Two Cases of Primary Osteolytic Intraosseous Meningioma of the Skull Metastasizing to Whole Skull and the Spine

  • We report here two cases of primary intraosseous meningioma with aggressive behavior. A 68-year-old man presented with a one year history of a soft, enlarging mass in the right parietal region. Magnetic resonance image (MRI) revealed a 6 cm sized, heterogeneously-enhancing, bony expansi1e mass in the right parietal bone, and computed tomograph (CT) showed a bony, destructive lesion. The tumor, including the surrounding normal bone, was totally resected. Dural invasion was not apparent Diagnosis was atypical meningioma, which extensively metastasized within the skull one year later. A 74-year-old woman presented with a 5-month history of a soft mass on the left frontal area. MRI revealed a 4 cm sized, multilobulated, strongly-enhancing lesion on the left frontal bone, and CT showed a destructive lesion. The mass was adhered tightly to the scalp and dura mater. The lesion was totally removed. Biopsy showed a papillary meningioma. The patient refused adjuvant radiation therapy and later underwent two reoperations for recurred lesions, at 19 and at 45 months postoperative. The patient experienced back pain 5 years later, and MRI showed an osteolytic lesion on the 11th thoracic vertebra. After her operation, a metastatic papillary meningioma was diagnosed. These osteolytic intraosseous meningiomas had atypical/malignant pathologies, which metastasized to whole skull and the spine.

Malignant Schwannoma of the Esophagus: A Rare Case Report

  • Neurogenic tumors are the most prevalent tumors of the mediastinum, and schwannomas are the most common type of neurogenic tumor. Primary neurogenic neoplasm of the esophagus is uncommon and malignant schwannoma of the esophagus is extremely rare. We report a case of a 27-year-old female presenting with dysphagia and palpitations who was found to have a lobulated tumor in the mediastinum that was compressing the esophageal lumen. The tumor was successfully treated surgically without recurrence. The final diagnosis, on histopathological examination of the specimen, was malignant schwannoma.

  • Malignant fibrous histiocytoma (MFH), a type of sarcoma, is a malignant neoplasm with uncertain origins that arise from both the soft tissues and the bone. The occurrence of MFH on the chest wall is extremely rare. We hereby report a case of a 72-year-old woman who was incidentally detected with MFH after a traffic accident.

Expression and Activity of the Na-K ATPase in Ischemic Injury of Primary Cultured Astrocytes

  • Astrocytes are reported to have critical functions in ischemic brain injury including protective effects against ischemia-induced neuronal dysfunction. Na-K ATPase maintains ionic gradients in astrocytes and is suggested as an indicator of ischemic injury in glial cells. Here, we examined the role of the Na-K ATPase in the pathologic process of ischemic injury of primary cultured astrocytes. Chemical ischemia was induced by sodium azide and glucose deprivation. Lactate dehydrogenase assays showed that the cytotoxic effect of chemical ischemia on astrocytes began to appear at 2 h of ischemia. The expression of Na-K ATPase ${\alpha}1$ subunit protein was increased at 2 h of chemical ischemia and was decreased at 6 h of ischemia, whereas the expression of ${\alpha}1$ subunit mRNA was not changed by chemical ischemia. Na-K ATPase activity was time-dependently decreased at 1, 3, and 6 h of chemical ischemia, whereas the enzyme activity was temporarily recovered to the control value at 2 h of chemical ischemia. Cytotoxicity at 2 h of chemical ischemia was significantly blocked by reoxygenation for 24 h following ischemia. Reoxygenation following chemical ischemia for 1 h significantly increased the activity of the Na-K ATPase, while reoxygenation following ischemia for 2 h slightly decreased the enzyme activity. These results suggest that the critical time for ischemia-induced cytotoxicity of astrocytes might be 2 h after the initiation of ischemic insult and that the increase in the expression and activity of the Na-K ATPase might play a protective role during ischemic injury of astrocytes.

A Study on Abdominal Temperature of Dysmenorrhea Patients

  • Dysmenorrhea is one of the common gynecologic disorders of menstruation women. Primary dysmenorrhea is menstrual pain without pelvic pathology. whereas secondary one is painful mestruation with underlying pathology. The cause of primary dysmenorrhea is increased endometrial prostaglandin. The mechanisims underlying secondary dysmenorrhea are not eluciated. There are many blood vessels under the skin and they play a very important role in the thermal control of peripheral part. The control of blood circulation is mainly controlled by autonomic nervous system and it is known that D.I.T.I. (digital infra-red thermographic imaging) is an objective method showing the body temperature. I observed the abdominal themerature of 49 patients complaning of dysmenorrhea who visited the gynecologic department of Pundang Oriental Medical Hospital during 1997. In order to rule out thermal abnormality due to obesity. the causes whose obesity index were above I. were excluded in this study. The mean temperature of lower abdomen of the dysmenorrhea group was $35.22{\pm}1.33^{\circ}C$ and control group was $36.01{\pm}0.74^{\circ}C$.

종합전문요양기관의 간호전달체계에 대한 실태조사 연구 (Survey on Nursing Care Delivery Systems of University Affiliated Hospitals in Korea)

  • Purpose: This study investigated nursing care delivery systems in 44 university affiliated hospitals and satisfactions with the systems perceived by 226 unit managers (head nurses) of general medical surgical wards. Methods: Data were collected with questionnaires consisting of checklists asking the unit managers their nursing care delivery systems and their satisfactions with the systems. Results: Four models of nursing care delivery systems (primary, modified primary, team, and functional models) were drawn from the participants' responses. Among the four key models 35% of the units adopted team model whereas 24.3% adopted primary model and 22.6% adopted modified primary model. In spite of 35% of team model being under use, 60.6% (n=137) of the unit managers answered the nursing delivery system of their units as team model and only 6.2% (n=14) answered their units having primary or modified primary models, instead of 46.9% combining both. In regard to the satisfaction, critical thinking ability of staff nurses (members in their units) was the most dissatisfactory area regardless of models of service delivery. Conclusion: Introducing team model supplemented with core concepts of primary model (primary team delivery model) into nursing practice will reform the workplace and therefore deliver safe health care services to patients.

유치 발거 원인에 대한 후향적 연구 (Reasons for primary teeth extraction in children : Retrospective study)

  • The purpose of this study was to investigate the principal reasons for primary teeth extractions and the tooth type extracted in children. 1159 patients were selected in this study. Dental records and radiographs were reviewed and age, gender, medical history, type of tooth extracted and the reasons for extraction were collected. The data were statistically analyzed using Chi-square test. Total 2078 primary teeth were extracted. Central incisors(34.1%) were most frequently extracted. Extractions due to physiological mobility(77.5%) were the most frequent followed by caries(13.8%), orthodontic(3.9%), trauma(1.7%). Reason for the extraction was different according to age (p = 0.000), but there was no difference according to gender (p = 0.109). While extractions due to physiological mobility predominated overall, reason for extraction was different according to the tooth type. There was no previous treatment in 54.6% of primary teeth extracted due to caries. Excluded physiological mobility, caries are the most common reason for extraction of primary teeth. The importance of preventive care should be emphasized in order to preserve primary teeth and improve children's oral health.

  • Purpose: The research looks at differences between the 'General ward' and the 'Comprehensive nursing ward' in Seoul Medical Center, regarding the facility improvements and changes in nursing services. It investigates and analyzes spatial problems and improvement needs through a survey and conduct investigation of staffs. It is to propose the primary data for the architectural planning of the future ward with the comprehensive nursing service. Methods: Targeting the comprehensive nursing ward, changes of the physical environment and spatial problems are analyzed through a field survey, behavior investigation, present-condition investigation, and floor plan analysis. Results: The workforce is increased by approximately twice the amount of the nursing staffs in the comprehensive nursing ward, compared to the pre-general ward. When utilizing the general ward, various spatial problems arise due to the restrictions of the facility condition. Because Sub N.S is an important facility as a part of the nursing work function in the ward of the comprehensive nursing service, the opinions of staffs must be considered when selecting a location and composing a space. Implications: It can be used as a primary data for the comprehensive nursing ward when architectural planning of a new hospital.

Neuroprotective effects of L-carnitine against oxygen-glucose deprivation in rat primary cortical neurons

  • Purpose: Hypoxic-ischemic encephalopathy is an important cause of neonatal mortality, as this brain injury disrupts normal mitochondrial respiratory activity. Carnitine plays an essential role in mitochondrial fatty acid transport and modulates excess acyl coenzyme A levels. In this study, we investigated whether treatment of primary cultures of rat cortical neurons with L-carnitine was able to prevent neurotoxicity resulting from oxygen-glucose deprivation (OGD). Methods: Cortical neurons were prepared from Sprague-Dawley rat embryos. L-Carnitine was applied to cultures just prior to OGD and subsequent reoxygenation. The numbers of cells that stained with acridine orange (AO) and propidium iodide (PI) were counted, and lactate dehydrogenase (LDH) activity and reactive oxygen species (ROS) levels were measured. The 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide assay and the terminal uridine deoxynucleotidyl transferase-mediated deoxyuridine triphosphate nick-end labeling assay were performed to evaluate the effect of L-carnitine (1 ${\mu}M$, 10 ${\mu}M$, and 100 ${\mu}M$) on OGD-induced neurotoxicity. Results: Treatment of primary cultures of rat cortical neurons with L-carnitine significantly reduced cell necrosis and prevented apoptosis after OGD. L-Carnitine application significantly reduced the number of cells that died, as assessed by the PI/AO ratio, and also reduced ROS release in the OGD groups treated with 10 ${\mu}M$ and 100 ${\mu}M$ of L-carnitine compared with the untreated OGD group (P<0.05). The application of L-carnitine at 100 ${\mu}M$ significantly decreased cytotoxicity, LDH release, and inhibited apoptosis compared to the untreated OGD group (P<0.05). Conclusion: L-Carnitine has neuroprotective benefits against OGD in rat primary cortical neurons in vitro.

보건진료전담공무원의 업무분석과 직무만족도 (Working Patterns and Job Satisfaction in Primary Health Practitioners)

  • Purpose: The purpose of this study was to identify the frequency and duration of primary health practitioners' work, and their job satisfaction, and to confirm differences in work and job satisfaction by type of primary health care post. Methods: Work frequency, duration of work, and job satisfaction were estimated by 371 primary health practitioners. Chi-square test and t-test were used to identify the differences in working patterns and job satisfaction by type of primary health care post. Results: Primary health practitioners were found to spend more time working with the elderly population than with students, pregnant women, children, people with disabilities, and multicultural families. Those in costal areas were more concerned with students than those working inland. In the latter group of practitioners, more time was spent working with patients with chronic diseases, pregnant women, women, children, multicultural families, and mental health clients. Also, the job satisfaction of inland primary health practitioners was significantly higher than that of costal practitioners. Conclusion: It is necessary to identify the characteristics of primary health practitioners' work, focusing on changes in the medical service environment. Furthermore, it is necessary to provide job training according to type of primary health care post, as practitioners' approaches should differ between posts.
