승모판막 재건술;인공판륜[prosthetic ring]을 이용한 수술례 (Mitral Valve Reconstruction; Result of Operation Using Prosthetic Ring)

  • Among 25 consecutive cases having undergone mitral valve surgery between March 1991 and June 1992 in Gill General Hospital, 11 patients[44%] who had undergone mitral valve reconstruction using prosthetic rings is evaluated and presented. Patients` mean age is 43 + 19 years[range:16-72], and they are consisted with 4 males and 7 females. Mitral valve insufficiency is due to degenerative disease in 6 cases[55%] and rheumatic disease in 5 patients[45%]. Carpentier`s functional classification I is 2 cases, II is 6 cases, and III is 2 cases. Surgical techniques include prosthetic ring annuloplasty[11 patients, 100%], chorda shortening[6, 55%], leaflet mobilization[4,36%], new chorda formation[2, 18%], chorda transposition[1, 9%] commissurotomy[3, 27%], and papillary muscle splitting[3, 27%]. Average number of mitral anatomic lesions per patient are 2.7 and we used average 2.8 procedures upon mitral valve apparatus per patient. There were no surgical mortality and no late valve related admission during the mean follow up period of 17 months. The mean functional class[NYHA] is 2.81 preoperatively and improved to 1.10 postoperatively. Doppler echocardiography showed much improvement from grade II MR [1 case], grade III MR [1 case] and 9 cases of grade IV MR to 6 cases of patients showed no MR, only trace MR in 4 cases, and grade I MR was found only in one patient with NYHA functional class II postoperatively. The postoperative mean mitral valve area is $2.10+0.28cm^2$. We conclude that mitral reconstruction is a predictable and stable operation.

판막주위 누출에 대한 다크론 확장 인공판막을 이용한 판막치환술 치험 (Trido Mitral Valve Replacement with Dacron Collar Prosthetic Valve due to Paravalvular Leak)

  • 승모판막의 인공판막 재치환술 후 발생하는 판막주위 누출은 드물지만 심각한 합병증이다. 판막주위 누출은 생존률의 증가나 증상호전을 위하여 적극적인 수술적 치료가 필요하다. 그러나 누출부의 단순한 봉합이나 첨포를 이용한 폐쇄는 판륜의 주위조직이 약화된 경우나 결손이 광범위한 경우에는 효과적이지 않다. 이에 저자들은 다크론 판 (Dacron sheet)으로 봉합륜(sewing ring)을 확장한 인공 기계 판막을 이용하여 판륜에 판막을 고정함과 동시에 다크론 판을 좌심방벽에 봉합하여 판막의 고정과 더불어 혈액의 누출을 방지하는 삼차 승모판막 재치환술을 시행하였다. 3례 모두 수술 후 특별한 문제없이 추적 관찰 중이다.

  • Duran ring을 이용하여 승모판막륜 성형술을 시행 받은 두 명의 소아 환자에서 술 후 8년과 5년 뒤에 판누스와 환자의 성장에 기인한 승모판협착이 발생하였다. 판누스와 Duran ring을 제거함으로써 승모판협착증은 호전되었다. 비록 어른에서 사용되는 충분한 크기의 판막륜 성형링이 사용되더라도 성장하는 소아에서는 시간이 지남에 따라 협착이 발생할 수 있다.

승모판막질환에 있어서 인공판륜을 이용한 승모판막재건술의 임상적 고찰 (Mitral Reconstruction Using Prosthetic Ring in Mitral Valvular Heart Disease)

  • 1994년 9월부러 1995년 8월까지 일년동안 부천 세종병원에서 시행된 승모판막질환 수술은 총 136례였으 며 이중 인공판륜을 사용하여 판막성형술을 시행된 44례를 대상으로 평가하였다 이 44례의 평균 연령은 38.2세(범위: 5세~63세)였으며 남성이 18례 여성이 26례였다, 사용된 인공판륜은 Carpentier ring이 32례, Dmm ring 이 12례였다. 판막질환의 원인은 류마치스가 30례(68%), 퇴행성 질환에 기인한 것이 13례(30%)였으며, 1례는 선천성 승 모판 부전증이었다. 판막질환의 형태로 보면 승모판막부전증이 33례(76%),승모판 협착증이 2 례(5%), 승모판 협착부전증이 9례(19%) 있었다. Carpentier의 기능적 분류는 I형이 5례(11%), II형이 24례(55%), III형이 4례(9%) 있었으며, II형과 III형의 혼합형이 11례(25%)에서 관찰되었고, 매 환자당 평균 3.7가지의 병변이 있었다. 승모판에 시행한 수술 수기는 전례에서 인공판륜성형술을 시행하였으며 한 환자 당 평균 3.4가지의 수기 를 사용하였다. 수술 사망은 2례에서 발생하였으며, 수술 후 승모판 부전증이 진행되어 2주에 시행한 재수술이 일 례 있 었다. 12개월의 추적 관찰에서 수술 전후의 NYHA 기능적 鈞畢\ulcorner평균 3.0에서 1.3으로 개선되었다. 심장 초음파 검사에서 술후의 승모판 면적은 2.07$\pm$0.11 cm2(평균 $\pm$ 표준오차)이었으며, 좌심실 수축력의 호전을 보였고, 판막부전의 정도는 전혀 판막부전 소견이 없어진 경우가 23례(53%), 경미한 폐쇄부전이 있는 경우가 18례(42%)였고, II도 부전의 소견을 보인 례가 2례(5%) 있었다.

Biometric analysis hand parameters in young adults for prosthetic hand and ergonomic product applications

  • This study aimed to evaluate the superficial anatomy, kinesiology, and functions of the hand to reveal its morphometry and apply the findings in various fields such as prosthetic hand and protective hand support product design. We examined 51 young adults (32 females, 19 males) aged between 18-30. Hand photographs were taken, and measurements were conducted using ImageJ software. Pearson correlation analysis was performed to determine the relationship between personal information and the parameters. The results of the measurements showed the average lengths of finger segments: thumb (49.5±5.5 mm), index finger (63.9±4.1 mm), middle finger (70.7±5.2 mm), ring finger (65.5±4.8 mm), and little finger (53.3±4.3 mm). Both females and males, the left index finger was measured longer than the right index finger. The right ring finger was found to be longer than the left in both sexes. Additionally, length differences between fingers in extended and maximally adducted positions were determined: thumb-index finger (56.1±6.2 mm), index-middle finger (10.7±4.1 mm), middle-ring finger (10.8±1.4 mm), and ring-little finger (25.6±2.7 mm). Other findings included the average radial natural angle (56.4°±10.5°), ulnar natural angle (23.4°±7.1°), radial deviation angle (65.2°±8.2°), ulnar deviation angle (51.2°±9.6°), and grasping/gripping angle (49.1°±5.8°). The average angles between fingers in maximum abduction positions were also measured: thumb-index finger (53.4°±6.5°), index-middle finger (17.2°±2.6°), middle-ring finger (14.3°±2.3°), and ring-little finger (32.1°±7.0°). The study examined the variability in the positioning of proximal interphalangeal joints during maximum metacarpophalangeal and proximal interphalangeal flexion, coinciding with maximum distal interphalangeal extension movements. The focal points of our observations were the asymmetrical and symmetrical arches formed by these joints. This study provides valuable hand parameters in young adults, which can be utilized in various applications such as prosthetic design, ergonomic product development, and hand-related research. The results highlight the significance of considering individual factors when assessing hand morphology and function.

경식도 심초음파 검사를 이용한 판막대치술 환자의 평가 (Postoperative Transesophageal Echocardiographic Evaluation in Patients with Cardiac Valve Replacement)

  • Since advent of the prosthetic cardiac valve replacement, much efforts for accurate assessing value function in-vivo have been attempted. To evaluate the postoperative functional and morphological status of the replaced cardiac valve prosthesis, 33 patients with valve replacement were studied by transthoracic and transesophageal 2-dimensional echocardiac imaging as well as by color Doppler flow velocity imaging. Twenty four patients had mitral valve replacement. 6 patients had aortic valve replacement and 3 patients had both mitral and aortic valve replacement. There were 34 mechanical and 2 biological prosthesis. Comparing to transthoracic echocardiography, transesophageal approach showed transvalvular regurgitant jet flow amid the prosthetic mitral valve ring during. systole and much clear visualization of cardiac chamber behind prosthesis which could give shadowing effect to ultrasound beam. According to the quantitative grading by the length and area of mitral regurgitant flow, 24 out of 27 mitral valves revealed mild degree regurgitation considered as physiological after prosthetic bileaflet valve replacement and the other 3 valves including 2 biological prosthesis had moderate degree regurgitation which was regarded as pathologic one. 2 cases of left atrial thromboses and 1 case of paravalvular leakage which were not visible by transthoracic approach were identified by transesophageal echocardiography in patients with mitral valve replacement and patients with aortic valve replacement respectively. We conclude that in patients with prosthetic mitral valve replacement, transesophageal 2-dimensional imaging with color Doppler can suggest reliable information beyond that available from the transthoracic access even though it gives patient some discomfort to proceed.

Study on the Diagnosis of Abnormal Prosthetic Valve

  • The two major problems related to the blood flow in replaced prosthetic heart valve are thrombus formation and hemolysis. Reliability of prosthetic valve is very important because its failure means the death of patient. There are many factors affecting the valvular failures and their representatives are mechanical failure and thrombosis, so early noninvasive detection is essentially required. The purpose of this study is to detect the various thromboses formation by using acoustic signal acquisition and its spectral analysis on the frequency domain. We made the thrombosis models using Polydimethylsiloxane (PDMS) and they are thrombosis model on the disc, around the sewing ring and fibrous tissue growth across the orifice of valve. Using microphone and amplifier, we measured the acoustic signal from the prosthetic valve, which is attached to the pulsatile mock circulation system. A/D converter sampled the acoustic signal and the spectral analysis is the main algorithm for obtaining spectrum. Then the spectrum of normal and 5 different kinds of abnormal valve were obtained. Each spectrum waveform shows a primary and secondary peak. The secondary peak changes according to the thrombus model. To quantitatively distinguish the frequency peak of the normal valve from that of the thrombosed valves, analysis using a neural network was employed. Acoustic measurement has been used as a noninvasive diagnostic tool and is thought to be a good method for detecting possible mechanical failure or thrombus.

Stability of the prosthetic screws of three types of craniofacial prostheses retention systems

  • Objectives: This study aimed to evaluate the stability of prosthetic screws from three types of craniofacial prostheses retention systems (bar-clip, ball/O-ring, and magnet) when submitted to mechanical cycling. Materials and Methods: Twelve models of acrylic resin were used with implants placed 20 mm from each other and separated into three groups: (1) bar-clip (Sistema INP, São Paulo, Brazil), (2) ball/O-ring (Sistema INP), and (3) magnet (Metalmag, São Paulo, Brazil), with four samples in each group. Each sample underwent a mechanical cycling removal and insertion test (f=0.5 Hz) to determine the torque and the detorque values of the retention screws. A servo-hydraulic MTS machine (810-Flextest 40; MTS Systems, Eden Prairie, MN, USA) was used to perform the cycling with 2.5 mm and a displacement of 10 mm/s. The screws of the retention systems received an initial torque of 30 Ncm and the torque values required for loosening the screw values were obtained in three cycles (1,080, 2,160, and 3,240). The screws were retorqued to 30 Ncm before each new cycle. Results: The sample was composed of 24 screws grouped as follows: bar-clip (n=8), ball/O-ring (n=8), and magnet (n=8). There were significant differences between the groups, with greater detorque values observed in the ball/O-ring group when compared to the bar-clip and magnet groups for the first cycle. However, the detorque value was greater in the bar-clip group for the second cycle. Conclusion: The results of this study indicate that all prosthetic screws will loosen slightly after an initial tightening torque, also the bar-clip retention system demonstrated greater loosening of the screws when compared with ball/O-ring and magnet retention systems.

O-ring Attachment를 이용한 무치악환자의 보철수복증례 (Prosthetic rehabilitation of the edentulous patients using O-ring attachment:A clinical report)

  • 무치악환자의 Implant보철수복 방법은 식립된 Implan의 수에 따라 다양하다. 많은 수의 Implant가 식립된 경우에는 고정성 보철물로 수복이 가능하지만 시술이나 기공상의 복잡성, 고가의 치료비, 보철물과 잔존치조제사이의 빈 공간으로 인한 비심미성, 발음의 어려움과 같은 여러 가지 문제점이 존재한다. 적은 수의 Implant가 식립된 경우는 Bar Retained Overdenture를 사용하지만 역시 기공과정이 복잡하며, Bar와 잔존치조제사이의 공간으로 인한 점막지지 면적의 감소와 보수가 어려운 단점이 있다. 여기에 비해서 O-ring Attachment를 이용하면 기공과정이 쉽고, 잔존치조제와 보철물 사이에 공간이 없으므로 점막지지 면적을 많이 얻을 수 있다. 또한 Implant식립의 수도 최소화할 수 있어 경제적이며, 문제가 발생한 경우에 쉽게 대처할 수 있는 장점을 지니고 있다. 본 증례들은 Steri-Oss Implant System의 O-ring Attachment를 이용하여 무치악환자를 보철 수복한 예로서 술후 기능적, 심미적으로 만족할 만한 결과를 얻었기에 문헌고찰과 더불어 발표하는 바이다.
