매복된 상악 중절치의 교정적 처치를 통한 치험례 (TREATMENT OF IMPACTED MAXILLARY CENTRAL INCISORS USING ORTHODONTIC TRACTIONS)

  • 매복은 임상 검사나 방사선 사진 검사 상에서 인식 가능한 맹출 경로 상에 장애물이나 이소 위치에 의한 치아 맹출의 정지를 의미한다. 상악 중절치의 치아 매복의 원인으로는 과잉치, 치아종, 치배의 이소위치, 외상 후 연조직 치유에 의한 폐쇄, 만곡치, 정지된 치근 발육 등이 있다. 매복된 치아는 인접치의 근심이동으로 인한 공간 상실, 부정교합, 인접치의 치근 흡수, 낭종의 형성 등을 유발할 수 있으므로 맹출장애에 대한 주의 깊은 관찰과 조기 진단이 중요하며 적절한 치료가 시행되어야 한다. 매복된 치아의 치료방법으로는 유치의 발거나 외과적 노출을 통한 맹출 유도, 경과관찰, 외과적 방법이나 교정력을 이용한 매복치의 재위치, 매복치의 외과적 발거 등이 있다. 이 중 교정적 견인은 매복된 치아가 원인을 제거한 후 또는 외과적으로 치관을 노출시킨 후에 일정기간의 관찰에도 맹출하지 않거나, 맹출경로가 정상에서 많이 벗어나 맹출이 기대되지 않을 경우 적용될 수 있다. 본 증례는 매복된 상악중절치를 교정적 견인을 통해 정상적인 맹출 방향으로 유도한 치험례들로 양호한 치료 결과를 보였기에 이를 보고하고자 하는 바이다.

  • 성공적인 상악전치부의 임플란트 식립은 많은 고려를 필요로 한다. 환자를 만족시키며 기능적이며 심미적인 보철수복을 위해서는, 치료에 앞서 정확한 진단이 선행되어야 하며 이는 구내스캔, 콘빔시티, 그리고 안면스캔을 중첩함으로 시도해 볼 수 있다. 구내스캔을 통해 얻어지는 환자의 교합양상과 콘빔시티에서 얻어지는 환자의 치조골의 양상 및 질, 그리고 안면스캔에서 얻어지는 환자의 특성과 구외평가를 어우르는 진단은 보다 성공적인 보철수복을 위한 발판이 될 수 있다. 다양한 정보의 중첩을 통해, 보철적으로 이상적인 치관의 위치를 우선순위에 두지만, 환자의 생물학적 한계를 포용하는 "탑다운"형식의 임플란트 진단은, 이를 고려한 서지컬 가이드를 3D 프린팅하고, 수술에 사용함으로써 구현되었다. 환자의 치조골 소실로 인한 임플란트 식립위치의 한계점은, 치아의 선각과 착시효과를 이용한 보철수복을 통하여 극복하려고 노력하였다. 이후, 임시수복물을 사용하고 환자가 가장 편하다고 느끼는 보철물을 장착한 뒤 디지털 교합장비(Arcus Digma II)를 이용하여 전방유도와 하악의 운동을 측정하여 이를 최종보철물에 적용하였다.

고교 남자 역도 선수의 척측 측부인대 초음파 검사 (Ultrasonographic Assessment of the Ulnar Collateral Ligament in High School Male Weight Lifters)

  • 목적: 남자 고교 역도 선수에게서 나타나는 주관절 부위의 변화와 내측 측부 인대 손상의 정도 및 빈도를 초음파 검사를 이용하여 확인하고자 하였다. 대상 및 방법: 평균연령 16.8세(범위 16세-18세) 고교 남자 역도 선수 15명(실험군, Group I) 및 평균 연령 16.7세(범위 16-17세)인 대조군(Group II) 9명을 대상으로 하여 주관절 부위 병력 청취, 신체검사, 단순 주관절 방사선 촬영, 외반 부하 방사선 검사 및 초음파 검사를 통하여 관절 주위 변화를 관찰하였다. 결과: 단순 방사선 촬영상 내측 관절 간격은 양군 간에 유의한 차이가 없었으나(3.6 mm, 2.7 mm; p>0.05), 외반 부하 방사선 사진상 실험군이 대조군에 비해 유의하게 증가 되었다(우측 평균 5.86 mm 및 3.52 mm, p<0.01, 좌측 평균 5.33 mm 및 3.64 mm, p<0.01). 초음파 검사상 실험군에서 내측 관절 간격이 유의하게 증가 되어 있었고(우측 평균 4.66 mm 및 3.29 mm, p<0.01, 좌측 평균 8 mm 및 3.38 mm, p<0.01), 척골 근위부가 유의하게 외측 이동되어 있었으며(우측 평균 0.73 mm 및 1.43 mm, p<0.01, 좌측 평균 0.96 mm 및 1.53 mm, p<0.05), 주관절 각변형 또한 실험군에서(우측 A형 6례, B형 9례, 좌측 A형 9례, B형 6례), 대조군(우측 A형 9례와 좌측 A형 9례)에 비해 유의하게 나타났다. 실험군에서 운동경력(p>0.1) 및 우세팔 여부(p>0.05)에 따른 유의한 차이를 보이지는 않았다. 결론: 남자 고교 역도 선수들의 주관절 내측 이완성의 빈도가 높았으며, 동적 초음파 검사를 이용시 보다 효과적인 평가가 가능할 것으로 생각되었다.

돌가자미 Kareius bicoloratus의 정자형성과정 중 생식세포, Cyst 상피세포 및 간질세포의 미세구조 (Ultrastructure of Germ Cells, Cyst Epithelial Cells and Interstitial Cells during Spermatogenesis of the Stone Flounder, Kareius bicoloratus)

  • 한국 서해안에서 채집한 돌가자미 Kareius bicoloatus의 정자형성과정 중 생식세포, Cyst 상피세포 및 간질세포들의 미세구조를 전자현미경 관찰에 의해 조사하였다. 제1차 정모세포에서 연접사복합체(synaptonemal complex)들이 성숙분열 중 전기의 쌍사기에 출현하였다. 특히, 감수분열을 위한 테스토스테론 호르몬을 분비하는 세포로 알려진 간질세포(Leydig cell)들은 제1차 정모세포 및 정세포 가까이에서 뚜렷하게 출현하였다. 포상핵을 가지는 이들 간질세포들은 소엽 간 간질조직 내의 섬유모세포들에 의해 둘러싸여 출현하였다. 특히, 이들 세포들은 타원형 또는 구상의 미토콘드리아들을 가지는데, 미토콘드리아들은 관상 크리스테(cristae)를 갖는 것이 특징이다. 또한, 활면소포체와 여러 개의 공포들도 세포질 내에서 출현하였다. 성장 발달 중인 정소 내에서 정소소엽 간 간질조직에서 나타나는 잘 발달된 간질세포들(스테로이드호르몬분비세포)은 포상의 핵과 관상 크리스테를 갖는 미토콘드리아를 가지며, 활면소체를 갖는 형태적 특징을 보였다. 정자형성과정 중, 제1차, 제2차 정모세포들은 cyst 상피세포 (Sertoli cell)들에 인접하거나 부착하여 나타났다. cyst 상피세포들의 핵은 길게 신장된 타원형 또는 삼각형의 핵을 가지며 세포질 내에 여러 개의 미토콘드리아가 출현하였다. 성장 발달 중인 정소 내에서 제2차 정모세포들과 정세포 가까이에 위치한 cyst 상피세포의 세포질 내에는 지방적, 미토콘드리아 로제트(mitochondeial rosettes)들과 글리코겐 입자들이 나타났다. 특히, 후기 성장 중인 정소 내에서 cyst 상피세포의 세포질 내에는 미토콘드리아, 소포체, 작은 지방적 및 다량의 글리코겐입자들이 출현하였다. 정세포 발달(정자변태) 후기에 기부중심립이 핵막에 부착하며, 원위중심립은 편모의 기저체를 형성하고, 편모축사를 형성하였다. 정자 첨체는 다른 경골어류에서와 같이 첨체를 형성하지 않았다. 정자 두부의 길이는 대략 $3{\mu}m$ 정도이며, 미부길이는 약 $30{\mu}m$ 정도이다. 정자 미부편모의 축사(axoneme)는 주변에 9쌍의 주변이중미세관 (nine outer doublet microtubule)이 있고 중앙에 두 개의 중앙미세관(two centrial singlet microtubules)으로 구성되어 9+2구조를 나타내었다. 본 종의 정자는 체외수정 종들이 갖는 두 개의 axonemal lateral fin을 가진다. 본 연구에서 여러 단계의 생식세포들 가까이에서 출현하는 cyst 상피세포(Sertoli cell)들은 영양공급, 식세포작용, 스테로이드형성의 3가지 기능을 나타내었다. 특히, 정소 발달단계 중 퇴화 회복기에 출현하는 cyst 상피세포들의 핵은 배정 후에는 모양이 불규칙하게 나타나며, 식세포작용을 나타내는 여러 특징들이 cyst 상피세포의 세포질 내에서 나타나고 있어, cyst 상피세포들은 배정 후 방출되지 않은 생식세포들의 퇴화 흡수에 관여하는 것으로 추정된다.

강화재의 사용 방법이 복합 레진 인레이 브릿지의 파괴 강도에 미치는 영향 (The effect of reinforcing methods on fracture strength of composite inlay bridge)

  • 본 연구는 복합레진 인레이 브릿지에서 강화재의 표면 처리와 사용 방법이 파괴 강도에 미치는 영향을 평가하였다. 본 연구에서 사용한 강화재료는 I Beam, U Beam, 1 + U Beam이었으며, 표면처리 방법은 Silane, Sandblast, Hole형성 (U beam)이었다. 강화 재료의 구성과 표면 처리 방법에 따라 총 11개의 실험군을 설정하였다. 상악 인공치 모형에서 제2소구치의 발거 상태를 가정하고 복합레진 인레이 브릿지 제작을 위하여 인접한 제1소구치에 DO, 제1대구치에 MO 와동을 형성하였다. 와동이 형성된 인공치 모형을 고무 인상체를 이용하여 석고로 제작하고, 각 실험군 별로 강화재료와 강화 재료의 표면 처리 방법에 따라 Tescera ATL (BISCO Inc. IL, USA) 복합레진을 사용하여 복합레진 인레이 브릿지를 제작하였다 그 후 시편을 복제모형에 인산아연시멘트로 합착하고 Universal testing machine (EZ Test, Shimadzu, Japan)을 이용하여 flexural stress를 가하여 파괴 강도를 측정하였으며 95% 유의 수준에서 one-way ANOVA/ Scheffes post-hoc test를 시행하여 통계 분석하였다. 다음과 같은 결론을 얻을 수 있었다. 1. 내부 강화재 I beam을 사용한 실험군이 유의성 있게 높은 파괴 강도 값을 보였다 (P<0.05). 2. 표면 처리 방법에 따른 차이는 나타나지 않았다 (P>0.05). 3. 복합레진 인레이 브릿지의 파괴는 강화 재료를 사용 시에는 복합레진과 강화 재료간에 분리 파괴가 나타났으며 사용하지 않은 경우에는 수직파괴 경향이 나타났다. 4. U beam에 유지 hole을 형성한 경우 파괴 강도 증가를 시키지 않았다.

High Resolution HC$_3$N Observations toward the Central Region of Sagittarius B2

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500,550,570 and 610nm) for blood glucose strip. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100,300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. Our department has developed an information system for routine hematology using down-sized computer system. We have used an IBM 486 compatible PC with 16MB main memory, 210 MB hard disk drive, 9 RS-232C port and 24 pin dot printer. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer. The frame and leaflet of the polymer valve were made from polyurethane, To evaluate the hemodynamic performance of the polymer valve a comparative study of flow dynamics past a polymer valve and a St. Jude Medical prosthetic valve under physiological pulsatile flow conditions in vitro was made. Comparisons between the valves were made on the transvalvular pressure drop, regurgitation volume and maximum valve opening area. The polymer valve showed smaller regurgitation volume and transvalvular pressure drop compared to the mechanical valve at higher heart rate.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm.
