions. Results: Thirty two studies were included. Seventeen studies used a single intervention such as aerobic dance, TaiChi, foot massage, aromatherapy, or a stress-reduction method. Fifteen studies used combined interventions, including education, exercise, counseling, support, yoga or meditation. The data on 47 outcome indicators and their effects were segregated into psycho/spiritual outcomes, stress coping, physical outcomes, cardiorespiratory function, symptom management, arm and shoulder functions, fatigue, and quality of life. Some interventions had positive effects on stress, fatigue, and functions of shoulder. Conclusion: Various interventions are available for breast cancer patients, and some have had positive effects. However, more studies are required to develop evidence-based practice guidelines for nursing interventions.
Background: Social support is an important factor in psycho-social well-being of cancer survivors. There is little information about level of social support and its predictors among cancer survivors in Iran or other Middle Eastern countries. The aims of present study were to determine the social support and its prediction factors among Iranian cancer survivors. Materials and Methods: In this descriptive-correlational study 187 cancer patients in one educational center and one private oncology office in northwest of Iran participated using a convenient sampling method. The data collection tool consisted of a researcher-prepared checklist and the Multidimensional Scale of Perceived Social Support Assessment (MSPSS). Data analysis was performed using SPSS statistical software with descriptive statistics and multiple linear regression analysis. Results: The total score of MSPSS was 68 from a possible score between 7 and 84. Participants believed that they received a high level of support from their family members and significant others. Multiple linear regression analysis showed that single and depressed cancer survivors and participants with lower levels of physical activity believed that they received lower levels of social support. Conclusions: Iranian cancer survivors receive high levels of social support and family members are the most important source of this support. In planning any supportive care program for Iranian cancer survivors this strength should be considered. Especially, single and depressed and patients with lower levels of physical activity need more attention.
End-of-life patients experience physical, mental, social, and existential distress. While medical personnel provide medication and care to alleviate patients' distress, listening to and interacting with patients remains essential for understanding their psychological condition. The most important tool, though difficult to implement in practice, is end-of-life discussion (EOLD). EOLD has been shown to have positive effects on end-of-life treatment choices, achievement of patients' life goals, improvements in the quality of life of patients and their families, and the prevention of depression and complicated grief among bereaved family members. EOLD is not often undertaken in clinical practice, however, due to hesitancy among medical personnel and patients for various reasons. In order to conduct an EOLD, the patient's judgment, psychiatric illnesses such as delirium and depression, and psychological issues such as the side effects of psychotropic drugs, denial, and collusion must be evaluated. Open and honest conversation, treatment goal setting, the doctor's familiarity with the patient's background, and attentiveness when providing information are important elements for any dialogue. Meaning-centered psychotherapy was developed to alleviate the existential distress of cancer patients, and its application may promote EOLD. The future development of meaning-centered psychotherapy in practice and in research is expected to further promote EOLD.
목적: 본 연구는 청소년 후기 말기 암 환자의 전반적인 경험을 이해하고 총체적인 고통을 경감하도록 돕는 정서적, 영적 돌봄 프로그램인 CD 프로그램을 개발하기 위한 것이다. 방법: 2006년 11월 1일부터 2007년 10월 30일까지 Keller와 Song의 ARCS이론과 Kang의 멀티미디어 코스웨어 설계 과정을 응용하여 플래쉬 도구를 활용한 CD 프로그램을 개발하였으며, 개발된 의미요법 프로그램명은 "청소년 후기 암 환자를 위한 "내 삶의 보물찾기""이었다. 프로그램 개발은 <계획>, <설계>, <개발>-CD프로그램, 사용자(간호사)용 교육지침서 및 학습자용 교육용 책자, <평가>의 4단계를 통해 이루어졌다. 결과: 본 연구에서 개발된 의미요법 CD 프로그램은 총 5개의 비밀을 찾는 과정으로 교육시간은 각 차시별로 $15{\sim}20$분으로 구성되었으며. "비밀"로 명명된 각 차시별 구성은 의미요법에 대한 프로그램의 제작목적을 알려주는 <처음>과 6컷 만화를 통한 동기유발을 유도하기 위한 <마음열기>, 각 차시별 교육목적과 의미요법 교육이 진행되는 영상물 상영 <알아보기>, 교육 내용에 대한 카타르시스를 위한 <웃음 송>, 교육내용을 자신에게 직접 적용해 보는 <경험하기>, 메인화면으로 돌아갈 수 있는 <끝>의 6단계로 이루어졌으며, 교육용 소책자(의미노트)를 제공하여 반복학습이 이루어질 수 있도록 하였다. 본 프로그램은 청소년 말기 암환자가 있는 임상현장인 병원학교 혹은 지역사회 어느 곳이든지 컴퓨터가 가능한 곳에서는 사용할 수 있도록 CD로 만들어졌으며, 프로그램은 신규 간호사도 사용하기 편리하게 자동과 수동 양방향의 기능을 가지고 매체조작이 용이하도록 제작되었다. 교육지침서는 전체 내용을 손쉽게 파악할 수 있도록 되어 있고, <웃음 송>에 대한 저작권 등록은 완료된 상태로 전국의 소아 청소년 종양병동에서 무료로 사용할 수 있도록 하였다. 결론: 본 프로그램은 간호사들이 교육에 대한 큰 부담없이 의미요법 프로그램을 쉽게 진행할 수 있도록 제작되어 있어 임상에서 청소년 후기 말기 암 환자의 정서적, 영적 간호중재 방법으로 유용하다고 판단된다. 또한 말기 암환자 뿐 아니라 청소년 후기에 만성 및 불치의 질병으로 고통을 받는 대상자에게도 적용할 수 있다고 생각된다.
지난 10년 동안 암 관리법에 의한 국가 정책에 따라 일본에서는 호스피스 및 완화의료가 급속히 발전해왔다. 완화의료 시설 및 입원 환자의 호스피스, 병원의 완화 의료팀, 가정 호스피스 기능을 갖춘 진료소의 수는 꾸준히 증가하고 있으며 국가 협회로부터 완화의료 전문가로 공인된 의사, 간호사, 약사의 수도 증가하고 있다. 학부, 대학원 및 계속적인 의학 교육을 통해서 교육프로그램 및 교육의 기회를 표준화하고 보급하기 위한 공동 노력이 이루어졌다. 일본의 연구 활동은 말기 섬망, 말기 탈수증, 완화적 진정, 임종 환자의 의료, 예후, 의사 소통, 정신 종양학, 지역적인 완화의료 프로그램의 분야에서 현저하게 기여하였다. 이 보고서에서는 일본에서의 주요 완화의료 환경, 특성, 국가 협회, 교육, 완화의료 연구에 초점을 맞추었다.
연구목적 유방암 환자가 치료 초기에 경험하는 디스트레스는 유방암 생존기까지 지속되는 만성 디스트레스의 위험요소로 알려져 있다. 본 연구는 유방암 첫 치료로써 수술을 받은 환자를 대상으로 디스트레스 온도계(distress thermometer, DT)를 이용하여 디스트레스의 정도와 이와 연관된 인자를 조사하는 것이다. 방 법 2013년 4월 1일부터 2017년 3월 31일 사이에 경북대학교병원에서 유방암 첫 치료로써 수술을 받고 일주일 이내에 있는 환자를 대상으로 하였다. DT로 디스트레스의 정도를 측정하였고 한국판 세계보건기구 삶의 질척도 단축형(Korean version of the World Health Oganization Quality of Life Scale Abbreviated Version, WHOQOL-BREF)으로 삶의 질을 측정하였다. 그 외 기본 인구학적 자료, 암 및 암 치료 관련 자료를 수집하였다. DT 점수 4점 이상인 대상자를 디스트레스 군으로 분류하였다. 디스트레군과 연관된 인자를 알기 위하여 디스트레스군과 비-디스트레스 군 사이의 변수 차이를 단변수 분석(univariable analysis) 하였다. 로지스틱 회귀분석을 시행하여 디스트레스 군을 예측할 수 있는 인자를 조사하였다. 결 과 대상자는 총 307명이었다. 이 중에서 DT에 응답하지 않은 39명, WHOQOL-BREF에서 6개 항목 이상 답을 하지 않아서 채점을 할 수 없었던 4명을 제외한 264명이 분석 대상자였다. 65.5%(n=173)가 디스트레스 군으로 분류되었다. 단변수 분석(univariable analysis)에서 디스트레스 군이 비-디스트레스 군과 비교하여 더 젊었으며(p=0.045), 배우자와 동거하지 않는 경우가 더 많았다(p=0.032). 삶의 질 영역에서 디스트레스 군이 전반적 삶의 질(p=0.009), 전반적 건강상태(p=0.005), 신체건강영역 p<0.001), 심리건강영역(p=0.002)에서 더 나쁜 삶의 질을 보였다. 로지스틱 회귀분석에서 60세 이상의 환자에 비하여 40-49세의 환자가 디스트레스에 이환 될 확률이 약 3배 높았고[OR=2.992, 95% confidence intervals (CI) 1.241-7.215], WHOQOL-BREF영역 중 신체건강영역이 디스트레스를 예측할 수 있는 인자로 나타났다(OR=0.777, 95% CI 0.692-0.873). 결 론 유방암 수술 치료 초기단계에서부터 상당한 수의 환자가 유의한 수준의 디스트레스를 경험하고 있다. 특히 생산성이 왕성한 삶의 주기에 있는 40대의 환자가 60대 이상의 환자에 비하여 수술 후 일주일 이내에 겪는 심리적 고통이 심할 가능성이 높다. 이러한 환자들에게서 수술 초기부터 신체적 삶의 질과 연관된 영역(예 : 통증, 불면, 피로 등)에 대해 평가하고 개입하는 것이 디스트레스 수준을 경감시킬 가능성이 있다.
