Resection and Observation for Brain Metastasis without Prompt Postoperative Radiation Therapy

  • Objective : Total resection without consecutive postoperative whole brain radiation therapy is indicated for patients with a single or two sites of brain metastasis, with close follow-up by serial magnetic resonance imaging (MRI). In this study, we explored the effectiveness, usefulness, and safety of this follow-up regimen. Methods : From January 2006 to December 2015, a total of 109 patients (76 males, 33 females) underwent tumor resection as the first treatment for brain metastases (97 patients with single metastases, 12 with two metastases). The mean age was 59.8 years (range 27-80). The location of the 121 tumors in the 109 patients was supratentorial (n=98) and in the cerebellum (n=23). The origin of the primary cancers was lung (n=45), breast (n=17), gastrointestinal tract (n=18), hepatobiliary system (n=8), kidney (n=7), others (n=11), and unknown origin (n=3). The 121 tumors were totally resected. Follow-up involved regular clinical and MRI assessments. Recurrence-free survival (RFS) and overall survival (OS) after tumor resection were analyzed by Kaplan-Meier methods based on clinical prognostic factors. Results : During the follow-up, MRI scans were done for 85 patients (78%) with 97 tumors. Fifty-six of the 97 tumors showed no recurrence without adjuvant local treatment, representing a numerical tumor recurrence-free rate of 57.7%. Mean and median RFS was 13.6 and 5.3 months, respectively. Kaplan-Meier analysis revealed the cerebellar location of the tumor as the only statistically significant prognostic factor related to RFS (p=0.020). Mean and median OS was 15.2 and 8.1 months, respectively. There were no significant prognostic factors related to OS. The survival rate at one year was 8.2% (9 of 109). Conclusion : With close and regular clinical and image follow-up, initial postoperative observation without prompt postoperative radiation therapy can be applied in patients of brain metastasi(e)s when both the tumor(s) are completely resected.

원발성 종격동종양 및 낭종의 외과적 치료 (Surgical Treatment of the Primary Mediastinal Tumors and Cysts)

  • 저자는 1985년 1월부터 1995년 12월까지 강북 삼성병원 흉부외과에서 수술치험한 42례의 원발성 종 격동 종양 및 낭종을 대상으로 후향적 임상분석을 실시하였다. 환자는27례의 남자와 15례의 여자로 구성되었다. 10개월의 영아부터 76세의 노인에 이르기까지 평균 연령은 40세 였다. 환자는 흉선종 12례 (28.6 %), 생식세포종 8례 (19.0 %), 원발성 낭종 7례 (16.7 %), 신경 종양 6례 (14.3 %)와 기타 종격동 종양 9례로 구성되었다. 종양의 3)례 (78.6 %)는 조직학적으로 양성이 었고, 9례 (21.4 %)는 악성 이 었다. 임상증세는 흉통, 호흡곤란, 기 침과 같은 호흡기증세로 주로 나타났다. 악성 종양을 지닌 환자는 전례 에서, 양성 환자는 55 %에서 입원당시에 증세가 나타났다. 환자 모두는 조직학적 진단과 절제를 위해 수술을 시행하였다. 2례의 유육종을 제외한 모든 양성의 종격동종양은 완전절제가 가능하였고 경과양호하였다. 그리고 일부 악성종양은 완전절제가 불가능하여 부분절제 및 항암 화학요법 혹은 방사선치료를 병용하였다. 술후 합병증은 7례(18%)였고 사망례는 없다. 결론적으로, 종격동종양은 개흥을 하기전에 다양하고 예측하기 어려운 愎昺㏏\ulcorner\ulcorner오랫동안 흉부외 과 의사에게는 관심의 대상이 되어왔으며, 적극적인 수술 및 다양한 병합요법으로 좋은 결과를 가져올 수 있다고 여겨진다.

  • Solitary fibrous tumors (SFTs) are rare neoplasms arising from mesenchymal cells. Although most SFTs are benign, rare cases of metastasis have been reported at various sites. Complete surgical resection is the mainstay of treatment for both primary and recurrent or metastatic SFTs. Herein, we present a case of an SFT initially identified in the anterior mediastinum that later developed multiple metastases, even to the thyroid gland. The patient underwent repeated surgical resection and is currently being followed up in an outpatient setting.

Pancreas-preserving limited duodenal resection: Minimizing morbidity without compromising oncological adequacy

  • Backgrounds/Aims: Pancreaticoduodenectomy is the most common procedure for the management of duodenal pathologies. However, it is associated with substantial morbidity and a low risk of mortality. Pancreas-preserving limited duodenal resection (PPLDR) can be performed under specific scenarios. We share our experience with PPLDR and its outcome. Methods: We retrospectively analyzed a prospectively maintained database of patients undergoing limited duodenal resection in the form of wedge (sleeve) resection or segmental resection of one or more duodenal segments from March 2016 to March 2021 at a tertiary care center in North India. Results: During the study period, 10 patients (including 9 males) underwent PPLDR. Five of these 10 patients showed primary duodenal or proximal jejunal pathology, while the remaining five had duodenal pathology involving an adjacent organ tumor. Four patients underwent wedge (sleeve) resection, while the remaining six underwent segmental duodenal resection of one or more duodenal segments. Mean hospital stay was 6 days (range, 3-11 days) without 30-day mortality. Morbidity occurred in 4 patients (Grade I-II, n = 3; Grade III, n = 1). All patients were alive and disease-free at the time of last follow-up. The mean follow-up duration was 23 months (range, 2-48 months). Conclusions: PPLDR is a safe and effective alternative for pancreaticoduodenectomy when selected carefully for specific tumor types and location.

종골에 발생한 골아세포종 (1례 보고) (Osteoblastoma in Calcaneus (A Case Report))

  • Benign osteoblastoma, a rare tumor which forms osteoid and woven bone, represents 3% of benign and 1% of all primary bone tumors. It can involve anywhere in the axial skeleton, which is most common in the spine (34%). But the lesion of the calcaneus is extremely rare. The ratio of males to females is 2:1, with 80 percent of the patients being under thirty years of age. Pain is the most common symptom and rarely pathologic fracture. Osteoblastoma is a benign tumor but has a high recurrence rate after intracapsular malignant change. Therefore, choice of surgical procedure is the 'enbloc' resection. Authors have experienced a case of osteoblastoma developed in calcaneus. The treatment was wide resection and triple arthrodesis. We have reported here the case in which osteoblastoma occured in calcaneus of the right foot.

늑골에 발생한 원발성 양성 종양[3 치험예] (Primary Benign Rib Tumors: 3 cases)

  • From Aug. 1975 to Aug. 1981,3 patients with primary benign rib tumors have been treated at the department of thoracic surgery, Paik Hospital, Seoul, Korea. Of these, one was aneurysmal bone cyst and two were fibrous dysplasia. Complete excision of the benign tumor including rib resection was performed in all patients and resulted in long term cure.

Clinical Outcomes of Surgical Treatment for Primary Chest Wall Soft Tissue Sarcoma

  • Background: This study investigated the clinical outcomes of surgical treatment of primary chest wall soft tissue sarcoma (CW-STS). Methods: Thirty-one patients who underwent surgery for CW-STS between 2000 and 2015 were retrospectively reviewed. The disease-free and overall survival rates were estimated using the Kaplan-Meier method, and prognostic factors were analyzed using a Cox proportional hazards model. Results: The median follow-up duration was 65.6 months. The most common histologic type of tumor was malignant fibrous histiocytoma (29%). The resection extended to the soft tissue in 14 patients, while it reached full thickness in 17 patients. Complete resection was achieved in 27 patients (87.1%). There were 5 cases of local recurrence, 3 cases of distant metastasis, and 5 cases of combined recurrence. The 5-year disease-free rate was 49%. Univariate analysis indicated that incomplete resection (p<0.001) and stage (p=0.062) were possible risk factors for recurrence. Multivariate analysis determined that incomplete resection (p=0.013) and stage (p=0.05) were significantly associated with recurrence. The overall 5- and 10-year survival rates were 86.8% and 64.3%, respectively. No prognostic factor for survival was identified. Conclusion: Long-term primary CW-STS surgery outcomes were found to be favorable. Incomplete microscopic resection and stage were risk factors for recurrence.

Preoperative Nodal 18F-FDG Avidity Rather than Primary Tumor Avidity Determines the Prognosis of Patients with Advanced Gastric Cancer

  • Purpose: This study investigated whether the metabolic avidity of primary tumors and/or metastatic lymph nodes (LNs) measured by $^{18}F$-fluorodeoxyglucose ($^{18}F-FDG$) positron emission tomography/computed tomography (PET/CT) was related to survival after surgery in patients with advanced gastric cancer (AGC). Materials and Methods: One hundred sixty-eight patients with AGC who underwent preoperative $^{18}F-FDG$ PET/CT and curative resection were included. The $^{18}F-FDG$ avidity of the primary gastric tumor and LNs was determined quantitatively and qualitatively. The diagnostic performance of $^{18}F-FDG$ PET/CT was calculated, and the prognostic significance of $^{18}F-FDG$ avidity for recurrence-free survival (RFS) and overall survival (OS) was assessed. Results: In all, 51 (30.4%) patients experienced recurrence, and 32 (19.0%) died during follow-up (median follow-up duration, 35 months; range, 3-81 months); 119 (70.8%) and 33 (19.6%) patients showed $^{18}F-FDG$-avid primary tumors and LNs, respectively. $^{18}F-FDG$ PET/CT showed high sensitivity (73.8%) for the detection of advanced pathologic T ($pT{\geq}3$) stage and high specificity (92.2%) for the detection of advanced pN (${\geq}2$) stage. $^{18}F-FDG$ avidity of LNs was significantly associated with RFS (P=0.012), whereas that of primary tumors did not show significance (P=0.532). Univariate and multivariate analyses revealed that $^{18}F-FDG$ avidity of LNs was an independent prognostic factor for RFS (hazard ratio=2.068; P=0.029). Conclusions: $^{18}F-FDG$ avidity of LNs is an independent prognostic factor for predicting RFS. Preoperative $^{18}F-FDG$ PET/CT can be used to determine the risk and prognosis of patients with AGC after curative resection.

Warthin씨 종양에서 발생한 편평상피암 1예 (A Case of Squamous Cell Carcinoma Arising in a Warthin's Tumor)

  • Warthin's tumor is the second most common benign tumor of the parotid gland, which consists of epithelial and lymphoid components. Malignant change is known to be extremely rare. In Korean literature, only a case of low grade adenocarcinoma arising from Warthin's tumor was reported. For squamous cell carcinoma, there has never been reported in Korea. The authors report a case of squamous cell carcinoma arising from Warthin's tumor in a 77-year-old male, who was treated with primary resection. The patient is well without any recurrence or metastasis after 15 months of follow-up.

The role of postoperative pelvic radiation in stage IV rectal cancer after resection of primary tumor

  • Purpose: To evaluate the effect of pelvic radiotherapy (RT) in patients with stage IV rectal cancer treated with resection of primary tumor with or without metastasectomy. Materials and Methods: Medical records of 112 patients with stage IV rectal cancer treated with resection of primary tumor between 1990 and 2011 were retrospectively reviewed. Fifty-nine patients received synchronous or staged metastasectomy whereas fifty-three patients did not. Twenty-six patients received pelvic radiotherapy. Results: Median overall survival (OS), locoregional recurrence-free survival (LRFS), and progression-free survival (PFS) of all patients was 27, 70, and 11 months, respectively. Pathologic T (pT), N (pN) classification and complete metastasectomy were statistically significant factors in OS (p = 0.040, 0.020, and 0.002, respectively). RT did not improve OS or LRFS. There were no significant factors in LRFS. pT and pN classification were also significant prognostic factors in PFS (p = 0.010 and p = 0.033, respectively). In the subgroup analysis, RT improved LRFS in patients with pT4 disease (p = 0.026). The locoregional failure rate of the RT group and the non-RT group were 23.1% and 33.7%, showing no difference in the failure pattern of both groups (p = 0.260). Conclusion: Postoperative pelvic RT did not improve LRFS of all metastatic rectal cancer patients; however, it can be recommended to patients with pT4 disease. A complete resection of metastatic masses should be performed if possible.
