  • Cervical pain is a rapid increase that is owing to a flexion-extension whiplash injury, unappropriated posture, chronical repetition injury from abdominal position of head and neck, excessive repeating work, chronical deficiency of excercise. Because of that is bring about muscle unbalance, tightness of cervical extensor muscle, weakness of cervical deep flexor muscles, instability of cervical region and reduction of proprioceptive sensor. Recent the role of muscle is more emphasized for preservation of sine stabilization. And cognition of integrated muscular system, importance for the operation and relation is increased to maintain stability of the motor system and pertinent function. Therefore we are going to introduce the sling exercise and stabilization exercise method for advanced efficient of cervical and upper limb and for the muscle strengthening to importance cervical stabilization through neurological program as control the reaction of cervical stabilization. Sling exercise therapy(SET) concept consists of a system of diagnosis and treatment. The system of diagnosis involves testing the muscle's tolerance through progressive loading in open and close kinetic chains. The SET system contains elements such as relaxation, increasing the range of movement, traction, training the stabilizing musculature, sensory-motor exercises, training in open and close kinetic chains, dynamic training of the mobilizing musculature, cardiovascular exercise, group exercise, personal exercise at home Sensory-motor training is an essential element of the SET concept. The emphasis is on closed kinetic chain exercise on an unstable surface, there by achieving optimum stimulation of the sensory-motor apparatus.

  • The main purpose of this study was to identify the effect of stress management program on the perceived stress, stress response, ways of coping of nurses.The data were collected from September 22 to December 21, 1997. The subjects were 45 emergency nurses of three university hospital located in seoul ; 25 assigned for the experimental group and 20 for the control group.The stress management program was composed of cognitive training, behavioral training and progressive muscle relaxation. The subjects of the experimental group carried out the 15-minute progressive muscle relaxation according to recorded-tape once a day for 6 weeks. The cognitive and the behavioral training were conducted by the investigator for 5~8 subjects at a time for 9 series, at an interval of 4~5days.The instruments used in the study was the perceived stress scale developed by Levenstein et al.(1993), Symptoms of stress Inventory(1991), and ways of coping scale developed by Lazarus & Folkman(1984).The data were analyzed by SAS program, using descriptive statistics, χ2-test, t-test, paired t-test.The results were as follows :1. The perceived stress of experimental group was lower than that of the control group. However, no significant difference between the two groups was revealed.2. The stress response of experimental group was reduced after stress management program and the stress response of control group was reduced after stress management program. but no significant difference between the two groups was revealed.3. The problem oriented coping of the experimental group was higher than that of the control group, however no significant difference between the two groups was revealed.4. The emotional oriented coping of the experimental group was lower than that of the control group, however no significant difference between the two groups was revealed.

수면증진 프로그램이 노인의 수면과 면역반응에 미치는 효과 (Effects of Sleep Promoting Program on Sleep and Immune Response in Elderly)

  • Purpose: This study was to examine the effects of sleep promoting program on sleep and the immune response in the elderly with insomnia. Methods: The study was designed as a quasi-experimental, nonequivalent control group pre-post test design. Thirty two elders who suffered from insomnia (16 experimental and 16 control subjects) were selected among the elders who had been enrolled in two churches. The subjects in the experimental group participated in a sleep promoting program which was composed of sleep hygiene education and progressive muscle relaxation for 4 weeks. Data were analyzed using the SPSS/WIN. Results: The experimental group showed higher sleep score than that of the control group (t=7.86, p=.00). The experimental group showed higher sleep satisfaction score than that of the control group (t=7.61, p=.00). The sleep promoting program was not effective in enhancing immune response. Conclusion: The sleep promoting program increased the sleep score, sleep-satisfaction score and B cell in immune response of elderly people suffering from insomnia. Therefore, sleep promoting program can be applied as an effective nursing intervention to promote sleep quality and sleep satisfaction.

  • Dysfunction of the anterior and dorsal muscles of the trunk have been studied in relation to low back pain of many years. Many muscles of the trunk are capable of contributing to the stabilization and protection of the lumbar spine, recent evidence has suggested that transversus abdominis may be critically involved and has been the focus of rehabilitation. The delay in onset of contraction of trunk muscles associated with movement of the upper or lower limb in patients with low back pain indicates a significant deficit in the automatic motor command for control of disturbance to the spine. The function of transversus abdominis has been largely ignored in the evaluation of spinal stabilization and protection. The most essential stabilizing muscles for the lumbar column are the transversus abdominis and the multifidus. Sling exercise therapy(SET) concept consists of a system of diagnosis and treatment. Positive impact of integrated amrita meditation technique on heart rate, respiratory rate and IgA on young healthy adults

  • The objective of the current study was to find out the effect of Integrated Amrita Meditation Technique (IAM) on blood pressure (BP), heart rate (HR), respiratory rate (RR) and IgA. One hundred and fifty subjects were randomized into three groups IAM, Progressive Muscle Relaxation (PMR) and Control. Baseline values were collected before giving the training for all the subjects and the IAM and PMR groups were given training in the respective techniques. BP, HR, RR and IgA were recorded manually at 0 h, 48 h, 2 months and 8 months after the first visit. HR was found to be reduced in the IAM group 48 h onwards and the fall sustained till 8 months (p < 0.05). IAM group showed significant drop when compared to the PMR group and control group in all the subsequent visits (p < 0.05). RR decreased significantly in the IAM group in the third and fourth visits (p < 0.05). RR of IAM showed significant decrease when compared to PMR and control from the third visit onwards. IgA showed significant increase in comparison with PMR and control in the third and fourth visits. BP did not show any difference in any of the visits. There was subject dropout from randomization to completion of the study, in all the three groups. The significant decrease in HR and RR and increase in IgA in the IAM group when compared to the PMR and control group shows the efficacy of the technique in reducing the physiological stress indicators for up to 8 months.

  • Oriental Medicine Based Psychotherapy is consisted of 8 sessions which are available for various kinds of patients(mood disorders, somatoform disorders, anxiety disorders, Hwabyung, adjustment disorders etc.). Through 8 sessions, patients were educated on the relationship of mind and body which is the basic concept of oriental medicine and also received several training such as Progressive Muscle Relaxation, Autogenic Training, Breathing Training, Meditation, Qi-training. After these several training they get to think of their problems and inner conflict with people and get some insight about themselves and others, it can help handling their problems. This case report is about the patient with lots of somatic complains through the whole body, who got better after treatment by oriental medicine based psychotherapy added on herbal medicine and acupuncture.

주장훈련이 간호사의 스트레스 반응과 주장행동에 미치는 효과 (The Effect of Assertiveness Training on the Stress Response and Assertive Behavior of Nurses)

  • This study was designed and carried out to investigate the effect of Assertiveness Training on the stress response and assertive behavior of emergency room nurses from September 22, to December 21. 1997. The subjects were 45 emergency room nurses of three general hospitals of Korea University Medical Center; 25 assigned for the experimental and 20 for the control group. The Assertive Training Program was composed of cognitive training, behavioral training and progressive muscle relaxation. The subjects of the experimental group carried out the 15-minute progressive muscle relaxation according to recorded-tape once a day for 6 weeks. The cognitive and the behavioural training were conducted by the investigator for 5-8 subjects at a time for 9 series, at an interval of 4-5 days. Post-measurement were administered to all sujects in two groups three weeks after the last session. The level of general stress (Cline, 1992), job stress (Kim & Koo, 1984), physiological response to stress (cortisol, epinephrine, norepinephrine), symptoms of stress (Lee, 1984) and assertive behavior (Kim, 1982) were measured before the first and after the last experiments. Data were analysed by x2, t, paired t, Wilcoxon signed rank tests Wilcoxon rank sums tests. The results are as follows : 1. The level of general stress of the experimental group was lower than that of the control group, however, no significant difference between the two groups (P=0.234) was revealed. 2. A significant reduction of job stress was revealed in the experimental group (P=0.017). 1) A significant reduction of interpersonal conflicts in the experimental group was revealed (P=0.018). 3. The physiological stress-response were analyzed as follows : 1) A decrease of Cortisol in both the experimental and the control groups was noticed, however, the difference between the two groups was not statistically significant (P=0.991). Pre-and post-measurement of the level of Corti sol revealed that a notable decrease in the experimental group and an increase in the control group with no significant difference in the day-day subjects (P=0.765), a significant decrease was noted on the level of Cortisol of the experimental group in the night-night nurses (P=0.036). 2) An increase of Epinephrine in both groups was noticed, however, a difference was not statistically significant between the two groups (P=0.082). Pre-and post-measurement of the level of Epinephrine revealed that a notable decrease in the experimental group and an increase in the control group with no significant difference in the day-day subjects (P=0.136). increases were noted on the level of Epinephrinel of both groups in the night-night nurses (P=0.136), 3) The level of Norepinephrine of the experimental group was significantly decreased (P=0.020). Pre-and post-measurement of the level of Norepinephrine revealed that a notable decrease in the experimental group and an increase in the control group with no significant difference in the day-day subjects (P=0.073). a significant decrease was noted on the level of Norepinephrine of the experimental group in the night-night nurses (P=0.036), 4. Symptoms of stress were reduced in both groups, with no significant differences between the groups (P=0.127). 1) The physical stress symptoms were reduced in both groups, with no significant difference (P=0.386), 2) The cognitive-behavioral stress symptoms were significantly reduced in the experimental group (P=0.037). 3) The emotional stress symptoms were reduced in both groups with no significant difference between the groups (P=0.110). 5. The assertive behavior was significantly increased in the experimental group (P=0.000). This study signified the Assertiveness Training as an effective mean of self intervention for the reduction of stress of nurses assigned to emergency rooms.

PTU-처치가 흰쥐대동맥의 수축 및 이완 반응에 미치는 영향 (Vascular Responses to Vasoactive Drugs in Propylthiouracil-Treated Rat Aorta)

  • Propylthiouracil(PTU)을 4주 및 12주간 투여한 흰쥐의 대동맥을 적출하여 혈관수축제와 이완제에 대한 반응을 관찰하여 다음과 같은 결과를 얻었다. PTU를 처치한 실험군에서는 대조군에 비해 현저한 체중저하와 혈중 갑상선호르몬의 감소를 보였다. PTU를 처치한 흰쥐의 대동맥에 대한 norepinephrine (NE)과, calcium및 potassium 이온에 의한 최대수축 반응은 대조군에 비하여 유의하게 감소되었다. 그러나 NE에 대한 중간유효량은 증가되었으나, calcium이온에 대한 중간 유효량은 유의한 차이가 없었다. 그리고 acetylcholine, histamine, isoproterenol 및 nitroprusside에 의한 대동맥의 이완작용은 대조군에 비해 증가된 경향을 보였다. PTU를 12주간 처치한 군에 있어서 acetylcholine에 의한 최대 이완반응은 대조군에 비해 유의하게 증가되었지만 다른 이완제에 의해서는 유의한 차이가 없었다. PTU를 4주간 처치한 군에 있어서는 대조군에 비하여 혈관이완제에 대한 중간억제량은 유의한 차이가 없었지만, 12주 처치군에 있어서는 isoproterenol 및 nitroprusside에 대한 중간억제량은 감소 되었으나 acetylcholine 및 histamine에 대한 중간억제량은 유의한 차이가 없었다. 이상의 결과로 미루어 보아 PTU-처치에 의한 혈관 반응성의 변화는 혈관 내피세포보다는 혈관 평활근세포자체의 변화에 기인되며, 이러한 세포내의 변화는 갑상선 기능이 저하된 후에도 계속되고 있음을 알 수 있었다.

급성기 정신분열병의 치료로서 단기적인 안구운동 민감소실 및 재처리요법에 대한 예비연구 (A Pilot Study of Brief Eye Movement Desensitization and Reprocessing(EMDR) for Treatment of Acute Phase Schizophrenia)

  • Objectives : Eye movement desensitization and reprocessing(EMDR) is a novel, time-limited psychotherapy originally developed for treatment of psychological trauma. The effectiveness of this therapy has been validated only for posttraumatic stress disorder ; however, EMDR is often applied to other psychiatric illnesses, including other anxiety disorders and depression. This pilot study tested the efficacy of EMDR added to the routine treatment for individuals with acute stage schizophrenia. Methods : This study was conducted in the acute psychiatric care unit of a university-affiliated training hospital. Inpatients diagnosed with schizophrenia were randomly assigned to either three sessions of EMDR, three sessions of progressive muscle relaxation(PMR) therapy, or only treatment as usual(TAU). All the participants received concurrent typical treatments(TAU), including psychotropic medication, individual supportive psychotherapy and group activities in the psychiatric ward. The Positive and Negative Syndrome Scale(PANSS), the Hamilton Depression Rating Scale and the Hamilton Anxiety Rating Scale were administered by a clinical psychologist who was blinded to the patients' group assignment. Results : Forty-five patients enrolled and forty patients(89%) completed the post-treatment evaluation. There were no between-group differences in the withdrawal rates of patients during the treatment or at the three-month follow-up session. All three groups improved significantly across each of the symptomatic domains including schizophrenia, anxiety, and depressive symptoms. However, a repeated measures ANOVA revealed no significant differences among the groups over time. Effect size for change in total PANSS scores was also similar across treatment conditions, but effect size for negative symptoms was large for EMDR(0.60 for EMDR, 0.39 for PMR and 0.21 for TAU only). Conclusion : These findings supported the use of EMDR in treating the acute stage of schizophrenia but the results failed to confirm the effectiveness of the treatment over the two control conditions in three sessions. Further studies with longer courses of treatment, more focused target dimensions of treatment, and a sample of outpatients are necessary.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. The implemented system allows users to register and retrieve patient information, medical images and diagnostic reports.
