청소년 간접흡연 예방 프로그램의 개발 및 효과 (Effectiveness of a Secondhand Smoking Prevention Program on Adolescents)

  • Purpose: This study has examined effectiveness of a secondhand smoking prevention program for adolescents. Methods: The study was done in a nonequivalent control group pretest/posttest design. The subjects of the current study were 198 middle school students from K city in the Gyeongbuk province. The students were assigned to an experimental group (100) and participated in the secondhand smoking prevention program or to a control group (98) and did not take part. Data collection was done from June to July 2012. A Chi-square test, independent t-test, paired t test and ANCOVA were used with the SPSS 18.0 program for data analysis. Results: There were significant differences between the groups in knowledge of short-term influence of secondhand smoking (F=9.65, p<.005), knowledge of long-term influence of secondhand smoking (F=15.53, p<.001), verbal coping skills for secondhand smoking prevention (F=16.35, p<.001), behavioral coping skills for secondhand smoking prevention (F=8.49, p<.005), and assertiveness of secondhand smoking prevention (F=17.30, p<.001) measurements. Conclusion: The secondhand smoking prevention program delivered to the adolescents is an effective method of encouraging secondhand smoking prevention and can be utilized as an effective nursing intervention for adolescents.

흡연부모 초등학생을 위한 간접흡연예방 프로그램 개발 및 효과 (Effects of Secondhand Smoking Prevention Education on Elementary School Students with Smoking Parents)

  • Purpose: This study was conducted to develop a secondhand smoking prevention education program for elementary school students with smoking parents and to identify its effects on their knowledge and coping skills related to secondhand smoke and their assertive behaviors towards secondhand smoke. Moreover, the study also intended to discover the program's impact on the students' coping assertiveness towards secondhand smoke from their smoking parents. Methods: For this nonequivalent control group pretest-posttest design study, elementary students who have smoking parents were recruited. They were assigned either to the experimental group (n=40) or to the control group (n=40). A secondhand smoking prevention education program was provided for the experimental group for 4 weeks. Results: The experimental group showed a significant increase in their scores for knowledge, behavioral coping skills, and coping assertiveness towards secondhand smoke from their parents. Conclusion: The findings show that this program could serve as an effective program for elementary students with smoking parents. Encouraging smoking parents to participate in the program could enhance the program's effect.

유아간접흡연 예방에 대한 부모의 참여의도 관련변인 연구 (A study on related factors of parent's participation intention for early childhood secondhand smoke prevention)

  • This study aims to 1) examine the difference of parent's participation intention for early childhood secondhand smoke prevention depending on sex, education level, income level, and smoking or nonsmoking of parent, parent's knowledge and attitude of early childhood secondhand smoke 2) estimate the influence of independent factors on parent's participation intention for early childhood secondhand smoke prevention centering around factors stated previously, and 3) suggest effective strategies to ultimately encourage parent's behavior for early childhood secondhand smoke prevention. A total of 279 parents with four- and five-year-old day care children participated in this study. The following are the main results of this study. First, the higher the level of income parent had, the higher the level of participation intention for early childhood secondhand smoke prevention parent had. Second, nonsmoking parent exhibited higher level of participation intention than smoking parent. Finally, parent's knowledge and attitude of early childhood secondhand smoke had influence on parent's participation intention for early childhood secondhand smoke prevention, but parent's attitude had a high degree of influence on parent's participation intention.

  • Objectives: The purpose of this study was to investigate the effect of a child-parent secondhand smoke(SHS) prevention program(C-PSHSPP). Methods: The participants were parents whose children were attending daycare centers located in S city. The institutions were selected using two-stage cluster sampling and data were collected using a structured questionnaire. Descriptive statistics were performed using SPSS version 18.0. Results: Number of cigarettes smoked per day, rate of smoking when with child decreased significantly. Parental awareness on SHS and scores on changes of experiential process increased significantly. Conclusions: From these results, it is suggested that the C-PSHSPP has beneficial effects in decreasing parental daily smoking amount at the same time increasing parental awareness on SHS and importance of smoking cessation. Therefore, C-PSHSPP can be recommended as a SHS intervention program for preschool children.

Do Sociodemographic Factors, Smoking Status, and Beliefs about the Health of Others Predict Attitudes about Smoke-free Air Policies in Various Settings?

  • Objectives: This study examined the extent to which attitudes about smoke-free air policies (SFAPs) in bars/restaurants, workplaces, all public places, and motor vehicles when minors are present can be explained by individuals' sociodemographic characteristics, smoking status, and beliefs about the health of others. Methods: Data were gathered from 359 individuals age 18 or older who attended the Lawrence County Fair in Indiana, United States, in July, 2009, an area where there were no SFAPs in place at the time of survey administration. Results: Multinomial logistic regression analyses indicated that perceived severity of secondhand smoke (SHS) on others, perceived responsibility of smokers for the harm their SHS causes to others, and perceived susceptibility of others to SHS exposure, along with education level and smoking status, significantly predict opposition to SFAPs in this population. Conclusions: The results of this exploratory study suggest the need for additional research related to attitudes about health policies as well as to the practical applications of these findings for smoke-free air advocacy.

Tobacco Use Among Students Aged 13-15 Years in South Korea: The 2013 Global Youth Tobacco Survey

  • Objectives: We examined the prevalence of tobacco use and exposure to secondhand smoke among middle-school students in Korea using the Global Youth Tobacco Survey (GYTS) in 2013. Methods: The GYTS in Korea was conducted between July and August 2013 by the Korea Centers for Disease Control and Prevention. Data were collected using a self-administered anonymous questionnaire from a nationally representative sample of middle-school students aged 13-15 years in sampled classrooms. Results: The GYTS in Korea was completed by 4235 students aged 13-15 years in 43 middle schools. Approximately one in five of the students (17.8%) reported that they had tried cigarettes in the past, while 5.2% reported currently being cigarette smokers. Current cigarette smoking was higher in boys (7.5%) than in girls (2.6%). Of the students, 29.7% had been exposed to secondhand smoke at home, 47.4% inside enclosed public places, and 53.9% in outdoor public places. Of the current cigarette smokers, 25.7% bought their cigarettes from a store despite a law prohibiting this. Additionally, 58.0% of students noticed point-of-sale tobacco advertisements or promotions, 66.8% of current cigarette smokers wanted to stop smoking, and 70.9% of students had been taught about the dangers of tobacco use in school. Conclusions: These findings provide an opportunity to develop, implement, and evaluate a comprehensive tobacco control policy. The results suggest that youth have relatively easy access to cigarettes and are regularly exposed to secondhand smoke in public places, as well as to point-of-sale tobacco advertisements and promotions. Strict enforcement of the ban on tobacco sales to youth, expanding smoke-free areas, and advertising bans are needed to reduce tobacco use among youth.

Policy Effects of Secondhand Smoke Exposure in Public Places in the Republic of Korea: Evidence from PM2.5 levels and Air Nicotine Concentrations

  • Objective: The purpose of this study was to evaluate secondhand smoke (SHS) exposure inside selected public places to provide basic data for the development and promotion of smoke-free policies. Methods: Between March and May 2009, an SHS exposure survey was conducted. $PM_{2.5}$ levels and air nicotine concentrations were measured in hospitals (n=5), government buildings (4), restaurants (10) and entertainment venues (10) in Seoul, Republic of Korea, using a common protocol. Field researchers completed an observational questionnaire to document evidence of active smoking (the smell of cigarette smoke, presence of cigarette butts and witnessing people smoking) and administered a questionnaire regarding building characteristics and smoking policy. Results: Indoor $PM_{2.5}$ levels and air nicotine concentrations were relatively higher in monitoring sites where smoking is not prohibited by law. Entertainment venues had the highest values of $PM_{2.5}$(${\mu}g/m^3$) and air nicotine concentration(${\mu}g/m^3$), which were 7.6 and 67.9 fold higher than those of hospitals, respectively, where the values were the lowest. When evidence of active smoking was present, the mean $PM_{2.5}$ level was 104.9 ${\mu}g/m^3$, i.e., more than 4-fold the level determined by the World Health Organization for 24-hr exposure (25 ${\mu}g/m^3$). Mean indoor air nicotine concentration at monitoring sites with evidence of active smoking was 59-fold higher than at sites without this evidence (2.94 ${\mu}g/m^3$ vs. 0.05 ${\mu}g/m^3$). The results were similar at all specific monitoring sites except restaurants, where mean indoor $PM_{2.5}$ levels did not differ at sites with and without active smoking evidence and indoor air nicotine concentrations were higher in sites without evidence of smoking. Conclusion: Nicotine was detected in most of our monitoring sites, including those where smoking is prohibited by law, such as hospitals, demonstrating that enforcement and compliance with current smoke-free policies in Korea is not adequate to protect against SHS exposure.

음식점 매출변화를 통해 살펴본 금연구역 정책의 경제적 영향 분석 (Assessment of the Economic Impact of the Non-smoking Area Policy on Restaurant Sales)

  • Purposes: Despite the positive health effects of both smokers and non-smokers, the non-smoking area policy is being negatively evaluated because of the vague fears of declining restaurant sales. The purpose of this study is to analyze the changes in sales of general restaurants(including liquor stores) and other restaurants that are considered to have the most applications of smoking cessation policy among the smoking facilities, and to examine the economic impact of the designation and expansion policies of non-smoking areas. Methodology: This study used the wholesale and retail trade survey data of the Korea National Statistical Office from 2011 to 2014 and analyzed 31,577 restaurants excluding missing values. For statistical analysis, t-test, ANOVA and Difference-in-differences(DID) models were used and the interaction term of area and year was entered. Findings: As the non-smoking area policy had been designated and expanded from December 2012 to December 31 2013, high restaurant sales in 2012 declined sharply in 2013. However, despite the expanding of the no-smoking area from January 1 2014 through December 31 2014, restaurant sales slightly recovered in 2014. In the case of other restaurants, there is no significant change in sales since the start of the policy in 2013. Practical Implications: The decline in restaurant sales due to the designation and expansion of non-smoking areas is temporary and can not be sustained over the long term. This result can be used to positively suggest negative perceptions of the designation and expansion policy of non-smoking areas. Also, this result can contribute to health promotion and smoking cessation policies by protecting non-smokers from the risk of secondhand smoking exposure and inducing smokers to decrease smoking rate and smoking amount.

일반계고와 특성화계고 남학생의 현재 흡연 영향요인: 제13차 청소년건강행태온라인조사(2017)를 활용하여 (Factors associated with Current Smoking among Male High School Students according to School Type: Using Data from the 13th(2017) Korea Youth Risk Behavior Web-based Survey)

  • Purpose: This study was conducted to examine the prevalence of current smoking and the factors that influence current smoking among male high school students in Korea according to school type. Methods: Data were extracted from the Korea Youth Risk Behavior Web-Based Survey (2017) and multiple logistic regression was used based on complex sample analysis. The study sample comprised of 15,777 male high school students: 12,583 from general high schools (GHS) and 3,194 from specialized vocational high schools (SVHS). Results: The prevalence of current smoking was significantly higher in SVHS students (19.6%) compared to GHS students (12.6%). Low academic achievement, depressive symptoms, current drinking, and high household economic status were commonly associated with current smoking in male students from both school types. However, exposure to secondhand smoking at home, non-exposure to anti-smoking advertisement, exposure to tobacco advertisement, participation in smoking prevention or cessation education were related to current smoking in GHS students, while living in rural area was associated with current smoking in SVHS students. Conclusion: To reduce current smoking among male high school students, it is necessary to develop smoking prevention policies reflecting the factors associated with current smoking that are different by school type.

금연서포터즈를 활용한 흡연예방 및 금연프로그램이 간호대학생의 흡연 지식 및 신념, 금연 자기효능감에 미치는 효과 (The Effect of Smoking Prevention and Smoking Cessation Program using Smoking Cessation Supporters on the Smoking Knowledge and Beliefs and Self-efficacy of Smoking Cessation of Nursing Students)

  • 본 연구는 금연서포터즈를 활용한 흡연예방 및 금연프로그램이 간호대학생의 흡연지식 및 신념, 금연 자기효능감에 미치는 효과를 평가하기 위해 실시되었다. 본 연구를 위해 개발된 흡연예방 및 금연프로그램은 흡연관련 일반적인 교육(1회 30분씩 8회)과 대학생 금연서포터즈의 활동으로 구성되었다. 30명으로 구성된 대학생 금연서포터즈는 팀당 6명, 총 5팀으로 구성하여, 팀별 온라인 동영상 홍보, 금연 홍보 및 캠페인 활동, 금연 도전자 모집 및 멘토링 등을 진행하였다. 연구결과, 금연서포터즈를 활용한 흡연예방 및 금연프로그램은 간호대학생의 흡연 지식을 높이고, 흡연에 대한 신념을 부정적으로 변화시키는데 효과적이었다. 특히 간호대학생을 대상으로 흡연에 관한 인식과 전반적인 실태를 파악하고 흡연예방 및 금연프로그램을 운영함으로써 비흡연학생의 흡연예방과 간접흡연으로 인해 타인의 건강까지 위협하고 있다는 사실을 인식하게 하였고, 흡연 학생에게는 금연동기부여와 금연 성공유지를 위한 방법을 제공하는 등 대학 내 금연문화 확산에 기여하였다.
