국화로부터 추출한 천연염료에 대한 급성경구독성 및 피부자극성 시험에 대한 연구 (Acute Oral Toxicity and Skin Irritation Studies on Natural Dyes Extracted from Chrysanthemum)

  • 본 연구는 SD 계통의 랫드에서 국화 염료인 백색국화, 백색국화 잎과 줄기, 황색국화의 급성경구독성 및 피부자극성을 평가하기 위해서 시행하였다. 수컷 랫드에 처치한 물질은 백색국화, 백색국화 잎과 줄기, 황색국화를 각각 1 ml/kg, 2 ml/kg를 D.W에 용해시켜 단회 경구 투여하였으며, 수컷 SD rats에 투여 후 14일 동안 관찰하였으며, 피부자극성 시험은 피부의 찰과 및 비찰과부의 대조부, 처치부에 국화 염료를 처치하여 3일 동안 관찰하였다. 단회 경구 투여 및 피부 자극성 시험 후 국화 염료의 안전성을 확인하기 위해 체중과 사료 섭취량, 임상증상, 안과학적 검사, 사망률, 피부자극성을 관찰하고, 랫드 피부에서의 병리조직학적 변화를 관찰하였다. 체중, 사료섭취량, 병리조직학적 변화가 대조군과 비교시 유의적인 변화가 나타나지 않았다. 또한 피부자극성을 확인 한 결과 1차 자극 지수가 0으로 비자극성 물질로 분류되었다. 따라서 본 연구를 통하여 국화에서 추출한 염료가 급성 경구독성 시 안전한 물질이며, 피부 자극성을 야기 하지않는 물질로 평가된다.

Head and neck extra nodal NHL (HNENL) - Treatment Outcome and Pattern of failure - A Single Institution Experience

  • Background: Extra nodal lymphoma (ENL) constitutes about 33 % of all non-Hodgkin's lymphoma. 18-28% develops in the head and neck region. A multimodality treatment with multi-agent chemotherapy (CT) and radiotherapy (RT) is considered optimum. Materials and Methods: We retrieved the treatment charts of patients of HNENL treated in our institute from 2001-2012. The charts were reviewed and the demographic, treatment details and outcome of HNENL patients were retrieved using predesigned pro-forma. Results: We retrieved data of 75consecutive patients HNENL. Median age was 47years (Range: 8-76 years). Of the 75 patients 51 were male and 24 were female. 55patients were evaluable. The patient and tumor characteristics are summarized in Table 1. All patients were staged comprehensively with contrast enhanced computed tomography of head, neck, thorax, abdomen, pelvis and bone marrow aspiration and biopsy 66 patients received a combination multi-agent CT with CHOP being the commonest regimen. 42 patients received 4 or lesser number of cycles of chemotherapy whereas 24received more than 4 cycles chemotherapy. Post radiotherapy, 41 out of 42 patients had a complete response at 3 months. Only 21patients had a complete response after chemotherapy. All patients received radiation (mostly involved field radiation) as a part of the treatment. The median radiation dose was 45 Gray (Range: 36 Gray-50 Gray). The radiation was planned by 2D fluoro simulation based technique in 37cases and by 3 Dimensional conformal radiation therapy (3DCRT) in 36 cases. Two patients were planned by the intensity modulated radiation therapy (IMRT) technique. IMRT was planned for one thyroid and one nasal cavity primary. 5 patients experienced relapse after a median follow up of 19 months. The median survival was not reached. The estimated two and three year survival were 92.9% (95%CI- 68.6- 95.35) and 88% (95%CI- 60.82 - 92.66) respectively. Univariate analysis revealed higher stage and poorer baseline performance status to be significantly associated with worse progression free survival. 5 patients progressed (relapse or primary disease progression) after treatment. Of the 5 patients, two patients were primary orbital NHL, two patients had NHL nasal cavity and one was NHL thyroid. Conclusions: Combined modality treatment in HNENL confers excellent disease control with acceptable side effects.

성견 구개부 골내고정원 장치에 가해진 즉시 교정력이 주위조직에 미치는 영향 (An effect of immediate orthodontic force on palatal endosseous appliance$(C-Palatal\;Plate^{TM})$ in beagle Dog)

  • 이 연구는 구개부 골내고정원 장치 C-Palatal $Plate^{TM}$에 가해진 즉시 교정력이 장치 주위 골조직 및 연조직의 치유 반응에 미치는 영향을 조직학적으로 관찰하고자 시행되었다. 4마리의 비글 성견을 대상으로 정중 구개부에 장치를 식립한 후, 교정력을 전혀 가하지 않고 8주 경과된 한 마리를 대조군으로 하고, 식립 직후부터 8주간 지속적인 교정력을 받은 한 마리, 18주간 지속적인 교정력을 받은 한 마리, 그리고 18주간 지속적인 교정력을 받은 후 장치가 제거되고 이후 4주간의 치유기간을 거친 한 마리, 총 세마리를 실험군으로 하여 동요도 검사, 방사선 사진 관찰, 그리고 조직학적 관찰을 시행한 결과 다음과 같은 결론을 얻었다. 1. 희생 후 조직괴의 측면 방사선 사진상에서, 대조군에 비하여 즉시 교정력을 받은 군에서는 플레이트의 수직적 변위가 발생하였으나 경미하였고, 동요도 검사시 모든 군에서 초기 안정성이 확인되었으며 실험기간 중 동요도의 증가는 없었다. 2. 상부 연조직의 치유상태는 실험군과 대조군 모두 병리적 변화가 관찰되지 않았다. 3. 골유착은 교정력을 가한 8주군과 18주군 모두에서 관찰되었고 18주 경과시 골유착의 분포가 더욱 증가되었다. 그러나 압박측과 긴장측간에 골유착 분포의 차이는 관찰되지 않았으며, 대조군이 실험군에 비하여 골유착에 불리한 것으로 나타났다. 4. 스크류 경부의 변연골은 대조군에서는 뚜렷한 변화가 없었던 반면, 8주군과 18주군에서는 압박측, 긴장측과 무관하게 부분적으로 변연골 침착 및 흡수가 일어났으나 경미하였다. 5. 비강으로 관통된 스크류에서, 8주군과 18주군에서는 스크류 표면을 따라 비강측으로 신생골 침착이 관찰되었으나, 대조군에서는 관찰되지 않았다. 6. 장치 제거 4주 후, 상피층은 긴밀하게 재생되었으나 결합조직은 성글고 불규칙한 배열을 보였으며, 장치를 둘러쌌던 결합조직피막이 잔존하였다. 스크류가 식립되었던 골부위에서는 왕성한 골개조가 진행되고 있었다. 즉시 교정력에 대하여 C-Palatal $Plate^{TM}$는 임상적으로 뿐만 아니라 조직학적으로도 충분한 고정원 가치를 지니는 것으로 확인되었다.

Lipomyces starkeyi KSM 22 Glucanhydrolase 용액의 치은 섬유아세포에 대한 영향 (Effects of Lipomyces starkeyi KSM 22 Glucanhydrolase on human gingival fibroblasts)

  • A novel glucanhydrolase from a mutant of Lipomyces starkeyi KSM 22 has additional amylase activity besides mutanolytic activity and has been suggested as promising anti-plaque agent. It has been shown effective in hydrolysis of mutan, reduction of mutan formation by Streptococcus mutans and removal pre-formed sucrose-dependent adherent microbial film and has been strongly bound to hydroxyapatitie. These in vitro properties of Lipomyces starkeyi KSM 22 glucanhydrolase are desirable for its application as a dental plaque control agent. In human experimental gingivitis model and 6 month clinical trial, mouthrinsing with Lipomyces starkeyi KSM 22 dextranase was comparable to 0.12% chlorhexidine mouthwash in inhibition of plaque accumulation and gingival inflammation and local side effect was negligible. This study was aimed to evaluate the cytotoxic effect of Lipomyces starkeyi KSM 22 glucanhydrolase on human gingival fibroblasts. Primary culture of human gingival fibroblasts at the 4th to 6th passages were used. Glucanhydrolase solution was made from lyophilized glucanhydrolase powder from a mutant of Lipomyces stakeyi KSM 22 solved in PBS and added to DMEM medium to the final concentration of 0.5, 1, and 2 unit. Cells were exposed to glucanhydrolase solution or 0.1 % chlorhexidine and the cells cultured in DMEM with 10% FBS and 1% antibiotics as control. After exposure, the morphological change, cell attachment, and cell activity by MTT assay were evaluated in 0.5, 1.5, 3, 6, 24 hours after treatment. The cell proliferation and cell activity was also evaluated at 2 and 7 days after 1 minute exposure, twice a day. The cell morphology was similar between the Lipomyces smkeyi KSM 22 glucanhydrolase groups and control group during the incubation periods, while most fibroblasts remained as round cell regardless of incubation time in the chlorhexidine group. The numbers of the attached cells in the glucanhydrolase groups were comparable to that of control and significantly higher than the chlorhexidine group. The numbers of the proliferated cells in the glucanhydrolase groups at 7 days of incubation were comparable to the control group and higher than the chlorhexidine group. The cell activity in glucanhydrolase groups paralleled with the increased cell number by attachment and proliferation. According to these results, Lipomyces starkeyj KSM 22 glucanhydrolase has little harmful effect on attachment and proliferation of human gingival fibroblasts, in contrast to 0.1% chlorhexidine which was cytotoxic to human gingival fibroblasts. Therefore this glucanhydrolase preparation is considered as a safe and promising agent for new mouthwash formula in the near future.

Efficacy and Safety of Neurokinin-1 Receptor Antagonists for Prevention of Chemotherapy-Induced Nausea and Vomiting: Systematic Review and Meta-analysis of Randomized Controlled Trials

  • Objectives: Can addition of neurokinin-1 receptor antagonists (NK1-RAs) be considered as an ideal strategy for the prevention of chemotherapy-induced nausea and vomiting (CINV)? Researchers differ on this question. Materials and Methods: Electronic databases were searched for randomized control trials (RCTs) that evaluated the effectiveness and safety of NK1-RAs in preventing CINV. The primary end point was complete response (CR) in the acute, delayed, and overall phases after chemotherapy. Subgroup analyses evaluated the types of NK1-RAs, routines of administration, types of malignancies, regimens used in combination with NK1-RAs, and age of patients included in the studies. The incidences of different types of adverse events were also extracted to estimate the safety of NK1-RAs. Results: A total of 38 RCTs involving 13,923 patients were identified. The CR rate of patients receiving NK-RAs was significantly higher than patients in the control groups during overall phase (70.8% vs 56.0%, P<0.001), acute phase (85.1% vs 79.6%, P<0.001), and delayed phase (71.4% vs 58.2%, P<0.001). There were three studies including patients of children or adolescents, the CR rate was also significantly higher in the treatment group (overall phase: OR=2.807, P<0.001; acute phase: OR=2.863, P =0.012; delayed phase: OR=2.417, P<0.001). For all the other outcomes, patients in the NK1-RAs groups showed improvements compared to the control groups (incidence of nausea: 45.2% vs 45.9%, P<0.001; occurrence of vomiting: 22.6% vs 38.9%, P<0.001; usage of rescue drugs: 23.5% vs 34.1%, P<0.001). The pooled side effects from NK1-RAs did not significantly differ from previous reports and the toxicity rates in patients less than eighteen years old also did not diff between the two groups (P=0.497). However, we found that constipation and insomnia were more common in the patients of control groups, whereas diarrhea and hiccups were more frequently detected in patients receiving NK1-RAs. Conclusions: NK1-RAs improved the CR rate of CINV. They are effective for both adults and children. The use of NK1-RAs might be associated with the appearance of diarrhea and hiccups, while decreasing the possibility of constipation and insomnia.

Improved Resource Allocation Model for Reducing Interference among Secondary Users in TV White Space for Broadband Services

  • In recent years, the Television White Space (TVWS) has attracted the interest of many researchers due to its propagation characteristics obtainable between 470MHz and 790MHz spectrum bands. The plenty of unused channels in the TV spectrum allows the secondary users (SUs) to use the channels for broadband services especially in rural areas. However, when the number of SUs increases in the TVWS wireless network the aggregate interference also increases. Aggregate interferences are the combined harmful interferences that can include both co-channel and adjacent interferences. The aggregate interference on the side of Primary Users (PUs) has been extensively scrutinized. Therefore, resource allocation (power and spectrum) is crucial when designing the TVWS network to avoid interferences from Secondary Users (SUs) to PUs and among SUs themselves. This paper proposes a model to improve the resource allocation for reducing the aggregate interface among SUs for broadband services in rural areas. The proposed model uses joint power and spectrum hybrid Firefly algorithm (FA), Genetic algorithm (GA), and Particle Swarm Optimization algorithm (PSO) which is considered the Co-channel interference (CCI) and Adjacent Channel Interference (ACI). The algorithm is integrated with the admission control algorithm so that; there is a possibility to remove some of the SUs in the TVWS network whenever the SINR threshold for SUs and PU are not met. We considered the infeasible system whereby all SUs and PU may not be supported simultaneously. Therefore, we proposed a joint spectrum and power allocation with an admission control algorithm whose better complexity and performance than the ones which have been proposed in the existing algorithms in the literature. The performance of the proposed algorithm is compared using the metrics such as sum throughput, PU SINR, algorithm running time and SU SINR less than threshold and the results show that the PSOFAGA with ELGR admission control algorithm has best performance compared to GA, PSO, FA, and FAGAPSO algorithms.

  • 최근 ATRP를 이용하여 단백질에 조절된 구조의 고분자를 연결시키는 연구가 활발히 진행되고 있다. 이때, ATRF 개시제는 단백질에 있는 일차아민에 아미드화 반응을 통하여 도입할 수 있다. 그런데, 형성되는 $\alpha$-할로 아미드의 경우, 상대적으로 늦은 개시 속도를 나타내는 것으로 알려져 있어 원하는 구조의 고분자를 얻는 것이 쉽지 않다. 따라서 본 연구에서는 아미드기를 가지고 있는 개시제를 이용한 [poly(ethylene glycol)methyl ether] methacrylate(PEGMA)의 ATRP 반응에 대한 적절한 반응 조건을 찾고자 하였다. PEGMA는 FEG를 가지로 가지는 단량체로서 고분자로 만들어졌을 때, 물에 잘 녹고 단백질과의 비특이적 상호작용이 적어 단백질과의 결합을 통한 바이오 응용에 널리 사용되고 있다. 아미드기를 가지는 개시제를 이용하여 PEGMA를 성공적으로 중합하기 위한 최적의 중합 조건은 할로겐 교환 반응과 활성 감소제를 사용해서 얻을 수 있었다.

Short-course palliative radiotherapy for uterine cervical cancer

  • Purpose: The purpose of this retrospective study was to evaluate the efficacy and feasibility of short-course hypofractionated radiotherapy (RT) for the palliation of uterine cervical cancer. Materials and Methods: Seventeen patients with cancer of the uterine cervix, who underwent palliative hypofractionated 3-dimensional conformal radiotherapy between January 2002 and June 2012, were retrospectively analyzed. RT was delivered to symptomatic lesions (both the primary mass and/or metastatic regional lymph nodes). The total dose was 20 to 25 Gy (median, 25 Gy) in 5 Gy daily fractions. Results: The median follow-up duration was 12.2 months (range, 4 to 24 months). The median survival time was 7.8 months (range, 4 to 24 months). Vaginal bleeding was the most common presenting symptom followed by pelvic pain (9 patients). The overall response rates were 93.8% and 66.7% for vaginal bleeding control and pelvic pain, respectively. Nine patients did not have any acute side effects and 7 patients showed minor gastrointestinal toxicity. Only 1 patient had grade 3 diarrhea 1 week after completion of treatment, which was successfully treated conservatively. Late complications occurred in 4 patients; however, none of these were of grade 3 or higher severity. Conclusion: Short-course hypofractionated RT was effective and well tolerated as palliative treatment for uterine cervical cancer.

Rediscovering A Path to Aging in Place: Development of Housing Cooperatives for Rural Elderly

  • Profit-keeping behaviors naturally occur in the market to satisfy consumers, and the logic behind it lies in the economies of scale. On the flip side, some commodities transacted in the market are not available or can not be easily acquired unless the demand is high enough. Under this proposition, some consumers rise and find their own solution to meet the services at a reasonable cost or at an adequate level. The commonly adopted way is to establish a cooperative, and it stirs purchasing power by pooling resources and further bargains price and service quality. As a consumer cooperative, housing cooperatives notably found in rural towns enable the elderly to continue independent living. This study is to take a closer look at residential life of the rural elderly in housing cooperatives. Utilizing in-depth focus group interviews with 40 residents in four housing cooperatives, this qualitative research draws main factors affecting the decision to move in, residential assessment, and strengths and weakness of living in a housing cooperative. The primary factor influencing the moving decision is to continue to independent living in a familiar community, and the bottom line is planning ahead. Frailty and bereavement are found to be the leading occasions for them to move. The participants are satisfied with the independent living arrangement, and particularly, cited such features as safety and security, elderly-friendly design, common spaces, freedom, social activities and efficient living. Also, it is stated that some cooperative natures such as control over the property and giving a voice on management render positive impacts on the satisfaction with communal living. In spite of all the benefits and strengths, participants face with a public notion that an independent living arrangement like a housing cooperative has never done before in rural towns, so that most people recognize it as part of dependent living arrangements like nursing home.

토끼 귀에 발생한 초기 창상 반흔에 베라파밀이 VEGF의 발현 및 세포자멸사에 미치는 영향 (Effect of verapamil on VEGF expression and apoptosis in early wound scarring of the rabbit ear)

  • Purpose: Excessive scarring in the forms of keloid and hypertrophic scar could be a consquence of the accumulation of granulation tissue cells due to aberrant control of apoptosis. Verapamil retard extracelluar matrix production and inhibits VEGF production in primary cultured keloid fibroblast. The object of this study was effect of verapamil on VEGF expression and apoptosis in early wound scarring of the rabbit ear. Methods: Full thickness wounds were created on the ventral side of 6 New Zealand rabbits's ear. 16 days after initial wounding verapamil and saline were injected each scars and scars were harvested 1 week, 2 weeks, 4 weeks later. The wounds were stained with hematoxylin and eosin, TUNEL stain, immunohistochemical stain for VEGF and calculated scar elevation index. Results: Histologic analaysis demonstrated significant reduction in inflammation, vascularity and improvement in dermal collagen organization in experimental group. In TUNEL staining apotosis positive cells were increased and immunohistochemial staining of VEGF demonstrated significant reduction of VEGF expression in experimental group. No significant difference was noted in scar elevation index between two groups. Conclusion: This study suggest that intralesional injection of verapamil on early wound scarring of the rabbit ear decreased VEGF production and increased apoptosis and have a benefit on the pathophysiology of scar formation.
