일부 농촌주민의 건강증진 생활양식 수행정도 (A Study of the Health Promoting Life Style in Rural Area)

  • 본 연구는 1995년 4월 1일부터 4월 30일까지 농촌지역인 청도군 각북면에 거주하는 20세에서 59세까지의 성인 1,252명 중 450명을 임의 추출하여 보건요원, 마을건강원, 보건진료원이 설문지로 직접 면담하여 조사한 411명을 대상으로 건강증진 생활양식 수행에 관련되는 여러 요인과의 관계를 분석한 결과를 요약하면 다음과 같다. 지각된 현재의 건강상태는 나쁜 편이 연령이 증가할수록 높게 나타났고 여자군이 높게 나타났다. 건강 증진 생활양식의 문항별 수행정도를 보면 "전혀 수행하진 않음"에 50% 이상 응답한 문항은 "하루에 3번 이상 심호흡"과 "담배 피우지 않음"에서 였으며, 50% 이상이 "항상 수행함"에 응답한 문항은 "하루에 세끼식사를 거르지 않음", "집에서 정성껏 조리한 음식을 섭취한다", "아침식사는 곡 먹음", "내의는 면제품으로 입고 자주 갈아 입음"이었다. 지각된 현재의 건강상태별 건강증진 생활양식의 문항별 수행정도는 건강상태가 좋을수록 수행도가 높았고 "건강문제에 대해서 전문가와 상담"은 나쁜 건강 상태에서 수행정도가 높았다. 성별에 따른 건강증진 생활양식 수행전도는 남자군에서는 "마음을 터놓고 이야기 할 수 있는 친구 있음", "마음이 맞는 사람과 모임을 가짐", "자신감 있는 생활", 자신의 생활(직업)에 만족감을 느낌", "목표를 가지고 생활", "하루에 3번 이상 심호흡", "건강과 관련된 서적이나 신문기사를 관심 있게 읽음", "일주일에 서너번 적어도 20분 동안 활발하게 운동", "정상 체중유지를 위해 노력함" 문항이 높았고, 여자군에서는 "건강문제에 대해 전문가와 상담"과 "술을 과하게 마시지 않음" 문항이 높았다. 연령 다른 건강증진 생활양식 수행정도는 "아침식사는 꼭 먹음", "일찍 자고 일찍 일어남", "적당한 수면을 취함"은 연령이 증가할수록 수행도가 높았고 "취미", "여가선용", "청결", "건강과 관련된 서적 읽음"은 연령이 낮은 중에서 높은 수행도를 나타냈다. 종교유무에 따른 건강증진 생활양식 수행정도는 종교 있는 군이 "하루에 3번 이상 심호흡", "정기적 혈압측정", "건강에 해롭다고 알려진 음식은 먹지 않음", "평소에 바른 자세로 앉거나 걸음" 문항에서 수행도가 높았다. 학력에 다른 건강증진 생활양식 수행정도는, 학력이 높을수록 높았고, 낮은 학력을 가진 근에서는 "일찍 자고 일찍 일어남" 문항의 수행도가 높았다. 결혼 유무에 따른 건강증진 생활양식 수행정도는 미혼군이 "하루에 3번 이상 심호흡", "일주일에 서너번 적어도 20분 동안 활발하게 운동", "가금 자기만의 시간과 생활을 가지면서 여유를 찾음", "나름으로의 긴장과 압박감을 풀 수 있는 방법을 찾으려고 노력함", "활동시 신체 각 부위를 골고루 움직임" 문항에서 수행도가 높았고, 기혼군에서는 "정성껏 조리한 음식 섭취", "하루세끼 식사를 거르지 않음", "일찍 자고 일찍 일어남", 문항의 수행도가 높았다. 가족수에 따른 건강증진 생활양식 수행정도는 "목표를 가지고 생활" 문항은 가족수가 많을수록 수행도가 높았다. 가족 형태에 다른 건강증진 생활양식 수행정도는 핵가족에서 "건강과 관련된 서적이나 신문기사를 관심 있게 읽음", "긍정적인 사고방식으로 생활함", "여가시 좋아하는 취미활동을 함" 문항이 높은 수행도를 나타냈고 대가족에서는 "하루세끼 식사를 거르지 않음"과 "아침식사는 꼭 먹음" 문항이 높은 수행도를 나타냈다. 수행에 영향을 미치는 요인의 다변량분석 결과는 건강상태가 좋을수록, 학력이 높을수록, 나이가 많을수록 수행도가 높았고 결정계수는 14.6%였다.

선거(選擧)의 거시경제적(巨視經濟的) 충격(衝擊)과 파급효과(波及效果) (The Macroeconomic Impacts of Korean Elections and Their Future Consequences)

  • 본고(本稿)는 선거가 선거기간을 전후해서 어떻게 경제에 충격을 주고 이러한 충격의 여파로 경제는 선거후에 어떻게 영향을 받는지를 "베이지안" 벡터자기회귀모형(自己回歸模型)을 이용하여 분석하였다. 계수(係數)를 수정하면서 선거기간에 대해 예측을 할 경우의 예측오차의 평균으로 선거의 충격효과를 계산해 보면 선거는 선거전, 선거분기 및 선거후분기에 본원통화(本源通貨), 물가(物價), 생산(生産), 이자율(利子率), 투자(投資) 등에 상당한 충격을 주는 것으로 나타났다. 과거의 선거(選擧), 특히 80년대 이후 선거중에는 대체로 현금통화(現金通貨) 및 본원통화(本源通貨)가 선거 기간중 증가하고 선거후 환수된다. 이에 따른 유동성(流動性)의 변화(變化)로 금리(金利)는 선거기간중 약간 하락하고 선거후 다시 증가하는 양상을 보였다. 선거기간중(選擧期間中) 생산(生産) 및 고용(雇傭)은 선거에 따른 운동원차출(運動員差出) 및 조업감소(操業減少) 등의 영향으로 감소하나, 선거후에는 선거중의 생산감소(生産減少)를 상쇄(相殺)할 만큼 증대(增大)하는 것으로 나타났다. 선거(選擧)에 따른 물가(物價)의 충격(衝擊)은 주로 선거전분기에 나타났다. 이러한 과거의 선거기간중의 양태가 1992년의 각 선거에서 되풀이되고 총선과 대통령선거가 실시될 경우를 가상해서 그 효과를 시산(試算)해 보면 선거(選擧)로 인한 상당한 유동성증대(流動性增大)가 본원통화를 중심으로 예상되는 가운데, 소비자물가(消費者物價)가 1992년중 약 2%포인트, 1993년에 2.5%포인트 상승할 것으로 나타났다. 반면 선거로 인한 소비증대(消費增大)나 생산감소(生産減少) 등의 효과는 상대적으로 작을 것으로 예상되는 한편, 자금흐름의 왜곡이 발생하여 유동성이 증가했음에도 불구하고 생산자금은 압박을 받게 되어 투자(投資)가 부진해질 것으로 나타났다. 또한 이러한 선거의 효과는 1992년중 지방자치단체장선거가 실시될 경우 크게 확대된다.

Tissue Doppler imaging을 이용한 소아 종양 환자에서의 심기능 평가 (Evaluation of cardiac function by tissue Doppler imaging in children with cancer)

  • 목 적 : 항암제와 방사선 치료의 개발, 골수나 조혈모세포 이식 기회의 증가로 소아 백혈병과 고형 종양을 가진 환아들이 장기 생존하게 되면서 치료에 따른 장기 부작용, 특히 심장 독성에 크게 관심을 가지게 되었다. 항암제에 의한 심장 독성을 파악하는데 가장 적절한 방법은 비침습적이면서 정량적으로 측정할 수 있는 심초음파라고 할 수 있다. 본 연구에서는 이미 소아암으로 진단을 받고 치료를 받는 중이거나 치료를 종료한 환아들을 대상으로 조직 도플러 영상을 사용하여 심실 기능을 평가하고, 향후 동질환의 환아에서 조기 및 후기 심독성을 조기에 발견하기 위한 방법으로 반복적인 조직 도플러 영상을 적용하는데 기여하고자 하였다. 방 법 : 2005년 4월 1일부터 7월 1일까지 계명대학교 동산의료원 소아 종양 크리닉을 방문한 항암치료 중이거나 항암치료 종료된 환아 23명을 대상으로 하였다. 전체 대상 환아군(1군)은 단축 분획 29% 이상(2군), 28% 이하의 군(3군)으로 나누고, anthracyclin 사용군(4군)과 미사용군(5군)으로 다시 나누었다. M-mode와 간헐파 도플러를 이용하여 단축 분획, 구혈률, 승모판막으로 유입되는 혈류의 E, A 속도, DT, IVRT를 측정하였고, 조직 도플러 영상을 이용하여 심첨 4, 2 방도에서 중격, 전벽, 하벽, 측벽의 기저부, 중간, 심첨 부위의 Sm, Em, Am 속도를 측정하였다. 측정한 값을 이용하여 modified Tei index, E/A, E/Em ratio를 구하였다. 결 과 : 환아의 평균 나이는 8년 4개월이었고 남아 12명, 여아 11명이었다. 항암제 중 anthracyclin을 사용한 환아(4군)는 13명이었고, 방사선 치료를 받은 환아는 4명이었으며 단축 분획이 28% 이하인 환아(3군)는 6명이었다. 대조군의 평균 나이는 6년 6개월이었고 남아 6명, 여아 3명이었다. 환아군과 대조군간에 성별, 나이에 있어서 유의한 차이는 없었다. 대조군과 1군의 E/Em ratio는 $7.06{\pm}1.64$, $6.46{\pm}1.85$로 두 군간에 P<0.05로 의미있는 차이가 있었으나 다른 측정값들은 유의한 차이가 없었다. 대조군과 2, 3, 4, 5군간에도 E/Em ratio는 P<0.05로 의미있는 차이가 있었다. 2, 3군과 4, 5군간에는 측정값들의 군간의 유의한 차이가 없었다. 결 론 : 좌심실 평균 이완기압을 반영한다고 알려져 있는 E/Em ratio에서 대조군과 항암 치료 군간에 차이가 확인됨으로써 항암 치료 이후의 심장 기능 부전은 수축기 기능 변화가 아닌 이완기 기능 변화가 먼저 오는 것을 알 수 있었다. 심장 이완기 기능을 평가하는 간헐파 도플러 측정값들에서는 두 군간에 차이가 없음으로 이완기 기능을 평가하는데는 조직 도플러 영상을 이용하는 것이 더 민감한 방법이었다. 따라서 항암 치료 이후에도 장기적으로 관심을 가지고 심장 기능에 대한 잦은 추적 관찰이 필요하겠고, 추적 관찰하는 방법으로는 조직 도플러 영상을 이용한 심초음파 검사가 비침습적이고 간편하며 유용한 방법일 수 있다.

뉴로피드백·뇌체조·한방차를 병행한 통합요법이 독거노인의 뇌기능 향상 및 삶의 질에 미치는 영향 (The Effect of the Integrated Therapy of Neurofeedback, Brain Gymnastics, and Oriental Herbal Tea on the Improvement of Brain Functions and the Quality of Life of Elders living alone)

  • 본 연구는 독거노인을 대상으로 뉴로피드백 훈련과 뇌체조 그리고 한방차를 병행한 통합요법을 적용하여 독거노인의 뇌기능 향상 및 삶의 질에 미치는 영향을 확인하기 위한 단일군 전후 설계인 실험 연구이다. 2014년 12월 22일부터 2015년 2월 28일까지 8주 동안, Y군에 거주하는 65세~90세의 독거노인 23명(남 10명, 여 13명)을 대상으로 뉴로피드백 훈련은 2 Channel Neurofeedback System을 이용하여 이완 집중 기억력을 중심으로, 뇌체조는 한국정신과학연구소에서 개발한 뇌체조 프로그램을 이용하여 각각 1회 30분, 주 2회, 8주간, 16회를 하였다. 그리고 한방차는 동의보감과 여러 선행연구를 토대로 개발하여 조제한 한방차를 8주간, 매일, 하루 3잔 음용하도록 하였다. 그 결과 통합요법이 뇌기능지수 중 긴장도와 항스트레스 지수가 유의하게 변화하여 뇌기능 향상에 긍정적인 효과가 나타났다. 또한 삶의 질에서는 혈압과 혈당이 낮아지고, 삶의 질, 일상생활, 우울증상 등에서 유의미한 결과가 나왔다. 결론적으로 통합요법이 독거노인의 뇌기능 및 삶의 질을 의미 있게 향상 시킬 수 있음을 보여주었다. 그러므로 뉴로피드백 훈련, 뇌체조 및 한방차를 병행한 통합요법이 독거노인들의 삶의 질을 높이는데 반드시 필요한 중재요법이며, 보다 실질적이고 체계적으로 심신을 치유하여 삶의 질을 향상시키게 할 것이다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. And the surface charge density (or the number of ionizable functional groups) of the microspheres synthesized by "two stage shot growth method" was 6~13 times higher than that of the microspheres synthesized by conventional seeded batch copolymerization. As a previous step for biomedical application, adsorption experiments of bovine albumin on microspheres were carried out under various conditions. The maximum adsorbed amount was obtained in the neighborhood of pH 4.5. Isoelectric point of bovine albumin is pH 5.0, so experimental result shows that it shifted to acid area. Our department has developed an information system for routine hematology using down-sized computer system. We have used an IBM 486 compatible PC with 16MB main memory, 210 MB hard disk drive, 9 RS-232C port and 24 pin dot printer. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer. To use as the structural matrix of the composite, collagen was purified from human umbilical cord. The obtained collagen was treated by pepsin to remove telopeptides, and finally, the immune-free atelocollagen was produced: The cross linked atelocollagen was highly resistant to the collagenase induced collagenolysis.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N.
