Depression masquerading as chest pain in a patient with Wolff Parkinson White syndrome

  • Wolff Parkinson White (WPW) syndrome is a condition in which there is an aberrant conduction pathway between the atria and ventricles, resulting in tachycardia. A 42-year-old patient, who was treated for WPW syndrome previously, presented with chronic somatic pain. With her cardiac condition in mind, she was thoroughly worked up for a recurrence of disease. As part of routine screening of all patients at our pain clinic, she was found to have severe depression as per the Patient Health Questionnaire-9 (PHQ-9) criteria. After ruling out sinister causes, she was treated for depression using oral Duloxetine and counselling. This led to resolution of symptoms, and improved her mood and functional capability. This case highlights the use of psychological screening tools and diligent examination in scenarios as confusing as the one presented here. Addressing the psychological aspects of pain and adopting a holistic approach are as important as treatment of the primary pathology.

  • Varicella (chickenpox) is a highly contagious airborne disease caused by primary infection with the varicella zoster virus (VZV). Following the resolution of chickenpox, the virus can remain dormant in the dorsal sensory and cranial ganglion for decades. Shingles (herpes zoster [HZ]) is a neurocutaneous disease caused by reactivation of latent VZV and may progress to postherpetic neuralgia (PHN), which is characterized by dermatomal pain persisting for more than 120 days after the onset of HZ rash, or "well-established PHN", which persist for more than 180 days. Vaccination with an attenuated form of VZV activates specific T-cell production, thereby avoiding viral reactivation and development of HZ. It has been demonstrated to reduce the occurrence by approximately 50-70%, the duration of pain of HZ, and the frequency of subsequent PHN in individuals aged ${\geq}50$ years in clinical studies. However, it has not proved efficacious in preventing repeat episodes of HZ and reducing the severity of PHN, nor has its long-term efficacy been demonstrated. The most frequent adverse reactions reported for HZ vaccination were injection site pain and/or swelling and headache. In addition, it should not be administrated to children, pregnant women, and immunocompromised persons or those allergic to neomycin or any component of the vaccine.

  • Understanding phosphorus metabolism in photosynthetic organisms is important as it is closely associated with enhanced crop productivity and pollution management for natural ecosystems (e.g., algal blooming). Accordingly, we exploited highly time-resolved metabolic responses to different levels of phosphate deprivation in Chlamydomonas reinhardtii, a photosynthetic model organism. We conducted non-targeted primary metabolite profiling using gas-chromatography time-of-flight mass spectrometric analysis. Primarily, we systematically identified main contributors to degree-wise responses corresponding to the levels of phosphate deprivation. Additionally, we systematically characterized the metabolite sets specific to different phosphate conditions and their interactions with culture time. Among them were various types of fatty acids that were most dynamically modulated by the phosphate availability and culture time in addition to phosphorylated compounds.

150 nm Pitch Measurement using Metrological AFM (길이 소급성을 갖는 AFM을 이용한 150nm 피치 측정)

  • Pitch measurements of 150 nm pitch one-dimensional grating standards were carried out using an contact mode atomic force microscopy(C-AFM) with a high resolution three-axis laser interferometer. It was called as 'Nano-metrological AFM' In Nano-metrological AFM, Three laser interferometers were aligned well to the end of AFM tip. Laser sources of the three-axis laser interferometer in the nano-metrological AFM were calibrated with an I$_2$-stablilzed He-Ne laser at a wavelength of 633 nm. So, the Abbe error was minimized and the result of the pitch measurement using the nano-metrological AFM has a traceability to the length standard directly. The uncertainty in the pitch measurement was estimated in accordance with the Guide to the Expression of Uncertainty in Measurement(GUM). The Primary source of uncertainty in the pitch-measurements was derived from repeatability of pitch-measurement, and its value was approx 0.186 nm. Expanded uncertainty(k=2) of less than 5.23 nm was obtained. It is suggested that the metrological AFM is a useful tool for the nano-metrological standard calibration.

Development of a Time-selective Self-triggering Water Sampler and Its Application to In-situ Calibration of a Turbidity Sensor

  • Seawater sampling is the primary task for the study of the marine environmental parameters that require shipboard or laboratory experiments for their analyses, and is also required for the calibration of some instruments for in situ measurement. A new automatic bottle (AUTTLE) is developed for seawater sampling at any desired time and water depth by self-triggering. Both any type of single or assembled mooring for 15 days and manual actuation by using a remote messenger as existing instantaneous single point water samplers are possible. Its sampling capacity and the resolution of time setting are 2 liters and 1 second, respectively. The result of a field experiment with an optical backscattering sensor (OBS) and a total of 14 AUTTLES for the in situ calibration of the OBS shows that the AUTTLE must improve our understanding on the behavior of the sand/mud mixtures in the environments with high waves and strong tides. The AUTTLE will serve as a valuable instrument in the various fields of oceanography, especially where synchronized seawater sampling at several sites is required and/or the information in storm period is important.

Research Through Empirical Analysis on SCM Efficiency Aspects of Retail Ready Packaging System

  • In this study, the relationship between packaging design and logistics efficiency raised in prior research was to be conducted through empirical analysis in terms of SCM efficiency. The main research contents were carried out in the pursuit of efficiency of the relevant store operation, relationship with SCM efficiency aspects, resolution of differences in recognition between manufacturers and distributors, establishment of automated systems for RRP production facilities and processes in the manufacturer's facilities, logistics costs, environment and quality safety, and promotion of RRP through the manufacturer's packaging system. As a result of the verification of SCM efficiency aspects through this study, logistics efficiency will be reduced in the case of the packaging system or delivery system, where only the operational efficiency of the store is the top priority(χ2 = 178.500, p < 0.001). This is because of the strong interaction between packaging and supply chain activities, many packaging systems affect the performance criteria of the supply chain, and the content that packaging personnel should ensure that the packaging system meets other essential requirements while meeting the primary goal of protecting the product is also a result consistent with the preceding study. An analysis of whether the RRP promotion through the manufacturer's packaging system would result in improved loading efficiency showed that there was a statistically meaningful difference at a significant level of 0.000, as χ2 = 140.133.

This measurement technique was named as the 'nano-metrological AFM'. In the nano-metrological AFM, three laser interferometers were aligned precisely to the end of an AFM tip. Laser sources of the three-axis laser interferometer in the nano-metrological AFM were calibrated with an I$_2$ stabilized He-Ne laser at a wavelength of 633 nm. Therefore, the Abbe error was minimized and the result of the pitch measurement using the nano-metrological AFM could be used to directly measure the length standard. The uncertainty in the pitch measurement was estimated in accordance with the Guide to the Expression of Uncertainty in Measurement (GUM). The primary source of uncertainty in the pitch-measurements was derived from the repeatability of the pitch-measurements, and its value was about 0.186 nm. The average pitch value was 146.65 nm and the combined standard uncertainty was less than 0.262 nm. It is suggested that the metrological AFM is a useful tool for the nano-metrological standard calibration.

Three-dimensional Turbulent Flow Analysis in Curved Piping Systems Susceptible to Flow-Accelerated Corrosion (유동가속부식이 잠재한 곡관내의 3차원 난류유동 해석)

  • The three-dimensional turbulent flow in curved pipes susceptible to flow-accelerated corrosion has been analyzed numerically to predict the pressure and shear stress distributions on the inner surface of the pipes. The analysis employs the body-fitted non-orthogonal curvilinear coordinate system and a standard $ {\kappa}-{\varepsilon}$ turbulence model with wall function method. The finite volume method is used to discretize the governing equations. The convection term is approximated by a high-resolution and bounded discretization scheme. The cell-centered, non-staggered grid arrangement is adopted and the resulting checkerboard pressure oscillation is prevented by the application of a modified version of momentum interpolation scheme. The SIMPLE algorithm is employed for the pressure and velocity coupling. The numerical calculations have been performed for two curved pipes with different bend angles and curvature radii, and discussions have been made on the distributions of the primary and secondary flow velocities, pressure and shear stress on the inner surface of the pipe to examine applicability of the present analysis method. As the result it is seen that the method is effective to predict the susceptible systems or their local areas where the fluid velocity or local turbulence is so high that the structural integrity can be threatened by wall thinning degradation due to flow-accelerated corrosion.

DEM generation from KOMPSAT-1 Electro-Optical Camera Data

  • The first Korean remote sensing satellite, Korea Multi-Purpose Satellite (KOMPSAT-1), is going to be launched in 1999. This will carry a 7m resolution Electro-Optical Camera (EOC) for earth observation. The primary mission of the KOMPSAT-1 is to acquire stereo imagery over the Korean peninsular for the generation of 1:25,000 cartographic maps. For this mission, research is being carried out to assess the possibilities of automated or semi-automated mapping of EOC data and to develop, if necessary, such enabling tools. This paper discusses the issue of automated DEM generation from EOC data and identifies some important aspects in developing a for DEM generation system from EOC data. This paper also presents the current status of the development work for such a system. The development work has focused on sensor modelling, stereo matching and DEM interpolation techniques. The performance of the system is shown with a SPOT stereo pair. A DEM generated from a commercial software is also presented for comparison. The paper concludes that the proposed system creates preferable results to the commercial software and suggests future developments for successful generation of DEM for EOC data.

  • PET detects only less than 50% of early gastric cancer and 62-98% of advanced gastric cancer. Therefore, mass screening programs are recommended for all adults over the age of 40 for early detection and early treatment of gastric cancer through endoscopy or various radiological tests. The most important step after being diagnosed with gastric cancer is accurate staging, which mainly evaluates tumor resectability to avoid unnecessary surgery. Important factors that affect tumor resectability are whether the tumor can be separated from adjacent organs or important blood vessels, the extent of lymph node metastasis, presence of peritoneal metastasis, or distant organ metastasis. To evaluate the extent of local tumor invasion, anatomical imaging that has superior spatial resolution is essential. There are a few studies on prognostic significance of FDG uptake with inconsistent results between them. In spite of lower sensitivities for lymph node staging, the specificities of CT and PET are very high, and the specificity for PET tends to be higher than that for CT. Limited data published so far show that PET seems less useful in the detection of lung and bone metastasis. In the evaluation of pleural or peritoneal metastasis, PET seems very specific but insensitive as well. When FDG uptake of the primary tumor is low, the distant metastasis is also known to show low FDG uptake reducing its detection. There are only a few data available in the evaluation of recurrence detection and treatment response using FDG PET.
