취약계층 아동집단의 비만예방을 위한 생태학적 요인과 해결전략 탐색: 지역사회 기반 참여연구 기반으로 (Ecological Factors and Strategies for Childhood Obesity Prevention Targeting Vulnerable Children: Using Community-Based Participatory Research)

  • Purpose: This study aimed to explore ecological factors and strategies for childhood obesity prevention targeting vulnerable children using a community-based participatory research (CBPR) methodology. Methods: The CBPR was conducted by following basic process steps. Participants were 12 community stakeholders such as community child center directors (n=4), vulnerable children's mothers (n=3), community health center officials (n=2), and lay health advisors (n=4); they were purposively sampled from K municipal county in Seoul, South Korea. The qualitative content analysis was performed to explore main themes of the ecological factors and strategies by using data obtained from 5 times of focus group interview. Results: Twelve ecological factors associated with childhood obesity prevention were identified: Intrapersonal factors including emotional overeating; interpersonal factors including permissive parenting style of children's eating behaviors; organizational factors including social workers' less educational opportunities; and community/policy factors including less government financial support. Four ecological strategies for childhood obesity prevention were addressed: Developing obesity prevention programs targeting vulnerable children' lifestyles; promoting parents' active participation in education; building healthy meal service environments through empowering social workers; and building supportive community environment and securing community resources for child obesity prevention. Conclusion: Our findings may be informative in terms of providing a comprehensive understanding of multi-level ecological barriers against vulnerable children' obesity prevention and, moreover, guiding multi-level strategies for preventing childhood obesity targeting children enrolled in community child centers.

산재원인을 최소화하기 위한 예방인자 및 전략의 우선순위 평가에 관한 연구 (A Study on the Priority Evaluation of Prevention Factors and Strategies to Minimize Industrial Accident Cause)

  • It is very important to evaluate the priority of prevention factors and strategies in order to minimize industrial accident. It provides decisive information for accident prevention and safety management. Therefore, this paper proposes evaluation method of the priority through statistic testing with prevention factors and strategies by the cause analysis of cause and effect models. Especially, this paper uses priority matrix criterion to justify application of rank and objectivity. This paper is based on the results of a questionnaire of workers and managers who are engaged by real manufacturing and construction industries with less than 300 workers in the central region of Korea, where most of the fatal accidents have happened. Finally, the result provides one way to implement safety management for industrial accident prevention.

Understanding the Biomechanical Factors Related to Successful Balance Recovery and Falls: A Literature Review

  • Background: Despite fall prevention strategies suggested by researchers, falls are still a major health concern in older adults. Understanding factors that differentiate successful versus unsuccessful balance recovery may help improve the prevention strategies. Objects: The purpose of this review was to identify biomechanical factors that differentiate successful versus unsuccessful balance recovery in the event of a fall. Methods: The literature was searched through Google Scholar and PubMed. The following keywords were used: 'falls,' 'protective response,' 'protective strategy,' 'automated postural response,' 'slips,' 'trips,' 'stepping strategy,' 'muscle activity,' 'balance recovery,' 'successful balance recovery,' and 'failed balance recovery.' Results: A total of 64 articles were found and reviewed. Most of studies included in this review suggested that kinematics during a fall was important to recover balance successfully. To be successful, appropriate movements were required, which governed by several things depending on the direction and characteristics of the fall. Studies also suggested that lower limb muscle activity and joint moments were important for successful balance recovery. Other factors associated with successful balance recovery included fall direction, age, appropriate protective strategy, overall health, comorbidity, gait speed, sex and anticipation of the fall. Conclusion: This review discusses biomechanical factors related to successful versus unsuccessful balance recovery to help understand falls. Our review should help guide future research, or improve prevention strategies in the area of fall and injuries in older adults.

Preventive and Risk Reduction Strategies for Women at High Risk of Developing Breast Cancer: a Review

  • Breast cancer is the most commonly diagnosed invasive cancer among women. Many factors, both genetic and non-genetic, determine a woman's risk of developing breast cancer and several breast cancer risk prediction models have been proposed. It is vitally important to risk stratify patients as there are now effective preventive strategies available. All women need to be counseled regarding healthy lifestyle recommendations to decrease breast cancer risk. As such, management of these women requires healthcare professionals to be familiar with additional risk factors so that timely recommendations can be made on surveillance/risk-reducing strategies. Breast cancer risk reduction strategies can be better understood by encouraging the women at risk to participate in clinical trials to test new strategies for decreasing the risk. This article reviews the advances in the identification of women at high risk of developing breast cancer and also reviews the strategies available for breast cancer prevention.

Mechanics behind Breast Cancer Prevention - Focus on Obesity, Exercise and Dietary Fat

  • Cancer prevention is rapidly emerging as a major strategy to reduce cancer mortality. In the field of breast cancer, significant strides have recently been made in the understanding of underlying preventive mechanisms. Currently, three major strategies have been linked to an increase in breast cancer risk: obesity, lack of physical exercise, and high levels of saturated dietary fat. As a result, prevention strategies for breast cancer are usually centered on these lifestyle factors. Unfortunately, there remains controversy regarding epidemiological studies that seek to determine the benefit of these lifestyle changes. We have identified crucial mechanisms that may help clarify these conflicting studies. For example, recent reports with olive oil have demonstrated that it may influence crucial transcription factors and reduce breast tumor aggressiveness by targeting HER2. Similarly, physical exercise reduces sex hormone levels, which may help protect against breast cancer. Obesity promotes tumor cell growth and cell survival through upregulation of leptin and insulin-like growth factors. This review seeks to discuss these underlying mechanisms, and more behind the three major prevention strategies, as a means of understanding how breast cancer can be prevented.

노인 낙상 - 넘어짐 그리고 인체손상의 과학, 넘어지면 다 죽는가?: 문헌 고찰 (Science of Falling and Injury in Older Adults - Do All Falls Lead to Death?: Literature Review)

  • Understanding sciences behind fall-related hip fractures in older adults is important to develop effective interventions for prevention. The aim of this review is to provide biomechanical understanding and prevention strategies of falls and related hip fractures in older adults, in order to guide future research directions from biomechanical perspectives. While most hip fractures are due to a fall, a few of falls are injurious causing hip fractures, and most falls are non-injurious. Fall mechanics are important in determining injurious versus non-injurious falls. Many different biomechanical factors contribute to the risk of hip fracture, and effects of each individual factors are known well. However, combining effects, and correlation and causation among the factors are poorly understood. While fall prevention interventions include exercise, vision correction, vitamin D intake and environment modification, injury prevention strategies include use of hip protectors, compliant flooring and safe landing strategies, vitamin D intake and exercise. While fall risk assessments have well been established, limited efforts have been made for injury risk assessments. Better understanding is necessary on the correlation and causation among factors affecting the risk of falls and related hip fractures in older adults. Development of the hip fracture risk assessment technique is required to establish more efficient intervention models for fall-related hip fractures in older adults.

체중 증가의 관련 요인과 예방책 (Factors Associated with Weight Gain and Its Prevention Strategies)

  • Weight gain is defined as the increase in body weight, increasing the prevalence of obesity, and results in metabolic diseases. Weight gain was reportedly caused by the interaction between the obesogenic environmental factors and individual metabolic factors. Sociodemographic and environmental factors (demographic factors, lifestyle/behavioral factors, food/nutritional factors, socioeconomic factors), drug-related secondary causes (some of the corticosteroids, antihyperglycemics, antihypertensives, antidepressants, etc.), and metabolic factors (aging and hormonal changes, menopause and decreased sex hormones, decreased adipocyte degradation, decreased fibroblast growth factor 21, central sympathetic nervous system hyperactivity, decreased sympathetic-adrenomedullary system activity) are significant factors related to weight gain. It is crucial to prevent weight gain and maintain an ideal weight, but studies on the risk factors of weight gain are insufficient. Therefore, this study evaluated the factors associated with weight gain to find strategies for preventing unnecessary weight gain.

노인자살 예방전략에 관한 모색 (Exploration of Prevention Strategies on Elderly Suicide)

  • 노인자살에 관한 연구는 2000년 이후부터 증가하고 있다. 우리사회에서 노인자살이 심각한 노인문제의 영역으로 주목받고 있음을 의미하며 동시에 이에 대한 정책적 노력이 필요함을 시사한다. 하지만 지금까지의 노인자살 연구들은 대부분이 노인자살에 관한 이론적 조망과 그에 따른 노인자살 요인들을 파악하는 데 집중되어 왔다. 그리하여 노인자살의 예방대책에 관한 논의는 개괄적이고 제안적인 수준에 머무르고 있다. 따라서 본 연구는 노인자살에 관한 선행연구들에서 파악된 자살요인들과 이를 기초로 제시된 정책 제언들을 종합적으로 분석하여 효과적인 노인자살의 예방전략을 모색하고자 한다. 이를 위하여 본 연구는 한국학술진흥재단에 등재된 학회지 중에서, 노인자살과 관련된 논문들을 추출하여 내용분석을 실시하였다. 분석결과로서 일반노인에게는 거시적 수준의 개입이 크게 요구되었으며, 잠재적 위험노인에게는 중간 수준과 미시적 수준 모두에서 노력이 요망되었다. 하지만 현실적인 측면에서 볼 때 무엇보다도 중간 수준의 2차적 개입이 우선되어야 함이 시사되었다. 본 연구에서 제안된 우선적 예방전략에 기초하여 향후 연구에서는 어떠한 구체적인 정책과 프로그램을 개발하여 어떻게 운영되어야 하는지에 대한 논의가 수반될 필요가 있을 것이다.

만성질환 위험요인 감소를 위한 전략과 방향 (Strategies to reduce risk factors of non-communicable diseases in South Korea)

  • Objectives: This review is to suggest strategies to reduce risk factors of non-communicable diseases (NCD) in South Korea. Methods: Prior research findings on the burden of NCD and associated risk factors and the effectiveness of intervention programs were reviewed. Strategies regarding the control of NCD risk factors were conceived. Results: The author presented research findings from the Global Burden of Disease study on the burden of non-communicable disease (NCD) and associated risk factors in South Korea. Strengths and limitations of population and high-risk strategies for preventing NCDs were introduced. The author also reviewed the evidence on the effectiveness of multiple cardiovascular risk factor interventions and community-based intervention programs on cardiovascular diseases conducted in industrialized countries. Finally, strategies to reduce NCD risk factors in South Korea were suggested. Conclusions: The evidence-based interventions and the importance of population strategies in NCD prevention were highlighted. The author indicated that strategies employed by unhealthy commodity industries to undermine effective public health policies and programs should be actively monitored. It has been suggested that effective high-risk strategies with ecological models to address social risks rather than medical risks among disadvantaged population should be further developed in South Korea.

고혈압과 당뇨병 건강 문제를 가진 성인의 치매 예방 행위 의도에 미치는 영향 요인 (The Factors Influencing Intention of Dementia Prevention Behavior in Adults with Hypertension or Diabetes)

  • Purpose: The purpose of the present study is to examine the factors influencing intention of dementia prevention behavior in adults with hypertension or diabetes. Methods: A total of 113 community-dwelling adults with hypertension or diabetes agreed to participate in this study. Data were analyzed using descriptive statistics, t-test, ANOVA, correlation, and multiple regression. Results: Self-efficacy and cues to action toward dementia prevention behavior were factors influencing intention of dementia prevention behavior of participations. Self-efficacy toward dementia prevention behavior was the most influential factor. These factors accounted for 22% of intention of dementia prevention behavior. Conclusion: These results suggest a need for strengthening self-efficacy to increase intention of dementia preventive behaviors. And dementia prevention programs should be developed in consideration of strategies to reinforce cues to action.
