성인 총폐정맥 환류이상 -1례 보고- (Total Anomalous Pulmonary Venous Connection in Adult -A Case Report-)

  • We experienced one case of TAPVC patient who was 39 years old man with mild dyspnea on exertion and easy fatigability. He was the oldest patient among TAPVC which was reported in Korea. Preoperative echocardiogram, cardiac catheterization and angiogram revealed supracardiac type TAPVC drained through left innominate vein. An anastomosis between common pulmonary venous trunk and left atrium, pericardial patch closure of ASD and ligation of left vertical vein were performed with extracorporeal circulation. The postoperative course was uneventful and discharged with excellent general condition and has been well during 8 months follow-up.

심실중격결손의 크기 측정에 있어서 술전 검사의 의의 (Significance of the Preoperative Examinations in Predicting the Defect Size of Ventricular Septal Defect)

  • We evaluated the correlationship between the predicted defect size at preoperative examination and the actual defect size at operation room, by examining 69 cases of ventricular septal defect operated at the deparment of Thoracic and Cardiovascular surgery, Kyungpook University Hospital from January 1988 to December 1990. We excluded cases associated with other cardiac anomalies. Of the 69 cases, 39 are male and 30 female, forming 1.3:1 sex ratio in males favor. Their age range from 6 months to 16 year, and 4.3 on the average Their body weights are from 6 to 45kg and 15 on the average. According to Soto`s classification, perimembranous type costitutes 42 cases[61%], doubly committed subarterial type 23 cases[33%], and muscular type 4 cases[6%]. The average diameter of defect size is 8.0$\pm$3.5mm measured in 2D-echocardiogram, 5.6$\pm$3.4mm in angiogram, and 7.4$\pm$4.4mm in operative field. There is statistically significant correlation between the size from 2D-echocardiogram and actual defect size[p=0.001], and no significant difference between the two. Especially in the cases without anurysmal formation, they are nearly the same. Cardiothoracic ratio, pulmonary to systemic flow ratio, pressure ratio and resistance ratio also have statistically significant correlation. Main pumonary artery to descending aorta diameter ratio is correlated with the actual defect size. There is statistically significant correlation between the size from angiogram and actual defect size with some difference.

  • We report on two women who underwent myocardial revascularization associated with antiphospholipid syndrome (APS) with different pathogenic patterns. The first woman presented with acute myocardial infarction, and preoperative angiograms demonstrated rapidly progressing coronary lesions, presumptive unstable plaque, and dissection. Operative findings, however, showed fresh thrombi in the coronary arteries, and she was diagnosed postoperatively as having APS. Her one-year angiogram demonstrated improved coronary lesions and a competitive flow pattern in the grafts. The second woman presented with unstable angina and had been treated for systemic lupus erythematosus and secondary APS for more than 14 years. She underwent myocardial revascularization due to accelerated coronary atherosclerosis. Her one-year angiogram demonstrated patent grafts.

복재정맥을 이용한 관상동맥 우회로 술 32례의 임상적 고찰 (Clinical Evaluation of 32 Cases of Aortocoronary Bypass with Sapheneous Vein)

  • From January 1990 to May 1993, 32 patients were underwent aortocoronary bypass with venous conduit using saphenous vein at Department of Thoracic and Cardiovascular Surgery, Masan Koryo General Hospital. There were 19 men, 13 women and a mean age was 57.8 years[range from 46 to 68]. The involved risk factors were as follows: smoking 15 cases, hypertension 14 cases, diabetic mellitus 6 cases, cholesterol[>240mg/dl] 13 cases, and triglyceride[>180mg/dl] 11 cases. The preoperative clinical status were chronic stable angina 4 cases, unstable angina 24 cases and myocardial infarction 4 cases. At angiogram, numbers that involved in coronary artery of narrowing more than 50% luminal diameter were 54 vessels[left anterior descending 30, right coronary artery 17, circumflex 7]. We performed 53 grafts in32 patients, average was 1.7 grafts per patient, 16 patients were single bypass, 11 patients were double bypass and 5 patients were triple bypass. The overall postoperative mortality was 3 %.

복부 대동맥에 발생한 혈관 내막 육종 의증 - 1예 보고 - (Suspected Intimal Sarcoma in the Abdominal Aorta -A case report-)

  • 68세 남자 환자가 흉통을 주소로 내원하여 시행한 심혈관 조영술에서 삼 혈관 질환이 발견되어 관상동맥 우회술을 계획하였다. 수술 전 검사로 시행한 컴퓨터 단층촬영 혈관 조영술에서 신동맥하 복부 대동맥부터 양측 총장골동맥에 걸친 거미줄 모양의 혈관 내 종괴가 발견되었다. 대동맥내 혈관 내막육종을 의심하고 심폐바이패스를 사용하지 않는 관상동맥 우회술과 함께 신동맥하 복부 대동맥과 양측 총장골동맥을 절제 후 치환하는 수술을 시행하였다. 병리검사 소견에서 대동맥 내 병변은 혈관 내막 육종이 의심되었다. 수술 후 3개월에 시행한 컴퓨터 단층촬영 혈관 조영술에서 잔존 또는 재발의 증거는 없었다.

뇌수막종으로 오인된 두개 내에만 발생한 Rosai-Dorfman Disease: 증례 보고 (Isolated Intracranial Rosai-Dorfman Disease Mimicking Meningioma: A Case Report)

  • Rosai-Dorfman Disease (이하 RDD)는 드문 조직구 증식성 질환이며 두개 내에만 발생하는 경우는 매우 드물다. 대부분의 두개 내 RDD는 균질한 조영증강을 동반하는 경막의 종괴들로 나타나서, 심한 경부 림프절증을 보이지 않는 경우에는 수술하기 전에 뇌수막종과의 감별이 어렵다. 저자들은 65세 남자 환자의 두개 내에만 발생한 RDD의 드문 증례에 대해 보고하고자 한다. 뇌 MRI에서 우측 전엽 원개에 경계가 분명한 조영증강되는 종괴의 형태로 보였으며 미세한 확산제한이 관찰되었다. 하지만 수술 전 뇌혈관조영상에서는 겨우 미세한 홍조만이 보였다. 비록 두개 내에만 발생한 RDD가 드물기는 하지만, 경막에 기반한 종괴가 뇌혈관조영상에서 저혈관성의 형태로만 보일 때 감별질환으로 생각할 수 있다.

중증 뇌동맥류 환자의 조기 수술에 있어서 뇌압 측정의 중요성 (Significance of Intracranial Pressure Monitoring in Early Surgery for Poor-Grade Aneurysm Patients)

  • Objective : Patients with poor grade aneurysm usually present with increased intracranial pressure(ICP), even those without an intracranial clot. Based on this fact, the present study investigated a significance of intracranial pressure monitoring in those patients. Patients and Methods : A total of 60 patients with Hunt and Hess Grade IV(50 patients) or V(10 patients) were treated for aneurysmal subarachnoid hemorrhage(SAH) during a 3-year-period, and intraparenchymal ICP was measured in the majority, immediately after arrival to the emergency room. Early surgery including intraoperative ventriculostomy was undertaken within 3 days after SAH. An ultraearly surgery was performed without preceding angiogram or ICP monitoring in patients with large sylvian hematomas, highly suggestive of middle cerebral artery aneurysm. Outcomes were assessed by the Glasgow Outcome Scale(GOS) at 6 months. Results : In overall, favorable outcome(GOS scores 1-2) was seen in 27(54.0%) of admission Grade IV and 1(10.0%) of admission Grade V patients. Of the 38 surgical patients with preoperative ICP monitorings, 25 patients (80.6%) exhibiting ICP values of less than 40mmHg showed favorable outcome, however, no patients with ICP values above 40mmHg recovered(Fisher's exact test, p=0.0001). Conclusion : It is concluded that a preoperative ICP above 40mmHg before ventriculostomy indicate significant vital brain destruction as intractable intracranial hypertension, and Grade IV patients at admission with an ICP below 40mmHg can be of benefit from early surgical intervention while Grade V patients still remains unfavorable.

Unilateral C1 Lateral Mass and C2 Pedicle Screw Fixation for Atlantoaxial Instability in Rheumatoid Arthritis Patients : Comparison with the Bilateral Method

  • Objective : Bilateral C1 lateral mass and C2 pedicle screw fixation (C1LM-C2P) is an ideal technique for correcting atlantoaxial instability (AAI). However, the inevitable situation of vertebral artery injury or unfavorable bone structure may necessitate the use of unilateral C1LM-C2P. This study compares the fusion rates of the C1 lateral mass and C2 pedicle screw in the unilateral and bilateral methods. Methods : Over five years, C1LM-C2P was performed in 25 patients with AAI in our institute. Preoperative studies including cervical X-ray, three-dimensional computed tomography (CT), CT angiogram, and magnetic resonance imaging were performed. To evaluate bony fusion, measurements of the atlanto-dental interval (ADI) and CT scans were performed in the preoperative period, immediate postoperative period, and postoperatively at 1, 3, 6, and 12 months. Results : Unilateral C1LM-C2P was performed in 11 patients (44%). The need to perform unilateral C1LM-C2P was due to anomalous course of the vertebral artery in eight patients (73%) and severe degenerative arthritis in three patients (27%). The mean ADI in the bilateral group was 2.09 mm in the immediate postoperative period and 1.75 mm in 12-months postoperatively. The mean ADI in the unilateral group was 1.82 mm in the immediate postoperative period and 1.91 mm in 12-months postoperatively. Comparison of ADI measurements showed no significant differences in either group (p=0.893), and the fusion rate was 100% in both groups. Conclusion : Although bilateral C1LM-C2P is effective for AAI from a biomechanical perspective, unilateral screw fixation is a useful alternative in patients with anatomical variations.

Unilateral Posterior Atlantoaxial Transarticular Screw Fixation in Patients with Atlantoaxial Instability : Comparison with Bilateral Method

  • Objective : Bilateral C1-2 transarticular screw fixation (TAF) with interspinous wiring has been the best treatment for atlantoaxial instability (AAI). However, several factors may disturb satisfactory placement of bilateral screws. This study evaluates the usefulness of unilateral TAF when bilateral TAF is not available. Methods : Between January 2003 and December 2007, TAF was performed in 54 patients with AAI. Preoperative studies including cervical x-ray, three dimensional computed tomogram, CT angiogram, and magnetic resonance image were checked. The atlanto-dental interval (ADI) was measured in preoperative period, immediate postoperatively, and postoperative 1, 3 and 6 months. Results : Unilateral TAF was performed in 27 patients (50%). The causes of unilateral TAF were anomalous course of vertebral artery in 20 patients (74%), severe degenerative arthritis in 3 (11%), fracture of C1 in 2, hemangioblastoma in one, and screw malposition in one. The mean ADI in unilateral group was measured as 2.63 mm in immediate postoperatively, 2.61 mm in 1 month, 2.64 mm in 3 months and 2.61 mm in 6 months postoperatively. The mean ADI of bilateral group was also measured as following; 2.76 mm in immediate postoperative, 2.71 mm in 1 month, 2.73 mm in 3 months, 2.73 mm in 6 months postoperatively. Comparison of ADI measurement showed no significant difference in both groups, and moreover fusion rate was 100% in bilateral and 96.3% in unilateral group (p=0.317). Conclusion : Even though bilateral TAF is best option for AAI in biomechanical perspectives, unilateral screw fixation also can be a useful alternative in otherwise dangerous or infeasible cases through bilateral screw placement.

미세수술을 이용한 하지의 재건시 삼차원 컴퓨터 혈관조영술의 유용성 (Usefullness with 3D CT Angiography in Microsurgical Reconstruction)

  • Preoperative angiography is frequently used in the planning of microsurgical reconstruction for identification of vascular abnormality that influence the planning of operation. But, recently 3D CT angiography is considered as new technique that can provide detailed information about vascular anatomy as well as soft and bony tissue without the risks of invasive angiography. 3D CT angiograms were performed in 19 patients before microsurgical reconstruction for the lower extremity and hand between May of 2003 and Oct of 2004. Sixteen of the studies were of the donor site and all of 19 studies were of the recipient site. No complications were found from the 3D CT angiograms. In one case of the bone exposed open wound, the injury of anterior tibial artery was identified and the zone of injury was adequately demonstrated. With the improvement in quality of CT imaging, 3D CT angiograms may provide a favorable alternative to invasive angiography. It is capable of providing high-resolution, three dimensional vascular imaging without the need for arterial puncture and prolonged post-procedure observation. The relation among blood vessels, bones, and soft tissue is well demonstrated in 3D CT angiogram. Also The acquisition time and examination cost were considerably lower in comparison with invasive angiography. In conclusion, this study demonstrates that 3D CT angiography may provide accurate, safe, and cost-effective preoperative imaging. The 3D CT angiography with relatively low morbidity, low cost, ease of image acquisition can have an broader role in microsurgical reconstructive surgery.
