Clinical Dental Hygienists' Experience of the Prevention Based Incremental Oral Health Care: Applying Focus Group Interviews

  • Background: In this study, we tried to comprehensively explore clinical dental hygienist's experience of a prevention-based incremental oral health care program, which was pilot-operated by dental clinics, define prevention-based incremental oral health care as experienced in the field, and identify factors to be considered. Methods: This study conducted a focus group interview with five dental hygienists who participated in an ongoing oral management pilot project in 2016. The interview was conducted by a researcher, and the co-research team attended as progress assistants and recorded characteristics of the participants, main dictations, and non-verbal characteristics. All interviews were recorded and underwent thematic analysis to examine the questions of the study as the main axis. Results: As a result of the study, 65 meaningful statements were extracted by code, integrated into 24 sub-categories, and structured into 11 categories. Finally, four keywords were drawn: characteristics, facilitating factors, conflicting factors, and improvement measures for prevention-based incremental oral health care. Regarding prevention-based incremental oral health care in dental clinics, dental hygienists were highly aware of the physical and mental burdens of personalized treatment and education for each individual. They were responsible for the patient and for facilitating changes in the behavior of the client, leading to professional satisfaction. The dental team's cooperation and supportive attitude were found essential to continue oral health care in the dental clinic. Conclusion: Through dental team-based treatment philosophy sharing and collaboration, it is possible to provide prevention-based incremental oral health care in dental clinics. In future, it is necessary to develop a system for establishing a sustainable preventative management system for public health promotion.

어머니의 취업상태와 자녀 구강건강상태의 관련성 (The association of employment status of mother and children's oral health)

  • Objectives : School-age children have mixed dentition and are sensitive to dental caries. Their dental condition is more important as it is directly related with oral health in their life, and parents' attention and education are necessary as the children can not manage their oral health voluntarily. Methods : To evaluate the effect of parents' occupational status on the oral health of children, this study conducted a questionnaire survey and analyzed the results along with the results of oral examination with 952 students and their parents of an elementary school located in Ulsan conducting school oral health projects on May, 2009. Results : As dental caries prevalence rate recorded 33.7%, 56.8% and 66.9% in non-dual income, professional dual income and non-professional dual income families, respectively, the rate of children of dual income families was significantly higher than that of non-dual income ones (p<0.05). Conclusions : For professional dual income families, although working mothers contribute to economic stability, they took less time to care or to educate their children compared to no-working mothers so that they need to pay more attention to oral health of children. In addition, more interest to and education for children of non-professional dual income families showing relatively lower socio-economic level and lack of time for children, were also necessary in school projects on oral health education and prevention.

Elderly Welfare System and Role of Dental Hygienists in Korea and Japan

  • In accordance with the aged society, oral health care for the elderly is considered important to maintain general health. Although the role of dental hygienists is essential for proper health management of the oral cavity, research on the care system for the elderly people's oral health and on the role of dental hygienists in the field of elderly welfare is still insufficient. Hence, the aim of this study is to investigate the status of Korean elderly welfare system and dental hygienists by comparing them with those in Japan, a precedent of aged society. First, we compared and investigated the Japanese long-term insurance system, which provides an institutional basis for a long-term care system for the elderly in Korea. Second, the elderly welfare law and care system, focusing on oral care, were examined. Lastly, in elderly care, we analyzed the distinctions between Korea and Japan regarding dental hygienists' role and scope of work. Taken together, as a precedent of aged society, Japan has shown well-specialized and systematic welfare for the elderly compared with Korea. With the development of the welfare system for the elderly in Japan, the role and the work scope of dental hygienists have been expanded to improve quality of life of elderly people, as a key professional for elderly oral care. Therefore, we should perceive the need for improvement of long-term care insurance and the expansion of dental hygienists' work in Korea. In conclusion, these results could be used as basic data for improving the elderly welfare system and developing dental hygienists in Korea.

부산광역시 초등학교 양호교사의 학교구강보건에 대한 태도와 의견 (Attitudes and Opinions about School Oral Health Care among Health Teachers of Elementary School in Busan)

  • Objectives : Because elementary school children are sensitive and prevalent to dental caries, oral health care program for the elementary school children (ESOHCP) should be met the first priority for the national oral health care program. This study was performed to proposed the guidelines for executing the efficient ESOHCP through the evaluation of present oral health care and education status based on the attitudes and opinions of health teachers. Methods : Subjects were 181 health teachers those are a part of the total 273 elementary school health teachers in Busan. From April to June 2001, questionnaire survey on the teacher's attitude and opinion, present status of oral health care and education in their own school was done. Data was analyzed by $\chi^2$-test, t-test, ANOVA using SAS (ver 6.21) program. Results : Mean age of the teachers was 40 years (24~58 years) and mean scale of the subject school was 32 classes (5~58 classes). Above 90% of them agreed with the necessity and importance of ESOHCP, but actual level of concerning was lower. They gained the informations about oral health mainly from media (33%), PC net (26%), academic society (25%), and more used internet (p=0.012) in younger generation. Though most of them (96%) had a specified school dentist, only 58% had the experience of periodic oral health examinations in their own school. The major experienced ESOHCP was fluoride brushing and contest to choose the best healthy teeth. Oral health education given by the teachers was not professional but their own. About half (42%) of the teachers had never taken education during last one year. Major education materials were VTR tape (79%), teeth model (64%), CD (55%), booklet (50%), etc., and the teachers' preference was also VTR tape (43%) and CD (41%). When they need education materials, they frequently request to dentist's association (36%), academic society (35%), but rarely to dental clinics or dentists (6%). They prefer that two times (46%) of annual oral health education, with the contents of regular brushing (53%), but lower regular visit to clinics (13%). The interest to oral health care (p=0.016) increased with teacher's age. And, they agreed regular students education in case of higher interest(p=0.044). Intention about join in the model school program was decreased with scale of school (p=0.002). Conclusions : This result showed that the present status of ESOHCP is still insufficient. It considered that the health teachers' interest and role is most important for the maintenance and success of ESOHCP. The professional and efficient education program would be focused on the health teachers to induce drive on ESOHCP.

요양보호사의 노인 구강보건에 대한 지식과 인식도 연구 (A study on the knowledge and awareness of care workers on the geriatric oral health)

  • 본 연구는 노인요양시설에 근무 중인 요양보호사의 구강보건지식정도와 인식도를 조사하여 구강보건의료서비스의 질적 향상과 발전 방안 연구를 목적으로 실시하였다. 연구방법은 D광역시 소재 노인요양시설(10개소)에 근무하는 요양보호사 중 연구 참여에 동의한 163명을 대상으로 2015년 5월 1일부터 5월 31일까지 설문조사한 자료를 SPSS Statistics 18. 프로그램을 이용하여 분석 하였다. 그 결과 요양보호사들의 구강보건지식은 총점 26점 만점에 20.09점(77.2%)로, 가장 높은 지식은 '올바른 칫솔질 방법' 93.0%, 가장 낮은 지식은 '구강보조용품사용 지식' 58.1%로 조사되었다. 구강보건교육경험이 있는 사람이 구강보건교육 요구도가 높았으며 (P<0.001), 구강보건교육경험횟수가 많은 사람이 구강보건지식(P<0.001), 요구도(P<0.01), 인지도(P<0.05)가 높아 유의하였다. 요양보호사들의 구강보건인식정도에 영향을 주는 것은 구강보건지식점수(P<0.01), 구강보건교육 요구도(P<0.01)가 높을수록 구강보건인식정도는 높은 것으로 나타났다. 따라서 향후 요양병원 장기입원환자의 구강특성을 고려한 효율적, 지속적, 실질적인 요양보호사의 구강보건교육이 이루어 질 수 있도록 하여야 할 것이다. 또한 구강보건교육의 담당자로 치과위생사의 역할이 필요 할 것으로 전문 인력의 배치를 위한 국가의 제도적 지원도 함께 이루어져야 할 것이다.

치의학대학원 학생의 장애환자에 대한 인식 조사 (DENTAL STUDENTS' PERCEPTIONS AND BEHAVIOR INTENTIONS TOWARDS PATIENTS WITH SPECIAL CARE)

  • The objective of this study was to explore perceptions of dental student of patients with special care. Their satisfaction with education, and their professional attitudes and behavioral intentions concerning treating these patients were the issues of this paper. Paper-and-pencil survey data were collected from 289 dental students at the school of dentistry, Seoul National University. Most respondents agreed that it is important to be educated about providing care for patients with special needs at the school. The higher grade students they are, they got the higher degree of understanding about treating these patients and the more satisfaction with the education. However, their intentions to treat these patients in their future professional lives were negatively correlated with degree of students. Based on these findings, it is recommended that school curriculum about treating patients with special care be reconsidered to develop students' comfort level in treating special needs populations.

일부 치과위생사의 노인 구강보건관리 제공 및 인식도에 대한 연구 (A study among dental hygienists on providing oral health care and awareness among the elderly)

  • Objectives: The purpose of this study was to evaluate dental hygienists understanding of the provisions of oral health care and awareness for the elderly. Methods: A survey was conducted among 285 dental hygienists, currently practicing around Gwangju province, Korea. The questionnaire contained questions on general characteristics of the subjects (5 items), oral health status of the elderly (5 items), and awareness (8 items) among the dental hygienists. The data were analyzed using a chi-square test. IBM SPSS 21.0 program was used for all statistical analyses. Results: The results showed that 47.7% of the dental hygienists took care of 1 to 5 elderly patients per day; they could not provide high quality oral health education to these patients because of lack of knowledge, lack of treatment time, and excessive workload. An analysis of the hygienists' professional educational experience related to oral healthcare of the elderly during university years showed that the older and more experienced hygienists' were less likely to have completed this education, i.e., 22.05% and 10.9%, respectively (p<0.001). Of all the dental hygienists, 62.8% were interested in oral health problems of the elderly, and 34.4% of dental hygienists for geriatric oral care were the most needed for geriatric oral care education (p<0.001). Conclusions: According to these results, it is important that dental hygienists participate in training on oral healthcare for the elderly and develop various programs for managing the oral health of the elderly.

Suggestions for the Work of Dental Hygienists and a Reflection of Health Insurance Charges according to Demographic Changes

  • Given that the demand for dental care for elderly individuals is expected to increase according to demographic changes, long-term roadmaps for the current health insurance system should be actively planned. The study aimed to investigate the health insurance system in Korea, compared to that of Japan, and suggest directions for the work of dental hygienists to provide basic data on efficient improvements in the health insurance system. Based on the collected data, the website was used to collect additional related data from Korea and Japan. The most common cause of death in Korea was malignant neoplasms, and most patients were hospitalized for diseases and disorders of the musculoskeletal system and connective tissue. Dental care covered by Korean medical insurance for individuals aged 65 years or older was treatment oriented, such as dentures and implants, while in Japan, treatment-intensive items, such as visiting a dental hygienist for guidance and provision of home-care professional oral hygiene treatment, were covered. The percentages of remaining teeth in Korea and the percentage of remaining 20 teeth were 68.9% in those aged 65 to 74 years and 51.2% in those aged 75 to 84 years in Japan. A strategy for promoting oral health among elderly individuals should be established, and a multilateral intervention approach is required to prevent oral problems from leading to deterioration of whole-body health. The role of dental hygienists in providing comprehensive assessment to elderly individuals is important. In order to provide systemic oral care, it is necessary to introduce oral care systems according to the national policy.

치주질환자 구강관리능력 향상을 위한 치면세균막관리(잇솔질교육)에 관한 연구 (The effect of plaque control (tooth brushing instruction) for oral health improvement on periodontitis patients)

  • Objectives : The aim of the trial was to study the effect of plaque control(tooth brushing instruction) for oral health improvement on periodontitis patients. Methods : 30 patients(35~65 years) with advanced periodontal disease were subjected to a baseline examination including assessment of oral hygiene status(O'leary index), gingival condition(bleeding). the assessment were repeated after 2, 4, 6 weeks. cognitive, behavioral and clinical outcomes were assessed in the primary care setting by questionnaire and clinical oral examination. Results : The oral hygiene status expressed as the individual mean percentage of tooth surfaces with plaque 59.3% at baseline and 21.2% after 6 weeks(p<0.001). The gingival bleeding status expressed as the individual mean point of interdental papilla 3.8 at baseline and 1.9 after 6 weeks(p<0.001). Also, the patients self-assessment about correct tooth brushing behavior was 3.9(5-point likert scales), satisfaction of plan practices 4.1(5-point likert scales), efficacy of tooth brushing instruction 8.7(10-point likert scales). Patients wanted to continuous participation. Conclusions : A verification of effect after individualized oral health instruction and repeated dental plaque control represented to significance on plaque control score, oral care practice and oral health recognition. The important oral care step against periodontal disease is to establish good oral health habits. Also, oral health behavior recognition is more important for the practice of oral health. Therefore professional plaque control and tooth brushing instruction absolutely need in improving oral health.

아동·청소년 학생치과주치의 사업의 문제점 및 개선방안 (Problems and improvements of a child·adolescent dental care service)

  • Objectives: This study identified problems and improvements in projects targeted to dental workers in a child-adolescent dental care service. Methods: The subjects were 40 dentists, 43 dental hygienists, and 51 others who participated in service in Seoul. A self-administered questionnaire was used to address the problems and improvements in the project. Descriptive statistics, chi-square test, and one-way analysis of variance were performed using SPSS 23.0. Results: Oral examination was performed by 99.2% of dentists, and some service items such as professional tooth cleaning, fluoride application, and scaling were performed by dental auxiliary personnel regardless of legal duties. The problem was that the subjects took a lot of time in providing services, and students or guardians participating in the project refused to receive preventive dental services. As regards improvements, the insurance fee was the highest (48.1%), followed by the provision of regular services (38.8%), and system improvement and administrative support (35.7%). Conclusions: To activate a child-adolescent dental care service, it is thought that continuous efforts such as providing financial support by the government, education to increase participants' understanding, and social consensus for the participation of service providers and beneficiaries are required.
