Recent Progress in the Care of Atopic Disease in Children (아토피성 질환관리의 최신 동향)

  • Background: Atopic dermatitis starts as an early childhood type I hypersensitivity to environmental allergens and is often the first step in the atopic march to develop into asthma and allergic rhinitis. Despite progress that has been achieved in management, this health problem remains poorly controlled and cause great pain and suffering for many children and their parents To introduce nurses to recent progress in pathophysiology, management and preventive measure of atopic disease. Search method: Systemic search was done using the PubMed and CINAHL from 1980 to 2005. In addition, historical references were taken from standard medical textbook. Results: In total, 30 relevant publications were located including primary research and review articles that cover the pathophysiology, management, and preventive measure of atopy. The evidence emerging from literature indicate that non-medical approaches such as breastfeeding and probiotics would improve management outcomes. Conclusion: The review suggests that breastfeeding and probiotic approaches would be the most effective preventive measures for children with atopic diseases.

  • PACS has been run at the Kyung Hee University Medical Center since 2001, and the installation and operation of PACS have contributed to automation and quantification of center's medical environment. In order to classify tile annual number of trouble cases processed by PACS, the authorshave made a classification code system which enabled detailed statistical processes for each section. Such process method has not only shown the management efficiency to trouble management of PACS, but also raised the interests in frequently occurring troubles, and enabled the prediction of troubles that may occur hereafter. Predictable troubles lead to preventive check, and this has direct effects on medical treatment and the hospital administration. The authors intend to arouse the necessity of preventive check of PACS by analyzing trouble management processes for the last 1 year.

The recent trend and determinants of service diversification in Korean hospital (우리나라 병원의 서비스 다각화추세와 관련요인분석)

  • Service diversification is recognized as an important strategy against turbulent environmental change. This study is designed to find out the trend of service diversification in Korean health care organizations and also to identify factors associated with the degree of service diversification. Data were collected from 69 hospitals out of 71 hospitals with over 300 beds. Important findings are summarized below. 1. Types of diversification are closely related to hospital size. Large hospitals have a tendency to provide sophisticated services requiring specialized skills and equipment, while small hospital have concentrated their efforts on health screening programs. 2. The more competitive and bigger hospitals are, the greater number of services that provide. Also, hospitals operating rational management information systems provide more services. Contrary to the expectation hospitals with a low performance during last 3 years showed more service diversification. 3. A trend of more diversification was observed in hospitals whose chief executive officer used a prospector strategy. 4. A multiple regression analysis revealed that bed size, competitive environment, degree of rational management, and the growth pattern were significantly associated with the service diversification.

Preventive Health Management Self-Efficacy related to Premature Labor (PHMSE-PL) scale for Korean women of childbearing age: instrument development and validation

  • Purpose: This study aimed to develop and examine the validity and reliability of a self-efficacy scale for preventive health management related to premature labor (PHMSE-PL) for women of childbearing age. Methods: Instrument development and validation were undertaken in three steps: conceptualization through a literature review and in-depth interviews, item generation and evaluation of content validity, and evaluation of construct validity and reliability. The content validity, factorial structure validity, and internal consistency reliability of the PHMSE-PL were evaluated, and cognitive interviewing was undertaken. Data were analyzed using confirmatory factor analyses, Cronbach's α, and 95% confidence intervals (CIs). Results: The content validity was assessed by experts and was strengthened through cognitive interviews with women of childbearing age. The PHMSE-PL comprised 34 items across five factors. The construct validity of the PHMSE-PL was supported. Cronbach's α for the total scale was .97 (95% CI=.96-.97). Conclusion: An evaluation of the psychometric properties of the PHMSE-PL scale found it to be a valid and reliable tool for women of childbearing age. The scale appears to be useful for women of childbearing age to self-assess their preventive health management self-efficacy related to premature labor and for health professionals to evaluate and promote women's preventive health management.
