Development of a Human Factors Investigation and Analysis Model for Use in Maritime Accidents: A Case Study of Collision Accident Investigation

  • In the shipping industry, it is well known that around 80 % or more of all marine accidents are caused fully or at least in part by human error. In this regard, the International Maritime Organization (IMO) stated that the study of human factors would be important for improving maritime safety. Consequently, the IMO adopted the Casualty Investigation Code, including guidelines to assist investigators in the implementation of the Code, to prevent similar accidents occurring again in the future. In this paper, a process of the human factors investigation is proposed to provide investigators with a guide for determining the occurrence sequence of marine accidents, to identify and classify human error-inducing underlying factors, and to develop safety actions that can manage the risk of marine accidents. Also, an application of these investigation procedures to a collision accident is provided as a case study This is done to verify the applicability of the proposed human factors investigation procedures. The proposed human factors investigation process provides a systematic approach and consists of 3 steps: 'Step 1: collect data & determine occurrence sequence' using the SHEL model and the cognitive process model; 'Step 2: identify and classify underlying human factors' using the Maritime-Human Factor Analysis and Classification System (M-HFACS) model; and 'Step 3: develop safety actions,' using the causal chains. The case study shows that the proposed human factors investigation process is capable of identifying the underlying factors and indeveloping safety actions to prevent similar accidents from occurring.

밀도기반 선박 도메인을 이용한 안전 성능 지표 활용성 연구 (A Study on the Applicability of Safety Performance Indicators using the Density-Based Ship Domain)

  • 선박 충돌 사고는 경제적 손실, 인명피해 등 다양한 부정적 상황을 초래할 수 있기 때문에 사고를 예방하기 위한 다각적인 노력이 필요하다. 따라서 사고 예방을 위한 연구가 활발히 이루어고 있으며, 본 연구에서는 선박 충돌 사고 예방을 위한 새로운 선행 지표를 제안한다. 기존 연구에서는 특정 해역에서 선박들간의 거리를 고려하여 충돌위험을 표현하였지만 이를 다른 해역에 적용하기 위해서는 모델을 새로 개발해야하는 단점이 존재한다. 본 연구에서는 선박 운항 정보인 AIS(Automatic Identification System) 데이터를 활용하여 해역의 환경과 운항의 특징을 포함한 밀도기반 선박 도메인 DESD(Density based Empirical Ship Domain)을 정의하였다. 각 해역별로 만들어진 2차원의 DESD를 대상으로 Deep Clustering을 적용하여 유사한 운항 환경을 가진 해역들을 군집화 한다. 군집화된 해역과 선박 충돌 사고의 연관성 분석을 통해 해역의 특징별로 사고의 발생여부가 달라짐을 통계적으로 검정하여 DESD가 사고의 선행 지표로서 활용될 수 있음을 증명하였다.

Indian Railways: Recent Trends in Control Accidents and Safety Measures for Passengers

  • Indian railways has been regularly in the news albeit for the wrong reasons. The frequency with which train accidents have been taking place has led to serious doubts in the public mind about the safety of rail travel and also the health of the network. Against this background, an attempt is made in this paper to assess the trends in railway accidents for the period from 2000-01 to 2009-10. The paper also highlighted the various measures taken by IR to prevent accidents to ensure safety to the public.

말비계 건설재해 특성 분석에 관한 연구 (A study on characteristics of accidents involving horse scaffolds at construction sites)

  • Many fatal and non-fatal injuries occurred as a result of unexpected falls from horse scaffolds during structural or finishing works such as painting, plastering, or plumbing. Therefore, this study is to improve safety standards for horse scaffolds used mostly in the interior finishing works. But hazard elements for the horse scaffolds has not been specifically identified. This study has analyzed the accidents of horse scaffolds to prevent construction accidents and the characteristics of a serious accidents related the horse scaffolds. It was suggested a safety management plan and a group of accidents characteristics. The results of this study are expected to be utilized as basic data for prviding a reference related to the horse scaffolds and to be unified the regulations associated with the horse scaffolds.

가스사고의 통계적 분석을 통한 사고 예방 정책 마련을 위한 기초 연구 (A Basic Research for Taking Precautionary Measures against Gas Accidents)

  • The quantity of the fuel gas consumed are trending upwards because it can be easily delivered but isn't deteriorated and doesn't have any environmental pollution. Though there are many advantages to use the fuel gas, because of its explosiveness and combustibility, it can be hesitate to consume more gas. So paying more attentions to prevent the fuel gas accidents is required. In this paper, we examine the present situation data about the fuel gas accidents and analyze those statistically using ANOVA. we confirm that there is an acceptable difference between the mean values of accidents classified by the kind of gas, the cause, the type and the place but isn't by month. It is expected that our result can be applied as preliminary data when mapping out a strategy for preventing the fuel gas accidents.

  • A forklift accident occurs in the logisitcs center. there are many kinds of accidents such as a casualty accident or a minor accident, etc. there are many other causes of accident, including poor visibility and habitual operational mistakes The result of the accident is the phenomenon of cutting, falling, scratching of the product. In the case of minor accidents, workers do not report accidents, so the occurrence of accidents is unknown. The customer receives the goods and then notifies them that they are defective product. After the product is returned, we can see that the forklift accident caused the problem of the product The result of the late response affects the image of the enterprise. In this study, we recognized the need for safety education to prevent accidents and found out which of the education items could raise awareness of safety. As result of the study, disinfection and forklift maneuverability regarding speed were mentioned as the most important items in education.

  • Investigation and analysis of accidents are critical elements of safety management. The over-riding purpose of an organization in carrying out an accident investigation is to prevent similar accidents, as well as seek a general improvement in the management of health and safety. Hundreds of workers have suffered injuries while installing, maintaining, or servicing machinery and equipment due to sudden re-energization of power lines. This study presents and analyzes two electrical accidents (1 fatal injury and 1 serious injury) that occurred because the power supply was reconnected inadvertently or by mistake.

건설 현장 강행공사 방지가 인적사고에 미치는 영향에 관한 연구 (A study on effects of the prevention of rush work of construction sites on human accidents)

  • In the construction sites, rush (speedy construction) work have recently been overused because of budget reductions by reducing the construction duration after the long-term economic recession and construction business stagnation with institutional change including the instruction of the duration shortening bidding system and the introduction of the post-construction sales system by the Korea Research Institute for Human Settlements. Due to this, negligent accidents have frequently occurred by forcing manpower, equipment, finance, and material procurement and pressing ahead with impractical construction wok at night according to the reduction of construction schedules. Therefore, this paper has the necessity of this study to seek for alternatives to prevent negligent accidents by finding factors related to rush work in the reality that the importance for preventing negligent accidents is more emphasized while they have not been reduced in construction sites.

화학산업 사고 예방을 위한 ESG 활용 필요성 연구 (A Study on tne Necessity of Using ESG to Prevent Accidents in the Chemical Industry)

  • 연구목적: 중대산업사고 예방을 위해 안전 분야에서의 ESG 활용 필요성 제시하고자 했다. 연구방법: 선행 연구 검토를 통해 강력한 중대사고 예방 제도인 중대재해처벌법을 검토했다. 그리고 미국과 한국의 중대사고 데이터 비교분석을 통해, 국내 화학산업의 주요 사고원인을 도출했다. 연구결과: 기업의 사고 예방을 목표로 하는 중대재해처벌법과 함께 ESG 경영을 통한 기업의 자발적인 안전관리를 유도할 필요성이 있다고 판단했다. 사고 데이터 분석을 통해서는, 미국 대비 국내 사고는 그 피해 규모와 사망자 수가 큰 것을 확인했다. 그 원인은 국내에서 인적 원인에 의한 사고가 많기 때문으로 해석했다. 결론: 중대재해처벌법 뿐만 아니라 기업 안전 관리의 자발성을 강화하고 적극적인 안전 경영을 유도하기 위해서는 'ESG 안전 평가'의 비중을 확대해야 한다. 사회 간접적 제재로써의 ESG를 활용하면 안전 분야에서 기업의 자발적이고 적극적인 안전 관리와 안전 투자 확대를 기대할 수 있을 것이다.

  • The number of human accidents in the construction industry is increasing every year, and it constitute the highest percentage among industry. This means that activities performed to prevent safety accidents in the country are not efficient to reduce the rate of accidents in the construction industry. In order to solve this issue, research has been conducted from various perspectives. But, research regarding to quantification model of human accidents is insufficient. the objective of this study is to conduct a basic study on quantification model development of human accidents. To achieve the objective, first, Cause of accident is defined the through literature review. Second, a basic statistic analysis is conducted to determine the characteristics of the accident causes. Third, the analysis is conducted after dividing into four categories : accumulate rate, season, total construction cost, and location. In the future, this study can be used as a reference for developing the safety management checklist for safety management in construction site and development of prediction models of human accident.
