청소년 임신에 따른 임산부와 신생아의 주산기 문제점 (Obstetric and Neonatal Outcomes of the Teenage Pregnancy)

  • 목 적 : 현재 국내 출산율은 계속 감소하고 있고 또 출산 연령이 계속 높아지고 있는 추세임에도 불구하고 10대 청소년들의 분만은 줄어 들지 않고 있다. 이러한 청소년 임신은 어린 신부와 그들에게서 태어난 신생아들에게도 의학적인 문제점뿐 아니라 사회적인 부담으로 작용하게 되므로 본 연구자들은 이러한 문제점들을 알아 보고자 하였다. 방 법 : 2004년 1월부터 2009년 7월까지 고려대학교 안산병원에 입원한 산부가 19세 이하로 청소년이었던 산부와 그들에게서 태어난 신생아를 대상으로 하였다. 대상 산부는 27명이었으며 대상 신생아는 28명이었고 이들의 의무기록을 후향적으로 분석하여 산부와 신생아의 특성, 주산기 합병증 및 지원했던 사회적 지원에 대해 조사하였다. 결 과 : 27명의 모든 산부는 미혼이었으며 이 중 11명은 결손 가정의 자녀였다. 임신으로 적절한 산전검사를 받은 산부는 3명뿐이었으며 특히 11명(40.7%)의 산부는 산전검사를 전혀 받지 않았다. 산부의 주산기 합병증으로 조산, 빈혈, 조기 양막 파수, 융모양막염 등이 있었다. 대상 신생아 28명 중 미숙아는 17명(60.7%)이었으며 극소 저체중 출생아는 10명(35.7%)이었다. 신생아의 합병증은 모두 미숙아에서 나타났으며 호흡곤란증후군 8명, 패혈증 3명, 치료가 필요했던 동맥관 개존증, 괴사성 장염, 뇌실 내 출혈 등이었다. 입원 중 사망한 환아는 1명으로 사인은 폐출혈이었다. 퇴원한 신생아 27명 중 14명(51.9%)이 입양되었거나 아동 보호 시설에 위탁되었고 친부모에게 양육된 경우는 12명(44.4%)이었다. 결 론 : 청소년 임신은 조산으로 출산하는 경우가 많고 산전 검사를 제대로 받지 못해 적절한 출산 준비 없이 신생아를 출산하게 되어 미숙아의 빈도가 높고 이로 이한 주산기 합병증이 많다. 또한 신생아가 미혼모에게서 출생하게 되므로 친부모에게 양육되지 못하고 입양되거나 아동 보호 시설에 위탁되는 경우가 많아 사회적 부담이 증가하게 된다. 따라서 이런 문제를 예방하기 위해 청소년에 대한 피임 교육과 함께 임신에 대한 적절한 정보를 제공하여 임신이 된 경우 적절한 관리를 받을 수 있도록 적극적인 사회 지지가 필요하겠다.

저출생 체중아 분마에 대한 임상적 고찰 (Clinical Observation on Delivery of Low Birth Weight Unfant)

  • A clinical study was made on 365 low birth weight infant and 406 normal birth weight infant who had been born at Kangnam St. mary's Hospital during past 3 years from Jan. 1, 1995 to Dec. 31, 1997. the data of this study were gathered through reviewing of medical records. 1. Comparison of general characteristic with of obstetric characteristic 1) Old maternal age, previous abortion and previous LBWI delivery in the group of low birth weight infant(LBWI) mother were more prevalent than those in the group of normal birth weight infant(NBWI)mother 2) Cesarean section, abnormal presentation and multiple pregnancy in the group of LBWI mother were prevalent than those in the group of NBWI mother. 3) regular antenartal care and visiting rate of tertiary hospital in the group of LBWI mother were more prevalent than those in the group of NBWI mother. 2. Frequency of low birth weight infant 1) Anmual average frequency of LBWI was 6.5% and monthly frequency was the highest in January and december. 2) The frequency of LBWI was the highest in 37-40wks of gestational age and was the highest in 2251-2500 gm of birth weight. 3) The frequency of congenital anomaly in the group of LBWI was more prevalent than that of NBWI. 3. Mortality rate of LBWI The mortality rate of LBWI was 9.2%. The highest mortality rate was noted before 27wks of gestational age, less than 1000gm of birth weight and within 12hrs of delivery. 4. The most common complication of pregnant women was pre-term labor, the most complication relating to placenta was premature rupture of membrane(PROM) and the most fetal complication was fetal distress in delivered LBWI. 5. Significant relating factors of low birth weight infant delivery were associated with maternal age, previous delivery, previous low birth weight delivery, pre-eclampsia, anemia, oligohydramnios, PROM, placenta previa, abruptio placenta, fetal sex, fetal distress and congenital anomaly.

초임부의 교육 요구와 만족도에 관한 연구 (A Study on the Educational Needs and Satisfaction of Primigravida during the Antepartal Period)

  • This study was carried out to identify the difference between educational needs and levels of satisfaction of primigravida during the antepartum period. The goal of the study was to obtain data needed to develope educational programs and to improve the nursing quality for antepartal clients. The subjects were 106 primigravida who attend antenatal clinic at 3 general hospitals in the Seoul area. The data was gathered using a questionnaire which, consist of 71 items, was developed by the researcher from May 4 to June 3, 1998. Results found are as follows : 1. The characteristics of subjects : The majority of subjects were aged 25-29 years(73.6%), college graduates(61.3%), jobless(68.9%), had no religion(38.7%), attended antenatal clinic regularly(91.5%), duration of pregnancy was 38-39 weeks(49.06%), pregnancy was planned(67%), and 42.5% have had some sort of prenatal education. 2. Over all level of educational needs was relatively high(Mn. 3.97) but the level of satisfaction was of average level(Mn. 3.01). Therefore, differences between the level of educational needs and satisfaction was significant(P=.0001). 3. The educational needs by category, the highest need was on birth preparation(Mn. 4.18), self care of the mother and infant care(Mn 4.10), health maintenance and promotion(Mn. 3.79), the lowest was care of discomfort(Mn 3.66). The level of satisfaction was higher on postpartum self care(Mn 3.15), but the lowest was infant care(Mn 2.84). Differences between educational needs and satisfaction by categories was significant(P=.0001). 4. Relationship between educational needs and levels of satisfaction among primigravida of different characteristics were as follows : 1) Educational needs of minor discomforts area were significantly high among age of 25-29(P=.0108), and over 35 years of age, was satisfied on preparation of labor & delivery, postpartum self care (P=.036, .02). 2) With regard to different level of education, middle school graduates had higher educational needs on discomfort care and postpartum self care(P=.0014, .014). College graduates have had higher educational needs on health maintenance(P=.008) and were more satisfied on preparation of birth(P=.0025) 3) With regard to whether pregnancy was planned or not, no difference was found on educational needs. But the levels of satisfaction was significantly higher in the group of planned pregnancy(discomfort care P=.0454, birth preparation P=.0256, postpartum self care P=.0092). 4) with regard to antenatal education, those who have had some sort of antenatal education, educational needs on birth preparation(P=.0345) was significantly high. And also the levels of satisfaction were significantly higher on every category(P=.0004-.0001). 5) No difference was found on educational needs or level of satisfaction by Job, religions, regularity of antenatal care and complication of pregnancy.

한방병원에 내원한 산후병 환자의 특성 및 증상에 대한 분석 (Analysis on Outpatients with Postpartum Disease at the Korean Gynecology Clinic of Traditional Korean Medicine Hospital)

  • Objectives: The purpose of this study was to analyze the current characteristics of outpatients with postpartum disease and provide a treatment instruction in the clinical field. Methods: To analyze characteristics of outpatients with postpartum disease, We searched medical records from January 1, 2015 to December 31, 2017 using Z34 (Supervision of normal pregnancy), O94 (Sequelae of complication of pregnancy), U327 (産後風) and 388 postpartum patients were analyzed. Results: 1. The general characteristics of the subjects are as follows. The average age was $33.73{\pm}3.62$ years old, 356 (91.75%) patients are full term pregnancy, 201 (51.81%) patients are first delivery, 79 (20.36%) patients had past history of abortion and 63 (16.24%) patients had past history of gynecologic disease. 2. The most subjects visited in April (10.31%). The mean duration from delivery date to visiting date was $131.5{\pm}214.6$ days. The mean number of visits after the initial visit was $1.91{\pm}1.7$. 3. The characteristics related with pregnancy and delivery are as follows. The number of subjects taken assisted reproductive technology was 19 (4.9%) and suffering from gestational disease was 28 (7.22%). The mean body weight difference between full term pregnancy and visiting date was $-8.24{\pm}3.57kg$. The number of subjects discharging lochia on visiting date was 167 (43.04%) and breastfeeding was 262 (67.53%). 4. The most chief complaint was arthralgia in 217 (55.93%) followed by general weakness, cold sensation, edema, sweating disorder, dizziness, lower abdominal pain, digestion disorder, heat sensation, defecation disorder and urination disorder. 5. The most accompanied symptom was arthralgia in 322 (82.99%), followed by sleep disorder, cold sensation, general weakness, sweating disorder, defecation disorder, edema, dizziness, digestion disorder, urination disorder, lower abdominal pain and heat sensation. 6. A total of 356 subjects were taken herbal medicine. The most prescribed herbal medicine to subjects was Gungguijohyeol-eum-gamibang (29.59%). Conclusions: These results could be helpful to diagnose and treat postpartum patients in Korean gynecologic clinical fields.

임신중 발생한 인공승모판 혈전증의 혈전 용해제를 이용한 치료 -치험 1례 보고- (Thrombolytic Therapy for Prosthetic Mitral Valve Thrombosis of Pregnant Patient -A Case Report-)

  • Prosthetic valve thrombosis is rare but it is one of fatal complication after heart valve surgery. Improvements of the valve design and the material have decreased the frequency of thrombosis but have not eliminated completely. And some cases of prosthetic valve thrombosis during pregnancy were reported inspite of adequate anticoagulation therapy.Urgent surgical intervention is indicated for prosthetic valve thrombosis but it is associated with high operative risk, therefore medical thrombolytic therapy such as urokinase or streptokinase therapy is regarded as an alternative therapy. This is a case report of the successful thrombolytic therapy for valve thrombosis in a pregnant patient after mechanical mitral valve replacement.

  • Adrenal rest tumors are a rare extra-adrenal complication of congenital adrenal hyperplasia (CAH) in women although they are more commonly found in the testes of male patients with CAH. An ovarian adrenal rest tumor (OART) may coexist with CAH or imitate its symptoms without CAH. In this case report, we present the case of a woman with OART without CAH, whose main complaint was infertility and who had a baby after successful surgical treatment.

  • Spontaneous hemothorax is rare, with limited data available on its etiology and treatment. We report a case of massive spontaneous hemothorax with a ruptured variceal phrenic vein during pregnancy, likely a complication of the Kasai procedure. Despite closed thoracostomy, the patient's symptoms and imaging findings did not improve. Emergent open thoracotomy and bleeding control were performed.

저체중아 출생과 관련된 산모의 특성 연구 (The Study of Maternal Characteristics of Low Birth-Weight Infant)

  • The purpose of this study was to investigate the characteristics of low birth-weight infants and their's mothers, and to identify the factors which influenced to delivery of L.B.W infants. The data derived from K. General Hospital and J. obs & gyn clinic in pusan from January, 1998 to August, 1998, which were from mothers of having B. W infants. The factors used for this study were characteristics of general, obstetrical, environmental aspects and physical and psychological life experiences during pregnancy. Analysis of data obtained were computerized statistically by using SPSS 7.5 WIN program. the data were analysed as number, frequency, percentage, t-test and ANOVA. The major results obtained of this study were as follows : 1) Gender that L.B.W infants were male in 47.2% and female in 52.8%, body weight of 2001~2500gm was above 8 in 68.5%, and below 5 in 1.8%. The L.B.W infants with complications were 7, which were 6.5%. The kinds of malformations were the Cleft palate & lip, Hyper-kalemia, Hypoglycemia, Meningocele, CHD, Down syndrome and each of them marked 0.9%. 2) In the general characteristics of pregnant women, the age group of 25~29 years was the most common as 46.3%. Over 35 years of age, elderly gravidas were in 7.5%. the height of 156~160cm was the most common as 52.8%. pregnant women of below 150cm height was in 3.7%. body weight of 51~55kg was the most common as 38%. pregnant women of below 45kg were in 19.4%. The women with smoking and drinking episodes during pregnancy were 1.9% and 25%. In the status of marriage, married women were in 95.4%, unmarried ones were in 1.9%, and unmarried couples were in 2.8%. Iin he obstetrical characteristics of pregnant women, pregnant women with gestational age under 37wks were in 45.4%, and the ones over 38wks were 54.6%. At the methods of delivery, normal spontaneous vaginal deliveries were in 51.9%, which were the most common, cesarian section deliveries were in 47.2%, and breech deliveries were in 0.9%. In the environmental characteristics of pregnant women, 40.8% of pregnant women lived in house or apartments with stairs, 23.1% of them lived in the high altitude. the pregnant women who ran a household without a helper were in 65.7%. In the pregnant women who had underwent life experiences of physical and psychological stress during pregnancy. life experiences of physical stresses were described as persistent fatigue due to lifestyle, traumatic experience, illness, move away with an effort, physical impact caused by discord. life experiments of psychological stresses were describeded as trouble with their husbands, discord with one's husband family, family problems, and conflicts due to environmental factors, etc. The number of the pregnant women who had complications during pregnancy was 32, which was 29.6% totally. Among them, pre-eclampsia was in 12.1% and the premature rupture of membrane in 7.4%. 3) In the analysis of the general, obstetrical, and circumstantial characteristics and L.B.W infants. There were statistical difference significantly between the gestational age of pregnant women(F=12.035, P=.000), and the status of marriage(F=3.207, P=.044), and maternal complication(t=2.344, P=.021) etc.

교통사고로 수상한 임산부의 특성 (Characteristics of Injured Pregnant Women by the Traffic Accidents)

  • Purpose: Trauma is one of the major causes of maternal and fetal mortality, and the most common cause of maternal trauma is a traffic accident. In Korea, data about traffic accidents in pregnant women are not widely collected and classified so far. Hence, we studied and analyzed the characteristics of injured pregnant women by the traffic accidents. Methods: From January 2002 to August 2011, pregnant women who were in traffic accidents visiting Emergency Department were studied. Pregnancy out come and the degree of the damage were determined through the retrospective analysis of the medical records. Results: The pregnant women who visited after traffic accidents were total 204 patients. Among them, 176 patients had no complication related to the traffic accidents, 28 patients had complications. The incidence of the complications in the 3rd trimester pregnants was statistically significant higher than that in the other trimesters. The analysis based on the mechanism shows more complications in the pedestrian injury. In the survey by the type of the vehicles, the complications from the trauma associated with a car had lower incidence. The patients arrived at the emergency center by walking had greater numbers than who arrived by an ambulance in the groups occurred the complications. The patients suffered complications who complained pain in trunk especially in abdomen and pelvis than in extremities and complained vaginal discharge, and those showed a statistically significant greater incidence. Conclusion: When pregnant women were injured by the traffic accidents, the factors related to the poor pregnant prognosis were trimester of pregnancy, means of visiting the emergency center, trauma mechanism, and complaining symptoms. Therefore, these factors may be used as a prognostic tool to predict an incidence of complications, length of hospital stay and rate of complications and can be used to plan for treatments.

산전관호 결과에 대한 고찰(임상 통계적 관찰에 의한) (Review of Prenatal Care on Clinical and Statistical Observation)

  • Anions a total of 1,410 women who were taken in th the Ilshin Maternity Hospital in Pusan for child-bearing front January 1 to Hay 31, 1970, review and observation were made on 653 mothers and 661 babies delivered (including twin) who had received parental care, and on 757 mothers and 773 babies delivered (including twin) who had not received parental care. The results are: 1. Out of 1,410 women 653 received parental care, The number is 46.4% 2. Most of them are between 25 and 29 years of age, and it shows the highest percentage. 3. Twenty five % of them visited the hospital care for the first time in the tenth month of pregnancy It was the highest percentage. 4. Those who visited the clinic only once for parental care during the pregnancy were 25.7%, the highest percentage. 5. Those who got parental care among primiparae were 43.7%, and among multiphase were 48.5% The rates are similar to each other. 6. Maturity rate of the babies delivered after receiving prenatal care: mature 91%, Immature- 1% and premature- 8%. 7. It was found that among primiparae were the higher frequency of prematurity. 8. Among babies delivered by mothers after receiving parental care, 8.1% were premature, while 16.1% of babies delivered by those who han't received the care were premature. 9. Mothers between 25 and 29 years of age brought birth to premature. babies most. 10. Frequency of prematurity based on the times of parental care received was: once or twice -8.7% 3 or 4 time-8.9%, 5 or times-10.3%, 7 or 8 times-4.5%, 11 or 12 times-2.7%, 13 or 14 times-3.8%. 11. Percental death rate shows 2.9% among the mothers received prenatal care, and 7.3% among those without parental care. 12. Average weight of a baby boy was 3,114 gm, and that of a baby girl was 3,021 gm. 13. Obstetric complication rate was 63,2% in the babies delivered by mothers with prenatal care, and 85.6% in those delivered by mothers without parental care.
