한국에서 Biochemical Recurrence의 진단에 대한 혈액 및 영상의학적 검사에 관한 비뇨기종양을 전공하는 의사의 트렌드에 대한 고찰 (The Trend of Uro-Oncologist About Blood Test and Imaging Studies for the Diagnosis of Biochemical Recurrence in Korea)

  • Purpose: The aim of this study is to investigate the criteria of biochemical recurrence (BCR) and follow-up periods and methods with and without blood and imaging test of urologic oncology before established guidelines of prostate cancer in Korea. Materials and Methods: In December 2015, we sent the questionnaire to urologic oncologist in academic hospital and received the answer from 108 urologic oncologist (50%). Also, we analyzed the data of 1,141 patients underwent radical prostatectomy in 2005 from Korean Medical Insurance. Results: In follow-up, 72 physicians (66.7%) performed blood test every 3 months, 51 physicians (47.2%) performed imaging study in case of BCR. Bone scan was the most common imaging study in the follow-up (74 physicians, 68.5%). But, bone scan was only performed in case of BCR (43 physicians, 39.8%). The criteria of BCR was PSA 0.2 ng/mL (75 physician, 69.4%), 76 physicians (70.4%) was performed different follow-up according to risk of patients. In Korean Medical Insurance data analysis, PSA were performed average 2 times every year and magnetic resonance imaging, computed tomography, Bone scan were performed average 0.1, 0.2, 0.1 times every year, respectively. Conclusions: The criteria of BCR and the follow-up of prostate cancer patients in Korea were similar Korean prostate cancer guidelines. Blood and imaging test might be increased compared to 10 years ago, it is necessary to compare the Korean Medical Insurance data between 10 years ago and present.

Interfraction variation and dosimetric changes during image-guided radiation therapy in prostate cancer patients

  • Purpose: The aim of this study was to identify volume changes and dose variations of rectum and bladder during radiation therapy in prostate cancer (PC) patients. Materials and Methods: We analyzed 20 patients with PC treated with helical tomotherapy. Daily image guidance was performed. We re-contoured the entire bladder and rectum including its contents as well as the organ walls on megavoltage computed tomography once a week. Dose variations were analyzed by means of Dmedian, Dmean, Dmax, V10 to V75, as well as the organs at risk (OAR) volume. Further, we investigated the correlation between volume changes and changes in Dmean of OAR. Results: During treatment, the rectal volume ranged from 62% to 223% of its initial volume, the bladder volume from 22% to 375%. The average Dmean ranged from 87% to 118% for the rectum and 58% to 160% for the bladder. The Pearson correlation coefficients between volume changes and corresponding changes in Dmean were -0.82 for the bladder and 0.52 for the rectum. The comparison of the dose wall histogram (DWH) and the dose volume histogram (DVH) showed that the DVH underestimates the percentage of the rectal and bladder volume exposed to the high dose region. Conclusion: Relevant variations in the volume of OAR and corresponding dose variations can be observed. For the bladder, an increase in the volume generally leads to lower doses; for the rectum, the correlation is weaker. Having demonstrated remarkable differences in the dose distribution of the DWH and the DVH, the use of DWHs should be considered.

전립선 암에서 Bicalutamide 사용으로 생긴 간질성 폐질환 1예 (Bicalutamide-induced Interstitial Lung Disease)

  • Androgen deprivation therapy, which is the standard treatment for metastatic prostate cancer, includes nonsteroidal antiandrogenic drugs, such as flutamide, nilutamide and bicalutamide. Of them, bicalutamide rarely induces interstitial pneumonia. We report a case of bicalutamide-induced interstitial pneumonia. A 68-year old male diagnosed with prostate cancer and multiple bone metastases presented with dry cough and low grade fever for 3 days. He had taken bicalutamide (50 mg/day) for 13 months. High resolution computed tomography revealed ground glass opacity in his right upper lung. The laboratory studies showed no eosinophilia in the serum and bronchoalveolar lavage fluid. Despite the use of antimicrobial agents for 2 weeks, the extent of the lung lesions increased to the left upper and right lower lung. He had no environmental exposure, collagen vascular disease and microbiological causes. Under the suspicion of bicalutamide-induced interstitial pneumonia, bicalutamide was stopped and prednisolone (1 mg/kg/ day) was initiated. The symptoms and radiologic abnormalities were resolved with residual minimal fibrosis.

국소 전립선암의 영구적 근접치료: 조기 결과 (Permanent Brachytherapy of Localized Prostate Cancer: Preliminary Results)

  • 목 적: 국소 전립선암의 영구적 근접치료 후 단기간 추적 관찰 동안의 생화학적 재발률과 부작용의 정도 및 이에 영향을 미치는 인자들에 대해 알아보고자 하였다. 대상 및 방법: 2007년 4월부터 2008년 12월까지 영구적 근접 치료를 시행 받은 환자는 67예이었다. 추적 관찰이 중단된 환자 5명, 이전에 방사선치료를 받았던 환자 2명을 제외한 60예 중 근치적으로 외부 방사선치료를 병용한 16예, 구제 요법으로 외부 방사선치료를 병용한 1예를 제외한 43예를 대상으로 종앙표지인자의 변화와 부작용의 발생 여부를 조사하였다. T1-T2a, Gleason score 6점 이하, prostate specific antigen (PSA) 10 ng/mL 미만의 조건을 모두 충족시킬 경우를 저위험군으로 정의하였으며, T2b-T2c, Gleason score 7점, PSA 10~20 ng/mL 중 한 가지 이상의 조건을 충족시키는 경우를 종간위험군으로 정의하였고, T3a 이상, Gleason score 8~10점, PSA>20 ng/mL 중 한가지 이상의 조건을 총족시키는 경우를 고위험군으로 정의하였다. 저위험군은 18명이었고, 중간위험군은 19명, 고위험군은 6명이었다. 처방선량은 145 Gy이었다. 결 과: 저위험군과 중간위험군에서 각각 1예씩 PSA가 2 ng/mL 이상 증가하여 생화학적 재발로 진단 받았으며(4.7%), 생화학적 재발률에 영향을 미치는 통계적으로 유의한 인자는 없었다. 급성 요로계 부작용의 경우 Radiation Therapy Oncology Group (RTOG) grade 1과 2가 각각 40예, 3예 있었으며, grade 2 이상의 급성 직장 부작용은 없었고 grade 1인 경우가 5예 있었다. 만성 요로계 부작용은 RTOG grade 1, 2, 3이 각각 1 예, 4예, 1예에서 나타났으며, RTOG grade 1, 2의 만성 직장 부작용이 각각 5예, 10예에서 나타났고 직장 요도간 장루가 생긴 grade 4인 경우가 3예 있었다. 부작용의 발생에 통계적으로 유의한 영향을 미치는 요인은 급성 직장염의 경우 전체 직장에서 최고 선량이 조사되는 직장 0.1 cc에 들어가는 최소 선량(p=0.041), 전체 직장 중 처방 선량의 150% 이상이 들어가는 부피(p=0.038), 치료 부위에 포함 된 직장 중 100% 선량이 들어가는 부피(p=0.047) 및 비율(p=0.019)이 있었으며, 급성 요로계 부작용의 경우 seed 개수(p=0.028)가 유의한 영향을 미쳤다. 만성 요로계 부작용에 유의한 영향을 미치는 인자는 없었으며, 만성 직장염의 경우 전체 직장의 100% 처방선량이 들어가는 부피의 비율(p=0,011), 치료 범위 직장에서 100% 처방선량이 들어가는 부피(p=0.023) 및 최고 선량이 조사되는 직장 0.1 cc에 들어가는 최소 선량(p=0.049)이 유의하게 나타났다. 결 론: 국소 전립선암 환자에서 영구적 근접치료는 저위험군 환자뿐 아니라 중간위험군이나 고위험군 환자에게도 선택적으로 적용 가능한 치료 방법이라고 생각되며 직장 부작용을 감소시키기 위한 노력이 필요하다고 생각된다.

  • 남성 유방암은 드물며 유방암의 약 1%를 차지한다. 유방 외 악성 종양이 남성 유방으로의 전이는 매우 드물다. 남자에서 유방으로 전이되는 가장 흔한 원발성 종양은 전립선암, 폐암, 위암, 대장암, 흑색종 및 육종이다. 남자에서 방광암의 유방으로의 전이 유방암은 드물게 보고되었고, 종괴가 아닌 침윤성 소견의 전이 소견을 보이는 경우는 매우 드물다. 방광암이 유방으로 전이된 59세 남자 환자의 유방촬영술 및 초음파에서의 영상 소견을 보고하고자 한다.

CDC6 mRNA Expression Is Associated with the Aggressiveness of Prostate Cancer

  • Background: Cell division cycle 6 (CDC6) is an essential regulator of DNA replication and plays important roles in the activation and maintenance of the checkpoint mechanisms in the cell cycle. CDC6 has been associated with oncogenic activities in human cancers; however, the clinical significance of CDC6 in prostate cancer (PCa) remains unclear. Therefore, we investigated whether the CDC6 mRNA expression level is a diagnostic and prognostic marker in PCa. Methods: The study subjects included 121 PCa patients and 66 age-matched benign prostatic hyperplasia (BPH) patients. CDC6 expression was evaluated using real-time polymerase chain reaction and immunohistochemical (IH) staining, and then compared according to the clinicopathological characteristics of PCa. Results: CDC6 mRNA expression was significantly higher in PCa tissues than in BPH control tissues (P = 0.005). In addition, CDC6 expression was significantly higher in patients with elevated prostate-specific antigen (PSA) levels (> 20 ng/mL), a high Gleason score, and advanced stage than in those with low PSA levels, a low Gleason score, and earlier stage, respectively. Multivariate logistic regression analysis showed that high expression of CDC6 was significantly associated with advanced stage (${\geq}T3b$) (odds ratio [OR], 3.005; confidence interval [CI], 1.212-7.450; P = 0.018) and metastasis (OR, 4.192; CI, 1.079-16.286; P = 0.038). Intense IH staining for CDC6 was significantly associated with a high Gleason score and advanced tumor stage including lymph node metastasis stage (linear-by-linear association, P = 0.044 and P = 0.003, respectively). Conclusion: CDC6 expression is associated with aggressive clinicopathological characteristics in PCa. CDC6 may be a potential diagnostic and prognostic marker in PCa patients.

Prostatic Cancer Presenting as an Isolated Large Lung Mass

  • A hidden primary tumor presenting as an isolated lung mass is a diagnostic challenge to physicians because the diagnosis of lung cancer is likely to be made if the histologic findings are not inconsistent with lung cancer. A large lung mass was found incidentally in a 59-year-old man. Although adenocarcinoma was diagnosed by percutaneous needle biopsy, thyroid transcription factor-1 (TTF-1) immunostaining was negative, raising suspicion that there was another primary site. There was no abnormal finding except for the lung mass on a $^{18}FDG$-PET/CT scan and the patient did not complain of any discomfort. Finally, prostatic cancer was confirmed through the study of tumor markers and prostate-specific antigen (PSA) immunostaining. Because of the rare presentation of a single lung mass in malignancies that have another primary site, physicians should carefully review all data before making a final diagnosis of lung cancer.

요로상피병변의 요세포학적 소견 (Urinary Cytologic Findings of Urothelial Lesions)

  • Urinary cytology is increasingly accepted as a diagnostic tool in the detection and follow-up of patients with bladder cancer. However, its value is reduced by several limitations, especially by the lack of cytologic criteria specifically reflecting the morphology of low-grade urothelial neoplasm. We reviewed histologically proven 50 cases of urine cytology with emphasis on cytologic findings of benign atypia and differential findings of urothelial neoplasm according to the grade. The diagnoses included 17 benign lesions(including 5 cases of urine calculi) and 33 malignant lesions (including 28 transitional cell carcinomas, 3 squamous cell carcinomas, 1 adenocarcinoma and 1 prostate adenocarcinoma), Diagnostic accuracy was 92%. Important cytodiagnostic criteria for benign atypia and low grade malignancy were cellularity, number of cell clusters, and morphology and arrangement of urothelial cells. The cytologic findings of urothelial neoplasms according to histologic grade were relatively well correlated with the histologic findings. However, the cytologic criteria were not sufficient to readily distinguish grade I from grade II. In view of this, we think that cytologic nomenclature "low-grade" and "high-grade" is a more reliable criterion. Recognition of subtle cellular morphologic features specific for urothelial lesions(including benign or malignancy) and proper fixation, processing and staining of specimen can expand the role of urinary cytology In detection and follow-up of patients.

Percutaneous osteoplasty for painful bony lesions: a technical survey

  • Percutaneous osteoplasty (POP) is defined as the injection of bone cement into various painful bony lesions, refractory to conventional therapy, as an extended technique of percutaneous vertebroplasty (PVP). POP can be applied to benign osteochondral lesions and malignant metastatic lesions throughout the whole skeleton, whereas PVP is restricted to the vertebral body. Common spinal metastases occur in the thoracic (70%), lumbosacral (20%), and cervical (10%) vertebrae, in order of frequency. Extraspinal metastases into the ribs, scapulae, sternum, and humeral head commonly originate from lung and breast cancers; extraspinal metastases into the pelvis and femoral head come from prostate, urinary bladder, colon, and uterine cervical cancers. Pain is aggravated in the dependent (or weight bearing) position, or during movement (or respiration). The tenderness and imaging diagnosis should match. The supposed mechanism of pain relief in POP is the augmentation of damaged bones, thermal and chemical ablation of the nociceptive nerves, and local inhibition of tumor invasion. Adjacent (facet) joint injections may be needed prior to POP (PVP). The length and thickness of the applied needle should be chosen according to the targeted bone. Bone cement is also selected by its osteoconduction, osteoinduction, and osteogenesis. Needle route should be chosen as a shortcut to reach the target bony lesions, without damage to the nerves and vessels. POP is a promising minimally invasive procedure for immediate pain relief. This review provides a technical survey for POPs in painful bony lesions.

Initial experience of magnetic resonance imaging/ultrasonography fusion transperineal biopsy: Biopsy techniques and results for 75 patients

  • Purpose: The aim of this study is to describe the technique and to report early results of transperineal magnetic resonance imaging and ultrasonography (MRI-US) fusion biopsy. Materials and Methods: A total of 75 patients underwent MRI-US fusion transperineal biopsy. Targeted biopsy via MRI-US fusion imaging was carried out for cancer-suspicious lesions with additional systematic biopsy. Detection rates for overall and clinically significant prostate cancer (csPCa) were evaluated and compared between systematic and targeted biopsy. In addition, further investigation into the detection rate according to prostate imaging reporting and data system (PI-RADS) score was done. Results of repeat biopsies were also evaluated. Results: Overall cancer detection rate was 61.3% (46 patients) and the detection rate for csPCa was 42.7% (32 patients). Overall detection rates for systematic and targeted biopsy were 41.3% and 57.3% (p<0.05), respectively. Detection rates for csPCa were 26.7% and 41.3%, respectively (p<0.05). The cancer detection rates via MRI fusion target biopsy were 30.8% in PI-RADS 3, 62.1% in PI-RADS 4 and 89.4% in PI-RADS 5. Rates of csPCa missed by targeted biopsy and systematic biopsy were 0.0% and 25.0%, respectively. The cancer detection rate in repeat biopsies was 61.1% (11 among 18 patients) in which 55.5% of cancer suspected lesions were located in the anterior portion. Conclusions: Transperineal MRI-US fusion biopsy is useful for improving overall cancer detection rate and especially detection of csPCa. Transperineal MRI-US targeted biopsy show potential benefits to improve cancer detection rate in patients with high PIRADS score, tumor located at the anterior portion and in repeat biopsies.
