2000년대 치주처치의 전략 (Periodontal Management strategies for the future in Korea)

  • 과학기술이 발전함에 따라 치주과학 영역에서도 1960년 이후 1980년대에 걸쳐 괄목할 만한 학문적 발전이 이루어졌으며 1990년에는 이에 의거한 치주질환의 예방 및 조절방법이 개발되어 사용되고 있다. 미래의 치주처치전략은 대상 지역의 특성과 보건환경에 따라 달라지므로 여기에서는 한국내 치주치료요구도와 치주치료 담당 일반치과의와 치주전문의의 수, 처치 내용에 대해 점검하고 현재의 치주처치 추세를 검토한 후 향후의 치료전략에 대해 언급하고자 한다. 1980년도 역학조사에 의하면 전체 인구의 82%가 치주질환(치은염+치주염), 35세 성인 인구의 30-40%가량이 치주염에 이환되어 있고 15세 청소년 인구중 0.1%가 유년형치주염에 이환되고 있다. 평균 수명이 증가하면서 65세 이상의 노년층도 전인구의 6%(성인 50%), 2000년도에는 6.9%(성인 52%)로 증가하고 치아보유율도 증가하므로 치주질환 치료요구도는 상승하리라 전망된다. 현재 한국내 치주전공 과정을 거친 치과의사는 약 780인 정도이다, 1996년 류의 연구에 의하면 이들도 대부분 일반치과의(개원의, 공증보건의 포함)로서 진료하고 있으며 의료보험하에서의 치주치료의 비중은 매우 낮다(4.66%). 포괄적 치주적 검사도 보편적이지 않고 대학병원급의 극히 일부에서만 이행되고 있는 실정이다. 향후의 치주질환이 처치전략에는 의료보험 운용시 치주적 배려와 간편한 치주검사과정을 통한 조기진단 및 조기치료, 진행기질환 및 난치성질환자의 치주전문으로의 의뢰가 포함될 것이며 각 환자에게는 개별적이며 임상시험 결과에 근거하는 evidence-based approach에 의한 치료선정 과정이 활용되리라 전망한다.

Subgingival pathogens in chronic periodontitis patients affected by type 2 diabetes mellitus: a retrospective case-control study

  • Purpose: The aim of this study was to compare the prevalence and bacterial load of 6 main periodontal pathogens between pairs of periodontal patients with and without type 2 diabetes mellitus. Porphyromonas gingivalis and Aggregatibacter actinomycetemcomitans genotypes were also investigated. Methods: Twenty patients affected by chronic periodontitis and type 2 diabetes were retrospectively selected and matched to 20 patients without diabetes on the basis of the degree and severity of periodontal disease. Microbiological data of subgingival biofilms were analysed and compared for the examined pathogens: A. actinomycetemcomitans, P. gingivalis, Prevotella intermedia, Treponema denticola, Fusobacterium nucleatum, and Tannerella forsythia. Results: The pairs were balanced in terms of demographic and clinical parameters, except for bleeding on probing and suppuration. In the microbiological test sites (4 for each patient), the mean probing pocket depth was 6.34±1.63 mm in patients with diabetes and 6.41±1.78 mm in patients without diabetes. No significant difference between pairs in the prevalence of P. gingivalis or the distribution of its genotypes was recorded. Patients with diabetes had a significantly greater amount of total bacterial load, P. gingivalis, T. denticola, T. forsythia, and F. nucleatum (P<0.05). Moreover, patients with diabetes had a higher number of sites with a greater cell count than patients without diabetes. When compared to the total bacterial load, only T. forsythia maintained its relative load in patients with diabetes (P=0.001). Conclusions: This retrospective matched study supports the hypothesis that microbiological differences exist among periodontal patients with and without diabetes mellitus.

성인의 치아우식증 및 치주질환에 영향을 미치는 요인 (Influencing factors to dental caries and periodontal diseases in Korean adults)

  • Objectives: The aim of this study was to investigate the factors related to dental caries and periodontal disease in Korean adults. Methods: The subjects were 5,149 adults over 19 years old who participated in the 5th Korea National Health and Nutrition Examination Survey 2012. Data were analyzed by chi-square test and logistic regression analysis using SPSS 18.0 statistical package program. The questionnaire consisted of 17 questions of independent variables and 2 kinds of dependent variables including dental caries and periodontal diseases by direct interview. The independent variables included 5 questions of general characteristics, 3 questions of health behavior, 5 questions of oral health behavior, and 4 questions of chronic diseases diagnosed by the doctors. Results: The prevalence rate of dental caries was higher in those who did not regularly work out and did not receive regular checkups including dental floss and dental brush use. The prevalence rate of periodontal diseases was higher in male adults, smokers, and those who were obese and had diabetes mellitus, poor oral health care habit without using dental floss and inter-dental brush. Conclusions: In order to prevent the dental caries in the adults, it is very important to do regular exercise and regular dental checkup. The best ways of dental caries prevention include tooth brushing after meal with use of dental floss and inter-dental brush. This study suggests that dental health promotion can be enhanced by smoking cessation, ideal body weight maintenance, timely tooth brushing after meal, and use of dental floss and inter-dental brush.

  • Background: Occupational injuries cause major health problems, which the developed, developing, and underdeveloped nations worldwide are facing today. The present study aimed to assess dental caries, periodontal health of stone mine workers, and the relationship between wasting diseases and the years of working experience. Methods: The study population comprised 510 men, selected based on the stratified cluster sampling procedure. Clinical oral examinations were carried out, and periodontal disease, dental caries, and wasting diseases were recorded. Results: Workers were in the age group of 17-56 years; the prevalence of dental caries in the workers was found to be 74%, with a mean decayed, missing, filled teeth index of 2.89. A periodontal pocket of more than 6 mm was observed in 6% of the workers. Conclusion: The oral health of mine workers is in a poor state; steps should be taken so as to provide basic medical and dental care facilities.

Periodontal pathogens and the association between periodontitis and rheumatoid arthritis in Korean adults

  • Purpose: Periodontitis and rheumatoid arthritis (RA) share a similar inflammatory pathogenesis. Porphyromonas gingivalis (Pg) can induce anticyclic-citrullinated peptide autoantibodies (anti-CCP antibodies), a key factor in the development of RA. This study aimed at evaluating the relationships between the 2 diseases and identifying the clinical implications thereof, with a focus on periodontal pathogens in Korean adults. Methods: A total of 260 RA patients and 86 age- and sex-matched control patients without arthritis were enrolled in this prospective cross-sectional study. Periodontal indices and the prevalence and amount of periodontal pathogens were compared between the groups. Correlations between periodontal and RA indices were examined, as were correlations between 9 periodontal pathogens and RA indices. Results: The RA group had significantly higher values than the control group for all investigated periodontal indices (P<0.05) except the number of teeth. The gingival index (GI) was correlated with the disease activity score 28 (DAS28) (r=0.125, P=0.049), RA disease duration (r=0.253, P<0.001), erythrocyte sedimentation rate (ESR) (r=0.162, P=0.010), and anti-CCP antibody titer (r=0.205, P=0.004). Probing pocket depth (PPD) was correlated with ESR (r=0.139, P=0.027) and anti-Pg antibody titer (r=0.203, P=0.001). Bleeding on probing (BOP) was correlated with DAS28 (r=0.137, P=0.030), RA disease duration (r=0.202, P=0.001), ESR (r=0.136, P=0.030), anti-Pg antibody titer (r=0.177, P=0.005), and anti-CCP antibody titer (r=0.188, P=0.007). Clinical attachment level (CAL) and periodontitis severity were correlated with anti-Pg antibody titer (the former r=0.201, P=0.002; the latter r=0.175, P=0.006). The quantity of Pg was positively correlated with the serum anti-Pg antibody titer (r=0.148, P=0.020). Conclusions: The GI, BOP, and PPD showed positive relationships with several RA indices. The anti-Pg antibody titer had positive relationships with PPD, BOP, CAL, and periodontitis severity. Thus, increasing values of periodontal indices could be used as a risk indicator of disease development in RA patients, and an increasing anti-Pg antibody titer could be considered as a warning sign in RA patients suffering with periodontitis.

The Relationship between Oral Health and Stroke in Adults Based on the 6th (2015) Korea National Health and Nutrition Examination Survey

  • Stroke is the second cause of death worldwide, although the survival period is increasing after the occurrence of stroke, severe physical disability is caused with aftereffect. Oral inflammation is not limited to the oral cavity, it can cause malignant changes in other tissues and organs. In previous studies, we confirmed the relationship between tooth loss and stroke due to periodontal inflammation. The purpose of this study was to investigate the relationship between oral health and stroke such as oral hygiene behavior, tooth loss and periodontal disease among Korean adults over 40 years of age. This study was analyzed using the 6th Korea National Health and Nutrition Examination Survey (KNHANES) data. A total of 3,389 adults over 40 years of age were analyzed as final subjects. Socioeconomic statuses and oral health status was analyzed using a complex sample analysis technique. Logistic regression was used to analyze the relationship of oral health and stroke, and 95% confidence intervals were computed using SPSS. When the prevalence of stroke according to oral hygiene behavior was checked, the prevalence of strokes was lower in subjects who had a lot of brushings per day and subjects who used oral hygiene products (p<0.05). The risk of stroke was 2.17 times (95% confidence interval, 1.43~3.28) higher in the group with less than 19 remaining teeth, but it was not statistically significant as a result of adjusting for age and sex, income level, education level, drinking and smoking (p>0.05). Loss of teeth was found to be associated with the risk factor of stroke. Therefore, loss of teeth due to periodontal disease is an additional issue that should be considered as a risk factor for stroke.

한국 여성의 불안 및 우울증상과 치주질환의 관련성 (Association of Anxiety and Depressive Symptoms with Periodontal Disease in Korean Women)

  • 본 연구에서는 심리적 요인에 대한 감수성이 높은 여성의 불안 및 우울증상과 치주질환의 관련성을 알아보고자 한다. 본 연구는 우리나라를 대표하는 국민건강영양조사 제 5기 1차년도(2010년) 8,958명 중 만 19세 이상 여자 3,551명의 자료를 분석하였다. 인구사회학적 특성 중 연령, 결혼상태, 소득수준, 교육수준, 경제활동 유무에 따라 불안 및 우울증상과 유의한 차이가 나타났다. 구강건강관련 특성 중 당뇨병 유무, 현재 흡연, 고위험 음주에 따라 불안 및 우울증상과 유의한 차이가 나타났다. 일일 칫솔질 횟수에 따라 불안 및 우울증상과 유의한 차이가 나타났으며, 치실 및 치간칫솔 사용 여부에 따른 유의한 차이는 없었다. 치실 및 치간칫솔을 사용하지 않는 행태가 치주질환 위험도를 증가시키는 것으로 나타났다(OR, 1.49; 95% CI, 1.14~1.95). 연령, 결혼 상태, 소득수준, 교육수준, 경제활동 유무, 당뇨 유무, 현재 흡연, 고위험 음주, 칫솔질 횟수, 치실 및 치간칫솔 사용 여부를 보정한 결과 불안 및 우울증상이 치주질환을 유의하게 증가시키는 것으로 나타났다(OR, 1.47; 95% CI, 1.04~2.09). 따라서 불안 및 우울증상이 있는 사람이 불안 및 우울증상이 없는 사람에 비해서 치주질환 유병률이 더 높게 나타나 그 위험성이 더 크므로, 치주질환을 더 효율적으로 관리하도록 정기적 치주관리 및 효율적 자가구강건강관리교육의 필요성이 요구된다.

Association between periodontal flap surgery for periodontitis and vasculogenic erectile dysfunction in Koreans

  • Purpose: The National Health Insurance Service-National Sample Cohort and medical checkup data from 2002 to 2013 were used to evaluate the association between periodontal surgery for the treatment of periodontitis (PSTP) and vasculogenic erectile dysfunction (VED). Methods: Bivariate and multivariate logistic regression analyses were applied to a longitudinal retrospective database to assess the association between PSTP and VED while adjusting for the potential confounding effects of sociodemographic factors (age, household income, insurance status, health status, residence area, and smoking status) and comorbidities (diabetes mellitus, angina pectoris, cerebral infarction, and myocardial infarction). Results: Among the 7,148 PSTP within the 268,296 recruited subjects, the overall prevalence of VED in PSTP was 1.43% (n=102). The bivariate analysis showed that VED was significantly related to PSTP (odds ratio [OR], 1.99; 95% confidence interval [CI], 1.38-2.06; P<0.001), and this was confirmed in the multivariate analysis after adjusting for sociodemographic factors and comorbidities (OR, 1.29; 95% CI, 1.06-1.58; P=0.002). Conclusions: Subjects with a history of periodontal flap surgery had a significantly higher risk of VED, after adjusting for potential confounding factors. Further studies are required to identify the key mechanisms underlying the association between severe periodontal disease and VED.

Analysis of Periodontitis Biomarker Expression in Gingival Crevicular Fluids

  • Background: Periodontal disease, also known as gum disease, is a major dental inflammatory disease with a very high prevalence; it is the main cause of tooth loss. Therefore, diagnostic biomarkers that can monitor gum inflammation are important for oral healthcare. Since the gingival crevicular fluid (GCF) adequately reflects changes in the periodontal environment, they have become a target for the development of effective diagnostic biomarkers for periodontitis. In the present study, the level of the target molecules suggested as diagnostic biomarkers for periodontitis were analyzed in GCF samples collected from healthy individuals and periodontitis patients. In addition, useful targets for the diagnosis of periodontitis were evaluated. Methods: GCF samples were collected from healthy individuals and periodontitis patients using absorbent paper points. SDS-PAGE and Coomassie staining were performed for protein analysis. The protein concentrations of GCF specimens were determined using the Bradford method. The levels of the target molecules appropriate for diagnosing periodontal disease were measured by ELISA, according to the manufacturer's protocol. Results: The protein concentration of GCF collected from periodontitis patients was 3.72 fold higher than that in an equal volume of GCF collected from healthy individuals. ELISA analysis showed that the level of interukin-6 (IL-6), IL-8, metalloproteinases 2 (MMP-2), MMP-9, tumor necrosis factor-alpha (TNF-α), azurocidin, and odontogenic ameloblast-associated protein (ODAM) were higher in the GCF samples from the periodontitis patients than in those from the healthy individuals. However, the level of IL-6 and TNF-α were relatively low (> 5 pg/ml). The prostaglandin E2 (PGE2) levels were not significantly different between the two GCF samples. Conclusion: These results indicate that IL-8, MMP-2, MMP-9, azurocidin, and ODAM are potentially useful diagnostic biomarkers for periodontitis; combining multiple biomarkers will improve the diagnostic accuracy of periodontitis.

The socioeconomic impact of Korean dental health insurance policy on the elderly: a nationwide cohort study in South Korea

  • Purpose: The purpose of this retrospective study was to investigate the relationships of types of dental insurance coverage in Korea with sociodemographic characteristics and the prevalence of systemic and oral diseases, as well as to evaluate the socioeconomic impact of Korean dental insurance policies. Methods: Sample cohort data from 2006 to 2015 were obtained from the National Health Insurance Service. Patients were divided into 2 groups. The exposed group comprised patients who received insurance benefits for complete dentures, removable partial dentures, and implant care, while the control group comprised patients who did not receive these benefits. The type of insurance coverage and the prevalence of systemic and oral diseases were compared between the 2 groups. Results: Patients who received benefits in the form of complete dentures, removable partial dentures, and implants had similar sociodemographic characteristics in terms of sex, age, income quintile, and type of insurance coverage to the control group. The prevalence of hypertension, anemia, renal disease, rheumatoid arthritis, osteoporosis, asthma, and cerebral infarction was higher in the exposed group than in the control group (P<0.05). The prevalence of periodontal diseases and dental caries was also higher in the exposed group. Conclusions: Korean dental health insurance policy has been beneficial for the medical expenses of low-income and elderly people suffering from a cost burden due to systemic diseases. However, since there is a tendency to avoid invasive interventions in older patients due to the high risk of systemic diseases, insurance coverage of dentures may be more helpful from a socioeconomic perspective than coverage of dental implant treatments.
