  • The purpose of this study is to design electrical stimulation system for pharyngeal dysfunction(dysphagia) in stroke patients. Pharyngeal muscle group activity is important, because contracting muscles provide the driving force at the initiation of the swallow and generate the pressure gradients necessary for bolus movement into the esophagus. Although we have many treatment methods for dysphagia, electrical stimulation system will be useful for stroke patients having dysphagia. Electrical stimulation can be divided into the body stimulation and electrodes. The body stimulation is divided again into frequency counter, time control and current measurement part. These parts are to control the current intensity, frequency and stimulating time. And they can be variable according to the patient's clinical assessment. The electrode plays a role to deliver the current from the system to the muscle. Also the position of the electrode can be variable according to the treatment method. We performed the clinical experiment with the stroke patient who has swallowing disorder. The videofluoroscopy was used for the observation. From the result of clinical experiment based on electrical stimulation, we expected that the dysfunction(in pharynx) level of the patient can be improved. However we could not have enough effectiveness of the treatment because of the number of patients, patient's adaptation and treatment period. We will design the optimized electrical stimulation system based on enough clinical experiment in the future.

간섭파가 노인의 교감신경계에 미치는 영향 (The Effects of Interferential Current Therapy on Sympathetic Nerve System in Senile patients)

  • The purpose of this study was to investigate the influence on sympathetic nerve system of interferential current therapy(ICT). The subjects were consisted of 20 senile patients, 10 males and 10 females with an average age of 71 years old. And systolic and diastolic blood pressure, temperature, heart rate, and respiratory were tested. The results were as follows: 1) Systolic and diastolic blood pressure were observed a statistical significance between before and during stimulation(p < .05). 2) Temperature was observed a statistical significance between before and during, and before and after 10 minutes stimulation(p < .05). 3) Heart rate and respiratory were not statistical significance(p > .05). These results are imply that electrical stimulation is directly or indirectly influence on sympathetic nerve system.

  • We have examined the electrical properties of arachidic acid Langmuir(L)films by using a displacement-current-measuring technique with pressure stimulation, displacement current peak appeared at a area per molecule around $90{\AA}^2$, possibly due to the orientational change in hydrophobic part of arachidic acid molecules. The displacement current is the transient current, it is generated when charged particles existing in single monolayers are displaced with the external stimulation. In this report, we mainly describe the displacement current generation from arichidic acid monolayers with pressure stimulation.

구형파 변조된 경두개 자기자극을 이용한 쥐의 감각피질 자극실험 (Mouse Somatosensory Cortex Stimulation Using Pulse Modulated Transcranial Magnetic Stimulation)

  • 본 논문에서는 쥐 뇌의 감각 피질을 자극하기 위해 변조신호를 이용하여 경두개 자기자극(TMS) 동물실험을 수행하였다. 제안한 TMS 시스템은 인버터, 변압기, 커패시터, 가변 인덕터, 자극 코일로 구성되어 쥐 뇌의 감각피질에 약 1.5 mT의 자기장이 생기도록 시스템을 설계하였다. 자극 신호는 구형파 변조되었으며, 반송 주파수는 85~91 kHz의 범위로 변화시키며, 자극 효과를 관찰하였다. 자극 결과 반송 주파수가 89 kHz 미만일 때에는 자극의 효과를 볼 수 없었고, 반송 주파수가 89 kHz 이상일 때에는 압력 자극에 반응하는 역치 값이 낮아지는 효과를 볼 수 있었다.

  • Recently, the study on development of electrical and electronic device is done to get miniature, high degrees of integration and efficiency by using inorganic materials. the study of Langmuir-Boldgett(LB) method that uses organic materials because of the limitation for the ultrasmall size. GaAsAl 레이저 자극이 흰쥐의 압통역치에 미치는 영향 (Effects of GaAsAl Laser on the Pressure Pain Threshold in Rats)

  • This study was designed to evaluate the analgesic effect of low power GaAsAl laser on the pain threshold of mechanical stimulation using different treatment points, acupuncture point (zusanli) and non-acupuncture points(back). Furthermore, we investigated the analgesic effect of low power GaAsAl laser using the different duration and intensity of laser in mechanical stimulation induced pain behavior. The results were summarized as follows: 1. The threshold of mechanical stimulation was significantly increased by GaAsAl laser stimulation into zusanli point after 15 and 30 min after laser stimulation(P<05). However, the laser stimulation into non-acupoint did not affect the pain threshold of mechanical stimulation. with dose dependent manner. 2. In order to investigate the analgesic effects of BV depending upon different intensities of laser stimulation, the experimental animals were divided into three groups: 3 mW treated group, 6 mW treated group and 10 mW treated group. The low power GaAsAl laser stimulation was applied into zusanli acupoint for 30 min with different intensity of laser stimulation. Six and ten mW of laser stimulation significantly increased the pain threshold of mechanical stimulation at 15 min after laser stimulation as compared to that of control group(P<.05). Moreover, the analgesic effect of 10 mW laser stimulation was maintained for 30 min after laser stimulation (P<.05). 3. Finally, we tested the analgesic effect of 10 mW laser stimulation using different duration such as 10 min, 30 min or 1 hr after application of mechanical stimulation. In 30 min treatment group, the pain threshold of mechanical stimulation was increased at 15min and 30min after laser stimulation(P<.05). However, laser stimulation for 60 min dramatically increased the pain threshold of mechanical stimulation at 0 min after laser stimulation and the analgesic effect of laser stimulation was observed until 1 hr after laser stimulation. In conclusion, these data apparently demonstrate that low power GaAsAl laser has analgesic effect on mechanical induced pain model in rats. In addition, the treated point, intensity and duration of laser stimulation should be concerned before clinical application for pain management purpose.

좌섬요통에 대한 BU 약침의 효과 (The Effects of BU Herbal Acupuncture on Low Back Pain of Sprain)

  • Objectives : This study was designed to estimate the effects of BU herbal acupuncture theraphy on low back pain of sprain patients by using Visual Analogue Scale(VAS) and pressure algometer. Methods : 13 volunteers were applied to BU herbal acupuncture stimulation on the BL23, BL24, BL25 and BL26 acupoints. The degree of improvement of low back pain of sprain was evaluated by VAS and pain pressure threshold(at the points of BL23 and BL24 before treatment, before 3rd treatment(after herbal acupuncture). Results : In VAS scores, there were significant differences between before and after herbal acupuncture stimulation. But there was no significant difference between before and after herbal acupuncture stimulation in pain threshold. Conclusions : The effectiveness of BU herbal acupuncture theraphy on low back pain of sprain was shown through VAS and pressure algometer. These imply that BU herbal acupuncture theraphy may be useful for low back pain of sprain. Further study is needed about BU herbal acupuncture.

The Effects of Repetitive Transcranial Magnetic Stimulation on Balance Ability in Acute Stroke Patients

  • PURPOSE: The aim of the present study was to determine whether high frequency repetitive transcranial magnetic stimulation (rTMS) can improve balance ability in acute stage stroke patients. METHODS: The study was conducted on 30 subjects diagnosed with hemiparesis caused by stroke. The experimental group consisted of 15 patients that underwent rTMS for 15 mins and the control group consisted of 15 patients that underwent sham rTMS (for 15 minutes). A 70-mm figure 8 coil and a Magstim Rapid stimulator was used in both groups. Patients in the experimental group received 10 Hz rTMS applied to the hotspot in the lesioned hemisphere in 10-second trains with 50-second intervals between trains, for 15 minutes (total 2,000 pulses). Both groups received conventional physical therapy for 30 minutes a day, 5 days a week, for 4 weeks. Static balance ability analysis was performed using the Gaitview system to measure pressure rate, postural sway, and total pressure, and dynamic balance ability analysis was performed to measure pressure variables using a balance system. RESULTS: A significant difference was observed in post-training gains for pressure rate, total pressure in static balance, and overall stability index in dynamic balance between the experimental group and the control group (p<.05). CONCLUSION: The results of this study indicate that high frequency rTMS may be beneficial for improving static and dynamic balance recovery in acute stroke patients.

Effects of Morphine on Somatosyrnpathetic Reflex and Arterial Blood Pressure Response Evoked by Stimulation of Peripheral Nerves

  • In the present study, the relationship between the somatosympathetic reflexes and arterial blood pressure responses to electrical stimulation of the peripheral nerve was investigated in cats anesthetized with ${\alpha}-chloralose$. Single sympathetic postganglionic fiber activities were recorded from the hindlimb muscle and skin nerves and also from the cervical and abdominal sympathetic chains. Effects of the morphine on responses of the sympathetic nerve and arterial blood pressure to activation of the peripheral $A{\delta}-$ and C-afferent nerves were analyzed. The following results were obtained. 1) Arterial blood pressure was depressed by peripheral AS-afferent stimulation (A-response) and was elevated during C-afferent activation (C-response). 2) Intravenously administered morphine enhanced the C-response while the A-response decreased insignificantly, Only the C-response was decreased by intrathecal morphine. 3) All the ten recorded cutaneous sympathetic fibers showed periodic discharge pattern similar to respiratory rhythm and five of them also showed cardiac-related rhythm. However, most of the muscular sympathetic fibers had cardiac-related rhythm and only four fibers showed respiratory rhythm. 4) Morphine decreased the sympathetic C-reflex elicited by the peripheral C-afferent activation and the abdominal sympathetic A-reflex was also decreased by morphine. From the above results, it was concluded that supraspinal mechanisms were involved in the enhanced arterial pressor response to peripheral C-afferent activation by intravenous morphine.

Assessment of Pulsed Magnetic Field Stimulus by Using Finger Photoplethysmogram and Pressure Pulse Waveform

  • Photoplethysmogram (PPG) and pressure pulse waveform (PPW) were compared and evaluated for the efficacy of stimulating knuckles by using the pulsed magnetic field. Both signals were observed simultaneously while the knuckles were exposed for 10 min to the pulsed magnetic field, with maximum field intensity of 0.8 T and transition time of 0.126 msec. After 5 min stimulation of the knuckles, the results showed that the aging indexes calculated from the second derivative of the PPG were increased from -1.913 to 0.072, and that of the PPW from -0.063 to 0.387. However, for the relatively long-term stimulation for 10 min, we found that the values of both the aging indexes of the second derivatives and augmentation index of the PPW returned to the starting level. The changes observed in characteristic factors such as the aging indexes of the second derivatives and augmentation index of the PPW indicate the potential of pulsed magnetic field stimulation as a therapeutic method for the treatment of patients with peripheral vascular disease.
