정신과에 의뢰된 내과계 환자들에 대한 치료적 접근 - 정신사회적 측면 - (Therapeutic Approaches to the Patients Who were Referred for Psychiatric Consultation from Medical Departments - Psychosocial Aspects in Consultation-liaison Psychiatry -)

  • To become an effective and successful consultation-liasion psychiatrist the psychiatrist should consider two aspects of consultation before he/she meets his/her patients. First the psychiatrist should understand the internal need and psychological state of patients who visited their physician before being refered to a psychiatrist So psychiatrists should be careful in the evaluation of the patient's intention whether they are willing to accept the psychiatric treatment approach or not Second the psychiatrist should understand the situation and the physician's internal need in the consultation. Psychiatrists should recognize whether there are any factors which interfere with the mutual understanding and cooperation between physicians and psychiatrist.

암 환자의 정신사회적 치료 (Psychosocial Management of Cancer Patients)

  • The author presented several points of experienced materials obtained from the joint meetings of consultation-liaison Psychiatric division with oncologic department in Kyung Hee University Hospital. The joint meetings which have been held regularly every other week were very helpful not only for the psychiatrist but also for the cancer department stags to manage the cancer patients actively. The joint meetings have been progressed into more cohesive and active communication as time goes by. Most of the subjects discussed in the meeting was focused on the psychological and behavioral problems of the cancer patients. Besides, the difficulties arising from the chemotherapy were discussed. Probably the most difficult task for the treatment team was to provide complete information while respecting the patient's right to deny their situation The liaison psychiatrist usually gave comments to the charge doctor and/or nusing staff how to evaluate the patient's behavior and what would be the influential factors in developing the doctor-patient relationship. It was found that many cancer patients and their family members had their own peculiar illness behavior and disease concept which led patients to take non-medical or moreover, anti-therapeutic care. The family members were found to play an influential role in the choice of treatment method and progression of the disease. Another role of the liaison psychiatrist in the oncology ward was to encourage the treatment team members. In the practical point of view, it was not easy for the liaison psychiatrist to have time regularly for the cancer patients to encourage the liaison activities. And it seems to be fundamental that the consultation fee for the liaison psychiatrist should be set up at the resonable level.

서울시 50대 정신건강검진사업에 대한 웹기반 조사 (Web-Based Survey on the Mental Health Screening of the Baby-Boomer Generation)

  • Objective : A pilot project entitled '50s mental health screening project' was conducted in Seoul. As a part of this project, we conducted surveys to obtain the opinions of psychiatrists regarding mental health screenings. Methods : A questionnaire was mailed to members registered with the Korean Neuropsychiatric Association, which enquired about the '50s mental health screening project'. Results: A majority (90.9%) of the subjects agreed on the purpose and implementation of the project. However, there were fewer individuals that actually showed the intent to participate (65.8%). The mean age of the group with the intention to participate was higher compared to the group with no intention to participate. The factors that negatively affected the intent to participate included prejudice and discrimination against psychiatry (3.795), low health insurance reimbursement (3.784), and inconvenience of reporting to public health centers (3.664). The most appropriate method that scored the highest for screening mental health was face-to-face consultation with a psychiatrist (3.889). Most of the participants agreed on the method of a self-reported survey along with an interview of psychiatrist (84.9%). Conclusion : Subjects were concerned about prejudice and discrimination against psychiatry, low health insurance reimbursement, and inconvenience of reporting to public health centers. These problems need to be supplemented. Face-to-face consultation with a psychiatrist may be considered an appropriate method for future mental health screening.

정신과 입원환자의 행동변화에 영향을 주는 요소에 관한 연구 (A Study of the Factor on Behavioral Change of the Psychiatric in-patient)

  • This article examined relationships between selected variables, such as demographic background, care, treatment variables, environmental characteristics, and patient's daily behavior and mood change. Relationship were determined between independent variabltherapeutic-rapeutie approach, demographic data, environmental management approach-,and dependent variable-patient's daily behavioral and mood change. 35 patients selected within some criteria in a psychiatric ward, were obserbed during 5 weeks by use of Wyatt's Behavior & Mood Rating Scale ac-cording to the object of the study. At the same time, the frequence of the care and treatment were collected. Criteria for sample selection and independent variables as an influential factor to the patient behavioral change, based on a literature revienw and clinical experiences. Pearson's correlation and multiple regression analysis were used to determine the influfntial factors to the patient behavioral change. Systematic reading (r=.8324), Psychiatrist's individual interview (r=.5764), tranquilizer (r=.3441) and hospitalization processing date (r=.4143) were related with patient's behavioral change. That is these 4 variables can be said to influence to the patient's behavior and mood. A stepwise multiple regression analysis of the effect of the independent varibles of systematic reading, psychintrists individual interview, tranquilizer and hospitalization processing date on the dependent variable, patient's behavioral change was carried out. Systematic reading with on R²of. 69 revealed to be the main influential factor to the patient's behavior and mood change, as the next factor psychiatrist individual interview. A total inclusion of these factors revealed a 73% prediction for the patient's behavior and mood change. But the most influential factor was the interaction of the systematic reading and psychiatrist's individual interview.

  • Music therapy had been widely used in the area of psychiatry. However recently, participation of psychiatrist has been decreased, due to recent trends of emphasizing biological psychiatry and rapid effectiveness of pharmacotherapy. A 46 years old male PTSD patient was admitted to psychiatric ward. He has suffered from PTSD symptoms, such as intrusive distressing recollection of his past traffic accident, social avoidance, insomnia and depression. Music therapy, which is conducted by a psychiatrist and a music therapist, was applied. This case suggests the positive role of music therapy in treatment of PTSD and emphasizes the importance of participation of psychiatrist, as an organizer during music therapy.

정신과 의사와 비정신과 의사의 항우울제 처방에 대한 연구 : 건강보험심사평가원 청구 데이터 중심으로 (The Epidemiology of Antidepressant Prescriptions in South Korea from the Viewpoint of Medical Providers : A Nationwide Register-Based Study)

  • Antidepressants are widely used to treat depression in Korea, however, only a few studies have focused on the provider of the treatment. The aim of the study is to compare the differences between patients who were prescribed antidepressants by psychiatrists and those who were prescribed antidepressants by non-psychiatrists in South Korea. Patients with a diagnosis of depressive disorder who had been newly prescribed antidepressants in 2012 were selected from the Health Insurance Review and Assessment Service database. They were classified into two groups depending on whether they received the antidepressant prescription from a psychiatrist or non-psychiatrist. Sociodemographic, clinical, and depression related cost has been investigated. Treatment resistant depression, which is defined as a failure of two antidepressant regimens to alleviate symptoms, was also investigated. Prescription adequacy was assessed based on whether a regimen was maintained for at least 4 weeks. Among the 834694 patients with pharmaceutically treated depression (PTD) examined in this study, 326122 (39.1%) were treated by psychiatrists. Patients who were treated by psychiatrists were younger and had more psychiatric comorbidities than those treated by non-psychiatrists. They had longer PTD duration (229.3 days vs. 103.0 days, p < 0.05) and a larger proportion of treatment resistant depression (9.3% of PTD) when compared to those patients treated by non-psychiatrists. The patients treated by psychiatrists had a smaller proportion of inadequate antidepressant use compared to those patients in the non-psychiatrist group (44.5% vs. 65.1%, p < 0.05). The costs related to depression corrected with PTD duration were higher in the non-psychiatrist group (32214 won vs. 56001 won, p < 0.05). Patients who receive antidepressants from psychiatrists are patients with more severe, treatment-resistant depression. Psychiatrists prescribe antidepressants more adequately and cost- effectively than non-psychiatrists.

한국에서 우울증의 비약물학적 치료의 현황과 요구도 (The Current Status and Requirements for Non-pharmacological Treatment of Depression in Korea)

  • Objectives : As a part of plan to develop evidence-based treatment guidelines for depression that is more suitable for Korean situation, we investigate the present status and client's requirements for non-pharmacological treatment of depression in Korean clinical situation. Methods : Subjects were patients with depression in 12 university hospitals which are located in metropolises in Korea. We analyzed the records from questionnaires about current clinical status and requirements for the non-pharmacological treatment of depression in Korea. Results : 50.8% of the subjects have experienced non-pharmacological treatments for their depression. The preference of non-pharmacological treatment method of depression is exercise/interesting activity, counseling by psychiatrists and psychotherapy, and the best effective treatment method is psychotherapy (Es=4.36). Actually, the mean consultation time by psychiatrist is $11.31{\pm}7.16$ min, and the appropriate consultation time for client's situation is $18.39{\pm}8.95$ min. During consultation, patients' satisfaction measurement for psychiatrist's explanation about pharmacological treatment is $64.17{\pm}27.11$, and satisfaction measurement for psychiatrist's counseling for their depression about personal problems, resent stress, interpersonal relationship is $61.66{\pm}26.63$. Conclusion : In Korea, many psychiatrists offered biologically oriented treatment to their patients with depression, and patients' satisfaction measurement about consultation by psychiatrists is low. Many patients wanted to combined pharmacological and non-pharmacological treatment for their depression, and aspired to information about complementary and self-help treatment methods. It is necessary to develop non-pharmacological treatment guideline for depression which reflect the clinical situation in Korea and meet Korean patients' need.

성형수술의 정신신체의학적 접근 (Psychosomatic Approach to Plastic Surgery)

  • 외모의 이상은 환자에게 정신적 손상 및 사회생활적응에 문제를 일으킬 수 있으므로 성형외과의사의 역할은 단순한 외형의 교정에 그치는 것이 아니라 그 환자의 삶의 질(quality of life)을 높이는 차원에서 환자의 정신적, 정서적 손상을 동시에 교정하여 사회생활에 보다 잘 적응하도록 하는 것이다. 성형수술 받는 환자는 다른 수술 받는 환자에 비해서 수술결과에 대한 기대수준이 높으므로, 의사는 수술 전에 충분한 설명을 해 주어야 하며 수술 후에도 의사 환자 관계를 계속 잘 유지하는 것이 필요하다. 수술결과에 불만족 하는 경우 다른 성형외과의사의 의견(second opinion)을 들어보고, 그래도 불만족 하는 경우 정신과의사의 자문을 받는 것이 순서이다.

정신건강에 대한 교사와 일반인의 태도 조사연구 (A Comparative Study of Attitude of Teachers and General Public Toward Mental Health)

  • In an attempt to recognize attitudes of teachers and general public toward mental health, III teachers of middle and high school and 130 general public were selected for this study from September 6, 1975 to October 18, 1975. Date were collected through "Mental Health Questionnaire"designed by Nounally. Results of the study were as follows : 1. The selected teacher group and general public group revealed almost similar altitudes toward psychotic patients, that is, positive aspects for employment problems and relationship toward other persons. 2. Both teacher group and general public group revealed the same attitudes toward neurotic patients, felt relatively more reliance on them, and showed much more positive attitudes toward neurotic patients than psychotic patients. 3. Both groups indicated that, for the security of mental health, It is best way to show him the right path and make sure that he follows it. As to mental health, most of the selective subjects consulted with psychiatrist about the problems and obtained the information through books. 4. Both teacher group and general public group identified mental illness being equal in seriousness to alcoholism or cardiac disease. In case of a close relative living in their home having mental disease, most of them responded to be glad to help him. 5. Both teacher group and general public group revealed intensively negative attitudes toward mental hospital in order of despair. danger, and filth, while positive attitudes toward psychiatrist.

The Psychiatrist and the Revolutionary: Frantz Fanon's Critique of Colonial Discourse

  • This article offers a reflection on Frantz Fanon's diagnosis and analysis of French colonialism in Algeria. We will attempt to demonstrate that there is a concrete and clear connection between Fanon as the psychiatrist diagnosing the devastating effects of the French colonial system, and his subsequent political involvement in the Algerian revolution. This is not to say that each part does not contain valuable insights in their own rights, but rather to stress that without being read together, as a whole, one would miss a significant element in the understanding of the importance Fanon's thought subsequently came to play in the emancipation struggles of the colonized worldwide. Furthermore, we argue that it is crucial to understand the intimate connection between Fanon's psychiatric work, his diagnosis of colonial mental disorders, as well as diagnosis of the colonial system as such, and then his political engagement, in order to understand the particular context in which he favourably discusses the use of violence in the name of fighting against the oppressive system of colonialism. Above all, we argue that Fanon's critique of colonialism continues to spark controversy because it still represents the most powerful and incisive analysis of, as well as answer to, the troubled relationship between the blessed and the wretched of the earth.
