급성 심정지로 나타난 연수경색 (Medullary Infarction Presenting as Sudden Cardiac Arrest: Report of Two Cases and Review of the Literature)

  • The causes of sudden death after medullary infarction involve arrhythmia, central respiratory failure, and dysautonomia. Sudden cardiac arrest in a medullary infarction is uncommon. Most of these cases experienced sudden cardiopulmonary arrest within 2 weeks from stroke onset as the extent of lesion increased. Here, we report two cases of medullary infarction presenting as sudden cardiac arrest. These cases indicate that medullary infarction could be one of the causes of sudden cardiac arrest.

  • We investigated the effects of bisphenol A (BPA), endocrine disruptor, on the mixed lymphocyte reaction and TNF-$\alpha$ production of antigen presenting cells in mice. Cells from mouse (C57BL/6) bone marrow were cultured with GM-CSF for 8 days and mature dendritic cells (DCs) were prepared. These DCs proliferation in response to Balb/c splenocytes was measured at 72 h of culture with BPA by tritiated thymidine incorporation ([3H]TdR) and [3H]TdR incorporation was determined by scintilation counting. Infectious Myositis of the Jaw Presenting as Trismus of Unknown Origin

  • Infectious myositis, an infection of the skeletal muscles, is a rare condition and potentially life-threatening if not detected and treated in the early stages. This clinical entity may arise from various pathogens, such as bacteria, fungi, parasites, and viruses. A propagation of contiguous infection, penetrating trauma, vascular insufficiency, or hematogenous spreading of microorganisms can cause infectious myositis. Though several cases have been reported in large muscle groups in the lower extremities, there are only a few reports on infectious myositis of the masticatory muscles. We report three cases of infectious myositis presenting jaw pain and trismus. Unlike a common head and neck infection caused by the spreading of odontogenic origin, the early diagnosis of infectious myositis was difficult because no specific lesion suspected to be the infection source was observed in the physical examination and the plain radiographs. Advanced imaging modalities such as computed tomography and magnetic resonance imaging, and laboratory evaluation is useful for the early detection of infectious myositis.

  • Tectal glioma is an indolent and benign tumor that occurs predominantly in the pediatric population. It arises in the tectum of the midbrain and, due to its location, contributes to the development of obstructive hydrocephalus, typically presenting with increased intracranial pressure (IICP) symptoms or signs. Here we report a rare case of tectal glioma that presented as adult-onset epileptic seizures without IICP symptoms and was treated with endoscopic third ventriculostomy and antiepileptic drugs.

Infected Left Atrial Myxoma Presenting Without Bacterial Growth on Blood Cultures: A Case Report

  • Although cardiac myxoma is one of the most common types of benign cardiac tumors, infected cardiac myxoma is very infrequent. The diagnosis of infected cardiac myxoma may be challenging because the presenting symptoms are non-specific and established management guidelines are lacking. This report describes a 39-year-old woman with a 5-month history of uncontrolled fever, chills, and myalgia who was diagnosed with myxoma and underwent mass excision. Although blood and urine cultures were negative for growing bacteria, a pathologic examination showed that the excised mass was a left atrial myxoma, with pan-bacterial polymerase chain reaction (PCR) of the surgical specimen revealing Haemophilus parainfluenzae at 99.87%, resulting in a diagnosis of infected cardiac myxoma. Laboratory tests, such as PCR, may supplement culture results in the diagnosis of infected cardiac myxoma.

육기(六氣)의 기질적(氣質的) 특성에 따른 한의 진단 활용성에 관한 연구 (A Study on Korean Medicine Diagnostic Application through Analysis of Temperamental Characteristics of Six Qi)

  • Objectives In this study, we tried to analyze the expression status and diagnostic application of the disease according to the characteristics of Six Qi. Methods Suwenxuanjiyuanbingshi (Exploration to Mysterious Pathogenesis and Etiology Based on the Plain Questions, 1152), Shoushibaoyuan (Longevity and Life Preservation, 1615), Dongeuibogam (Treasured Mirror of Eastern Medicine, 1613) and two modern works on Six Qi were analyzed for this study. Results Some substrate characteristics presenting Six Qi were extracted from original literatures. Also, we found that the characteristics of Six Qi can be applied to clinical trials by presenting them as three criteria as wind, humidity, and temperature. Conclusions This study suggests.

A Case of Primary Pancreatic Lymphoma Presenting with Obstructive Jaundice

  • A 55-year-old man was admitted to the hospital for jaundice. Computed tomography (CT) scans showed a diffuse mass in the pancreas and peripancreatic area, with infiltration to of the whole pancreas, and overall reduced enhancement compared to normal pancreas. Esophagogastroduodenoscopy revealed elevated mucosal lesion covered hyperemic mucosa at duodenal bulb and ulcerative lesion at body of stomach. Endoscopic ultrasonography revealed an irregular mass with unclear boundaries was observed within the pancreas. Abrupt narrowing of mid to distal common bile duct was seen and the stricture was caused by compression of pancreatic mass. Plastic stent was inserted and clinical improvement was achieved including resolution of jaundice. The patient is currently being treated with combination of rituximab, cyclophosphamide, doxorubicin, vincristine, and prednisolone. We report a case of primary pancreatic lymphoma presenting with obstructive jaundice.

Renal artery stenosis presenting as congenital nephrotic syndrome with hyponatremic hypertensive syndrome in a 2-month-old infant: a case report

  • Here, we present the case of a 2-month-old male infant with hyponatremic hypertensive syndrome resulting from stenosis of the right proximal and mid-renal arteries. The patient exhibited nephrotic-range proteinuria, low serum albumin, increased serum creatinine, and elevated renin and aldosterone levels. Doppler ultrasonography and computed tomography angiography revealed decreased vascular flow in the small right renal artery. Following a successful percutaneous balloon angioplasty, the patient experienced a decrease in blood pressure and normalization of serum electrolyte levels within a few days. However, it took 3 months for the proteinuria to resolve completely. This case is significant as it represents the first reported instance of a neonate presenting with clinical features resembling congenital nephrotic syndrome caused by renal artery stenosis that was successfully treated with percutaneous renal angioplasty.

Synthetic Approach to 99mTc-labeled SPECT Radiotracers with Multi-nitroimidazoles for Hypoxia

  • Hypoxia, defined as the deficiency of oxygen, is a significant hallmark of cancers presenting in the majority of solid tumors. Detection of tumor hypoxia is essential in cancer diagnosis to prevent cancer progression, metastasis, and resistance to cancer therapies in clinical practices. Single-photon emission computed tomography (SPECT) is one of the methods studied and applied for hypoxia detection with the use of radiolabeled imaging agents in which 99mTc is the common radioisotope used for radiolabeling. Nitroimidazoles are the hypoxia-targeting moieties presenting in numerous 99mTc-radiolabeled imaging agents due to their bio-reducible ability in hypoxic environments. Recently, in addition to 99mTc-labeled radiopharmaceuticals containing one nitroimidazole unit, there has been considerable attention given to 99mTc-radiopharmaceuticals bearing two or more nitroimidazole units. This review summarizes the synthesis of hypoxia-targeting chelators and radiolabeling processes to produce these 99mTc-radiopharmaceuticals for SPECT imaging.
