Initial experience with Scepter Mini dual lumen balloon for embolization of cerebrovascular diseases

  • Objective: Endovascular treatment of cerebrovascular diseases is often challenging due to small caliber, tortuous distal vessels. Several devices and techniques have evolved to overcome these challenges. Recently, a low profile dual lumen microballoon catheter, specifically designed for distal navigation is employed for neurovascular procedures. Due to its recent advent, scarce data is available on clinical utility and safety of Scepter Mini. The aim of this case series is to report our initial experience with Scepter Mini in the management of various cerebrovascular diseases. Methods: All interventional neurovascular cases performed using Scepter Mini between January 2020 till April 2021 were included. Data regarding patient demographics, procedural details and complications was retrospectively collected from patient's electronic medical record and procedure reports. Results: Total twelve embolization procedures were performed in eleven patients, including six brain arteriovenous malformation, two dural arteriovenous fistula, one vein of Galen malformation and three hyper-vascular glomus tumor embolizations. All procedures were successfully performed with adequate penetration of the embolic agent. Complete embolization was performed in six procedures, while intended partial embolization was performed in the rest of procedures. Scepter Mini was solely used in ten procedures, however in the other two embolization procedures it was used as an additional conjunct tool to complete the intended embolization. No balloon related complication was observed in any procedure. Conclusions: Scepter Mini dual lumen microballoon catheter is safe and feasible for delivery of liquid embolic agents for cerebrovascular embolization procedures.

  • When the nature of measuring a performance variable is destructive or very expensive, it is attractive to use a surrogate variable which is highly correlated with the performance variable and less expensive to measure. In this paper, we propose rectifying screening procedures using the performance and surrogate variables. Two screening procedures are considered; a statistically-based screening procedure to reduce the current proportion of nonconforming items to a specified lower value after screening, and an economically-based screening procedure where the screening limit is determined so that the expected cost is minimized. It is assumed that the surrogate variable given the performance variable is normally distributed with known mean and standard deviation. For two screening procedures, methods of finding the optimal solutions are presented and numerical examples are also given.

일반외과 영역 다빈도 수술률의 소규모 지역간 변이 (Small Area Variation in Rates of Common Surgery in General Surgery Department)

  • This study was performed to investigate small area variation in rates of three common surgeries in general surgery department across 156 small areas. Three surgery rates were analyzed; Anal and/or stomal procedures, inguinal and/or femoral hernia procedures, appendectomy. Methods: We used health insurance claims data and the number of patients were 13,845, 2,154 and 7,151 persons respectively. Surgery rates were directly standardized with age and sex and logistic regression was used to analyze surgery rates. Results: Mapping of the surgery rates, there was small area variation in anal and/or stomal procedures. The clinic which was located in more competitive market and chose DRG payment system tends to do more anal and/or stomal procedures. There was no factor except DRG bed rates had effect on rate of inguinal and/or femoral hernia procedures. Conclusions: Findings of this study will contribute to developing investigation method on small area variation and policy to reduce the variation such as developing evidence based medical practice guideline.

  • After analyzing each procedure from standard instrument procedures used at Incheon International Airport, which is believed to have caused noise problems in and around Ganghwa island, we came to a conclusion that some Standard Instrument Departure Procedures and Standard Instrument Approach Procedures need to be revised, and therefore we made some propositions with regard to noise abatement procedures(CDA, RNAV approach etc.). Additionally, we evaluated viability of the proposed flight procedures by conducting a number of demonstrations with use of flight simulation.

  • To evaluate seismic performance of reinforced concrete piers two procedures for capacity spectrum method are presented. The capacity spectrum procedures include the reduction factor-ductility-period($R_{\mu}-{\mu}-T$)relationship in order to construct the inelastic demand spectra from the elastic demand spectra. Application of the procedures are illustrated by example analysis. Maximum displacements estimated by the procedures are compared to those by inelastic time history analysis for several artificial earthquakes. The results show that the maximum displacements estimated by the procedures are, on overall, smaller than those by the inelastic time history analysis.

Biotrainer를 이용한 치과외래환자의 수지피부온도변화에 관한 연구 (The Change of the Digital Skin Temperature in Dental Outpatients by Biotrainer)

  • In order to evaluate objectively the anxiety level in dental treatment, the author used Biotrainer(BF-120R), one of the skin temperature biofeedback apparatus, to examine 68 dental outpatients on their digital skin temperature change due to routine consecutive dental procedures(oral examination, anesthetic injection, cavity preparation, fissure sealing, polishing). The subjective anxiety level change was also evaluated by visual analog scale. The obtained results were as follows : 1. The skin temperature decreased through consecutive procedures and the temperature in each procedure decreased until 60sec, and then increased on 120sec. 2. The temperature changes in Preparation and Injection were greater than those in other procedures. 3. Generally, male exhibited more change of skin temperature in all procedures than female. 4. Type II, continuously decreasing after procedure, occupied the most in all patients and yhad the lowest beseline temperature. 5. The anxiety level of before-procedure was higher than that of after-procedure and the levels in Injection and Preparation were higher than in other procedures.

Procedures for determination of elastic curve of simply and multiple supported beams

  • In this paper two procedures for determination of the elastic curve of the simply and multiple supported beams are developed. Determination of the elastic curve is complex as it requires to solve a strong nonlinear differential equation with given boundary conditions. For numerical solution the initial guess of the slope at the end of the beam is necessary. Two procedures for obtaining of the initial guess are developed: one, based on transformation of the supported beam into a clamped-free one, and second, on the linearization of the problem. Procedures are applied for calculating of elastic curve of a simply supported beam and a beam with three supports. Obtained results are compared. Advantages and disadvantages of both methods are discussed. It is proved that both suggested procedures give us technically accurate results.

한국, 일본, 중국, 미국의 식품위생법 비교 (Comparison of Sanitary Codes of Retail Eood Establishments of Korea,)

  • Sanitary codes of retail ffod establishment of Korea, Japan, China, and America were reviewed in order to figure out the differences of the codes. The codes of Korea & Japan are similar in many aspects. The code of America regulates food safety procedures in detail and are easy to interpret. The code of China is broad and not specific in the procedures. Korean code deals with many administration affairs and Japanese code deals with food test and business. Chinese code also deals with administration and standards. American code defines 90 different terms while the codes of rest of the countries define only few terms. For sanitization American code specifies the procedures in specific terms in detail but others do not specify the procedures. For facilities, the American code specifies location, material and procedures but other codes also specify the material but the contents of the codes are not so much specific to compare with American code.
