상악 완전 무치악 환자에서 구강 스캐닝 디지털 워크플로우를 활용한 완전 고정성 임플란트 수복 증례 (Fixed implant rehabilitation of maxillary edentulous patient using intraoral scanning digital workflow: a case report)

  • 편악 무치악 환자에서 완전 고정성 임플란트 보철 치료를 위해 다수의 임플란트를 식립할 때에는 악궁의 해부학적 구조, 대합치의 상태, 교합 평면 등을 고려한 최종 보철물 형태를 미리 결정해야 한다. 이 때 임시 보철물의 형태 및 교합 정보는 최종 보철물의 디자인에 유용하게 활용될 수 있다. 본 증례에서는 구강 스캐닝 디지털 워크플로우를 통해 무치악 스캔 데이터와 임시 보철물의 스캔 데이터를 중첩하였으며, 이를 통해 임시 보철물의 정보를 최종 보철물의 디자인에 반영하였다. 또한 구강 스캔을 통해 임시 의치 정보를 획득하여 임플란트의 위치와 각도를 계획하고, 수술 가이드를 사용하여 사전에 계획한 3차원적 위치에 임플란트를 식립하였다. 본 증례에서는 이와 같이 디지털 기술을 활용하여 심미적, 기능적으로 만족할 만한 임상적 결과를 얻었기에 이를 보고하는 바이다.

인체 중이 임플란트 개발에 관한 연구 (A Study on the Development of a Human Middle Ear Implant)

  • This paper describes the design of the ossicular replacement prosthesis for the people who have problem of hearing because of middle ear disease. Also the simulation results of the designed product for verification by finite element analysis software (ABAQUS) are presented. New model is applied to middle ear FE analysis which was generated in previous study. The full replacements of ossicular prosthesis for ossicles (malleus, incus and stapes) are made of Hydroxyapatite and Titanium. Although other existing prosthesis models consider only sound amplification effect, current type has damper system which is operating on the audible frequencies. High frequency sound transmitted to inner ear can be reduced and the prosthesis and inner ear can be prevented from damage.

  • The above-Knee Prosthesis has been used by the handicapped person and become a important part of their life. But uniform above-knee prosthesis only increases inconvenience And so the tool that can estimate and help to design of suitable prosthesis for user need to be developed. The simulator developed in this research is composed of two part. One is hardware that can realize various walking motions. The other is software that can display and analyze the results of walking mot ion. Three motors constitute hardware of Simulator. Two motors are used to realize heap motion that need two degree of freedom and the rest one used to realize swing motion. Software of Simulator display results of three motor trajectories and walking mot ion of hardware using computer graphic. Therefore, The simulator developed in this research which is able to realize human gait and results are analyzed through simulation program at PC will be some help to design and produce of prosthesis suitable to user.

Restoration using dental CAD/CAM system in severe tooth wear patient: A case report

  • In a situation where a vertical dimension has to be increased, the normal occlusion, function and aesthetics should be restored after taking into consideration the neuromuscular system and temporomandibular join. In order to do this, the provisional prosthesis should be made according to the patient's information. Moreover, the period of adjustment should be made through the provisional prosthesis modification, if necessary. The patient is then transferred to the final prosthesis to obtain results that are satisfactory. In the modern-day world, provisional prosthesis are replicated using three dimensional scanning and computer aided design/computer aided manufacturing (CAD/CAM) then made into final prosthesis. The adaptability of stomatognathic system can be judged by the PMMA provisional restoration. Functional and aesthetical aspect can be obtained. The adjustment period can be shortened by an intraoral correction and proper wear between provisional restorations. By using the CAD/CAM technology that transfers the information right into the final prosthesis.

A Digitally Designed All-on-4 Restoration with Screwmentable Concept

  • An all-on-4 restoration allows edentulous patients to use a fixed prosthesis with a minimum number of implants. These implant-supported fixed complete dentures have traditionally been fabricated as screw-retained or cement-retained prostheses. However, it is difficult to passively fit the long-span full-arch prosthesis using the screw-retained type restoration, and predictable retrievability is not obtained with the cement-retained type. This case report describes a prosthesis fabricated using a combination of the two retention types. The screwmentable method allows the implant-supported fixed complete denture to achieve a passive fit at the connection with retrievability. In addition, a framework with an optimized size was designed by using digital dental technology.

  • The axial contour and proximal contact of the prosthesis affect periodontal health, food impaction, and patient satisfaction. This narrative review provides a summary of articles regarding the axial contour and proximal contact of a fixed dental prosthesis on periodontal health and physiologic function. By acquiring a comprehensive understanding of the axial contour and proximal contact of teeth and prostheses, as well as their functional significance, the virtual design of fixed dental prosthesis can be optimized to maintain periodontal health and promote physiologic function effectively.

Guide flange prosthesis for early management of reconstructed hemimandibulectomy: a case report

  • Surgical resection of the mandible due to presence of benign or malignant tumor is the most common cause of the mandibular deviation. Depending upon the location and extent of the tumor in the mandible, various surgical treatment modalities like marginal, segmental, hemi, subtotal, or total mandibulectomy can be performed. The clinicians must wait for extensive period of time for completion of healing and acceptance of the osseous graft before considering the definitive prosthesis. During this initial healing period prosthodontic intervention is required for preventing the mandibular deviation. This case report describes early prosthodontic management of a patient who has undergone a reconstructed hemi-mandibulectomy with modified mandibular guide flange prosthesis. The prosthesis helps patient moving the mandible normally without deviation during functions like speech and mastication.

최종 보철물에 대한 고려 없이 전악 임플란트 식립된 환자의 고정성 보철 수복 증례 (Fixed prosthesis restoration in edentulous patient fully implanted without considering definitive prosthesis: A case report)

  • 임플란트를 이용한 완전 무치악 환자의 치료에 있어 가장 중요한 것은 최종 보철물의 형태이며, 최종 보철물의 형태가 결정된 후 잔존골 분석 및 식립할 임플란트의 종류, 개수, 위치 등에 대한 선택이 이루어져야 한다. 본 증례는 상악 우측 측절치를 제외하고 전악에 임플란트가 식립된 채로 내원한 환자에 대해 고정성 보철 수복한 증례이다. 최종 보철물에 대한 고려 없이 임플란트가 식립되었기 때문에 먼저 총의치를 제작하여 안모 분석 및 고정성 보철 수복 가능성을 평가하였다. 고정성 보철 수복이 가능하다고 판단되어 진단 납형, 방사선 영상 및 디지털 분석을 통해 최종 보철물에 사용할 수 없는 임플란트를 치료계획에서 제외한 후 고정성 임시 보철물을 제작하였다. 4개월 동안 고정성 임시 보철물을 사용하면서 안정적인 교합과 심미적인 만족을 보였기에 CAD/CAM (Computer aided design and computer aided manufacturing)을 이용하여 지르코니아 최종 보철물로 이행하였고, 치료 종료 후 3개월 동안의 평가에서 만족스러운 결과를 얻었다.

신체 힘에 의해 동작되는 부분 의수를 위한 부족구동 손가락 메커니즘 (Underactuated Finger Mechanism for Body-Powered Partial Prosthesis)

  • This paper presents an anthropomorphic finger prosthesis for amputees whose proximal phalanx is mutilated. The finger prosthesis to be proposed is able to make the amputees to perform the natural motion such as flexion/extension as well as self-adaptive grasping motion as if normal human finger does. The mechanism of finger prosthesis with three degrees-of-freedom (DOFs) consists of two five-bar and one four-bar linkages. Two passive components composed of torsional spring and mechanical stopper and only one active joint are employed in order to realize an underactuation. Each passive component is installed into the five-bar linkage. In order to activate the finger prosthesis, it is required for the user to flex and extend the remaining proximal phalanx on the metacarpophalangeal (MCP) joint, not an electric motor. Thus the finger prosthesis conducts not only the natural motion according to his/her intention but also the grasping motion through the deformation of springs by the object for human finger-like behavior. In order to reveal the operation principle of the proposed mechanism, kinematic analysis is performed for the linkage design. Finally both simulations and experiments are conducted in order to reveal the design feasibility of the proposed finger mechanism.

BLDC 모터 기반 동력 의족의 구성과 토크 추정을 활용한 유각기의 임피던스 제어 (The Structure of a Powered Knee Prosthesis based on a BLDC Motor and Impedance Control using Torque Estimation on Free Swing)

  • This paper presents the design of a lab-built powered knee prosthesis based on a BLDC motor, a sensored impedance control using a force sensor, and a sensorless impedance control through torque estimation. Firstly, we describe the structure of the lab-built powered knee prosthesis and its limitations. Secondly, we decompose the gait cycle into five stages and apply the position-based impedance control for the powered knee prosthesis. Thirdly, we perform an experiment for the torque estimation and the sensorless impedance control of the prosthesis. The experimental results show that we can use the torque estimation to control the low impedance during the swing phase, although the estimated torque data has a delay compared with the measured torque by a load cell.
