Marine accidents are increasing year by year, and various accidents occur such as engine failure, collision, stranding, and fire. These marine accidents present a risk of large casualties. It is important to prevent accidents beforehand. In this study, we propose a modeling to predict the occurrence of marine accidents by applying the Markov Chain Process that can predict the future based on past data. Applying the proposed modeling, the probability of future marine accidents was calculated and compared with the actual frequency. Through this, a probabilistic model was proposed to prepare a prediction system for marine accidents, and it is expected to contribute to predicting various marine accidents.
The present study was motivated by the needs to make diversified scientific approaches toward influential factors like human, technical, organization, policy and environmental on the basis of the specialized information concerning the cause for industrial accidents in a measure to prevent the falling accident which has the biggest priority among the three major frequent industrial accidents. In this connection, diverse policies have been practiced in the meantime, with little effects in fact to reduce occurrence of industrial accidents, seemingly because such policies have been practiced on the direct dimension, instead of tries to nip in the bud of fundamental causes. This study was thus conducted with a view to determine the causes that have influence on falling accidents from the overall context and unearth the factors requiring management with priority. For this aim, "Fall from Height"(2003), a study by HSE, UK was applied to various conditions of korea as an experiment to search for fundamental causes for falling accidents. The major findings of this study might be summed up as exploring a main critical path that has influence upon falling accidents and suggesting effective ways to cut down through the critical path.

As the land price in the downtown area increases, buildings are becoming bigger, deeper and higher. Consequently, the importance of underground construction has increased. Although construction engineers make every effort to complete underground construction without any problem, construction failures like landslides and the collapse of a retaining wall occur because of the uncertainty of the soil conditions as well as the unexpected risks of excavation work. In order to prevent potential excavation accidents, it is essential to understand the causes and impacts of such accidents. However, there are only a few examples of construction failures, which show the economic impact on accidents during excavation because of the sensibility of the information. This paper presents two cases of excavation accidents, which were investigated by construction insurance company. The compensation for the accidents paid by the insurance company was compared with the estimated costs calculated based on the estimation method for excavation accidents proposed by our previous study. The comparison results showed that the estimate calculated by our method was much less than the actual compensation because the estimate solely focused on the construction costs whereas the compensation included other external factors.
Since cranes are a kind of complex human-machine systems, it is almost impossible to completely secure safety with current technologies. Therefore, managerial interventions to prevent human errors are needed for safely operating a crane. The Occupational Safety and Health law states that cabin-type crane operators should have crane drivers' licence and crane-related operators (e.g., pendent-type crane operators, slinging workers) should take a special safety training. However, statistics on industrial accidents showed that fatalities due to crane accidents (185 accidents occurred during 2013~2017) were the highest among hazardous machinery and equipment. To effectively control the crane-related accidents, voices of crane workers need to be analyzed to investigate the current status. This study surveyed perceived causes of crane accidents and status of special safety training for crane workers of 387. The survey revealed that 24.3% of the respondents experienced crane accidents and 31.4% eye-witnessed crane accidents. 79% of the respondents pointed human errors such as improper crane operation and improper slinging as the first cause. Lastly, only 16.7% of the respondents took a professional special safety training; but the rest took lecture-based or incomplete education. The findings of the present study can be applied to improve crane-related policies and special safety training systems.

Helicopter pilots are required to perform many visual workloads in topographical avoidance, flight path modification and navigation, because helicopters operate at very low altitudes. The helicopter-specific instability also require the pilot to have precise perception and control. This has caused frequent human error in helicopter accidents. In Korea, two to three cases have occurred annually on average over the past 10 years, and this trend has not decreased. The purpose of this study was to identify human error risks in advance to prevent helicopter accidents and to help develop measures for missions and mission phases with high risk of human error. Through the study, the tasks and mission phases where accidents occur frequently were classified and the risk of human error was calculated for each mission phases. To this end, the task of frequent accidents during helicopter missions was first identified, detailed steps were classified, and the number of accidents was analyzed. Next, the AHP survey program was developed to measure the pilot's risk of human error and the survey was conducted on the pilots. Finally, the risk of human error by helicopter mission and by mission phases calculated and compared with the actual number of accidents.

Purpose: This study investigates human error accidents in the Korean railway sector, emphasizing the need for systematic management to prevent such incidents, which can have fatal consequences, especially in driving-related jobs. Research design, data and methodology: This paper analyzed data from the Aviation and Railway Accident Investigation Board and the Korea Transportation Safety Authority, examining 240 human error accidents that occurred over the last five years (2018-2022). The analysis focused on accidents in the driving, facility, electric, and control fields. Results: The findings indicate that the majority of human error accidents stem from negligence in confirmation checks, issues with work methods, and oversight in facility maintenance. In the driving field, errors such as signal check neglect and braking failures are prevalent, while in the facility and electric fields, the main issues are maintenance delays and neglect of safety measures. Conclusions: The paper concludes that human error accidents are complex and multifaceted, often resulting from a high workload on engineers and systemic issues within the railway system. Future research should delve into the causal relationships of these accidents and develop targeted prevention strategies through improved work processes, education, and training.

In the event of a marine pollution accident, Korea, which is bordered by the sea on three sides, would suffer tremendous damage. According to an analysis of marine pollution accidents in Korea, most pollutants were small fishing boats and tugboats. In particular, urgent safety measures are required due to the high incidence of marine pollution accidents caused by stranded tugboats. This study aims to decrease secondary marine pollution accidents by preparing measures to prevent accidents by stranded tugboats, the main source of pollution accidents. To this end an experiment was conducted to mark 63 locations where stranded accidents have occurred on the GPS plotters of 61 tugboats operating in Boryeong sea area. Its effectiveness was then investigated. The result showed that there was no stranded accident by the subject ship during the 20-month experiment period. As a result of survey of the captains, 36 out of 40 respondents (90%) stated that the experiment was significantly helpful in ensuring safe navigation, thus confirming its effectiveness. Various measures were proposed to prevent stranded accidents, including providing information to manufacturers and users by marking the location of grounded accidents on GPS plotters and posting the location information of unknown reefs on the official website of institution Korea Hydrographic and Oceanographic Agency.

To prevent the electrical accidents in the laboratory, we identify problems for periodic inspections of the electric field and develop a fuzzy inference system that can be practically applied to check items. Focusing on electrical safety in the lab environment, we draw check items that can be applied in common and develop a standard checklist that is consistent with the laboratory electrical safety and the periodic inspections. Using the standard checklist we select the items that may contain a linguistic ambiguity and define the membership functions for these items. We also have a safety rating defined by the membership function. Using these fuzzy variables we form the fuzzy rules in the form of 'If-Then' and develop a fuzzy inference system through the fuzzy engine. From this, electrical accidents could be prevented in advance continuously by managing the intelligent and efficient inspection and electrical safety to prevent the electrical accidents in the laboratory.
The damage of safety accidents at home can be minimized by preventing accidents in advance as well as by coping with the accidents promptly through the help of safety management system. In this paper, we identify several services to prevent and cope with the accidents that may occur at home by leveraging sensors and wireless network technology available at home network environments. In order to provide these services, we propose a context-aware system for safety management at home. The proposed system awares the dangerous situations through the information collected by the sensors in home network environment. The proposed system tries to not only prevent the accidents but also to cope with the situations using the information of rescue targets and the location where the accident occurs. We evaluate the appropriateness of sensors by implementing the proposed environments. Through OPNET simulation experiments, we also measure the amount of network traffic in order to confirm that the bandwidth of IEEE 802.15.4 is enough for the proposed system. Finally, we determine more appropriate wireless data transmission mechanism with respect to the end-to-end delay and battery consumption for the proposed system.
According to the statistics, occupational fatal injuries by mobile cranes were about 12 per year in whole industrial. Mobile cranes are widely used in various parts of industries to improve the efficiency of the work. However considerable number of fatal injuries happen each year during the operation of the machines. In this study, the current regulations to be adequate in industrial site have to be renew in order to prevent the fatal injuries by mobile cranes. Fatal injury analyses were conducted with several accident cases by the mobile cranes. For each accident, the causes of the injuries were examined and proper safety measures were proposed. In this study, the mobile crane showed a high fatality rate in industrial accidents and no detailed cause analysis of fatal accidents was conducted in terms of unsafe acts or conditions. This study proposed a revision of the standard guideline as an accident prevention measures through in-depth analysis of fatal accidents. First, among the mainly five machines caused the accidents, mobile crane was higher for the second showed 0.6% for number of fatalities compared to number of mobil cranes and for the third showed 11% for number of fatalities compared to number of injuries. Second, main cause of cognitive engineering agenda was visibility, responsibility, affordance. As the measures to prevent accidents before starting operation, alternative revision for the fool proof including visibility, responsibility, affordance etc. for the fool proof measures was proposed. Third, alternative revision as cognitive accident prevention for the fail safe measures was proposed.
