A study of facial soft tissue of Korean adults with normal occlusion using a three-dimensional laser scanner (3차원 레이저 스캐너를 이용한 한국 성인 정상교합자의 안면 연조직에 대한 연구)

  • Developments in computer technology have made possible the 3-dimensional (3-D) evaluation of hard and soft tissues in orthodontic diagnosis, treatment planning and post-treatment results. In this study, Korean adults with normal occlusion (male 30, female 30) were scanned by a 3-D laser scanner, then 3-D facial images formed by the Rapidform 2004 program (Inus Technology Inc., Seoul, Korea.). Reference planes in the facial soft tissue 3-D images were established and a 3-D coordinate system (X axis-left/right, Y axis-superior/inferior, Z axis-anterior/posterior) was established by using the soft tissue nasion as the zero point. Twenty-nine measurement points were established on the 3-D image and 43 linear measurements, 8 angular measurements, 29 linear distance ratios were obtained. The results are as follows; there were significant differences between males and females in the nasofrontal angle $(male:\;142^{\circ},\;female:\;147^{\circ})$ and transverse nasal prominence $(male:\;112^{\circ},\;female:\;116^{\circ})$ (p<0.05). The transverse upper lip prominence was $107^{\circ}$ in males, $106^{\circ}$ in females and the transverse mandibular prominence was $76^{\circ}$ in both males and females. Li-Me' was 0.4 times the length of Go-Me'(mandibular body length) and the mouth height was also 0.4 times the width of the mouth width. The linear distance ratio from the coronal reference plane of FT, Zy, Pn, ULPm, Li, Me' was -1/-1/1/0.5/0.5/-0.6 respectively. The 3-D facial model of Korean adults with normal occlusion were be constructed using coordinate values and linear measurement values. These data may be used as a reference in 3-D diagnosis and treatment planning for malocclusion and dentofacial deformity patients and applied for 3-D analysis of facial soft tissue changes before and after orthodontic treatment and orthognathic surgery.

A STUDY ON CORRELATIONSHIP BETWEEN CRANIOFACIAL GROWTH PATTERN AND SYMPHYSIS MORPHOLOGY (악안면 성장양상에 따른 하악이부 헝태에 관한 연구)

  • Craniofacial growth pattern is an important diagnostic data in the course of orthodontic diagnosis and treatment planning ; it also has great influence in the establishment of occlusion as well as shaping and development of face. There have been many studies to classify different craniofacial growth patterns and attempts to predict growth patterns. This study aimed to correlate craniofacial growth pattern and symphysis morphology. 120 adult patients with age from 19 to 39 (mean age : 23.1) were chosen as subjects , using lateral cephalometric films. their anterior to posterior facial height ratios were calculated. They were divided into 3 groups - clockwise growth pattern with $56\%-62\%$(36subjects), counter-clockwise growth pattern group with $65\%$-80\%$(43subjects) and normal growth pattern group with $62\%-65\%$(41subjects). Symphysis morphology and Prominence evaluation in each subject were studied and the following conclusions were drawn : 1. In comparison of symphysis morphology between the sex groups, men showed large symphysis height and prominence. 2. Concerning the symphysis morphology, the clockwise growth pattern group showed larger height, H/D ratio and actual length but smaller depth, angle, effective length and E/A ratio compared to the counter -clockwise growth pattern group. 3. Those with smaller prominance of symphysis showed clockwise growth tendency and those with larger prominance showed counter-clockwise growth tendency.

Impact of face masks on spectral and cepstral measures of speech: A case study of two Korean voice actors (한국어 스펙트럼과 캡스트럼 측정시 안면마스크의 영향: 남녀 성우 2인 사례 연구)

  • This study intended to verify the effects of face masks on the Korean language in terms of acoustic, aerodynamic, and formant parameters. We chose all types of face masks available in Korea based on filter performance and folding type. Two professional voice actors (a male and a female) with more than 20 years of experience who are native Koreans and speak standard Korean participated in this study as speakers of voice data. Face masks attenuated the high-frequency range, resulting in decreased Vowel Space Area (VSA) and Vowel Articulation Index (VAI)scores and an increased Low-to-High spectral ratio (L/H ratio) in all voice samples. This can result in lower speech intelligibility. However, the degree of increment and decrement was based on the voice characteristics. For female speakers, the Speech Level (SL) and Cepstral Peak Prominence (CPP) increased with increasing face mask thickness. In this study, the presence or filter performance of a face mask was found to affect speech acoustic parameters according to the speech characteristics. Face masks provoked vocal effort when the vocal intensity was not sufficiently strong, or the environment had less reverberance. Further research needs to be conducted on the vocal efforts induced by face masks to overcome acoustic modifications when wearing masks.

Voice Quality of Dysarthric Speakers in Connected Speech (연결발화에서 마비말화자의 음질 특성)

  • This study investigated the perceptual and cepstral/spectral characteristics of phonation and their relationships in dysarthria in connected speech. Twenty-two participants were divided into two groups; the eleven dysarthric speakers were paired with matching age and gender healthy control participants. A perceptual evaluation was performed by three speech pathologists using the GRBAS scale to measure the cepstrual/spectral characteristics of phonation between the two groups' connected speech. Correlations showed dysarthric speakers scored significantly worse (with a higher rating) with severities in G (overall dysphonia grade), B (breathiness), and S (strain), while the smoothed prominence of the cepstral peak (CPPs) was significantly lower. The CPPs were significantly correlated with the perceptual ratings, including G, B, and S. The utility of CPPs is supported by its high relationship with perceptually rated dysphonia severity in dysarthric speakers. The receiver operating characteristic (ROC) analysis showed that the threshold of 5.08 dB for the CPPs achieved a good classification for dysarthria, with 63.6% sensitivity and the perfect specificity (100%). Those results indicate the CPPs reliably distinguished between healthy controls and dysarthric speakers. However, the CPP frequency (CPP F0) and low-high spectral ratio (L/H ratio) were not significantly different between the two groups.

Effects of Semi-Occluded Vocal Tract Exercise in Patients with Functional Aphonia (반폐쇄성도훈련이 기능적 실성증 환자의 음성 개선에 미치는 효과)

  • Background and Objectives : Functional aphonia is characterized by incomplete closure of the vocal folds. Semi-occluded vocal tract exercise (SOVTE) allows smoothly vocal folds collision without damage to the vocal folds tissues to produce normal vocal intensity. The purpose of this study is to report the effect of SOVTE in patients with functional aphonia. Materials and Method : Seven patients diagnosed with functional aphonia were treated with 1-3 voice therapy sessions using voiced lip-trill, humming, Lax Vox in SOVTE. To assess the effectiveness of semi-occluded vocal tract exercise, cepstral analysis and auditory perceptual assessment were performed before and after voice therapy. Results : F0 (fundamental frequency), CPP (cepstral peak prominence) and L/H ratio (low/high spectral ratio) were significantly increased, while CPP Standard deviation, L/H ratio Standard deviation were decreased. In addition, 'Grade', 'Breathiness' and 'Asthenia' were significantly decreased in the GRBAS scale after SOVTE (p<0.05). Conclusion : In our study, SOVTE seemed to be effective to elicit voice quickly and promote vocal folds vibration without muscular effort in patients with functional aphonia.

Implantation of a Newly Designed Supratarsal Gold Weight versus the Traditional Pretarsal Model for the Correction of Long-standing Paralytic Lagophthalmos: A Retrospective Cohort Study

  • Background The study determined to compare the clinical outcomes of traditional gold weight implantation for the correction of paralytic lagophthalmos with those of a newly designed model. Methods In this retrospective cohort study, we enrolled 30 patients (76% females; average age 60.8 ± 12 years) with facial palsy who underwent implantation of either the traditional pretarsal gold weight (PT group; n = 15) or a new supratarsal model (ST group; n = 15) from May 2014 to April 2019. The main outcome measures were the 12-month postoperative weight prominence, weight migration, improvement of lagophthalmos, upper eyelid contour, and upper eyelid ptosis. The secondary outcome was long-term (24 months) reoperative rate. Results The new model group had significantly better eyelid contour (risk ratio [RR] 3.16, 95% confidence interval [CI] 1.62-6.15, p = 0.001), less weight prominence (RR 1.74, 95% CI 1.13-2.70, p = 0.013), less weight migration (RR 1.31, 95% CI 1.12-1.54, p = 0.001), and less eyelid ptosis (RR 2.36, 95% CI 1.21-4.59, p = 0.011) than the traditional model group. Improvement of lagophthalmos was not statistically significant between the two groups (RR 1.44, 95% CI 0.72-2.91, p = 0.303). The 24-month reoperative rate was 53.3% in the PT group versus 13.3% in the ST group (RR 2.00, 95% CI 1.15-3.49, p = 0.015). Conclusion The newly designed supratarsal gold weight showed superior postoperative outcomes than the standard traditional model.

  • In this study, 186 cases of spontaneous pneumothorax experienced at the Dept. of thoracic and Cardiovascular surgery, chosun University Hospital during from Jan. 1978 to Mar. 1987 were analysed retrospectically. The results were as follows: 1. The sex ratio of the stadied patient was 2.5:! in male prominence and the incidence and the incidence was highest in the adolescence between 20-29 year of age. 2. The clinical symptoms were frequently Dyspnea (52.2%) and Chest pain (38.7%). The etiologic factors were as follows, Tuberculous origin in 55.9%, Unknown origin in 17.2%, Bullae in 11.8%. 3. In the method of treatment, closed thoracotomy was the most effective procedure in achieving the expansion of collapsed lung in 87.1%. Other methods were bed rest with high oxygen inhalation, thoracentesis and open thoracotomy. 4. The incidences of complication were developed in 12.4% and recurrent rates were seen in 9.7%.

Correlation between Physical Fatigue and Speech Signals (육체피로와 음성신호와의 상관관계)

  • This paper deals with the correlation between physical fatigue and speech signals. A treadmill task to increase fatigue and a set of subjective questionnaire for rating tiredness were designed. The results from the questionnaire and the collected bio-signals showed that the designed task imposes physical fatigue. The t-test for two-related-samples between the speech signals and fatigue showed that the parameters statistically significant to fatigue are fundamental frequency, first and second formant frequencies, long term average spectral slope, smoothed pitch perturbation quotient, relative average perturbation, pitch perturbation quotient, cepstral peak prominence, and harmonics to noise ratio. According to the experimental results, it is shown that mouth is opened small and voice is changed to be breathy as the physical fatigue accumulates.

  • In-process evaluation equipment for the prominent discrete tone (PDT) emitted by small motors or actuators such as HDD(Hard Disk Driver), ODD(Optical Disk Driver) or linear motors has been developed and estimated on the basis of Prominence Ratio (PR) method in ECMA 74. Correlation of the evaluation equipment was performed in comparison with the method by ECMA 74 in anechoic chamber. Coherent function between two systems was applied in order to compensate traceability of ECMA 74 in anechoic chamber. Usability of the developed system with estimated Gage R&R shall be approved with real samples in the mass production line.

Measuring Correlation between Mental Fatigues and Speech Features (정신피로와 음성특징과의 상관관계 측정)

  • This paper deals with how mental fatigue has an effect on human voice. For this a monotonous task to increase the feeling of the fatigue and a set of subjective questionnaire for rating the fatigue were designed. From the experiments the designed task was proven to be monotonous based on the results of the questionnaire responses. To investigate a statistical relationship between speech features extracted from the collected speech data and fatigue, the T test for two-related-samples was used. Statistical analysis shows that speech parameters deeply related to the fatigue are the first formant bandwidth, Jitter, H1-H2, cepstral peak prominence, and harmonics-to-noise ratio. According to the experimental results, it can be seen that voice is changed to be breathy as mental fatigue proceeds.
