우리나라 성인의 구강질환과 만성질환의 관련성 (The association of oral diseases and chronic diseases in Korean adult population)

  • Objectives : The purpose of this research is to use data from the third year of the 4th National Health and Nutrition Examination Survey to study relationship between oral disease and chronic disease that generally persist from 6 months to over a year, or more specifically, circulatory disease, diabetes, and osteoporosis. Methods : Of the data from the third year of the 4th National Health and Nutrition Examination Survey, 7,893 adults over 19 years old who completed medical examination, health survey, and nutrition survey were selected as the final research subjects. Relationship between chronic disease and oral disease was analyzed by cross tabulation (2-test) and logistic regression analysis using SPSSWIN ver 18.0. Results : 1. Differences in the rate of prevalence of periodontal disease and the rate of prevalence of missing teeth were statistically significant with respect to age, gender, marital status, education level, residential area, income level, and occupation. Rate of prevalence of dental caries was statistically significant with respect to age, education level, and income level. 2. After examining the relationship between existence of chronic disease as diagnosed by doctor with oral disease, rate of prevalence of periodontal disease and missing teeth, hypertension, hyperlipidemia, stroke, myocardial infarction, angina, diabetes, and osteoporosis showed statiscally significant difference. 3. Examination of rate of prevalence of chronic disease with respect to oral disease, periodontal disease and missing teeth exerted statistically significant influence on hypertension, hyperlipidemia, stroke, myocardial infarction, angina, diabetes, and osteoporosis (p<0.05), while dental caries did not have statistically significant effect. 4. Analysis of coupling effect of periodontal disease and missing teeth on chronic disease showed that they were related in all chronic diseases examined in this study (hypertension, hyperlipidemia, stroke, myocardial infarction, angina, diabetes, and osteoporosis) (p<0.05). Conclusions : Periodontal disease and missing teeth were found to increase the rate of prevalence of chronic disease.

  • Purpose: The purpose of this study was to investigate the relationship between periodontal disease, diabetes and chewing problems in Korean adults over the age of 19 using the 6th National Health Nutrition Survey. Methods: Data from the 6th National Health and Nutrition Survey (Ministry of Health & Welfare, 2013; 2014; 2015) were used. In this study, 17,101 adults aged 19 and older were included in the study to determine the relationship between diabetes and chewing problems in Korean adults. Results: Diabetes and chewing problems have been associated with periodontal disease. Diabetes was 0.719 times lower(p<0.001) in periodontal disease than in the case of diabetes. Chewing problem was 1.360 times(p<0.001) periodontal disease prevalence compared to 'not at all uncomfortable'. It was found that the prevalence of periodontal disease was 2.139 times(p<0.001) compared to 'not at all uncomfortable'. It was found that the prevalence of periodontal disease was 2.296 times higher(p<0.001) compared to 'not at all uncomfortable'. It was found that the prevalence of periodontal disease was 2.119 times higher(p<0.001) compared to 'not at all uncomfortable'. Conclusion: Diabetes and chewing problems were found to be related to periodontal disease, and as reported in previous studies, diabetes and chewing problems related to oral disease need to be prevented and treated with regular checkups. In addition, based on the research results, it can be used as basic data for the health business plan that can maintain and manage health.

  • Objectives: The purpose of this study was to confirm the relationship between socioeconomic characteristics and the prevalence of periodontal disease, with a focus on employment status, in a representative sample of Korean adults aged 19-39 years. Methods: Data were obtained from 3,178 adults who completed the Korean National Health and Nutrition Examination Survey, health-related questionnaires, and an oral examination. Socioeconomic, demographic, and oral health-related behavior data were adopted as independent variables. The chi-square test was used to assess differences in the distribution of socioeconomic status, oral health behavior, and nutrition factors according to employment status. Logistic regression analysis was performed to confirm the relationships between the prevalence of periodontitis and the related variables. Results: Significant differences in sex, educational level, income, marital status, smoking status, prevalence of periodontitis, number of dental check-ups in a year, and unmet dental care were confirmed according to employment status. The prevalence of periodontal disease was not significantly related to employment status. Conclusions: These results suggest a significant difference in periodontal disease according to socioeconomic factors in Korean adults. However, the prevalence of periodontal disease was not related to employment status. To prevent the progression of periodontal disease, attention should be paid to oral health, time available for dental treatment, smoking cessation, and use of floss.

  • Objectives: The purpose of this study is to evaluate the relationship between socioeconomic characteristics and prevalence of periodontal disease in a representative sample of Korean adults older than age 20. Methods: Data of 3,837 adults were collected by the six Korean National Health and Nutrition Examination Survey, which was conducted in 2015. Socioeconomic, demographic, and oral health-related behavior data were collected as independent variables. We determined frequencies, percentage, and determining statistical significance using multiple regression analysis. Results: Prevalence of periodontal diseases showed statistically significant difference in accordance with sex, age, socioeconomic and demographical characteristics and oral health-related behavior. It was confirmed that the prevalence of periodontal diseases was increased in the lower educational level and income (OR, 1.478 and 1.520) after adjusting for conditions such as age, sex, recent dental check-ups, visiting dental clinic, tooth brushing frequency, use of self-care devices. Conclusions: The prevalence of periodontal disease was related with socioeconomic factors in Korean adults. Therefore, differentiated oral health service policies and dental health education among adults with lower education and income is required in order to reduce the prevalence of periodontal disease.

  • The auther had examined the periodontal tissues of 273 nurses and students of nursing school in National Medical Center. the obtained results were as follows; 1. The ppevalence rate of the periodontal disease was 87.55% on the average in Korean women who are in the twenties. 2. The prevalence rates of the periodontal disease of the old women were highre than that of young women.

유지치주치료기간과 치은연하세균 출현율의 관계 (Relation between the interval of supportive periodontal therapy and the prevalence of the subgingival microflora)

  • This study was performed to evaluate the relation between the interval of supportive periodontal therapy and the prevalence of the subgingival microflora. The subgingival plaques from 108 patients were used in the study. Control group were the patients with no periodontal treatment and test groups were assigned into 3 groups according to the period of recall check: group 1; 1-2 months, group 2; 3-4 months, group 3; 6months or more. The polymerase chain reaction (PCR) used for direct identification of periodontal pathogens (P. gingivalis, T. forsythensis, T. denticola) in subgingival plaque. The results of this study were as follows. 1. The prevalence of P. gingivalis, T. forsythensis, T. denticola in control group were 100%, 87%, 90%. 2. In clinical parameters such as plaque index, gingival index, bleeding on probing, control group was not significant different with group 1 but Significant different with group 2, group 3. 3. In group 1, the majority of P. gingivalis had type II fimA. 4. When group 3 were compared with group 1, the prevalence of P. gingivalis increased. But the prevalence of P. gingivalis with type II fimA, which have the virulence factor, decreased. 5. We were unable to find the correlation between P. gingivalis with type IV fimA and periodontal disease. 6. The prevalence of T. forsythensis, T. denticola in test group were 85%, 93% or more. From the above results, we were able to find the relation between the interval of supportive periodontal therapy and the prevalence of the subgingival microflora and the need of the strict supportive periodontal therapy to prevent recurrence of periodontal disease, because there were high prevalence of periodontal pathogens.

The Effect of Metabolic Syndrome on Periodontal Disease in Korean Adults: Based on the Data from the 6th Korea National Health and Nutrition Examination Survey (2013∼2015)

  • Background: The purpose of this study was to evaluate the association between metabolic syndrome and periodontal disease in Korean adults. Methods: This study analyzed the screening data of 12,686 adults aged ≥19 years, including demographic characteristics, lifestyle habits, and each component of metabolic syndrome, obtained from the sixth Korea National Health and Nutrition Examination Survey database. Periodontal health status was measured by the community periodontal index. Subjects with three or more risk factors were considered as having metabolic syndrome. The Rao-Scott chi-square test was performed to assess the relationships between demographic characteristics, lifestyle habits, components of metabolic syndrome, and periodontal disease. Logistic regression analysis was performed based on the complex sample to evaluate the relationship between metabolic syndrome and periodontal disease. Results: The prevalence of periodontal disease was higher among the subjects with advancing age, lower average household income and education level, those working in production, residents of eup-myeon areas, in past or current smoker, those with excessive alcohol consumption habit in a week, and reduced brushing frequency and the use of oral care products (p<0.001). Each component of metabolic syndrome was associated with higher prevalence of periodontitis in the subjects with abnormal than in those with normal levels (p<0.001). The prevalence of periodontal disease in subjects with metabolic syndrome was approximately 1.443 times higher than that in normal subjects (odds ratio,1.443; p<0.001). Conclusion: This study confirmed the association between metabolic syndrome and periodontitis in Korean adults, and further studies will be needed to determine the causal relationship between the two conditions.

Prevalence and risk indicators of peri-implantitis in Korean patients with a history of periodontal disease: a cross-sectional study

  • Purpose: The aim of this study was to analyze the prevalence and risk indicators of peri-implantitis in Korean patients with history of periodontal disease. Methods: A total of 444 patients with 1,485 implants were selected from patients who had been treated at the Department of Periodontology, Chonbuk National University Dental Hospital between July 2014 and June 2015. A group with a history of peri-implantitis (HP) (370 patients with 1,189 implants) and a group with a current peri-implantitis (CP) (318 patients with 1,004 implants) were created based on the radiographic and clinical assessments of implants. The prevalence of peri-implantitis was calculated at both the patient and implant levels. The influence of risk variables on the occurrence of peri-implantitis was analyzed using generalized estimating equations analysis. Results: The prevalence of peri-implantitis in the HP and CP groups ranged from 6.7% to 19.7%. The cumulative peri-implantitis rate in the HP group estimated with the Kaplan-Meier method was higher than that in the CP group over the follow-up period. Among the patient-related risk variables, supportive periodontal therapy (SPT) was the only significant risk indicator for the occurrence of peri-implantitis in both groups. In the analysis of implant-related variables, implants supporting fixed dental prosthesis (FDP) and implants with subjective discomfort was associated with a higher prevalence of peri-implantitis than single implants and implants without subjective discomfort in the HP group. The presence of subjective discomfort was the only significant implant-related variable predictive of peri-implantitis in the CP group. Conclusions: Within the limitations of this study, the prevalence of peri-implantitis in Korean patients with a history of periodontal disease was similar to that reported in other population samples. Regular SPT was important for preventing peri-implantitis. Single implants were found to be less susceptible to peri-implantitis than those supporting FDP. Patients' subjective discomfort was found to be a strong risk indicator for peri-implantitis.

조사연도에 따른 흡연 유형과 치주질환의 관련성 분석: 제4기, 제5기 국민건강영양조사를 이용하여 (The Association between Types of Smoking and Periodontal Disease according to the Survey Year Using the Fourth and Fifth Korea National Health and Nutrition Examination Surveys)

  • 본 연구는 국민건강영양조사 제4기와 제5기(2007년~2012년) 자료를 이용하였으며, 구강검진 조사자료가 제공되지 않은 2011년 자료를 제외하고, 20세 이상의 성인 11,643명을 대상으로 연도별 흡연 유형별 치주질환의 추이를 살펴보고, 치주질환에 영향을 주는 인자를 평가하였다. 5개년도를 통합한 자료에서는 비흡연자에 비해 직접흡연자는 단순, 다중 로지스틱 회귀분석 모두에서 치주질환이 있을 aOR이 1.78, 1.77이었으며(p<0.001), 연도별 경향성을 보았을 때, 여자에 비해 남자가 치주질환이 있을 aOR은 1.12로 나타났다(p<0.001). 연도별 흡연 유형에 따라 치주질환 유병률은 치주질환이 있는 집단에서 직접흡연자의 비율이 현저하게 증가추세를 보였다. 또한 주요인자를 보정한 연도별 흡연 유형과 치주질환의 관련성을 단변량 분석한 결과, 비흡연자에 비해 직접흡연자의 치주질환이 있을 aOR은 2007년도에 1.99로 가장 높게 나타났다. 반면에 간접흡연자가 비흡연자에 비해 치주질환이 있을 확률은 통계적으로 유의하지 않았다. 인구사회학적 변수와 임상적 변수를 보정한 다변량 로지스틱 분석결과, 2007년도를 제외하고는 치주질환 발생에 있어 직접흡연자의 위험비는 통계적으로 모두 유의하였다. 반면 치주질환 발생에 있어 간접흡연의 영향력은 2010년도에서만 1.68의 aOR로 통계적 유의성을 보였다(p=0.032). 결론적으로 본 연구를 통해 연도에 따라 간접흡연의 노출이 증가되고 있음을 간접적으로 예측할 수 있었고, 직접흡연자에 비해 통계적 영향력의 크기는 뚜렷하지 않지만 치주질환에 있어 위험인자로서 독립적으로 고려되어야 함을 확인할 수 있었다. 중대구강병인 치주질환의 다양한 위험요인을 규명하고, 이를 예방하기 위한 측면에서 간접흡연과 치주질환의 명확한 기전과 영향력 크기를 밝히기 위한 근거자료 마련이 지속화되어야 할 것이다.

중장년층 당뇨병환자의 구강관리실태와 치주질환 (Oral care status and periodontal disease of middle aged diabetic patients)

  • Objectives: The purpose of the study was to identify the oral care status and periodontal disease of middle aged daibetic patients in Korea. Methods: The primitive data were taken from 238 diabetic people in the 5th Korea National Health and Nutrition Examination Survey(KNHANES) in 2010 - 2012. The variables included general profiles(gender, age, occupied area, education, and smoking), oral care status(brushing time, brushing time, use of oral hygienic supplies, availability of oral hygienic supplies, oral examination, and recognition of oral health state), and diabetes management(morbidity period, treatment, education, and glycosylated hemoglobin levels). Results: Most of the subjects took gargling more than twice a day and it accounted for 73.5%. Oral hygienic supply users accounted for 24.8%, and most of them used floss brush. Oral examination was done in 19.3% of the respondents and 55.0% thought that their oral health status were very poor. The prevalence rate of the periodontal diseases was 46.6%. Female tended to have a higher rate of periodontal disease than the male. Oral hygienic supply users had lower rate of periodntal disease than those who did not use. Those who had poor subjective oral health status had higher prevalence rate of periodontal disease than those who did not. Conclusions: Periodontal disease is closely related to good oral care of middle aged diabetic patients. It is necessary to develop the program for the prevention of the periodontal disease for the diabetic patients.
