Purpose: To analyze health programs of the PHCP (Primary Health Care Posts) Method: From August 2006 to July 2007, data on the general quality and health program of the PHCP was requested by official letter and replies were received via E-mail. From December 8 to December 30, 2007, data from 1,268 (66.8%) PHCP out of 1,897 PHCP were analyzed using SPSS 12.0 Win program. Results: The average population covered by each PHCP is 878.3 people. For the health and special programs, Community Health Practitioners report high motivation for programs on health promotion, management of chronic illness, social welfare (40-50%). Demand by the residents was reported at 10% and increases in the health of the residents were attributed to high interest and demand. Volunteer work was 83.3% for bathing, 54.5% for equipment support and 46% for exercise programs. As elders make up 30% of the population in rural areas, there is an increasing demand for volunteer work in bathing programs. Conclusions: As the number of elders in the population increases and there is an increased need for more medical treatment for older people who are sick, the role of PHCP must be strengthened to include visits to homes of community residents. Where financial support for the PHCP is difficult, it is necessary to develop sound data on demographic characteristics of the population in order to develop efficient and effective health promotion programs. The finding that 54.7% of the population need management of chronic illness has difficulty in seeing a physician indicates a need to enhance the health care delivery system by strengthening the role of the Community Health Practitioners and including them in the civil service system to ensure stability of the PHCP.

Purpose: This study was performed to identify job competencies and needs for job education perceived by new community health practitioners. Methods: This study used a qualitative research design. Eight new community health practitioners participated in this study. Data were collected through in-depth interviews and analyzed using content analysis. Results: Five job competencies were identified in this study, including primary care, public health management, interpersonal relationship, teaching and counseling, and leadership. The contents of job education that they needed were management of major symptoms and chronic diseases, understanding and prescription of medications, emergency responses and care, management of endemic diseases, planning and management of public health programs, writing official documentsand computer works, and leadership training. The learning methods they preferred were connecting theory and practice, situation- or case-based learning, skill- or practice-based learning, and increased opportunities of clinical practice. Conclusion: The findings of this study provided the direction of job education for new community health practitioners. Job education for new community health practitioners needs to consider the job competencies and educational needs identified in this study.
The purpose of this study was to analyze the level of the services provision of community health practitioners (CHP) and to find out the influence factors on the services provision of CHP. In this study the dependent variables were the level of community health services(CHS), maternal and child health services(MCH), family planning services(FPS), primary care services(PCS) and the ratios of preventive health services(PHS). And independent variables were predisposing, community demographic and task factors. For this analysis, atepwise regression was used. Data collected for the study on reorganization of health centers organization in 1985 was partly used. The findings of this study can be summarized as follows : First, total variance of independent variables for CHS, MCH, FPS, PCS and PHS are shown 62.5 percent, 58.3 percent, 41.8 percent, 17 percent and 61.9 percent respectively. Second, the most important variables which explain CHS, MCH, FPS, PCS and PHS was ratios of household contacted ($R^2$=0.289), marital status ($R^2$=0.177), marital status($R^2$=0.167), ratios of household contacted($R^2$=0.119) and management of preventive health services($R^2$==0.203) respectively. The independent varivbles used in this analysis presented that the explnining for the provision of preventive health service are more influenced than primary care services. In summary this analysis suggests that the level of preventive health services provision of CHP is low and the provision of primary care services compared with preventive health services are occurred independentely. In the future, the strategies for active preventive services by CHP must to be strengthened.
Health policy is directed to equity in the provision of primary care for rural people before the year of 2, 000. This study aimed to define and identify the need for physician's care by using empirical data, and suggested an alternative of the primary care delivery system in rural areas to the government. 1. Twenty percent of the study population wanted to obtain any form of medical care services. : 9.3 percent of the population was in need for physician's care; 15 percent of the need was met by physicians, while 85 percent remained unmet at the time of survey in 1979. 2. For meeting all the need for physician's primary care, 2.9 annual physician visits per capita are demanded. An alternative, which was devised in some favourable way at reasonable cost in rural settings, was suggested. It was to deploy the physician extender such as community health practitioner in the infrastructure of the health care delivery system, whose supervision is provided by physician, based on experience of the KHDI health demonstration project. 3. One physician, two community health practitioners and two community health aides should be assigned in distant locations for meeting all the estimated need for physician's primary care for 10, 000 rural underserved residents.
This study was conducted to follow the patient referral system operated from the primary health care facilities to the hospital in a rural area of Korea. The subject for this study was sampled from a community health development project carried out by the Korea University in Yeoju Kun, Kyonggi Province. The data of referred patients from primary health care facilities were collected during the period from January 1989 to December 1989. The data was sorted out by a computer system using Database package. The results of this study were summarized as follows: 1. Characteristics of the referred patients were: males $32.0\%$. and females $68.0\%$. The more elderly of the patients visited to the hospital after having been referred there by CHPs or public physicians, $25.9\%$ has been to hospital on at least one previous occasion as against $74.1\%$ for whom it was there first visit. 2. The majority of patients who were referred to a hospital where: medicine $44.3\%$ and orthopedics$16.4\%$, major diseases were : diseases of digestive system $(21.3\%)$ ; symptoms and ill defined conditions $(17.3\%)$ ; diseases of the muscular skeletal system and connective tissue$(14.2\%)$. chronic illness was $82.0\%$ and acute illlness was $18.0\%$. 3. From Community health practitioners more patients referred than the public physicians. Categoris of diseases of the referred patients were different between community health practitioners and public physicians. Due to the. respective differences between the medical restrictions put on the nursing staffs at the community health practitioners and public physicians. From this study it was recommended to define the reason of differences between ~he two groups in futher study. Study as to 1) why one group should be referring more for hospital treatment than the other. And 2) why the two agencies should be referring different diseases.
By the rural area health care special law in 1980, Primary health care posts were established in rural areas as fundamental elements of the national health system. Nurses have been deployed to the posts after taking an education course mandated by the special law. However, health care posts have confronted environmental changes over the past 30 years such as an aging and decreasing rural population and advanced traffic systems, which make it necessary to reshape their form and role. Therefore, some guidelines are suggested for future role enlargement of health care posts by analyzing their current management and duties. The guidelines are as follows: 1) enlarging the portion of prevention and management of chronic degenerative diseases, 2) development and practice of diverse health promotion programs, 3) extension of primary health care for the increasing older population, 4) development of health programs for married immigrants, 5) practice of timely maternal child health programs, 6) development of adequate health care posts for low-income people in rapidly urbanizing rural areas and in poor areas in big cities, and 7) revision of laws and institutional arrangements for the role enlargement of health care posts to match social changes and customer needs.
Purpose: The purpose of this study was to examine the trends of disease of residents who had treatment in primary health care posts in N-county over the past seven years (1997-2003). The data will assist in planning responses to changes in the health care environment and in planning health promotion programs. Method: A retrospective descriptive survey was conducted of the computerized records of primary health care done by community health practitioners over the last seven years. Cross analysis was conducted among the data using SAS, and the results were displayed in frequencies and percentages. The data were collected from May to September 2004. Results: The results of the study are as follows: 1. The six most frequent diseases were ranked as follows: diseases of the 1) respiratory system, 2) musculoskeletal system and connective tissue, 3) digestive system, 4) skin and subcutaneous tissue, 5) circulatory system, and 6) symptoms, signs and abnormal clinical and laboratory findings. Especially in 2002, circulatory system related disease rose two steps in the ranking and digestive system related disease went a step down. 2. The patients' use of primary health care posts over the past seven years continued to increase on the whole. Conclusions: According to these results, each primary health care post should carry out special health promotion programs that fit the local society of N- county and are in accordance with changes in health care needs.
Purpose: The study was done to compare the status of dispatch of public officials in charge of health care in response to COVID-19 according to location of primary health care posts and the practitioners' knowledge, attitude and support related to COVID-19. Methods: Among the 193 practitioners surveyed, 175 people in Chungcheong-do, Jeolla-do and Gyeongsang-do were selected for final analysis according to work status and differences in knowledge, attitude and support. Data were analyzed using χ2 and one-way ANOVA, followed by a Scheffé-test. Results: There was a statistically significant difference by region in the number of days and places for dispatch teams to respond to COVID-19. In particular, in the case of support, the overall score was low, but Gyeongsang-do, which had the longest number of dispatch days due the occurrence of a large-scale confirmation of patients, showed higher support compared to other regions. Knowledge of COVID-19 was high, but there were no regional differences. Conclusion: Regarding performance of COVID-19 response tasks, the support level was low, indicating that psychological support and appropriate compensation from the local government are required. In addition, it is necessary to prepare a system for the provision of alternative services to prevent a work gap in health clinics.
Purpose: The aims of this study were to identify successful strategies and propose a community-based participatory global health project model for primary health care capacity development. Methods: The study used case study methodology. A The unit of analysis was an international cooperation health project entitled "Community-based Primary Health Care Improvement in San Lorenzo, Ecuador" using community-based participatory research conducted in 2007~2008. Data were collected through windshield surveys, focus group discussion, key informant interviews, and provider surveys. Results: Identified successful strategies for the international cooperation health project were reciprocal partnership between researchers and community, partners' capacity building, south-to-south cooperation, and continuous monitoring and feedback. Community participation was found to be an essential tenet to guarantee the improvement of primary health care in the underserved rural community. Evidence from the activities of community health practitioners in Korea was applicable to the development of training programs for primary health care providers in Ecuador. Conclusion: Strategies for strengthening primary health capacity may be tailored depending on socio-cultural, political, and economical situations of each country. The model, however, would be applicable to the entire process of community-based global health projects in underserved rural communities of other countries.

Based on literature, status and role of the NP in America was reviewed. The process of developing NP program in America suggests us many things. In America, nurse practitioners have sustained a mutually beneficial status with their patients for over thirty years. Excel fence in academic education and clinical training will enable nurse practitioners to continue to provide quality health care. The magnitude changes in the health care system of the United States, the challange of providing real access of health care continues. Lack of access to adequate primary care was the driving force in the initial 1965 Federal Involvement in developing the NP role. In 1993 President Bill Clinton's health care reform initiative provided policy support for NPs as primary care providers. The Institute of Medicine explicitly recognized NPs as an integral part of the primary care team. In addition, several national reports recognized NPs as affordable, accessible, high-quality care providers. The recent passage of direct Medicare reimbursement for NPs reflected public policy statements coincided with and likely contributed to a growth spurt in the NP workforce. From 1965 to 1977 NP programs offered traditional primary care clinical tracks(adult, family, woman's health, and pediatrics) for relatively small clusters of students in a variety of institutional settings. From 1978 to 1990 these educational programs were incorporated into graduate schools of nursing. By 1990 the majority of NPs received educational preparation in master's-level nursing programs. A new emphases was placed on postmaster's NP programs designed for master's prepared clinical nurse specialists and nurse managers. he the health care system shifted hospital nursing resources toward community-based care, these master's -level nurses sought additional NP preparation. NP educational programs are defined as the educational structure in which one or more NP clinical tracks are offered. NP clinical tracks, in turn, offer curriculum and supervised clinical experiences that match standards in specific practice areas such as family(FNP), adult(AUP), geriatrics(GNP), pediatrics(PNP), women's health (WHNP), neonatal (NNP), and acute care(ACNP). There were indications that NP practice was expanding into new clinical areas as evidenced by new types of tracks, particularly in acute care and psychiatry. The increase in acute care NP students likely reflects the increased demand from hospitals and other acute care settings. In Korea, change of nurse's role into nurse practitioner's role may have many difficulties. The need of health consumer, policy support of government, approval of medical care team are all essential component. Every nursing personnel make effort to planning the new health care delivery system.
